보건소 비만예방관리 사업 현황 및 요구도 분석 (Current Status and Needs Assessment for Obesity Prevention and Management Project at Public Health Centers)

  • Purpose: The obesity prevention and management program led by public health centers are important in the community. This study aims to identify the current status of obesity prevention and management programs at public health centers and perceptions regarding facilitators and barriers when implementing programs. Methods: This study used a concurrent mixed methods design. A survey was conducted to investigate the current status and infrastructure of the obesity prevention and management program at eighty-three public health centers nationwide. Nine program managers and six local residents with experience in the program were interviewed by using a semi-structured questionnaires. Results: Most of the infrastructure facilities for the program were inadequate, and insufficient budgets and lack of professional staff were identified as barriers. Facilitators included diversification of program delivery methods, operator competence, and visible outcomes and rewards. For the effectiveness of the program, it is vital to have adequate assistants, a sufficient budget, various promotional methods, and connections with various institutions in the community. On this basis, it is acknowledged that the public health center serves as a platform for preventing and managing obesity in the community. Conclusion: It is expected that infrastructure improvements in public health centers and the link with community resources are needed. In addition, there is an urgent need to set standards for obesity prevention and management programs implemented in public health centers to 'resolve regional disparities'.

  • 미래 사회로 접어들면서, 건강한 삶의 증대를 위한 노력은 현대인들의 주요 관심 분야이다. 특히, ICT 기술과 경쟁력 있는 의료산업 환경을 융합하여 건강한 삶을 위한 기술 개발은 차세대 성장 동력으로 자리잡고 있다. 따라서, 본 논문에서는 건강 검진 프로세스에서 검진 결과에 대한 인공지능 기반의 데이터 분석을 적용하여 종합 판정의 신뢰성을 향상시킬 수 있는 지식 베이스 모델링을 구축하는 연구를 수행하였다. 이를 위해, 딥러닝 분석을 통한 알고리즘을 설계하여 검사 결과지수를 산출, 검증하고, 판정 지식을 통한 종합 검진 정보를 제공하는 모델링을 연구하였다. 제안한 모델링의 적용을 통해, 국민 건강에 대한 빅데이터 분석, 활용이 가능하여 의료비 절감 및 건강 증대의 효과를 기대할 수 있다.

The effect of peri-implant bone exposure on soft tissue healing and bone loss in two adjacent implants

  • Purpose: The purpose of this study was to evaluate the soft tissue and bone change around two adjacent implants in onestage implant surgery. Methods: Eleven subjects (7 males, 4 females) who were needed placement of 2 adjacent implants in the molar area were included. The two implants were placed with the platform at the level of the alveolar crest. The interproximal bone between the 2 implants was not covered with gingiva. After surgery, an alginate impression was taken to record the gingival shape and radiographs were taken to evaluate implant placement. Using a master cast, the gingival height was measured at baseline, 4 weeks, and 12 weeks. In the radiograph, the alveolar bone level was measured at the mesial and distal side of both implants at baseline and 12 weeks. Results: The exposed bone was covered with gingiva at both 4 and 12 weeks. Loss of alveolar bone around implants was found in all areas. The alveolar bone level in the exposed bone area did not differ from that in the non-exposed area. Conclusions: This study showed that the alveolar bone level and gingival height around 2 adjacent implants in the exposed bone area did not differ from that in unexposed bone area.

모션인식을 활용한 Human UI/UX를 위한 IoT 기반 스마트 헬스 서비스 (IoT based Smart Health Service using Motion Recognition for Human UX/UI)

  • 본 논문에서는 모션인식을 활용한 Human UI/UX를 위한 IoT 기반 스마트헬스 서비스를 제안한다. 현재까지 M2M기반의 u-healthcare에서 적용되는 센서 네트워크에서는 TCP/IP 프로토콜이 아닌 non-IP프로토콜을 이용하고 있다. 그러나 서비스의 이용 확대와 IoT 기반의 센서 네트워크 관리를 용이하게 하기 위해서는 다수의 센서들의 인터넷 연결이 반드시 요구된다. 따라서 센서들에 의하여 측정된 자료들을 인터넷과 통신하는 것은 물론 이동이 가능해야 하기 때문에 네트워크 이동성을 고려한 IoT 기반 스마트헬스 서비스를 설계하였다. 또한 IoT 기반 스마트헬스 서비스는 기존의 헬스케어 플랫폼과는 다르게 바이오 정보뿐만이 아니라 동작감지를 위한 스마트 헬스 서비스를 개발하였다. u-healthcare에서 사용되는 WBAN 통신은 일반적으로 많은 네트워크화된 장치 및 게이트웨이로 구성된다. 본 논문에서 제안하는 방법은 WBAN 센서 노드간의 이동성을 지원하는 기술을 이용함으로써 무선 환경에서도 동적 변화에 쉽게 대응할 수 있고, 사용자의 동작감지를 통해 체계적인 관리가 이루어진다.

Framework of Health Recommender System for COVID-19 Self-assessment and Treatments: A Case Study in Malaysia

  • This paper proposes a framework for the development of the health recommender system, designed to cater COVID-19 symptoms' self-assessment and monitoring as well as to provide recommendations for self-care and medical treatments. The aim is to provide an online platform for Patient Under Investigation (PUI) and close contacts with positive COVID-19 cases in Malaysia who are under home quarantine to perform daily self-assessment in order to monitor their own symptoms' development. To achieve this, three main phases of research methods have been conducted where interviews have been done to thirty former COVID-19 patients in order to investigate the symptoms and practices conducted by the Malaysia Ministry of Health (MOH) in assessing and monitoring COVID-19 patients who were under home quarantine. From the interviews, an algorithm using user-based collaborative filtering technique with Pearson correlation coefficient similarity measure is designed to cater the self-assessment and symptoms monitoring as well as providing recommendations for self-care treatments as well as medical interventions if the symptoms worsen during the 14-days quarantine. The proposed framework will involve the development of the health recommender system for COVID-19 self-assessment and treatments using the progressive web application method with cloud database and PHP codes.

Protection Motivation Theory and Rabies Protective Behaviors Among School Students in Chonburi Province, Thailand

  • Objectives: The aim of this study was to predict rabies protective behaviors (RPB) based on protection motivation theory (PMT) among fourth-grade students at schools in Chonburi Province, Thailand. Methods: This cross-sectional study was conducted from December 2020 to February 2021. A multistage sampling technique was used for sample selection. The questionnaire was divided into socio-demographic data and questions related to PMT and RPB. Descriptive statistical analysis was conducted using the EpiData program and inferential statistics, and the results were tested using the partial least squares model with a significance level of less than 5%. Results: In total, 287 subjects were included, of whom 62.4% were girls and 40.4% reported that YouTube was their favorite media platform. Most participants had good perceived vulnerability, response efficacy, and self efficacy levels related to rabies (43.9, 68.6, and 73.2%, respectively). However, 54.5% had only fair perceived severity levels related to rabies. Significant positive correlations were found between RPB and the PMT constructs related to rabies (β, 0.298; p<0.001), and the school variable (S4) was also a predictor of RPB (β, -0.228; p<0.001). Among the PMT constructs, self efficacy was the strongest predictor of RPB (β, 0.741; p<0.001). Conclusions: PMT is a useful framework for predicting RPB. Future RPB or prevention/protection intervention studies based on PMT should focus on improving self efficacy and response efficacy, with a particular focus on teaching students not to intervene with fighting animals. The most influential PMT constructs can be used for designing tools and implementing and evaluating future educational interventions to prevent rabies in children.

A Cross-Sectional Analysis of Breast Reconstruction with Fat Grafting Content on TikTok

  • As of November 2021, TikTok has one billion monthly active users and is recognized as the most engaging social media platform. TikTok has seen a surge in users and content creators, ranging from athletes to medical professionals. In the past year, content creators have utilized the app to advocate for social reforms, education, and other uses that were not previously considered. Breast cancer is the most commonly diagnosed cancer in women, with an expected 281,550 new cases of invasive breast cancer in 2021. As more individuals with breast cancer choose to undergo resection, the demand for autologous fat grafting in breast reconstruction has increased due to the natural look and feel of breast tissue. The purpose of this article is to analyze content related to breast reconstruction with fat grafting found on TikTok and recommend methods to improve patient education, care, and outcomes. We searched TikTok on November 1, 2021, for videos using the phrase "breast reconstruction with fat grafting." The top 200 videos retrieved from the TikTok search algorithm were analyzed, and all commentaries, duplicates, and nonrelevant videos were removed. Video characteristics were collected, and two independent reviewers generated a DISCERN score A total of 131 videos were included in the study. They were found to have a combined 1,871,980 likes, 41,113 comments, and 58,662 shares. The videos had an average DISCERN score of 2.16. Content creators had an overall low DISCERN score in items involving the use of references, disclosure of risks for not obtaining treatment, and support for shared decision-making. When stratified, the DISCERN score was higher for videos created by physicians (DISCERN average 2.48) than for videos created by nonphysicians (DISCERN average 1.99; p < 0.001).

치과 보철물 거래 플랫폼 서비스의 중요성과 요구사항: 치과 전문가 설문조사 (Importance and requirements for dental prosthesis order platform services: a survey of dental professionals)

  • 목적: 본 연구는 치과 보철물 거래 플랫폼 서비스의 중요성을 이해하고, 보다 넓은 적용을 위해 필수 요소를 파악하는 것을 목표로 하였다. 연구 재료 및 방법: 치과 의사, 치과 기공사, 치과 위생사, 그리고 치과 산업 종사자들을 대상으로 치과 보철물 주문 및 관련 플랫폼 서비스에 대한 견해를 평가하기 위해 설문 조사를 실시하였다(총 53명). 이를 위해 설문지는 치과 전문가의 검토를 거쳐 작성되었고, Cronbach's alpha coefficient를 사용하여 설문지의 신뢰도를 평가하였다. 요인 분석을 통해 총 변동의 88.417%를 설명하는 5개 범주의 57개 요인을 도출했다. 설문 조사는 온라인 설문지 플랫폼을 통해 실시되었으며, 데이터 분석은 통계 소프트웨어를 사용하여 one-way analysis of variance과 Tukey's honestly significant difference test을 사용하여 수행되었다(α = 0.05). 결과: 주문 시에 정확한 정보 입력, 효과적인 의사소통, 변형 없는 치과 인상체 전달, 데이터 전송 및 저장의 편리성, 안정적이고 합리적인 가격의 플랫폼 서비스 제공이 필수 요소로 확인되었다(P < 0.05). 또한, 이러한 항목의 중요성에는 연령, 치과 직업, 경력 경험에 따른 유의한 차이가 관찰되었다(P < 0.05). 결론: 치과 보철물 주문 플랫폼 서비스에 대한 치과 종사자들의 요구사항은 안정성과 경제성, 그리고 치과 보철물 데이터 전송 및 저장의 용이성이었다. 이러한 결과는 앞으로 치과 보철물 주문 및 관련 플랫폼 서비스의 개발 및 개선을 위한 중요한 지표가 될 수 있다.

COVID-19 기간 동안 보건진료소를 이용하는 마을 주민의 미충족 보건의료서비스 이용 경험: 앤더슨 행동모델을 기반으로 한 혼합연구 (Experiences of Unmet Healthcare Service Utilization in Rural Populations Using Primary Health Care Posts during the COVID-19: A Mixed Method Study Based on Andersen's Behavior Model)

  • Purpose: The purpose of the study quantitatively investigates the experience of unmet healthcare service utilization by rural populations in vulnerable areas during the COVID-19 pandemic based on Andersen's behavior model. At the same time, this study attempts to describe the experiences of unmet healthcare service utilization among participants in vulnerable rural areas by analyzing qualitative contents through open-ended question. Methods: Data were collected from October to November 2022 using Qualtrix, a web-based survey platform. A total of 863 participants completed an online survey. Quantitative data were analyzed using 𝑥2 test and logistic regression analysis. Qualitative data were analyzed using content analysis. Results: The factors affecting participants' unmet healthcare service utilization were type of residential area and underlying disease. The qualitative analysis identified; four categories and nine sub-categories. Conclusion: Based on these findings, it is necessary to develop a disaster nursing response model according to the type of residential areas and the number of people.

사물인터넷 기반의 일상 건강정보 수집을 위한 스마트 홈 테스트베드 구축 (Study on Building Smart Home Testbed for Collecting Daily Health Condition based on Internet of Things)

  • 사물인터넷 기술의 발달로 의료서비스의 질을 높이기 위한 정보통신기술과 의료서비스의 결합 시도가 증가하고 있다. 사물인터넷 기술을 활용하면 개인의 건강 정보를 일상생활에서 지속적으로 수집 할 수 있으며, 이에 대한 분석을 통해 의료서비스 품질을 향상 시킬 수 있을 것으로 기대하고 있다. 하지만 일상 생활에서의 건강정보 수집을 위해 활용할 수 있는 사물인터넷 기기들의 부족과 사용자 편의성 저하 등의 문제와 더불어, 개인 건강 정보에 대한 개인정보 보호 문제가 이러한 서비스들의 연구 개발 및 적용에 걸림돌이 되고 있다. 따라서 본 연구에서는 사용자가 자연스러운 일상 생활을 영유하는 상황에서 건강관련 정보를 상시 수집하고 이를 개인 저장소에 저장하여 필요시에만 의료 종사자에게 제공할 수 있는 일상 건강정보 관리 서비스 시나리오를 구성하고, 이를 위한 사물인터넷기반 스마트 홈 테스트베드를 구축하였으며, 현재 사용자 실험을 진행 중에 있다. 본 연구 결과를 기반으로 추후 의료기기 제조업체, 병의원, 보험사 등과의 연계를 통해 일상의 건강정보에 기반한 고품질 의료 시범서비스 제공 방안을 모색하고 있으며, 다양한 건강관련 사물인터넷 기기의 활성화 및 일상 건강정보 분석을 통한 스마트 헬스케어 서비스의 활성화에 기여할 수 있을 것으로 기대한다.
