Reimbursement of Digital Therapeutics: Future Perspectives in Korea

  • Digital health is rapidly growing worldwide and its area is expanding from wellness to treatment due to digital therapeutics (DTx). This study compared DTx in the Korean context with other countries to better understand its political and practical implications. DTx is generally the same internationally, often categorized as software as a medical device. It provides evidence-based therapeutic interventions for medical disabilities and diseases. Abroad, DTx support entailed state subsidies and fundraising and national health insurance coverage. In the case of national health insurance coverage, most cases were applied to mental diseases. Moreover, in Japan, DTx related to hypertension will possibly be under discussion for national health insurance coverage in 2022. In overseas countries, coverage was decided only when the clinical effects were equivalent to those provided by existing technology, and in the UK, real usage data for DTx and associated evaluations were reflected by national health coverage determination. Prices were either determined through closed negotiations with health insurance operating agencies and manufacturers or established based on existing technology. Concerning the current situation, DTx dealing with various diseases including hypertension are expected to be developed near in the future, and the demand for use and compensation will likely increase. Therefore, it is urgent to define and prepare for DTx, relevant support systems, and health insurance coverage listings. Several support systems must be considered, including government subsidies, science/technology funds, and health insurance.

  • Purpose: This study examined the effects of stress vulnerability and parental burnout on the mental health of women with early school-aged children, with a focus on the mediating role of spirituality. Methods: A survey was conducted among 171 women with early schoolaged children in Gyeonggi Province, Gangwon Province, and Seoul. Data were collected from September to December 2022 using the Korean-Symptom Check List 95, the Parental Burnout Assessment, and the Spirituality Assessment Scale. The data were analyzed using structural equation modeling with SPSS/WIN 22.0 and AMOS 20.0. Results: The study model demonstrated a good fit, explaining 40.5% of the variance in mental health through stress vulnerability, parental burnout, and spirituality. Spirituality had a significant direct impact on mental health. Additionally, participants' spirituality directly influenced their mental health, while stress vulnerability and parental burnout indirectly affected their mental health and were mediated through spirituality. Conclusion: Stress vulnerability and parental burnout are negatively associated with mental health, while spirituality partially mediates these effects. Implementing a program to promote spirituality is suggested to assist mothers in recognizing the value and meaning of parenting activities during nursing interventions for mental health.

산업보건 위험성평가 기법의 현장 적용에 관한 연구 (A Study on the Field Application of Occupational Health Risk Assessment Method)

  • Scientific exploration of how occupational health risks relate with occupational illnesses are essential for mitigating health-related issues in industries. This study analyzed the risk scores obtained by occupational health risk assessments at 3,172 manufacturing companies and examined their effects on occupational illness. Statistical analyses revealed that companies with an occupational health manager (scored 89.1 out of 100) had significantly higher activity scores of health management compared to those without (78.2). However, companies with a history of occupational illness (79.1) or those classified as high-risk industries (85.2) had significantly lower activity scores than their counterparts (81.7, 87.3). In addition, regression analyses using factor analysis showed that latent risk factors such as cardiovascular disease/job stress, health management, and musculoskeletal problem significantly influenced the risk of occupational illness. The activity factors such as health management, work environment management, and regulatory complaisance significant impacted the reduction of occupational illness. The findings of this study can be used to improve the occupational health risk assessment method and utilized in effectively managing occupational risks in industries.

  • The use of caffeine as a psychoactive stimulant in tea has been observed to have serious negative effects in humans' systems such as respiratory, nervous, cardiovascular, renal and skeletal systems. This study was carried out to assess the levels of caffeine in 10 different tea brands available in local market in Dutse, Jigawa State, Nigeria. Quantitative analysis of caffeine was performed by a simple and fast UV-Vis spectrophotometric methods using different solvents for extraction. The caffeine content in all the tea samples analyzed in this study were below the maximum allowable limits set by the USFDA. Tea have been associated with adverse health effects and the claims made by manufacturers about the benefits of tea do not highlight risks associated with excessive consumption of a combination of the ingredients contained in tea. Long term effects of tea consumption of children and young people have not been adequately studied. Therefore, it is recommended that further research be carried out on the adverse effects of energy drinks on children. Research is also needed to be done on the effects of the combination of ingredients on health and excessive consumption of those ingredients to children and adolescents. People need to be educated and given proper awareness on the health risks associated with caffeine containing beverages.

  • Scientists have long used conventional toxicological methods to establish 'safe levels of exposure' for chemicals presumed to have threshold health effects or doses below which significant effects are unlikely to occur. These same methods cannot be used to establish safe levels of exposure for non-threshold pollutants. such as carcinogens. Therefore. Federal regulatory agencies in the United States are using risk assessment methods to provide information for public health policy decisions concerning increases in risk associated with increases in exposure to carcinogenic and other non-threshold pollutants. Acceptable exposure/risk levels are decided by policymakers who consider descriptions and estimates of risks together with social and economic benefits from the uses of the chemical. 1bis paper focuses on the development of quantitative risk assessment approaches by Federal regulatory agencies in the United States, and identifies the mathematical models currently being used for risk extrapolation. including their inherent uncertainties. The uncertainties and limitations of these methods have led some scientists to question the utility of quantitative risk extrapolation. The experience of the; U.S. Environmental Protection Agency (EPA). as summarized in this paper. can provide a realistic basis for evaluating the pros and cons. Finally. shortcomings in current risk assessment methods and their use in policy decisions are explored. and areas for possible improvement. given current scientific knowledge. are identified.

  • Objectives: This research is part of a study to be conducted over five years starting from 2017 by the Ministry of Environment on the development of technologies to evaluate the impact of chemical accidents on the human body. Methods: For this research, a five-stage specific study method was developed. Results: In brief, the developed health risk assessment method can be summarized as follows. First, a health risk assessment system was built based on the guidelines set forth by the USA NRC/NAS. Second, based on the disease manifestation theory, the health risk assessment method was divided into 1) a carcinogenic health risk assessment method focused on all carcinogens except non-genotoxic carcinogens and 2) a non-carcinogenic health risk assessment method focused on noncarcinogens including non-genotoxic carcinogens. Third, the detailed contents of the health risk assessment method were developed in four stages(hazard identification, dose-response assessment, exposure assessment, and risk determination) through theoretical consideration of the assessment of the level of health risk related to chemical exposure. Finally, a health risk assessment methodology, classified into stages to address acute, subacute/subchronic, and chronic conditions was developed after considering the physicochemical behavior of hazardous chemicals upon implementation of countermeasures after a chemical accident. Conclusions: A method to evaluate the health risks related to toxic chemicals generated by chemical accidents was developed. This study was performed with the purpose of developing a mathematical health risk assessment method to evaluate the health effects of exposure to hazardous chemicals upon implementation of emergency countermeasures after chemical accidents.

건강관련 식품표시가 구매 전 품질평가와 제품 사용 및 구매태도, 구매의도에 미치는 영향: 계획적 행동이론을 바탕으로 (Effects of Health-Related Food Labeling on Quality Assessment Before Purchase, Attitudes towards Using and Purchasing Products, and Purchase Intentions Based on the Theory of Planned Behavior)

  • 소비자는 건강식품을 구매할 때, 건강관련 식품표시 뿐 아니라, 제품의 맛과 가격을 함께 고려하고, 과거구매경험과 주변 사람들의 의견에 영향을 받는다. 본 연구는 이러한 점에 초점을 맞추어 계획적 행동이론의 모형에 맛, 건강 기대도와 가격수용도 등의 구매 전 품질평가 변수와 제품사용태도, 과거구매경험 변수를 추가하고, 구조방정식모형 분석을 통해 제품 선택 시 건강관련 식품표시의 영향을 살펴보았다. 건강관련 식품표시는 유기가공표시, 영양강조표시, 식품첨가물표시로 구성하였고, 자녀용으로 슬라이스 치즈를 월 1회이상 구입하는 기혼여성 300명을 대상으로 온라인으로 설문조사를 수행하였다. 본 연구결과, 건강관련 식품표시에 대한 인식은 맛과 건강에 대한 기대와 가격수용도를 높여 제품사용 및 구매태도와 구매의도에 긍정적 영향을 미쳤다. 특히, 건강관련 식품표시를 인식하는 것만으로도 별도의 품질평가 없이 제품사용에 대한 호의적 태도가 형성되는 것으로 나타났다. 가격수용도는 제품사용 및 구매태도에, 과거구매경험은 제품사용 및 구매태도와 구매의도에 직접적으로 정적인 영향을 미쳐 계획적 행동이론 확장모형의 주요 변수로 파악되었다. 또한, 구매의도에 가장 큰 영향을 미치는 변수는 제품구매태도로 나타났고, 지각된 행동통제의 구매의도에 대한 영향은 유의하지 않았다. 본 연구는 계획적 행동이론을 기반으로 맛, 가격 등 구매 전 품질평가와 과거구매경험 변수를 포함하여 건강관련 식품표시와 제품선택에 대한 새로운 이론적 틀을 구축하였다는 점에서 의의가 크다.

Assessment of Occupational Health Risks for Maintenance Work in Fabrication Facilities: Brief Review and Recommendations

  • Background: This study focuses on assessing occupational risk for the health hazards encountered during maintenance works (MW) in semiconductor fabrication (FAB) facilities. Objectives: The objectives of this study include: 1) identifying the primary health hazards during MW in semiconductor FAB facilities; 2) reviewing the methods used in evaluating the likelihood and severity of health hazards through occupational health risk assessment (OHRA); and 3) suggesting variables for the categorization of likelihood of exposures to health hazards and the severity of health effects associated with MW in FAB facilities. Methods: A literature review was undertaken on OHRA methodology and health hazards resulting from MW in FAB facilities. Based on this review, approaches for categorizing the exposure to health hazards and the severity of health effects related to MW were recommended. Results: Maintenance workers in FAB facilities face exposure to hazards such as debris, machinery entanglement, and airborne particles laden with various chemical components. The level of engineering and administrative control measures is suggested to assess the likelihood of simultaneous chemical and dust exposure. Qualitative key factors for mixed exposure estimation during MW include the presence of safe operational protocols, the use of air-jet machines, the presence and effectiveness of local exhaust ventilation system, chamber post-purge and cooling, and proper respirator use. Using the risk (R) and hazard (H) codes of the Globally Harmonized System alongside carcinogenic, mutagenic, or reprotoxic classifications aid in categorizing health effect severity for OHRA. Conclusion: Further research is needed to apply our proposed variables in OHRA for MW in FAB facilities and subsequently validate the findings.

사상의학의 체질에 따른 식품분류와 태음식 섭취가 각각 체질의 혈액 생화학적 지표 및 건강상태에 미치는 영향 (The Food Classification in Sasang Constitution and Effects of Tae-eum Constitutional Diet on the Blood Biochemical Parameters and Health Status)

  • Presently Sasang constitutional medicine draws public attention and a diet based on this theory in gaining popularity. However, scientific data are not available to probe the effects of constitutional diet and the correlations between constitution and food consumptions. Thiry-four healthy subjects[26 females and 8 males] with Tae-eum(females: 5, males: 7), So-yang(females: 9)and So-eum(females: 12, males: 1), were studied. Subjects were not told the kind of constitutional diets they were consuming. Tae-eum constitutional diet was given to all subjects more than 2 meals a day for 8 weeks. The Sasang constitutional classification, food frequency questionnaire and food habits were assessed. The anthropometric assessment, dietary assessment, health status assessment and blood analysis were carried out before and after taking the Tae-eum constitution diet. In case of females, the body weights and BMI were different among the constitutional groups in the following order : Tae-eum constitutional diet. In case of females, the body weights the body weights and BMI were different among the constitutional groups in the following order: Tae-eum>So-yang>So-eum. In case of males, the body weights and BMI of Tae-eum were significantly higher than those of So-eum. In general, habitual food consumption of all of these four groups were very close to those for each of Sasang constitutional types described by Sasang medicine. After 8-weeks of Tae-eum diet, the health status and blood biochemical parameters were not significantly changed.

Toxicological Effects of PFOS and PFOA on Earthworm, Eisenia fetida

  • Perfluorinated Compounds (PFCs) are anthropogenic compounds found in trace amounts in many environmental compartments far from areas of production. Along with the highly persistent nature of PFCs, there are increasing concerns over the potential adverse effects of them on the ecosystems. Most of highly fluorinated compounds degrade into PFOS and PFOA that are very stable compounds hard to break down. So, in this study, we tried to determine the toxicity of PFOS and PFOA in the terrestrial invertebrate. Acute toxicity test using earthworm, Eisenia fetida, was performed according to the OECD test guideline 207 (Earthworm, Acute Toxicity Tests). In the 14 day acute toxicity tests, the highest concentration causing no mortality and the lowest concentration causing 100% mortality of PFOS were 160 and 655 mg/kg (dry weight), respectively. And the highest concentration causing no mortality and the lowest concentration causing 100% mortality were 500 and 1,690 mg/kg (dry weight), respectively in the PFOA-exposure group. 14 day-LC50 values were estimated at the level of 365 and 1,000 mg/kg (dry weight) in the PFOS and PFOA-exposed group. These results indicate that under laboratory conditions PFOS is about 3 times more toxic to earthworms than PFOA. Based on known environmental concentrations of PFOS in the soil of Korea, which occur in the 0.42~0.73 ng/L range, there is no apparent risk to terrestrial invertebrate, earthworms. However, further work is required to investigate long-term effects on these and other terrestrial organisms.
