목회자의 목회간호에 대한 역할기대 (Pastor's Expectations from Parish Nurses)

  • Parish nursing is a community health nursing role developed in 1983 by Lutheran chaplain Granger Westberg. An increasing emphasis on holistic care, personal reseponsibility for a healthy lifestyle, and changes in healthcare delivery systems have undoubtedly facilitated the establishment and nurturance of an innovative nursing role in the community. Parish nurses are functioning in a variety of church congregations of various denominations. The parish nurse is a educator, a personal health counselor, a coordinator of volunteers. The parish nurses helps people relate to the complexed medical care system and assists people to integrate faith and health. The autors conducted a study on pastor's expectations from parish nurses. Results of this study will be useful to those instrumental in planning, initiating, supporting, and evaluating a parish nurses program The research was done on 130 pastors in Taegue and Kyong Sang Buck Do, of various ages ranging from their 20's to 60's: and pastoring churches of various sizes, ranging from under 100 to over 300 members. 94.6% agreed that they needed a parish nurse on their staff; and 86.2% said they wanted to start a parish nurse program in their churches if certain basic conditions were met. The pastors responded that some would hire the nurses on a full-time basis(22.3%), a part -time basis (37.7%) or use volunteer nurses (40%). The pastors said they would expect the following from a parish nurse: health counselling (80.0%) regular health check-ups (78.5%) health care for the elderly (78.5%) health information and education (72.3%) hospice care (72.3%) visiting sick church members at home (69.2%) arranging and training volunteers to help the seek (59.2%) health care for expectant mothers (50.0%) introducing and taking people to health care facilities (46.2%) The pastors were surveyed about specific areas of health education they would want the parish nurse to teach(for example, high blood pressure and heart disease prevention and management(76.2%) ; stress management(74.6%); and diabetes prevention and management(73.8%). The pastors were surveyed about specific areas of health counselling they would expect the parish nurse to do (for example, drug abuse, (73.1), alcohol abuse(64.6%), marriage conflict(60.0%), recovery after the loss of a loved one(56.9%), and women's conflict with parents-in-law(53.8%). The pastors were surveyed about types of things they would want included in regular health check-ups, what they would want a parish nurse to do on home visits, and what they would want included in home care for the elderly. They were also surveyed on what kind of spiritual care they would like parish nurses to give. Most (90.7%) wanted their parish, parishioners to be involved in the parish nurses program as volunteers, and in a variety of ways(such as visiting sick in their homes(68.5%) and helping with housework(63.1%) and taking sick people to health facilities(60%). Parish nurses role, activities, and boundaries of practice should be continuously monitored and refined and a 'case manager' should be conceptualized as an additional or all-encompassing role. An initial parish / community needs and readiness assessment should be done prior to establishing a program to detemine if the congregation is ready, willing, and able to support such a position for at least a 2 to 3 year period.

보건 영양사의 직무만족도 및 자기진단평가 (The Job Satisfaction and Self-assessment of Public Health Nutritionists)

  • This study was carried out to investigate the general characteristics of public health nutritionists, the current status of nutrition services operation, the recognition about nutrition services of public health center related man power, the job satisfaction and self-assessment and the need for a retraining course of public health nutritionists. The subjects were 58 public health nutritionists who responded to the questionnaire distributed at the annual retraining program in 1998. The results of this study are summarized as follows. 1) 46.5% of the nutritionists were 26-30 years old, 62.1% were 4 year university graduates 74.1% were food and nutrition majors and 51.7% were daily workers. 2) Among the public health center-related manpower, the recognition about nutrition services was the highest for the manager of family health section, followed by the head of public health centers, and then the general nurse in public health centers. 3) The ranking of the reasons for job satisfaction of the public health nutritionists was, relationship with colleagues(3.84), inhabitants response after nutrition counselling(3.53), specialized value realization/conviction about duty(3.35), contents of the work(3.10), value achievement(3.08), self achievement/development(3.00), self discretion(2.92), participation in policy decisions(2.90), work load(2.75), chance of retraining and acquisition of new information(2.73), working environment(2.69), supervisio $n^port by superiors(2.67), salary(2.38), supply of necessary education material, technique(2.37), and budget security(2.22). 4) The satisfaction of the inhabitant's responses after nutrition counseling was the highest among the 4 year university graduates(p<0.05), the satisfaction of the specialized value realization/conviction about duty was the highest among the nutritionists 26-30 years old(p<0.05). Food and nutrition majors(p<0.05) and those having worked less than 3-5years at public health centers also showed much satisfaction(p<0.05). Satisfaction with the salary was the lowest among the food and nutrition majors(p<0.01) and daily workers(p<0.001). The satisfaction with the participation in policy decisions was the lowest among the daily workers(p<0.01). 5) The ranking for the level of self-assessment were, nutrition and dietetic practice(2.92), communication(2.80), management(2.77), public health science and practice(2.66)(p<0.01). The general characteristics such as the level of education, major, employment condition, current public health center's tenure, and charge experience of the nutrition guidance work were not significantly related to self-assessment except the management part(p<0.05). The higher the satisfaction of specialized value realization/conviction about the duty, the better the total score on the self-assessment(p<0.05)..

건설 단계별 안전보건대장을 활용한 중·소규모건설현장의 안전성 확보 방안 (A Study on Securing Safety at Small and Medium-sized Construction Sites using The Safety and Health Register at Each Construction Stage)

  • 지난 20년간 건설산업의 재해는 절대적인 수치에서는 감소했다. 그러나 중·소규모 건설현장에서의 발생한 재해는 건설산업 전체의 약 80% 이상을 차지하고 있으며 지속적인 증가세를 나타내고 있다. 건설산업에서의 재해 저감을 위해 고용노동부는 건설업에서 상대적으로 막강한 권한을 보유한 발주자에게 안전에 대한 책임을 의무화하여 선제적으로 재해 예방을 하도록 단계별 안전보건대장 작성 제도를 법제화하였다. 제도의 활성화를 위해서는 많은 연구들이 필요하지만, 지금까지 안전관리계획, 유해·위험방지계획 등과 관련한 연구는 많았지만 안전보건대장과 관련한 연구는 없었다. 따라서 본 연구에서는 중·소규모 건설현장의 재해 저감을 위한 방안으로 발주자의 안전보건관리 절차와 연계한 개선된 안전관리 절차를 제시하였다. 공사단계의 안전관리는 공종별 교육, 안점점검을 하도록 하여 현장상황과 일치하는 관리가 되도록 하였으며, 모든 정보는 전통적인 방식의 서류형태의 문서를 대체하여 데이터 기반의 절차가 되도록 하여 관리자의 업무부담을 낮추었다. 개념적인 절차의 제시로 향후 추가 연구가 필요하지만, 중·소규모 건설현장의 안전관리 지원 정보시스템 개발을 위한 기초자료로 활용될 것으로 기대한다.

거주성 관점의 미국 이재민 임시대피시설 공간계획 관련 지침분석 (Analysis of the Space Planning Guidance about the Temporary Shelter Emphasizing Habitability for Disaster Victims in U.S.)

  • The purpose of this study was to analyze the information about the disaster temporary shelter space planning guidance described in the U.S. shelter guidelines in terms of the habitability for disaster victims and to address the implications and potential impacts of its findings for the improvement of the shelter design standards and guidelines in Korea. The researchers reviewed four federal and non-governmental organization shelter field and design guidelines and attributes regarding the shelter space planning were classified in accordance with four habitability categories: Safety, Health, Convenience, and Comfort. Three major findings emerged from this study: 1)A total of 72 items about the shelter space planning were extracted from guidelines, and the majority of items are concerned with dormitory areas and sanitary spaces. Other items were about accessibility, children respite care area, signage, health service areas, food preparation and serving areas, parking and drop-off areas, registration and waiting areas, shelter manager and staff areas, lounge and storage areas, and household pets area. 2)Most of these items are strongly related to the convenience category(66.7%), followed by comfort(40.3%), safety(30.6%), and health(25.0%). 3)The habitability of the disaster temporary shelter can be established with considerations on the needs of disaster victims and vulnerable people, minimum occupancy space per person, separate sanitary spaces for the privacy, safe and convenient access routes to the shelter, and the provision of children areas for safety and comfort. The study findings will contribute to the development of the disaster shelter guidelines in Korea by suggesting shelter space planning indicators related to the habitability for the governmental and non-governmental organizations' immediate and systematic responses to the disaster.

PSR 모델을 적용한 근로환경조사 지표 개발 (A Study on Health-related PSR Model using Korean Working Conditions Survey Data)

  • 본 연구에서는 업종에 따른 다양한 근로환경 요인들을 파악하고 관련 분야의 산업안전보건 정책을 판단할 수 있도록 근로환경조사 기초자료를 분석하여 사회요인을 고려한 지수 및 지표 개발을 목표로 하였다. 지표개발을 위해 OECD에서 제시하는 지표개발 가이드라인과 해외의 지표개발 사례들을 벤치마킹하여 수행되었다. OECD의 압력-상태-대응 모형을 벤치마킹하여 건강과 관련 있는 지표들을 제시하였다. 근로환경조사의 압력-상태 건강관련 지표는 압력은 물리적 위험환경, 노동시간, 업무환경, 사회 환경으로 구성되었고, 이에 따른 상태 항목은 건강관련 항목으로 정신건강, 신체건강, 건강문제로 인한 결근, 업무 만족으로 구성되었다. 그 결과 임금근로자, 상용근로자, 관리자, 사무종사자, 전문가 및 관련 종사자, 50세 미만 근로자의 압력-상태 지표가 상대적으로 양호하였다. 반대로, 50세 이상, 자영업자, 일용 직, 농림어업 숙련직, 단순노무, 장치 기계 조립 근로자의 압력-상태 지표가 상대적으로 취약한 것으로 나타났다.

학교급식의 관리현황과 문제점에 관한 고찰 (Consideration on the Point at Issue and management State of School Lunch)

  • This paper studies regarding the problems of transmitting disease through food and its prevention in terms of current problematic food poisoning and the transfer of bacterial disease while providing lunch for school children. As the problems of school lunch program, the number of food poisoning and of the transfer of bacterial disease is highly increasing because of the shortcomings of systematic safety management toward food and of the lack of meal supplier's awareness about sanitation.For the prevention the following suggestions are provided:1. It is necessary to shift from the system of meal supply for nutrition to that of meal supply for health. 2. A system that offers a responsibility for nutrition to that of meal supply to a person in charge of hygienist, or a cook, including the a nutritionist, is needed. 3. It is necessary to gain the help from the department of sanitation in city or country for the sanitation guidance and inspection of school meal supply, not to totally rely on the local ministry of education. 4. A manager or employees related to school/group meal supply need to get education constantly on sanitation and public information. 5. For the long term, a systematic sanitary management needs to be accomplished by introducing HACCP system.

  • 본 연구에서는 창업보육센터 자체의 조직과 창업보육센터에 입주하여 기술개발과 경제활동에 힘쓰고 있는 입주업체의 조직과 구성원들의 운영 활동의 문제점을 살펴보고, 두 조직간의 상호관계를 도출하였다. 또한 나타난 제반 문제점들에 대한 해결방안 및 조직의 구성, 체계적인 관리방법 등을 통해 지역 창업보육센터의 자립 경영 방향을 제시하였다. 이를 구체적으로 살펴보기 위하여 창업보육센터 입주기업들을 유형별로 4가지 형태로 나누어 분류하였고, 실제 한 대학의 예를 들어 각 유형 별 기업의 특징과 장 단점 그리고 발전방안을 제시하였다.

피부미용사의 손 부위 통증에 관한 연구 (A Study on Pain Characteristics in Parts of the Hand among Estheticians)

  • Objectives: The purpose of this study was to investigate estheticians' self-reported pain symptoms and pain levels according to the parts of the hand. Methods: Data were collected through a questionnaire survey among 116 estheticians from 31 skin care centers and spas. Information about general characteristics, job characteristics and work characteristics were based on data from the self-administered questionnaire. To determine the self-reported pain symptoms and pain levels according to the parts of the hand, a Cornell Musculoskeletal Discomfort Questionnaire was used. Results: The subjects had the greatest self-reported pain symptoms in the thumb(Right: 53.4%, Left: 46.6%) and felt the highest level of pain in the wrist(Right: 4.87 points, Left: 4.71 points). Physical burdens and age had statistically significant effects on the pain level in the right hand, while physical burdens and the manager rather than the director had statistically significant effects on the pain level in the left hand. Conclusions: Since the thumb and wrist have the greatest subjective pain symptoms in the hand, the use of proper tools is recommended in order to reduce the usage of the thumbs during massage, as is regular stretching to protect the wrists.

  • 최근의 의료정책과 정보기술의 발달은 병원이 주변 환경에 맞춰 비용을 줄이고 의료의 질을 향상시킬 필요를 가지게 한다. 즉, 정책과 기술의 변화로 병원 업무가 단순한 진료비 계산과 보험청구중심에서 벗어나 경영정보시스템(MIS), 의료영상저장전송시스템(PACS), 처방전달시스템(OCS), 전자의무기록시스템(EMR), 의사결정지원시스템(DSS) 등이 개발되고 있다. 특히 유비쿼터스 네트워크 및 관련기술과의 융합은 의료정보시스템을 의료 IT의 관련 정보시스템들과 통합되는 방향으로 진화해가고 있으며, 앞으로도 그 가속도는 더할 전망이다. 이러한 변화와 인터넷 환경의 발달은 의료정보 시스템의 근본적인 변화를 요구한다. 모바일 의료정보시스템은 기존에 의료정보 환경에서 구축되었던 병원의 시스템을 PDA 등의 모바일 환경으로 구축하는 것을 말한다. 기존 시스템의 모바일 네트워크 환경을 통해 언제, 어디서든지 의료진의 접근이 가능하게 됨으로써 업무의 효율을 높임은 물론이고 실시간 업무 처리 및 유지보수 비용의 절감을 통한 수익성 증대에도 중대한 역할을 하게 된다. RFID는 자동인식 및 데이터 획득 기술로 사람의 작업이나 판단을 궁극적으로 제외하고 객체가 갖고 있는 정보를 자동적으로 취득, 온라인으로 관련 데이터를 자동처리 시스템 구현의 핵심요소 기술이다. 본 논문에서는 RFID 응용 서비스가 실용화하고 있는 실정에서 통합의료정보시스템을 위한 환자 진료의 서비스 강화를 도모하도록 RFID 기반의 서버 및 모바일 클라이언트 의료정보시스템을 구현하고, 실제 병원내의 여러 디바이스가 연결된 데이터베이스를 통합적으로 관리하는 환자진료 및 실시간 원무 관리의 자동화를 위한 태그 매니저(Manager)와 기존의 EMR, HIS, PACS의 호환을 위한 DB 서버 에이전트를 설계 및 구현하였다. 다양한 의료정보시스템에서 유비쿼터스 컴퓨팅 환경을 위한 모바일 의료정보시스템의 RFID 응용시스템 은 환자의 진료카드에 태그를 부착하여 기본적인 환자의 접수, 진료, 검사의 대기시간의 단축을 위한 데이터를 처리한다.
