A review of the qualification criteria for the national examinations for emergency medical technicians (응급구조사 국가시험 응시 자격기준에 관한 고찰)

  • Purpose: Emergency medical technicians (EMTs) have so far been trained as professionals under the same conditions, with no change in the 1995 Act. We aimed to find ways for them to secure expertise in accordance with social needs by strengthening the quality of the EMT education. Methods: This is a descriptive study comparing the operation status of the national emergency medical examination conducted by the Korea Health Personnel Licensing Examination Institute, and the national examinations of EMT paramedic and EMT basic. The scope of the national examinations for EMT was compared by subject and area. Results: The national written exam for EMT paramedic consists of five areas. EMT basic does not include basic medicine in three related subjects, 11 areas, and 18 detail areas. Paramedic care does not include advanced pediatric life support. In addition, nine areas and 20 detail areas are not included. Conclusion: The study suggests the need for institutional supplementation so that those who have completed EMT basic and the subjects prescribed by the ordinance of the Ministry of Health and Welfare at universities, etc., in the Higher Education Act can take the EMT paramedic national exam.

National Health Insurance System of Korea: Resource-Based Relative Value Scale and a New Healthcare Policy (우리나라의 건강보험 수가 시스템: 상대가치 그리고 새로운 건강보험 보장성 강화 대책)

  • The resource-based relative value scale (RBRVS) compares the value of a medical practice to the consumption of resources, which consist of the work of the physician, practice expenses, and professional liability insurance. At the time of the 2nd revision of RBRVS, the fee for radiological examinations had been reduced due to the high preservation rate. In RBRVS, practice expenses account for most of the compensation of radiological examinations, and physicians' work is relatively undervalued. A new healthcare policy (Moon Jae-In care) consists of the expansion of the National Health Insurance (NHI) coverage, reduction of patient charges for the vulnerable class, and support for catastrophic medical expenses. However, Moon Jae-In care is expected to negatively affect the NHI in Korea financially. The expansion of the insurance coverage for ultrasonography and MRI examinations is a significant part of the Moon Jae-In care, and radiological societies should establish fair compensations for physicians' work within the field of radiology while implementing the Moon Jae-In care.

The Factors Affecting Health-related Quality of Life in Urban Working Elderly (도시거주 일하는 노인의 건강관련 삶의 질 영향요인)

  • The purpose of this study was identify the factors affecting health related quality of life in urban working elderly. This study used the data from the 6th Korea National Health and Nutrition Examinations Survey and analyzed the demographic characteristics, work related factors, health related factors and health-related quality of life in living urban and aged 65 and over. Data were analyzed using the IBM SPSS 18.0 program. As a result, there were significant differences according to age, sex, marital status, generation composition, education level, family income, house ownership, allowance, private medical insurance, smoking, alcohol, stress. The affecting factors of health-related quality of life were sex, education level, allowance, private medical insurance. According to the results of this study, the health-related quality of life of the elderly should be improved by considering gender specific characteristics and economic status.

A Study on Oral Health Behavior of the Industrial Workers (산업체 근로자의 구강보건행태에 관한 조사 연구)

  • This study was conducted to provide basic data for improvement of oral health of workers, by investigating oral health behaviors of industrial workers. About sixty percent(60.9%) of subjects experienced in having dental examinations and 54.3% of them didn't receive follow-up treatments after the dental examinations. Also, those who brushed teeth twice per week(58.6%) and averagely, earned 2~3 million won of monthly income(p<.01), showed a higher frequency of tooth brushing. Experienced workers(p<.01) were more interested in oral health. Those who were engaged in other jobs and had less than 10~20 years of experience(p<.05), had lots of experience in staying away from work. More aged group recognized that oral health education is necessary(p<.05). The population of oral examination and health instruction may have a favorable impact on maintenance of oral health status and improvement in quality of life.

Comparison on health behaviors and health checking by status of workers and age of single households (1인가구의 고용형태 및 연령에 따른 건강관련 생활습관 및 검진행태 비교)

  • In this study we conducted a questionnaire survey by purposive quota sampling(n=566) by status of workers(permanent workers, temporary workers and business owners) and age group(20-59 years old) for single households engaged in economic activities in order to compare the health behaviors and health checking by employment type and age. By employment type, the sleep time of permanent workers was significantly less than that of other groups(p<0.001), and the rates of health and oral examinations in permanent workers were significantly higher(p<0.001). By age group, as the age increased, the oral condition was worse(p<0.001), but the rates of not taking oral examinations were significantly higher(p=0.023). This study can be meaningful as basic data for making a healthy living environment according to various characteristics of single households.

A Study on Scheduling Periodic Examinations for the Early Detection of Breast Cancer in Korea (유방암 조기진단을 위한 검진주기 결정에 대한 연구)

  • Objectives: The purposes of this study were to propose a screening schedule for the early detection of breast cancer among Korean women, as based on the statistical model, and to compare the efficacy of the proposed screening schedule with the current recommendations. Methods: The development of the screening schedule for breast cancer closely followed the work of Lee and Zelen (1998). We calculated the age-specific breast cancer incidence rate from the Korea Central Cancer Registry (2003), and then we estimated the scheduling of periodic examinations for the early detection of breast cancer, using mammography, and based on the threshold method. The efficacy of the derived screening schedule was evaluated by the schedule sensitivity. Results: For estimating the screening schedule threshold method, we set the threshold value as the probability of being in the preclinical stage at age 35, the sensitivity of mammography as 0.9 and the mean sojourn time in the preclinical stage as 4 years. This method generated 14 examinations within the age interval [40, 69] of 40.0, 41.3, 42.7, 44.1, 45.4, 46.7, 48.0, 49.3, 51.0, 53.2, 55.3, 57.1, 59.0 and 63.6 years, and the schedule sensitivity was 75.4%. The proposed screening schedule detected 85.2% (74.5/87.4) of the cases that could have been detected by annual screening, but it required only about 48.7% (14.0/30.0) of the total number of examinations. We also examined the threshold screening schedules for a range of sensitivities of mammography and the mean sojourn time in the preclinical stage. Conclusions: The proposed screening schedule for breast cancer with using the threshold method will be helpful to provide guidelines for a public health program for choosing an effective screening schedule for breast cancer among Korean women.

A Study on the Status and Improvement Direction of Radiographic Imaging Examination Assessment in Korea Medical Institutions (한국 의료기관의 방사선 영상검사 평가 현황 및 과제)

  • This study was conducted to analyze the status radiological imaging examinations assessment in Korea medical institutions conducted in the public sector and suggest a direction for improvement. Among the assessment of medical institutions, the main assessment related to radiographic imaging examinations are the certification evaluation of medical institutions and the adequacy assessment of radiographic imaging examinations. The certification evaluation of medical institutions evaluates the image inspection operation process, provision of accurate results, and compliance with safety management procedures. In the assessment of adequacy of radiographic imaging examinations, structural indicators related to manpower and equipment, patient evaluation implementation rate, and exposure reduction programs were included. However, for safer and higher-quality radiological imaging examinations, it is necessary to increase the participation rate of medical institutions in certification evaluations. In addition, it is necessary to improve the manpower indicator, and incentive payments can be considered to induce quality improvement of medical institutions in the future. Integrated management of radiation exposure at the national level should also be carried out simultaneously.

A Study on Optimum kVp in Four Spot Films of Stomach Examinations (위(胃) 사등분할촬영시(四等分割撮影時) 적정(適正)한 관전압(管電壓)에 관(關)한 검토(檢討))

  • In an attempt to provide optimum kVp for four spot films of stomach examinations, we measured experimentally film density and scatter radiation with field size. And to investigate the effect of concentrations in barium sulphite and kVp in spot films of stomach fluoroscopy were carried out and the following results were obained. 1. The entire density of film by field size has the sharpest increase from $10cm^2$ to $100cm^2$, and relatively flattened curve beyond $500cm^2$ in field size. 2. The quantity of scatter radiation reaching an X-ray film depends upon field size: the larger the fields, the more scatter radiation. 3. It is necessary for increasing 3 to 5kVp as for the absence of barium sulphite and 5 to 7 kVp in the case of 20 per cent and 25 per cent in barium sulphite concentrations to produce uniform density in the four spot films for stomach fluoroscopy.

Prediction of Physical Examination Demand Using Text Mining (텍스트 마이닝을 이용한 건강검진 수요 예측)

  • Recently, physical examinations have become an important strategy to reduce costs for individuals and society. Pre-physical counseling is important for an effective physical examination. However, incomplete counseling is being conducted because the demand for physical examinations is not predicted. Therefore, in this study, the demand for physical examination was predicted using text mining and stepwise regression. As a result of the analysis, the most recent text data showed a high explanatory power of the demand for physical examination. Also, large amounts of data have high explanatory power. In addition, it was found that the high frequency of the text "health food" reduces the number of health examination customers. And the higher the frequency of the text of the word "food", the lower the number of physical examination customers. However, when the word "wild ginseng" was exposed a lot on Twitter, the number of physical examination customers visiting hospitals increased. In other words, customers consume efficiently by comparing the health examination price with the price of consumer goods. The proposed research framework can help predict demand in other industries.

  • The study analyzes the effects of quality factors of health check-up service applied to military officer every year on the value and the behavioral intention of consumers so that it is be able to suggest alternatives to realize a systematic and realistic health check-up system. The survey period was from September 27 to October 4, 2016 and 294 out of 300 patients were finally selected; Frequency analysis, ANOVA analysis, Pearson's correlation analysis and multiple regression analysis were performed. In conclusion, the Age(p<0.05), Number of examinations(p<0.01), Medical communication(p<0.01), Quality of service(p<0.001), Medical Services Value(p<0.001) were drawn as characterized results by sectors regarding health screening service quality and customer satisfaction and behavioral intention. In the future, the military should improve the quality of education and publicity programs to improve the awareness of health checkup system, and improve quality of health checkup service to detect diseases early.
