의학기술의 발전과 소득수준의 증가로 "건강하게 오래살기"에 관심이 높아지면서 적극적으로 건강을 증진하고 유지하는 웰니스가 확대되고 있다. 또한 맞춤형 의료서비스에 대한 수요가 증가하고 방대한 의료 빅 데이터를 이용한 질병 예방의 움직임도 나타나고 있다. 이 논문에서는, 의료 시장에서 주요 관심분야로 부각되고 있는 웰니스를 지원하기위해 빅 데이터 기반의 의료 질 향상을 통한 환자중심의 의료서비스를 목적으로 한다. 환자를 약물에 의존적으로 치료만 하는 것이 아니라 식생활 개선을 기반으로 질병예방과 치료를 위해 빅데이터를 분석한다. 개인 트윗터를 분석해서 일상생활정보를 획득하고 웰니스 사전을 기반으로 질병예방과 치료를 목적으로 한다. 효율적인 빅데이터 분석을 위해 하둡노드를 증가하면서 데이터 처리시간을 실험하였다. 실험결과 저장시간의 경우 63%, 데이터 통합의 경우 18%, 전체 테스트 시간을 기준으로 26%로 하나의 노드로 처리하는 경우보다 세 개의 노드로 처리하는 것이 효율적임을 실험을 통해 확인하였다.
Background: This paper aims to demonstrate current health expenditure (CHE) and National Health Accounts of the years 2015 constructed according to the SHA2011, which is a new manual of System of Health Accounts (SHA) that was published jointly by the Organization for Economic Cooperation and Development (OECD), Eurostat, and World Health Organization in 2011. Comparison is made with international trends by collecting and analysing health accounts of OECD member countries. Particularly, financing public-private mix is parsed in depth using SHA data of both HF as financing schemes as well as FS (financing source) as their revenue types. Methods: Data sources such as Health Insurance Review and Assessment Service's publications of both motor insurance and drugs are newly used to construct the 2015 National Health Accounts. In the case of private financing, an estimation of total expenditures for revenues by provider groups is made from the Economic Census data; and the household income and expenditure survey, Korean healthcare panel study, etc. are used to allocate those totals into functional classifications. Results: CHE was 115.2 trillion won in 2015, which accounts for 7.4 percent of Korea's gross domestic product. It was a big increase of 9.3 trillion won, 8.8 percent, from the previous year. Government and compulsory schemes's share (or public share) of 56.4% of the CHE in 2015 was much lower than the OECD average of 72.6%. 'Transfers from government domestic revenue' share of total revenue of HF was 17.8% in Korea, lower than the other contribution-based countries. When it comes to 'compulsory contributory health financing schemes,' 'Transfers from government domestic revenue' share of 14.9% was again much lower compared to Japan (44.7%) and Belgium (34.8%) as contribution-based countries. Conclusion: Considering relatively lower public financing share in the inpatient care as well as overall low public financing share of total CHE, priorities in health insurance coverage need to be repositioned among inpatient care, outpatient care and drugs.
Purpose: This study identified words closely associated with the keyword "dietary supplement" (DS) using big data in Korean social media and investigated consumer perceptions and trends related to DSs before (2019) and after the coronavirus disease 2019 (COVID-19) pandemic (2021). Methods: A total of 37,313 keywords were found for the 2019 period, and 35,336 keywords were found for the 2021 period using blogs and cafes on Daum and Naver. Results were derived by text mining, semantic networking, network visualization analysis, and sentiment analysis. Results: The DS-related keywords that frequently appeared before and after COVID-19 were "recommend", "vitamin", "health", "children", "multiple", and "lactobacillus". "Calcium", "lutein", "skin", and "immunity" also had high frequency-inverse document frequency (TF-IDF) values. These keywords imply a keen interest in DSs among Korean consumers. Big data results also reflected social phenomena related to DSs; for example, "baby" and "pregnant woman" had lower TD-IDF values after the pandemic, suggesting lower marriage and birth rates but higher values for "joint", indicating reduced physical activity. A network centered on vitamins and health care was produced by semantic network analysis in 2019. In 2021, values were highest for deficiency and need, indicating that individuals were searching for DSs after the COVID-19 pandemic due to a lack an awareness of the need for adequate nutrient intake. Before the pandemic, DSs and vitamins were associated with healthcare and life cycle-related topics, such as pregnancy, but after the COVID-19 pandemic, consumer interests changed to disease prevention and treatment. Conclusion: This study provides meaningful clues regarding consumer perceptions and trends related to DSs before and after the COVID-19 pandemic and fundamental data on the effect of the pandemic on consumer interest in dietary supplements.
Purpose: This study aimed to examine factors influencing health-related quality of life (HRQOL) and compare them between young-aged and old-aged patients with cancer. Methods: Data of 291 patients (young-aged: 168, old-aged: 123) were obtained from the 10th wave of the 2015 Korea Health Panel Survey. The HRQOL was measured using the Korean version of Euro-QoL-5D. Independent t-test, analysis of variance, and multiple regression analysis were performed to identify factors influencing HRQOL. Results: The average HRQOL score was 0.87±0.10 and 0.82±0.15 among young-aged and old-aged, respectively. The factors differed partially between the two groups. For young-aged, the influencing factors were activity restriction, subjectively perceived health status, and smoking. For old-aged, the influencing factors were activity restriction, subjective health status, and unmet healthcare needs. Conclusion: Strategies to improve the HRQOL of elderly adults need to be developed considering the age group. Additionally, studies that include clinical factors such as symptoms are required to prepare need-based practical approaches for better quality of life of such patients.
본 논문에서는 혼자하는 헬스케어가 아닌 지인 또는 가족과 함께하는 헬스케어 시스템을 구축하는 것으로 능동적인 건강관리가 가능한 스마트 헬스케어 기기에 대한 연구한다. 노부모를 모시는 젊은 사용자를 타겟으로 놀이형 건강관리 시스템으로 직관적인 UI 설계를 통해 남녀노소 상관없이 가족단위의 건강관리 서비스 개발한다. 건강정보 자동수집 및 실시간 피드백이 가능하고, 중계기를 통한 데이터 종합 및 분석할 수 있다. 또한 체계화된 데이터베이스를 빅데이터의 형태로 활용할 수 있다. 제시한 서비스를 활용하면 건강관리를 통한 질병예방효과와 의료비에 대한 절감효과를 누릴 수 있고, 자동관리로 사용자의 편의 극대화와 수요 증가 효과를 기대할 수 있다. 이를 기반하는 기술을 응용한 제품의 개발기간 단축 및 저가격화와 경쟁력 강화를 통해 핵가족화되어 가는 시대에 세대간의 소통을 이끌어 낼 수 있다는 장점을 가진다.
As of 1 July 2000 a big reform was introduced into the Korean health care system: the separation between prescribing and dispensing of drugs (SPD reform). There was, however, a big financial stake associated with pharmaceuticals, particularly before the reform, because physicians as well as pharmacists were allowed to purchase drugs at much lower costs than the insurance reimbursement. In this respect, this study focuses on the change in income and profit of both doctor's clinics and pharmacies after the reform. Data from National Health and Nutritional Survey by the ministry of health and welfare were used to estimate the income or expenditure that are financed by out-of-pocket payment of the patients, while national health insurance data etc. were used for the estimation of the income or expenditure that is financed by insurers. Average annual income per doctor's clinic increased from 299 million won to 338 million won for the three years between 1998 and 2001, whereas average annual income per pharmacy increased enormously from 60 million won to 305 million won for the same period. Average annual 'profit' increase per each doctor's clinic caused by the reform itself was estimated to range from 50 to 83 million won, while that per each pharmacy, from 23 to 87 million won. In sum, while both doctor's clinics and pharmacies are beneficiaries of the SPD reform, its positive impact is particularly prominent on the latter.
The purpose of this study is to prepare basic data that can be applied to the development of personalized programs in which chronic patients can actively participate in health care on their own, by analyzing the relationship between health literacy, level of metal health, and level of life health of patients with chronic diseases. For the study, the Korean Medical Panel's annual data(Version 2.1) was used, and 4,095 people aged 19 or older with chronic diseases and without disabilities were extracted, and frequency analysis, t-test, ANOVA, and chi-squared goodness of fit test, etc. were performed with IBM SPSS Statistics 26.0. As a result, it was found that the higher health literacy, the higher level of mental health and level of life health. In addition, the distribution between health literacy, level of mental health, and level of life health was found to be different from each other. Respondents with higher ability to health literacy tend to evaluate level of metal health and life health lower, and the rate of change in this trend was relatively higher than the rate of change in the tendency to evaluate level of mental health and life health higher in respondents with lower ability to health literacy.
본 논문에서는 하둡 플랫폼에서 비용 효율적 빅데이터 분석을 수행하기 위한 클러스터 규모의 설정 방안을 연구한다. 의료기관의 경우 진료기록의 병원 외부 저장이 가능해짐에 따라 클라우드 기반 빅데이터 분석 요구가 증가하고 있다. 본 논문에서는 대중적으로 많이 사용되고 있는 클라우드 서비스인 아마존 EMR 프레임워크를 분석하고, 비용 효율적으로 하둡을 운용하기 위해 클러스터의 규모를 산정하기 위한 모델을 제시한다. 그리고, 다양한 조건에서의 실험을 통해 맵리듀스의 실행에 영향을 미치는 요인을 분석한다. 이를 통해 비용 대비 처리시간이 가장 효율적인 클러스터를 설정함으로써 빅데이터 분석시 효율성을 증대시킬 수 있다.
피부상태의 진단과 관리는 뷰티산업종사자와 화장품산업종사자에게 그 역할을 수행함에 있어서 매우 기초적이며 중요한 기능이다. 정확한 피부상태 진단과 관리를 위해서는 고객의 피부상태와 요구사항을 잘 파악하는 것이 필요하다. 본 논문에서는 피부상태 진단 및 관리를 위해 소셜미디어의 빅데이터를 사용하여 피부상태 진단 및 관리를 지원하는 빅데이터기반 피부관리정보시스템 SCIS를 개발하였다. 개발된 시스템을 사용하여 텍스트 정보 중심의 피부상태 진단과 관리를 위한 핵심 정보를 분석하고 추출할 수 있다. 본 논문에서 개발된 피부관리정보시스템 SCIS는 빅데이터 수집단계, 텍스트전처리단계, 이미지전처리단계, 텍스트단어분석단계로 구성되어 있다. SCIS는 피부진단 및 관리에 필요한 빅데이터를 수집하고, 텍스트 정보를 대상으로 핵심단어의 단순빈도분석, 상대빈도분석, 동시출현분석, 상관성분석을 통해 핵심단어 및 주제를 추출하였다. 또한 추출된 핵심단어 및 정보를 분석하고 산포도, NetworkX, t-SNE 및 클러스터링 등의 다양한 시각화 처리를 함으로써 피부상태 진단 및 관리에 있어 이를 효율적으로 사용할 수 있도록 하였다.

Comorbidity isthe simultaneous presence of two chronic diseases or conditions in a patient. As part of a larger research study, the aims of this study were to explore comorbid conditions in intensive care unit (ICU) patients and to compare the comorbidity across different demographic groups, and to determine what comorbid health problems coexisted in the patients with hospital-acquired pressure injury (HAPI). The average number of comorbid conditions was 6.4 with range from 0-20 in the ICU patients. African American patients had significantly more comorbid health problems than other race/ethnicity groups. Asian and Hispanic female patients showed higher comorbidity than male patients across age. The patients with HAPIs had significantly more comorbid health problems than the patients without HAPIs -- the average numbers were almost two-fold. We found comorbid health problems that existed with HAPI in ICU patients. 'Other diseases of lung' and 'Disorders of fluid, electrolyte, and acid-base balance' were most frequently coexisting health problems in the ICU patients with HAPI. Exploratory plots are helpful to discover patterns or hypotheses relevant to clinical management in critical care. Inclusion of patients' comorbid health problems to ICU HAPI risk assessment may be helpful. Identification of patients at a high risk for the development of HAPI and the early preventative interventions can help reduce length of stay as well as costly complications.
