Purpose: This study was conducted to investigate the worker's service quality of the elders at a nursing home. Methods: A total of 371 workers, who were engaged in caring for the elders at a nursing home in B-Metropolitan city and Kyungnam, answered the questionnaires regarding the knowledge & skills of service, information cognition and service quality for the elders related to their clients. Results: Service quality for the elders was significantly correlated to the level of knowledge and skills, as well as the information cognition for the elders, and frequency of educational participation. Service quality for the elders was statistically significant predictors of knowledge and skills, and information cognition for the elders. These two variables accounted for 37% of the variance in service quality for the elders. Conclusion: In order to improve the worker's care level, the workers engaged in caring for the elders should make the efforts to be given a nursing education related to the knowledge & skills, as well as the information for the elders.
Purpose: The purpose of this study was to investigate the predictors of tuberculosis (TB) drugs in health care workers diagnosed with latent tuberculosis Infection (LTBI). Methods: This study was a descriptive correlation study. Data were collected by using a self-report questionnaire. The collected data were analyzed by -test, t-test, and logistic regression analysis. Results: There were involved 160 participants, 35 (21.9%) who took the TB drugs, and 125 (73.8%) who did not take the TB drugs. The predictors on taking TB drugs in health care workers with LTBI were subjective norms (OR=3.33, p<.001), perceived sensitivity (OR=2.67, p=.026), perceived barrier (OR=0.14, p=.014), and unmarried (OR=4.69, p=.006) than married, health care worker category 2 group (OR=5.84, p=.015) and 1 group (OR=4.25, p=.022) than 3, 4 group, sleep over 7 hours (OR=4.11, p=.022) than less 7 hours sleep. Conclusion: In order to promote the use of TB drugs in health care workers with LTBI, it is necessary that take intervention strategies to increase the subjective norms and perceived sensitivity and to decrease the perceived barriers.
Purpose: The purpose of this study was to identify the risk perception, safety climate and preventive behaviors of COVID-19 infections, and to verify the factors influencing the adoption of preventive behaviors by health care workers. Methods: A mixed-method approach was used based on a survey carried out in a general hospital. Quantitative data (N=181) were collected through a questionnaire and analyzed using multiple regression. Qualitative data (N=8) were collected through individual interviews and analyzed through Colaizzi's phenomological research method. Results: The factors influencing adoption of preventive behaviors by health care workers were safety climate (β=.41, p<.001), education (β=-.20, p=.025), work unit (β=-.16, p=.032), and risk perception (β=.15, p=.020). The explanatory power was 28.3%, and they have shown that these health care workers felt 'the threat of a pandemic like a battlefield', experienced 'struggle with quarantine rules' and realized 'the reality of infection control and the reorganization for change'. Conclusion: The findings of this study indicate that to increase the adoption of infection control preventive behaviors by health care workers when faced with new infectious diseases in the future, it is necessary to establish organizational support and a safe climate. This study confirms the need for preemptive support and education.
Background: In this study, wage status and wage determinants of care workers were analyzed. Methods: The analysis used database (DB) of long-term care institutions, DB of long-term care institutions, DB of long-term care workers, DB of health insurance qualification, and contribution possessed by National Health Insurance Services. We analyzed the wage status of the care workers from 2009 to 2016 through basic analysis and estimated the factors affecting the wage of the long-term care facilities' care workers using pooled ordinary least squares. Results: The monthly average wage of care workers was raised from Korean won (KRW) 1.37 million in 2009 to KRW 1.52 million in 2016, and the working hours were shortened by 20 hours from 207 hours to 187 hours. Hourly wages increased by KRW 1,329 from KRW 6,831 in 2009 to KRW 8,160 in 2016. The average monthly wage of care workers was affected by gender, age, years of employment, monthly working hours, establishment type, city size, institutional size, the grade of the institution, and management status. In particular, the wage level of the care workers was high when the larger the size of the institution, the better the management status (fill rate), the establishment type is "government and local government" and "corporation," the institutional rating is high, and the facility manager has the first grade of the social worker license. Conclusion: The government should consider aggressive policies to improve the treatment of care workers as well as the quality of long-term care services so that there will be more long-term care facilities that are guaranteed social publicity above a certain level.
Purpose: The purpose of this study was to understand care workers' fall-related knowledge and caring behaviors for fall prevention in nursing home. Methods: This study was a descriptive survey study. Data have been collected from 125 care workers in 5 nursing homes. Data were analyzed using frequency, t-test, one-way analysis of variance (ANOVA), and Pearson correlation coefficients. Results: 1) The average percentage of correct answers for care workers' fall-related knowledge was 72.2%. There were significant differences in fall-related knowledge by age, experiences of re-education about care for fall injuries, and experiences to transfer fall patients to another hospital. 2) The average level of care workers' caring behaviors for fall prevention was 3.81 points out of 4. There were significant differences by experiences of education about care for fall injuries in nursing home and concern of fall injury prevention. 3) There was a significant correlation between fall-related knowledge and caring behaviors for fall prevention (r=.320, p<.001). Conclusion: The level of fall-related knowledge is positively correlated to the level of caring behaviors for fall prevention. Therefore, nurses should prepare education programs to increase care workers' fall-related knowledge and caring behaviors for fall prevention.
Purpose: This study identified the prevalence of depressive symptoms and explored correlates of depressive symptoms among Korean women care-workers caring older adults living in community. Methods: A total of 465 participants were recruited for the study. Depressive symptoms was measured by the CES-D10 and distress was assessed using a single question given 5 Likert scale. Results: The prevalence of depressive symptoms and distress among care-workers were 32.5% and 32.0% respectively. Work environment safety, violence exposure experience, work-family conflict, weekly care work time, and poor health status were significantly associated with depressive symptoms. Only violence exposure experience and poor health status were associated with distress. Conclusion: Our findings suggest more attention on mental health of care-workers and their risky work condition such as violence.
Occupational health services in Korea have been operated as dual types : one is operated by occupational health care manager and the other is health care agency without their own personnel. The performance of occupational health service should be different due to the variety of characteristics of health care manager and workplace, qualification of health care manager. This study is to analyze performance of occupational health care services with a particular consideration of job performance shape and efficiency, based on comparing those two types of health care management to show on the basic data for the settlement of more qualitative health care management system at workplace. For this study, total 391 places in Seoul and Inchon city area ; 154 places (39.4%) managed by designated health care manager and 237 places (60.6%) by the agency with their commission are selected as research samples. Tools for data collection are questionnares that have been investigated during the period of 20 September 1993-20 December 1993. Those data are compared with percentiles, mean, standard deviation and B/C ratio using SPSS PC program. Conclusions observed from the tests and each comparison could be summerized as follows : 1. Occupational health care have been accomplished at workplaces with designated people than with agencies people, and coverage rate of the occupational health care services has differences, due to management types. The reason of these results is due to visit only one or two times monthly by the agencies, while their own health care manager obsess, at the workplaces all the times. 2. Most of the expense for environmental control of all health care services expenditures shows that there is almost no fundamental improvement because more expenses are needed for procuring personal protective equipment and measuring work environment instead of environmental improvement. 3. It is investigated how much the cost of occupational health care services needs per worker, and calculated how much the cost needs per service hour per worker. The results from this show that the cost of occupational health services at workplaces with their own managers used less than the cost of health care agencies, eventually the former gives better services with less cost than the latter. 4. Benefit/Cost ratio is also produced by total benefit/total cost. The result from the above way reads 4.57 as a whole, while their own manager having workplaces reads 4.82 and the agencies do l.56. Even if their own manager performing workplaces spent more cost, this system produces more benefit than the agencies management. 5. The B/C ratio for medical organization such as local clinic, health care center and pharmacy shows more than or equal to at the workplaces controlled by the agencies. It is inferred that benefit would be much less than the cost used, with so being inefficient. 6. It is assumed that the efficiency ratio of health education is equal to reduction rate of workers medical organization visit. Estimated reduction rate 5%, 10%, 15%, show that the efficiency ratio of health education have an effect on producing benefits. It is estimated that more benefit can be produced if more qualitative education will be provided for enhancing health care efficiency. 7. Results of this study cannot be generalized because there are large scale of deviation in case of workplaces with less than 300 full time workers, but B/C ratio reads 2.69 as a whole and 3.25 at workplaces with their own health care manager are higher than 1.63 at the workplaces manged by the agencies. Finally, all the benefit concerning health care services could not be quantified, measured and shown on the value of money. This is a reason that a considerable part of benefits are so underestimated. This is also thought that measurement tools should be developed for measuring benefits of health care services with a comprehensive quantification. in the future. It is also expected that efficiency of occupational health care services should be investigated using cost-effectiveness analysis.
Objectives: The aim of this study was to analyze the accessibility of dental care services among individuals with precarious employment in South Korea. Methods: We used the $9^{th}$ wave of the Korean Health Panel data (2015) and included 7,736 wage and non-wage earners in our study. We determined precariousness in the labor market as a combination of employment relationship and job income, and categorized individuals based on this into the following four groups: Group A comprising those who report job and income security, Group B comprising those who experience job insecurity alone, Group C comprising those who report a stable job but low income, and Group D comprising those who experience both job and income insecurity. Accessibility to dental care services was determined by experience of unmet dental care needs and unmet dental care needs caused primarily by financial burden. Logistic regression analyses were used to assess the effect of precarious work on access to dental care services. Results: Individuals with job insecurity (Group B; OR=1.445; 95% CI=1.22-1.70) and both job and income insecurity (Group D; OR=1.899; 95% CI=1.61-2.24) were more likely to have unmet needs than the comparison group. Both groups B and D were also 2.048 (95% CI=1.57-2.66) times and 4.435 (95% CI =3.46-5.68) times more likely, respectively, to have unmet dental care needs caused by financial burden. Education status, health insurance, and health status were all also effective factors influencing unmet dental care needs. Conclusions: Unstable employment and low income resulted in diminished access to dental care services. Therefore, governments should consider health policy solutions to reduce barriers preventing individuals with employment and income instability from accessing adequate dental care.
Purpose: This study was conducted to identify health problems and support received from the health and welfare service using MDS-HC(Minimun Data Set for Home Care) in the aged living at home. Method: Eighty-one elderly persons were selected from those listed in community-welfare service centers in Seoul and Kyunggi Province between December 2002 and January 2003. Result: Eleven health problems per elderly person on average were identified, and the frequent care needs were in order: preventive health measure, health promotion, visual function, depression & anxiety, communication disorders, social function, pain, environmental assessment, oral health, cognition and falls. The number of health problems by the level of ADL was ‘ADL 1(Independence)’ 9.87, ‘ADL 2(Partial independence)’ 12.78, ‘ADL 3(Dependence)’ 13.73. Utilization of formal health & welfare services among the elderly was ‘meals on wheels’ 40.7%, ‘home helper’ 38.2%, ‘visiting of social welfare worker’ 21.0%, ‘physical therapy’ 19.6%, ‘day care center’ 12.3%, ‘volunteer's service’ 9.9%, ‘home visiting care’ 3.7%, ‘occupational therapy’ 3.7%, and ‘speech therapy’ 2.5%. Conclusion: The results suggest that using the MDS-HC 2.0 is applicable to help decide criteria for both health and welfare service supplied to the elderly.
Objectives: This study aims to examine the effect of psychological capital on turnover intension and job satisfaction by aged care facility workers. Methods : Samples were obtained from 147 aged care facility workers in one county of Kyungsangbukdo. The study conducted a correlation and a regression analysis to learn how psychological capital (self-efficacy, hope, optimism, resilience) would affect turnover intension and job satisfaction. Results : First, age and marital status turned out to have a positive (+) influence on job satisfaction which would be negative (-) affected by tenure of office. Second, hope, sub-variable of Psychological Capital, affected a negative (-) influence on turnover intension. Third, resilience, sub-variable of Psychological Capital, affected a positive(+) influence on job satisfaction. Conclusions: It is suggested to develop the psychological capital of aged care facility workers, effective welfare system and educational program are required for better job satisfaction.
