Rapid industrialization has induced the migration of rural people to urban areas. Such migration has created enlarged the existing low income group. Residents of low income area have increased health risk owing to their poor living environment, low income. overwork and inappropriate health care. The general objective of this study was to group the pattern of medical care utilization of low income group. The specific objectives were to identify disease prevalence and medical care utilization of low income group. To meet the objectives of this study, household interview method was applied. A total of 1845 households in 5 areas such as Bongchon 5th Dong, Bongchon 2nd Dong, Sanggae 5th Dong, Sanggae 4th Dong, and Shinrim 7th Dong were visited and interviewed by field team during the period from April 19 to May 3. 1989. The major findings obtained from the information collected were as follows : The Number of room per household used was one to two rooms. The employment state of the head of household disclosed that 88.6% had a job and the remaining 11.4% were unemployed. The average monthly income was 502,770won. however, 30% of the total income was less than 300,000 won in Bongchon 5th dong area. and 34.5% in Shinrim 7th Dong area. 41.3% of households had debts, which was consisted of household expense(33.4%), income formulation(22.7%) and medical care cost(15.9%) etc. Prevalence rate of diseases during the preceding 30days before the date of the household interview was 387.7 per 1000 persons. The prevalence rate of female was higher than that of male. 8.9% of the sick persons wasn't receiving any medical treatment, and the main reasons of which were lack of economic availability(43.3%) and feeling of non treatment needed(33.7%). According to the study results it was found that the prevalence rate of chroic diseases and the disabled in low income resident areas was higher than that in the other areas. Therefore, the health status of this group should be improved through PHC approaches. In addition. in order to prevent the diseases and promote the health of those people, the health center as well as health subcenter should be strengthened.

Purpose: To evaluate survival rates and prognostic factors related to treatment outcomes after bladder preserving therapy including transurethral resection of bladder tumor, radiotherapy (RT) with or without concurrent chemotherapy in bladder cancer with a curative intent. Materials and Methods: We retrospectively studied 50 bladder cancer patients treated with bladder-preserving therapy at Keimyung University Dongsan Medical Center from January 1999 to December 2010. Age ranged from 46 to 89 years (median, 71.5 years). Bladder cancer was the American Joint Committee on Cancer (AJCC) stage II, III, and IV in 9, 27, and 14 patients, respectively. Thirty patients were treated with concurrent chemoradiotherapy (CCRT) and 20 patients with RT alone. Nine patients received chemotherapy prior to CCRT or RT alone. Radiation was delivered with a four-field box technique (median, 63 Gy; range, 48.6 to 70.2 Gy). The follow-up periods ranged from 2 to 169 months (median, 34 months). Results: Thirty patients (60%) showed complete response and 13 (26%) a partial response. All patients could have their own bladder preserved. Five-year overall survival (OS) rate was 37.2%, and the 5-year disease-free survival (DFS) rate was 30.2%. In multivariate analysis, tumor grade and CCRT were statistically significant in OS. Conclusion: Tumor grade was a significant prognostic factor related to OS. CCRT is also considered to improve survival outcomes. Further multi-institutional studies are needed to elucidate the impact of RT in bladder cancer.

Purpose: The purpose of this study was to evaluate the prognostic value of the lymph node ratio (LNR), which was defined as the proportion of involved nodes of all dissected nodes, in pN1 breast cancer. Materials and Methods: We retrospectively analyzed the clinical data of patients with pN1 breast cancer (n = 144) treated at Keimyung University Dongsan Medical Center, Daegu, Korea between 2001 and 2010. The median age was 46 years (range, 27 to 66 years). The LNR was 0.01-0.15 (low LNR) in 130 patients and >0.15 (high LNR) in 14 patients. Sixty-five patients (45.1%) had T1 tumors, 74 (51.4%) had T2 tumors, and 5 (3.5%) had T3 tumors. Eighty-eight patients (61.1%) underwent total mastectomy and 56 (38.9%) underwent partial mastectomy. Fifty-nine patients (41.0%) underwent radiotherapy and 12 (8.3%) underwent regional radiotherapy. The median follow-up period was 65 months. Results: The 5- and 10-year disease-free survival (DFS) rates were 92.7% and 82.4%, respectively. Univariate analyses revealed that high LNR (p = 0.004), total mastectomy (p = 0.006), no local radiotherapy (p = 0.036), and stage T2 or T3 (p = 0.010) were associated with worse DFS. In multivariable analysis, only high LNR (p = 0.015) was associated with worse DFS. Conclusion: High LNR is an independent prognostic factor in pN1 breast cancer and could be an indication for adjuvant radiotherapy in these patients.

Background: Reactive oxygen species (ROS) are known to be related to cardiovascular diseases. Many studies have demonstrated that angiotensin-converting enzyme inhibitors have beneficial effects against ROS. We investigated the antioxidant effect of captopril and enalapril in nitric oxide mediated vascular endothelium-dependent relaxations. Materials and Methods: Isolated rabbit abdominal aorta ring segments were exposed to ROS by electrolysis of the organ bath medium (Krebs-Henseleit solution) after pretreatment with various concentrations (range, $10^{-5}$ to $3{\times}10^{-4}$ M) of captopril and enalapril. Before and after electrolysis, the endothelial function was measured by preconstricting the vessels with norepinephrine ($10^{-6}$ M) followed by the cumulative addition of acetylcholine (range, $3{\times}10^{-8}$ to $10^{-6}$ M). The relevance of the superoxide anion and hydrogen peroxide scavenging effect of captopril and enalapril was investigated using additional pretreatments of diethyldithiocarbamate (DETCA, 0.5 mM), an inhibitor of Cu/Zn superoxide dismutase, and 3-amino-1,2,4-triazole (3AT, 50 mM), an inhibitor of catalase. Results: Both captopril and enalapril preserved vascular endothelium-dependent relaxation after exposure to ROS in a dose-dependent manner (p<0.0001). Pretreatment with DETCA attenuated the antioxidant effect of captopril and enalapril (p<0.0001), but pretreatment with 3AT did not have an effect. Conclusion: Both captopril and enalapril protect endothelium against ROS in a dose-dependent fashion in isolated rabbit abdominal aortas. This protective effect is related to superoxide anion scavenging.
Objectives : Therapeutic effect of Balance Appliance of functional crebrospinal technique (FCST for meridian and neurologic yinyang balance was observed in two refractory torticollis cases. Methods : A unidentified severe torticollis two cases with several months of duration was managed by the Balance Appliance on temporomandibular joint (TMJ), combined with acupuncture and manual medicine. Results : Assessment was made by self assessment of subjective symptoms and clinical observation. The patient reported over-90% remission and returned to ordinary daily life after $3{\sim}5$ months of therapy, which effect was reported to maintain for 6 months. Conclusions : An impressive effect was observed and further clinical and biological research on FCST is expected.

The purpose of this study was to investigate the treatment status and its related factors of the newly detected rural hypertensives through community health promotion program. A questionnaire survey and blood pressure measurement were performed to 6,977 residents of a rural area, and 282 hypertensives detected by blood pressure measurement were selected as subjects of the study. The study employed the health belief model as a hypothetical model. The major results of this study were as follows: The proportion of person experienced treatment among hypertensives was 12.0%. Treatment experience rate was significantly related with age and educational level(p<0.01). That is, if they were older, lower educational level, the treatment experience rate was higher. The major reasons of no treatment were 'they had not hypertensive symptoms ' (45.6%), 'their blood pressure was not high so much that they received treatment ' (43.2%). The chief facilities for treatment were public health institutions(57.9%) such as health center and health subcenter, and hospital/ clinics(29.8%). The treatment experience rate was higher when they had higher perceived severity for hypertension, lower perceived barrier to treatment, although statistically not significant. Treatment experience rate was significantly related with cues to action and health education experience(p<0.05). That is, if they had hypertension related symptoms such as headache previously, patients suffered from hypertension complication and health education experience for hypertension, the treatment experience rate was higher. In multiple logistic regression analysis for treatment experience, having a cerebrovascular patient in their acquaintance and the experience of health education for hypertension were significant variables. On consideration of above findings, it would to be essential to provide knowledge about hypertension and its treatment, and severity of hypertension complications through health education.
About 380 dental hygienists who were working in public health centers in 2004 joined this study with questionnaires about the evaluation of job training programs regarding oral health care. The findings are as follows. 1. As for the general characteristics of subjects including location, age and career, the dental hygienists in Public Health Subcenters outnumbered the dental hygienists in Public Health Centers, as the former accounted for 61.1 and the latter 35.3 percent. And the dental hygienists in their 36~40s made up the largest age group. The group in their 11~15 years career revealed 48.4, over 16 years career 35.7, under 10 years career group 15.9 percent, respectively. 2. The portion of certification in the respondents was 28.9 percent, as compared to the 71.1percent of non-certification. The major reasons of non-certification were not-applying(44.6%), participation rejected (29.3%). 3. Freqently requested curriculums were school based oral health program(4.42, the highest by Linkert 5 point scale), followed by oral health education(4.41), public oral health services for the toddler and for the elderly(4.04), for the disabled(3.92), oral health planning evaluation(3.85) and oral health survey& investigation(3.69). The gap between Dental hygienists at Public Health Center and Public Health Subcenter was statistically significant different(p < 0.001) in oral health survey& investigation and oral health planning evaluation.

To exam infestation rate and removal rate of head louse, we did delousing at 27 primary schools and 19 kindergartens established as an annex to the primary school in Namwon-shi, Jollabuk-do, Korea, from May to December 1995. Total of 3123 subjects were examined ed for the presence of head lice and nits The overall infestation rate was 9.6%, 9.5% in primary school children and 10.1% in kindergarten children. Among 299 infested children, 55 children evaluated by live nit had louse, and 79 children had dead nit. According to distractive research, Ibaek-myon and Songdong-myon revealed high infestation rate of 27.3%, and 21.7% respectively. Infestation rate of primary school was 0 to 50.9%, kindergarten was 0 to 100%. Infestation rate of female children were higher than male in all grades. When we executed one time treatment with PARA$^{(R)}$ aerosol(bioallethrin with piperonyl butoxide), treatment rate of 65.1% was examined, 82.8% was examined in three times of treatment. We concluded that 3 times of treatments interval of 14 days were more effective than one time in the case of treatment of only infested children. In order to prevent the spread of head lice, education of school hygiene and suitable delousing attempts should be executed in the present situation.

In order to find out the status of routine-immunization in a rural area, this study was performed, through analyzing the data which was obtained from the immunization register of infants who was born at Su-Dong myun in 1996 and 1997, managed by Su-dong Myun health subcenter. The results are as follows. 1. B.C.G immunization rate was the highest such as 52.2% within 1 month and next order such 34.8% at 2 month in 1996. In 1997, the highest such as 73.8%, almost all, within 1 month. 2. D.P.T immunization rate in 1996 showed, almost all, the highest at 3 month(79.4%) for 1st dose and at 5 month(78.4%) for 2nd dose. However, the rate for 3rd dose showed the highest at 7 month(51.4%), and next order at 8 month(35.1%) and at 6 month(13.5%). D.P.T immunization rate in 1997, similarly showed the highest at 3 month(81.8%) for 1st dose, at 5 month(71.2%) for 2nd dose and at 7 month(71.4%) for 3rd dose. 3. Hepatitis B immunization rate showed the highest at birth at once or within one week(87.0%) for 1st dose in 1996 and (94.7%) in 1997. The rate for 2nd dose showed the highest at 2 month(51.7%) in 1996 and (50.0%) in 1997, and next order at 1 month(44.8%) in 1996 and (34.4%) in 1997. The rate for 3rd dose showed the highest at 3 month(54.8%) in 1996 and 5 month(54.8%) in 1997, and next order at 5 month(25.8%) in 1996 and at 3 month(26.0%) in 1997. 4. Measles immunization rate was 76.1% in 1996. The rate(76.1%) by the kind of vaccine was the highest with measles-MMR(34.8%), and with MMR(32.6%) and next order with measles(8.7%). The rate by measles immunization time(month) was the highest such as 35.0% at 9 month and 10 month respectively and the rate by MMR was the highest at 16 month(35.5%), and 15 month(22.5%), 13 month (12.9%) and 14 month(12.9%) in next order.

Objectives: This study was conducted to analyze the dental care utilization patterns and related factors of the rural residents. Methods: The data collected by interview and self-administered questionnaire survey of 524 peoples of Seongju county in Gyeongsanbuk-do. The summarized results are as follows. Results: The rate of persons who experienced the oral disease was 52.5% during 1 year and it was at most in the age group of 40-49. The rate of persons who had experienced the oral disease were investigated according to general characteristics, perception of oral health, being of regular treatment facility. Therefore the rate of persons who had experienced the oral disease was significantly higher the younger peoples, worse oral health status and being of the regular treatment source than the other groups. During 1 year period, 64.0% of the cases had treated the perceived oral disease, 36.0% did no action at all during last year. Among respondents, 49.4% had treated their oral disease at dental clinics, 8.0% had treated at community health center or subcenter and remains did not treated at all. The results of logistic regression analysis suggested that statistically significant factors in dental health care utilization were educational level, degree of pain, oral health status and regular treatment facility. Therefore the dental health care utilization rate was higher at groups with the high educational level, serious pain, better oral health status and being of the regular treatment source than other groups. 45.5% of the rural residents did not treat their oral disease immediately due to the no identified need, limitation of time(19.2%), economic limitation(19.2%), and geographical limitation(9.0%). Conclusions: In consideration of above findings, we may conclude that oral health community program to prevent oral diseases should be intensified, oral health education to raise oral health knowledge should be performed periodically.
