The Trend and International Comparison of Overseas School Health Indicators (학교보건지표의 해외 동향과 국제 비교)

  • Purpose: The purpose of this study was to identify the trend of overseas school health indicators and to investigate indicators comparable internationally. Methods: Using the key words, school, health, indicators and policy through formal literature and web sites, the resources were used after the completeness of resources such as the presence of author, year published and place, and reproducibility was evaluated. Results: In overseas, the interest of school health indicators has increased gradually since 1960. Quality indicators as well as quantitative indicators are important as the good school health indicators. The overseas school health indicators have been very comprehensive, not only including students, but also including the expanded population such as school personnel, parents, family and community, process and outcomes, policies, social and cultural environment. The trend of school health research is from traditional issue-based to indicatorbased which makes comprehensive interpretation including development of school health service and life satisfaction. Among internationally comparable indicators, Health Behaviour in School-aged Children (HBSC) and Global School-based Student Health Survey (GSHS) were chiefly for students' health and behavior level, and the School Health Service Survey (SHS) was for school health service personnel and policy. Conclusion: Characteristics of overseas school health indicators were expanded population, and comprehensive and internationally comparable indicators. Therefore, Korea school health indicators need to be comprehensive using expanded population and qualitative indicators, and consider standardized indicators comparable internationally.

Analysis of Community Health Status and Related Factors Using Community Health and Social Indicators (지역사회 보건사회지표를 이용한 지역사회 건강수준 관련 요인 분석)

  • Purpose: The purpose of this paper was to investigate community health status and related factors using community health and social indicators. Method: Data sources were reviewed and data for 10 categories, 75 indicators were collected. Community health status and health-related factors were categorized, and the means and standard deviation of individual indicators were obtained and standardized scores were calculated. In addition, through factor analysis of individual indicators by category using the scores and using the resultant factor coefficients as weights, indexes were calculated by area. Correlation and regression were analyzed. Result: Each indicator was highly correlated with each index, and the indexes were highly correlated with one another. Correlation coefficients were above 0.8 between community health index and population, education, housing, and economy, between population and education, housing and economy, between education and housing and economy, and between housing and economy, environment and industry. But multicollinearity was not found in the result. Significant factors on community health index were population, health personnel and facilities, education, housing and economy, and R-square were 92.4%. Conclusion: Health determinants such as population, health personnel and facilities, education, housing and economy could be influencing factors on community health in community level. These results showed the importance of intersectoral collaboration within a local government. Overall community health can be enhanced by intersectoral collaboration.

Self-Care and Associating Factors in Hemodialysis Patients (혈액투석 환자의 자기관리 수행도와 이에 영향을 미치는 요인)

  • Self-care and the performance of their own role might be important for the prevention of complications and improvement of quality of life in hemodialysis patients with chronic renal failure(CRF). To improve well-being and quality of life for the patients, the author estimated the level of self-care and associating factors through a questionnaire. The information was composed of the knowledge for hemodialysis and renal disease, the level of self-care, health belief, supports from the family, disease-related stresses, personal characteristics, medical history, relationships with medical personnel, etc. The data was gathered from 126 hemodialysis patients who were undergoing hemodialysis in one university hospital and five hospitals in Kyungsangnam-Do area from December 1997 to January 1998, and was analyzed by PC SAS program(version 6.12) with the level of significance($\alpha$=0.05). The mean age of subjects was 47.0$\pm$13.5years with no significant difference in gender distribution. The mean duration of hemodialysis was 39.0 months, and their frequencies of hemodialysis were more than three times per week(77.0%). Only 21.4% had the specific education on hemodialysis and CRF. In the level which was expressed as the score out of 100, the mean of knowledge was 90.7$\pm$9.1 and the mean of self-care was 73.9$\pm$12.7, that means, they only partially carried their knowledge into practice. They showed a significant correlation between knowledge and health belief($\gamma$=0.282); self-care and health belief($\gamma$=0.357), family supports and knowledge($\gamma$=0.221), self-care($\gamma$=0.402), health belief($\gamma$=0.431); and health belief and stress($\gamma$=-0.361). Age, religion, marrital status, education, and relationships with medical personnel showed positive correlations, and smoking showed negative correlation with self-care. In the multiple regression with the level of self-care as dependent variable, and each of the characeristics as independent variables, supports from the family($\beta$=6.615=0.158), the experience of disease specific education($\beta$=4.959), relationships with medical personnel($\beta$=6.615), current smoking($\beta$=-6.986), and current drinking ($\beta$=-7.095) were detected as significant factors. The value of R-square was 34%. In summary, to promote the level self-care and to improve the well beings and Quality of life for the hemodialysis patients, it would be emphasized that they terminate smoking and drinking, and it would be recommended that the education programs and supports from the family be strengthened. And, because there was a considerable difference between the level of knowledge and self-care, it would also be emphasized to propose the education programs which focused on execution. In addition to that, there is a need to improve relationships between the patients and medical personnel through positive changes in the attitudes of the medical personnel.

Factors Influencing the Knowledge of Health Insurance Standard and Health Insurance Application (요양급여비용 산정기준의 지식수준과 건강보험 실무적용에 영향을 미치는 요인)

  • This study aims to figure out the current state of health insurance education provided to dentists and personnel in charge of insurance claim as well as the effects of experiences in education on their knowledge of health insurance standard and actual application. As a result of analysis on 100 data sheets related to dentistry and 100 data sheets related to personnel in charge of insurance claim (a total of 200 data sheets), following results were generated. Insurance claim personnels showed higher score than dentist in knowledge of health insurance standard and it was significant statistically. Knowledge of insurance claim personnel showed significant differences in educational experiences, in health insurance for the latest three years and average hours of education per session whereas that of dentists did not indicate any significant differences. The level of practical application of dentists was significantly different (p<0.05) according to experiences in insurance claim, and that of insurance claim personnel significantly varied (p<0.05) depending on age, experiences of insurance education and average hours of education. The longer average hours of education, the higher level of practical application. Experiences of insurance education turned out to affect on the knowledge of health insurance standard significantly (p<0.05) on the part of dentists and insurance claim personnel, and the level of experiences in insurance claim and knowledge of health insurance standard on the part of dentists and insurance claim personnel turned out to affect on practical application significantly (p<0.001). Judging from the result, continuous education needs to be conducted in order to enable dentists and insurance claim personnel to maintain the level of knowledge about health insurance and practical application.

Organizational Determinants related with Relative Efficiency of the Community Mental Health Centers (지역사회 정신보건센터의 상대적 효율성에 영향을 미치는 조직관련 특성)

  • This study was conducted to explore the relationship between the efficiency and its organizational determinants of tile Community Mental Health Centers(CMHCs). Data are obtained from 81 personnel of 27 CMHCs from Sept. to Oct. in 1999(19 in Kyunggi province, 7 in Seoul City and 1 in Chunchon, Kangwon province). Major findings of this study are as follows. 1. DEA is a mathematical programming technique that optimizes the relative efficiency ratio of inputs over outputs for each decision-making unit(DMU). It produces a summary scalar efficiency ratio for each DMU of CMHCs. It assessed multiple inputs and multiple outputs simultaneously, and compared to specific peer group of CMHCs. 2. Organizational determinants of DEA efficiency of CMHCs we proved as advertisement(+), location of CMHCs(in public facility)(+), area of facility(+), period of operation(+), job satisfaction(+), clarity of work-role(vague), cohesion(-), rate of certified personnel(+), number of referral(+), and voluntary service time(-).

Performance of Administrative Personnel in Hospitals (병원행정부서인력의 업무수행수준분석)

  • The hospital is characterized by it's remarkable labor industry and human resources input by unit. Recently, the administrative personnel are recognized as important staff to provide a hospital guidance to consumer and also easiness for consumer's visit to hospital. The objectives of this study is to find the performance of the administrative personnel in hospitals. The unit of analysis is the hospitals and data was collected form 144 staffs in 5 hospitals. Self administered questionnaire was given to analyze the general characteristics of staft such as age, sex, education, experience, and performance level in terms of frequency, ability, necessity of tasks. The major findings are as follows: 1. The 5 major tasks such as general affairs, insurance related affairs, hospital statistics, admission/discharge, and analysis of treatment cost were analyzed. Performance level of these tasks were not showed consistent level. It means that the same task was showed both high level performance and low level one. 2. The higher rates of performance level, ability and necessity were found, below 29 years of age, junior college graduates and university hospital than that of general one. 3. Factors mostly affected to performances were found as characteristics of hospital, age and education. 4. Concerning the various manpower management, On the job training, incentive mechanism and colose relationship among units were somewhat lack. In conclusion, most responded administrative personnel have performed actively in dealing with their tasks. however, the performance level and ability of the same task were showed differently, it means that such routine works were not standardized. Therefore, standardization and specification of tasks should be developed to strength the performance. Finally, this study is the first attempt to find out the performance of the administrative staffs and the study results imply that further study could be neeed to promote the performance of administrative personnel efficiently and effectively.

The Relationship of Value Added to Personnel Expenses and Operating Margin in Hospitals (의료기관의 인건비투자효율과 의료이익률 간의 관계)

  • The study intends to conduct an analysis of relations between efficiency of investment to human resources and the operating margin in hospitals. The analyzed results are as follows: First, it is found out that an index related to labor productivity(the monthly value added per bed, the value added ratio to gross revenue), and an index for efficiency of human resources(value added to personnel expenses), do not have a significant difference by years. Second, labor productivity, indicating the efficiency of human resources, does not have a significant difference between regions and between hospital types. But there is a significant difference according to types of establishment: private hospitals have higher labor productivity(efficiency of human resources) than corporate hospitals. The hospital size is small have significantly higher labor productivity. As a result of a follow-up check, it is found out that there is separation between a group with more than 200 beds and a group with less than 200 beds. Third, at the relations between the indices related to value-added productivity and the operating margin that the higher the value added ratio to gross revenue and the higher labor productivity, the higher the operating margin. Especially, labor productivity(value added to personnel expenses), an index for the efficiency of human resources, out of all the indices related value added productivity, has the most significant influence on the operating margin.

Studies of Nasal Carriage and Antimicrobial Susceptibility of Test Staphylococcus aureus in College Students and General Hospital Personnel in Incheon City (인천시내 일부 종합병원 종사자와 대학생의 비강내 Staphylococcus aureus의 보균상태 및 향균제에 대한 감수성)

  • Nasal carrige of Staphylococcus was studied in relation to its significance as a source of the Staphylococci that caused hospital-acquired infection. Due to the trend of the increasing r esistance of S. aureus to many antimicrobial agents, it is necessary to study the sensitivity to antibiotics of this infectious microorganisms. 50 persons from general hospital and 50 college students were the object of this study. The following results were obtained 3 0 Strains of S. aureus were isolated. The rate of S. aureus nasal carrying were 26% in college students and 34% in hospital personnel. S. aureus which showed resistance to penicillin were 90%, tetracyclin 43%, erythromycin 37% and oxacillin 17%. The number of penicillin resistance of S. aureus were 11 (84%) in college students and 16 (94%) in hospital personnel. The number of strains of penicillin resistant S. aureus which produced 13-1actamase were 9 (82%) in college student and 14 (88%) in hospital personnel. Methicillin-resistant S. aureus (MRSA) which showed resistance to erythromycin and penicillin G were 100%, tetracyline, cephalothin and clindamycin were over 40% respectively, gentamicin 20%, SAM 20% and chloramphenicol 0%.

Survey of Korean daycare personnel on safe sleep practices related to sudden unexplained infant death: a cross-sectional exploratory study

  • Purpose: Safe sleep practices (SSP) are among the main strategies to reduce sleep-related sudden unexplained infant death (SUID). Daycare personnel must be knowledgeable and trained in SSP related to SUID. This study explored the experience, knowledge, and confidence regarding SSP associated with SUID of daycare personnel. Methods: A cross-sectional survey was conducted with 395 staff members at 61 daycare centers to measure their experience related to SSP (10 items), related to sleep position and location, bedding materials, and other topics; knowledge of SSP (18 items); and confidence in SSP (1 item) related to SUID. Results: A substantial proportion (23.6%) of respondents used the lateral or prone positions for infant sleep. On average, 4.5 bedding materials were used for infant sleep. Participants showed a lack of knowledge about SSP as indicated by a 56.6% knowledge of SSP related to SUID correct answer rate. Personnel who received SUID education were more knowledgeable and had more confidence regarding SSP than those who did not. More knowledge and confidence related to SSP were associated with better adherence to SSP. Conclusion: Standard SSP guidelines should be developed based on South Korea's culture of childcare for educating both childcare professionals and parents at home.

A Gap Analysis between Inpatients' and Personnel's Perception of Hospital Foodservice Quality (병원급식서비스의 질 인식에 대한 입원환자와 종사자간 괴리 분석)

  • The purposes of this study were to analyze the gap between foodservice personnel and inpatients, to urge foodservice providers to reconsider by identifying the problems in service delivery for customer satisfaction, and to deduce the priority for foodservice quality improvement. The results of this study can be summarized as follows : the average perception score of personnel (4.32 out of 5) was higher than that of customers (3.90). In particular, the customers' perceptions of 17 attributes, which included 'removal service of tray by foodservice personnel', 'nutrition and health-related information service', 'handling inpatient's complaint ASAP', 'delicious meals' and 'salty enough meals' and so on, was significantly lower than personnel's. Both service providers and customers perceived that 'personnel attitude' was the highest and 'meal quality' was the lowest among the 4 factors, but there was significant difference on 'meal quality'(p < .001), 'customer reception' (p < .001) and 'personnel attitude' (p < .05) between the two groups. As a results of quadrant analysis, 'removal service of tray by foodservice personnel', 'handling inpatient's complaints ASAP' and 'meal service according to doctor's orders were categorized into Quadrant A with meaning of high personnel's perceptions and low customers'. Therefore service providers have to perceive the gap between the two viewpoints and grant priority to these attributes in improving foodservice qualify.
