Factors associated with Suicidal Ideation by Age Group among Korean Adults: Using the Fifth Korea National Health and Nutrition Examination Survey(KNHANES V, 2012) (한국 성인의 연령계층별 자살생각 관련 요인: 국민건강영양조사 제5기 3차(2012) 자료를 이용하여)

  • Objectives: This study aimed to investigate factors influencing suicidal ideation among Korean adults. Methods: Cross-sectional data from the fifth Korea National Health and Nutritional Examination Survey conducted in 2012 were analyzed in this study. The subjects were categorized into three groups by subjects' age(defined as young adults(20-39), middle-aged adults(40-59), and the elderly(60 or more). Data were analyzed by frequency, Chi-square test and multiple logistic regression using SPSS 18.0. Results: The factors influencing suicidal ideation were different among three age groups. The influencing factors were (1) gender, marital status, household income, perceived stress and depression in the young adult group, (2) household income, perceived health status, perceived stress and depression in the middle-aged group, (3) gender, regular physical activity, smoking, perceived health status, perceived stress and depression in the elderly group. Conclusions: The health education considering the characteristics of each age group should be developed and applied to prevent adults' suicidal ideation because the factors influencing suicidal ideation were revealed differently between the age groups.

Cancer screening rate in people with diabetes in the Korean population: results from the Korea National Health and Nutrition Examination Survey 2007-2009

  • OBJECTIVES: To investigate the screening rates for gastric, breast, and cervical cancer in people with diabetes compared with people without diabetes. METHODS: Data from the Korea National Health and Nutrition Examination Survey (2007-2009) were used. Cancer-free men who were 40 years old and over and cancer-free women who were 30 years old and over were included. The lifetime screening rate and regular screening rate were compared in people with and without diabetes. RESULTS: Fewer people with diabetes than people without diabetes had ever received cancer screening (53.5 vs. 59.5%, p<0.001 for gastric cancer; 60.5 vs. 71.5%, p<0.001 for breast cancer; and 49.1 vs. 59.6%, p<0.001 for cervical cancer). Fewer people with diabetes than people without diabetes received the recommended screenings for gastric cancer (38.9 vs. 42.9%, p<0.001), breast cancer (38.8 vs. 44.6%, p<0.001), and cervical cancer (35.1 vs. 51.2%, p<0.001). In subgroup analyses according to socioeconomic factors, the lifetime and recommended screening rates were lower in the diabetic population in most socioeconomic subgroups. In the multivariate analysis adjusted for socioeconomic factors, people with diabetes showed lower lifetime screening rates for gastric and cervical cancer (odds ratio [OR], 0.8; 95% confidence interval [CI], 0.7 to 0.9 and OR, 0.7; 95% CI, 0.6 to 0.9), and lower regular screening rates for breast and cervical cancer (OR, 0.7; 95% CI, 0.6 to 0.9 and OR, 0.7; 95% CI, 0.5 to 0.9). CONCLUSIONS: The cancer screening rate in people with diabetes was lower than in people without diabetes. Considering the higher cancer risk in people with diabetes, efforts to increase the screening rate in this high-risk population should be implemented.

Prevalence Estimation of Several Chronic Diseases through Community Based Health Examination Survey (지역사회 주요 만성질환의 유병률 추정 - 구리시 '97 건강진단조사 결과를 기초로 -)

  • Objectives. The health examination survey was conducted in order to estimate the prevalence of major chronic diseases in a community Methods. A multi-stage stratified probability sample of 1,656 persons, aged from 20 to 69 years, was drawn out from Kuri City. All sampled persons were personally informed of the purpose, content and procedures of the survey and mobile examination center was run between 6 AM and 9 PM for the convenience of the participants. Results. 854 persons (339 males and 515 females) completed this survey with a response rate of 53.4%. Prevalent diseases in male were hypertension(19.0%), HBs Ag carrier(9.8%), impaired glucose tolerance(8.7%), diabetes mellitus(6.3%), hypercholesterolemia(3.4%), anemia(2.7%) and degenerative arthritis(2.6%) in order of prevalence rate. Prevalent diseases in female were osteoporosis(22.5%), impaired glucose tolerance(17.3%), anemia(12.8%), hypertension(12.4%), degenerative arthritis (6.6%), HBs Ag carrier(5.8%), diabetes mellitus(5.4%) and hypercholesterolemia(3.5%) in order of prevalence rate. Conclusions. This results suggest that hypertension was most prevalent in adult male whereas diabetes mellitus including IGT, osteoporosis were most prevalent in adult female, in Kuri City.

Development of Web-based Multimedia Content for a Physical Examination and Health Assessment Course (웹기반의 건강사정 멀티미디어 컨텐츠 개발)

  • Purpose: This study was to develop Web-based multimedia content for Physical Examination and Health Assesment. Method: The multimedia content was developed based on Jung's teaching and learning structure plan model, using the following 5 processes: 1) Analysis Stage, 2) Planning Stage, 3) Storyboard Framing and Production Stage, 4) Program Operation Stage, and 5) Final Evaluation Stage. Results: The web based multimedia content consisted of an intro movie, main page and sub pages. On the main page, there were 6 menu bars that consisted of Announcement center, Information of professors, Lecture guide, Cyber lecture, Q&A, and Data centers, and a site map which introduced 15 week lectures. In the operation of web based multimedia content, HTML, JavaScript, Flash, and multimedia technology(Audio and Video) were utilized and the content consisted of text content, interactive content, animation, and audio & video. Consultation with the experts in context, computer engineering, and educational technology was utilized in the development of these processes. Conclusions: Web-based multimedia content is expected to offer individualized and tailored learning opportunities to maximize and facilitate the effectiveness of the teaching and learning process. Therefore, multimedia content should be utilized concurrently with the lecture in the Physical Examination and Health Assesment classes as a vital teaching aid to make up for the weakness of the face-to- face teaching-learning method.

Breast Cancer Knowledge, Perception and Breast Self-Examination Practices among Yemeni Women: an Application of the Health Belief Model

  • Background: The incidence of breast cancer is rapidly increasing in Yemen with recent indications of constituting one-third of female cancers. The main problem in Yemen remains very late presentation of breast cancer, most of which should have been easily recognisable. Since stage of disease at diagnosis is the most important prognostic variable, early diagnosis is an important option to be considered for control of breast cancer in low resourced settings like Yemen. In the present study, we aimed at describing breast cancer knowledge, perceptions and breast self-examination (BSE) practices among a sample of Yemeni women. Materials and Methods: This cross-sectional study covered 400 women attending four reproductive health centres in Aden, Yemen through face-to-face interview using a structured questionnaire during April - July 2014. We collected data on sociodemographic characteristics, knowledge about breast cancer, and screening practices as well as respondents' perceptions based on the five sub scales of the Health Belief Model (HBM): perceived susceptibility; perceived severity; perceived barriers; perceived benefits; and self-efficacy. The response format was a five-point Likert scale. Statistical Package for Social Sciences (SPSS 20) was used for statistical analysis. Statistical significance was set at p<0.05. Logistic regression analysis was conducted with BSE as a dependent variable. Results: The mean age of women was 26.5 (S.D=5.6) years. The majority (89.0%) had never ever performed any screening. Two-thirds of respondents had poor knowledge. Perceived BSE benefits and self-efficacy and lower BSE barriers perception were significant independent predictors of BSE practice. Conclusions: Poor knowledge and inadequate BSE practices are prevailing in Yemen. The need for implementing culturally sensitive targeted education measures is mandatory in the effort to improve early detection and reduce the burden of breast cancer.

Associations of Blood Lead and Cadmium Levels with Hypertension using the Korea National Health and Nutrition Examination Survey III-VI (국민건강영양조사 자료를 활용한 혈 중 납과 카드뮴의 고혈압과의 관련성)

  • Objective: A significant association between blood lead levels and hypertension has been reported in many studies. The relationship between cadmium and hypertension has been debated as well. We aimed to study the association of lead, cadmium, and both with hypertension in the Korean general population. Methods: We examined 5,967 adult men and 6,074 women who participated in the Korea National Health and Nutrition Examination Survey III-VI (2005, 2008-2013 years). Logistic regression models were used to examine the relationship between blood lead concentration and blood cadmium concentration and hypertension using logtransformed blood lead and cadmium concentrations as independent variables after covariate adjustment. Results: Adjusted for general characteristics, the odds ratio of log-lead to hypertension was 2.71 (1.82-4.03), and log-cadmium to hypertension was 2.52 (1.83-3.47). Estimates were found to be statistically significant (p<0.001). When a multiple logistic model was applied, the odds ratio of log-lead and log-cadmium for hypertension were 2.24 (1.50-3.36) and 2.24 (1.62-3.10), respectively. The standardized estimate coefficients of log-lead and logcadmium for hypertension were 4.77 and 6.65, respectively. Conclusion: We observed the association of blood lead concentration, blood cadmium concentration, and both with hypertension. This study suggests that exposure to lead and exposure to cadmium are both risk factors for hypertension.

Lack of Health Insurance Increases All Cause and All Cancer Mortality in Adults: An Analysis of National Health and Nutrition Examination Survey (NHANES III) Data

  • Background: Public use National Health and Nutrition Examination Survey (NHANES III) and NHANES III linked mortality data were here applied to investigate the association between health insurance coverage and all cause and all cancer mortality in adults. Patients and Methods: NHANES III household adult, laboratory and mortality data were merged. Only patients examined in the mobile examination center (MEC) were included in this study. The sampling weight employed was WTPFEX6, SDPPSU6 being used for the probability sampling unit and SDPSTRA6 to designate the strata for the survey analysis. All cause and all cancer mortalities were used as binary outcomes. The effect of health insurance coverage status on all cause and all cancer mortalities were analyzed with potential socioeconomic, behavioral and health status confounders. Results: There were 2398 sample persons included in this study. The mean age was 40 years and the mean (S.E.) follow up was 171.85 (3.12) person months from the MEC examination. For all cause mortality, the odds ratios (significant p-values) of the covariates were: age, 1.0095 (0.000); no health insurance coverage (using subjects with health insurance), 1.71 (0.092); black race (using non-Hispanic white subjects as the reference group) 1.43, (0.083); Mexican-Americans, 0.60 (0.089); DMPPIR, 0.82, (0.000); and drinking hard liquor, 1.014 (0.007). For all cancer mortality, the odds ratio (significant p-values) of the covariates were: age, 1.0072 (0.00); no health insurance coverage, using with health coverage as the reference group, 2.91 (0.002); black race, using non-Hispanic whites as the reference group, 1.64 (0.047); Mexican Americans, 0.33 (0.008) and smoking, 1.017 (0.118). Conclusion: There was a 70% increase in risk of all cause death and almost 300% of all cancer death for people without any health insurance coverage.

Expanding the Supply of Home Health Nurses : Post-Master's Program and Certification Examination (가정전문간호사 공급 확대를 위한 방안: 석사후과정과 자격시험)

  • Purpose: The study aim was to examine the home healthcare system and relevant education, as well as the special certification examination, and propose a plan to increase the supply of home health nurses. Methods: A narrative literature review was conducted using data from research articles, Korean and U.S. educational institutes and organizations, Korean national statistical data, government press releases, and related medical legislation. Results: Between 2005 and 2006, 763 home health nurses were certified through the special qualification examination; however, in the 16 years from 2007 to 2023 (after the graduate-level program was established), a total of 555 home health nurses were certified, with an average of approximately 35 per year. Currently, 790 home health nurses are working at 194 medical institutions nationwide. Relatively few institutions exist in rural areas, and the supply of home health nurses is low. Only seven educational institutions offer home health nurse certification programs, with a total of 77 designated students. In contrast to Korea, post-master's certification courses are offered in the U.S.. Conclusion: To expand the supply of home health nurses, we recommend revising the rules for the special qualification examination and introducing a post-master's certification program for home health nurses. Future studies should provide additional education for applicants from other specialties in post-master's certification programs.

Health Beliefs and Breast Self-Examination among Undergraduate Female Students in Public Universities in Klang Valley, Malaysia

  • Background: Breast cancer is the most frequently occurring cancer in women and the most common cause of cancer death worldwide. Materials and Methods: A cross-sectional study was carried out among 792 female undergraduate students in public universities in Klang Valley, Malaysia, from January to April 2011. Data were collected using a validated questionnaire developed for this study. Results: The mean age of respondents was $21.7{\pm}1.2$ years. Most of them were single (96.8%), Malay (91.9%) and 150 (19.6%) claimed they had practiced BSE. There was a significant differences between performers and non-performers correlated to age, marital status, check breast by doctor, and being trained about BSE. Performers had lower mean scores for perceived barriers and susceptibility and higher mean score for confidence. Stepwise logistic regression analysis yielded four significant predictor variables. Conclusions: Overall our findings indicate that the practice of BSE while perceived as being important is not frequently practiced among female in Malaysia. Targeted education should be implemented to improve early detection of breast cancer.

  • Objectives This study was purposed to develop the clinical practice guideline for examination of Sasangin disease and diagnosis for Sasang Constitution. Methods "Donguisusebowon" and many articles were reviewed and examined for developing clinical practice guidelines. And "Guideline for Educating Public Health Korean Medical Doctors on Diagnosis of Sasang Constitution" was basically used to develop clinical guidelines. Results & Conclusions By researching and discussing the examination of Sasangin disease and diagnosis for Sasang Constitution, we make the principle of the clinical practice guideline, including methods using body shape, facial shape, vocal characteristic and personality etc.
