Testing for Self-care Ability Measurement in an Aged Health Center - A Delphi Technique - (일개 노인보건센터에서의 자가관리능력 측정도구 검증 - 델파이기법을 중심으로 -)

  • Purpose: To find out whether the self-care ability measurement used for visiting health care provide an appropriate standard to select service methods using a Delphi survey and to offer basic data that can improve the measurement. Method: A Delphi survey was done with 39 people consisting of theoretical professionals, field managers, and practitioners engaged in visiting health care. Result: It was found that items and weighted points in the currently used self-care ability measurement needed adjustment. Therefore, a modified evaluation standard was presented by domains, items and detailed items. Finally, the self-care ability measurement is composed of 4 domains, 11 items and 19 detailed items. Conclusion: This study is significant in that it presented an evaluation standard applicable directly to business practice by supplementing problems in the existing self-care ability measurement. This study suggests the need to develop various measurements for elderly households based on community.

  • The objective of this study is the developement health & disaster monitoring measurement using ubiquitous active communication digital datalogger system for monitoring measurement of railway construction sites. For the replacement of current passive data communication, ubiquitous active communication digital datalogger system is studied for the first time with in a country. Therefore data communication method and analyzing program of automatic measurement data is developed for the global positioning automatic digital datalogger system. The results of this study will be using both real time automatic monitoring measurement and health & disaster monitoring measurement of railway structures.

Impact of Health Insurance Type on the Quality of Hemodialysis Services: A Multilevel Analysis (의료보장 형태가 혈액투석 서비스 제공에 미치는 영향에 대한 다수준 분석)

  • Objectives: This study aims to compare quality indicators for the hemodialysis services between patients with health insurance and those with medical aid. Methods: This study used data from sampled hospitals that provided a hemodialysis service. A total of 2287 patients were selected, and the information for hemodialysis service has been granted from medical record reviews. A multi-level regression analysis was used to examine the differences in process and outcome indicators for hemodialysis between patients with health insurance and those with medical aid. Process indicators were defined as: frequency of hemodialysis, hemodialysis time, erythropoietin (EPO) use, measurement of hemodialysis dose at least once a month, measurement of phosphate at least once every three months, and measurement of albumin at least once every three months. Outcome indicators were defined as: hemodialysis adequacy, anemia management, blood pressure management, and calcium, phosphate and nutrition management. The total scores for outcome indicators ranged from 0 (worst) to 4 (best). Results: There was a significant difference in the measurement of hemodialysis dose at least once a month between patients with health insurance and those with medical aid (OR 0.66, 95% CI = 0.43 - 0.99). However, frequency of hemodialysis, hemodialysis time, EPO use, measurement of phosphate at least once every three months, measurement of albumin at least once every three months, hemodialysis adequacy management, Hb${\geq}$11 g/dL, blood pressure within the range of 100-140 /60-90 mmHg, calcium x phosphate${\leq}$55 $g^2/dL^2$ and albumin${\geq}$4 g/dL were not significantly different between the groups. Conclusions: There were no significant differences in outcome indicators for hemodialysis between the groups. Further studies are warranted into the mechanism that results in no differences in the outcome indicators for hemodialysis.

Regulatory Compliance for the Working Environment Measurement System in Korea (작업환경측정제도 규제 순응 실태)

  • The regulatory recognition, regulatory approval, and observance for employer, inspector, and worker groups were investigated for the Korean measurement system in working environments. The employer, worker, and inspector showed an agreement of 90~97 % for the measurement of working environments. This survey showed that the measurement system of working environments has been settled in Korea. However, this survey showed an agreement of 81~87 % for reporting the results of working environment, involvement of worker representative, improvement of working environment, and holding presentations for working environment measurement. The employer recognized well for the measurement of working environments, involvement of worker representative, improvement of working environment, and holding presentations for working environment measurement, but the employee did not recognize well them as much as the employer did. Thus, the improvements for the recognition for involvement of worker representative, improvement of working environment, and holding presentations for working environment measurement by the employee are needed.

  • Objectives: Time location data are critical for accurately estimating personal exposures. This review papers summarized various measurement methods of time location pattern for air pollution exposure assessment. Methods: Forty manuscripts (papers, books and reports) were reviewed to comprehensively describe time location measurement methods. Results: This review included traditional methods such as time activity diary, questionnaire, observation, focus group and newly developed technical methods including global positioning system, web, radio frequency identification and ultrasound detection. Some research applied a combination of methods. Conclusion: Although various methods have been used to collect time location data, further development of accurate measurement methods for time location data is needed.

  • I. Background and Purpose Health Indicator system and measurement of health status are an important fields in national health fields. This study reviewed the overall concepts of health and health indicators, health indicator system. The purposes of this study are to build the conceptual health framework, and suggest a health indicator system, in order to correspond to the situation of national health and the demand of International organizations. II. Scope and Contents The scope of this study ; - Review of tile conceptual health framework, health indicators, and health Indicator system - Selection and development of tile new individual health indicators - Suggestion of tile revised health indicator system III. Results of Study This study intented to build the conceptual and framework of national health and provide the measurement tools of health status. This study developed the health indicator system through the conceptual and hierarchial approach to national health. The health indicator system contains 6 concern: areas and each sub-areas. The major concern areas are health state and behavior, deathㆍdiseaseㆍdisability, health care utilization, health resources, health expenditure and finance, other affecting factors on health. This health indicator system is corresponding to the situation of health status patterns and the demand of international organizations. And this health indicator system is considering the present health data production system and the availability of health data.

Trend and Implication of OECD Hospital Performance Project (OECD 병원 성과 프로젝트의 동향과 국내 시사점)

  • The Organization for Economic Cooperation and Development, which has continuously evaluated the performance of healthcare systems, has recently invested much effort into hospital performance measurement. The purpose of this paper is to introduce the hospital performance measurement programs operated by international organizations or at the national level based on the OECD's hospital performance project. Health Insurance Review & Assessment service (HIRA)'s quality assessment was analyzed based on the analytical framework of the OECD's hospital performance project. The hospital performance measurement programs of WHO, Canada, Australia, United States and United Kingdom are briefly explored, in view of the conceptual framework, key performance dimensions and indicators that are currently in use. The OECD suggested seven key dimensions of hospital performance: timeliness, efficiency, continuity, effectiveness and appropriateness, staff orientation, patient orientation and safety. The analysis of the quality assessment program of HIRA, which operates 36 diseases and procedures and 347 indicators, shows that the numbers of indicators are relatively small in the areas of safety, patient centeredness and efficiency. Continuity of care and staff orientation are not fully developed also, but the situations are similar in other countries. In conclusion, hospital performance measurement using stable and comprehensive data should be developed to improve overall system performance, and discussions on a conceptual framework that can lay out directions and key performance domains need to take into place.

A Review of a System for Improving the Reliability of Domestic Measurement Results regarding the Work Environment (국내 작업환경 측정결과 신뢰성 향상을 위한 제도 고찰)

  • Objectives: Improvement of the reliability of work environment measurement results is one of the principal subjects to be resolved in the field of industrial hygiene. Thus, this study was conducted in order to review domestic programs for enhancing the reliability of work environment measurement results and propose a more efficient management plan for workplace monitoring. Materials and Methods: The current domestic programs reviewed in this study are the quality control system and evaluation system for industrial hygiene laboratories, the reliability system accompanied by KOSHA (the Korea Occupational Safety and Health Agency), the system of permissible exposure limits and observed inspection system for permissible exposure limits. Their contents and data were analyzed by reviewing articles, reports, and guidelines related to reliability improvement for work environment measurement results. Results: As a result of the literature review, all the domestic programs investigated in this study had both merits and demerits in terms of considering the reliability of work environment measurement results. In particular, the fact that preliminary notification is provided to the head of a workplace before implementation would be an obstacle for elevating the reliability of work environment measurement results. Conclusions: Based on the review results obtained from this study, it is proposed that on-site measurement without preliminary notification should be considered for a more efficient inspection system that would enhance the reliability of workplace monitoring.

An investigation on the Improvement of the Working Environment Measurement Reporting Policy (작업환경측정 보고제도 개선 방안 도출을 위한 조사 연구)

  • Objectives: In order to reduce the burden on employers and increase the reliability of measurement results, improvements to the provisions related to the work environment measurement reporting system, such as the current Occupational Safety and Health Act and its Enforcement Rules, are planned. This study aimed to suggest improvements for the work environment measurement reporting system through a survey and Delphi investigation. Method: This survey included workplaces (health managers), national institutions (the Ministry of Employment and Labor) that use the results of the work environment measurement reporting system for policy and supervision purposes, and work environment measurement institutions that enter the results were included. In addition to the survey, we tried to derive results through meetings with stakeholders and expert advisory meetings. Results: It is difficult to abolish or partially improve the reporting system under the Enforcement Regulations of the Occupational Safety and Health Act at this point because the opinions of workplaces, supervisory agencies, and measuring agencies differ in terms of its intended purpose and use. In the case of high-exposure harmful factors (over 50% on the basis of exposure) in the "comprehensive opinion" described in the work environment measurement results table, it is necessary to insert unit of work with exposed harmful factors, exposure factors, and current conditions in checklists or tables so that they can be reflected in government policies. In the case of workplaces that are feared to be highly exposed to substances subject to measurement, it seems desirable to improve them so that industrial health instructors registered with the Korea Safety and Health Agency or local labor offices can provide technical guidance. As an improvement plan to increase the reliability of data and the use of big data, it is necessary to improve the input method for processes and jobs. Conclusion: The laws and regulations of the work environment measurement reporting system are difficult to revise due to a lack of consensus among current stakeholders, but improvements can be achieved by improving the Ministry of Employment and Labor's notifications and other means. In addition, in order to effectively utilize the data from the K2B system, it is necessary to improve the input method for processes and jobs.

  • The major goal of caring the elderly is not to cure the patient, but rather to improve or maintain the individual functional ability. With the aged population increases, there is increasing consensus among health care workers about the importance of developing a tool to measure and evaluate the health status of the elderly. As many writers is pointing out, care of the elderly embraces the spectrum of physical-social-psychological problems and an enormous number of variables could be introudeced into the health worker's routine measurement. But such an expansive approach to measurement could have disadvantages. The purpose of this study is to introduce the measurement tools of physical functioning, perhaps the most important measure required in long-term care, which are separated into three categories of measures; general physical health, ADL, IADL. It is important that all health workers who provide care to the elderly incorporate the concepts of functional health status into the routine assessments. And continuous study about reliability and validity is needed to apply those foreign tools to Korea.
