Lessons from Chile: The Impact of Privatization of Health Insurance on Women's Health (의료보험 민영화가 여성의 건강에 미치는 영향 : 칠레의 사례를 중심으로)

  • Chile has been the first country in Latin America which has built a two-tiered health care system by partially privatizing the health insurance sector. Despite the intial decrease of health expenditure, more researches now show that health inequality within the Chilean health sector has been augmented with privatization of its insurance system. To explore such inequality, this article looks into the impact of privatization of health insurance on women's health. The author argues that privatization has intensified medicalization of women's body and, consequently, it worsened women's health in Chile. This article contributes to a more comprehensive understanding of market-oriented health care reform by linking it with medicalization process.

Analysis of Connection Centrality Degree of Hot Terminologies According to the Discourses of Privatization of Health Care (의료민영화 논의에 따른 이슈용어의 연결 중심성 분석)

  • The purpose of this study was to review the agreement and disagreement logics on privatization of health care to bring quality enhancement of medical service and alienated area without medical services at the same time, to identify the core keywords through language network analysis a kind of contents analysis on the editorials dealing with privatization of health care and hospitals for profit published on the major daily newspapers for the recent three years, and to find out what is the core of the controversy through the connection centrality analysis of core keywords. Conclusively, it was found from the centrality analysis that "medical service," "hospital," "privatization," "privatization of health care," "hospital for profit" and "Government" were situated in the center of the controversy. It is natural that keywords such as "medical service," "hospital," "privatization," "privatization of health care"and "hospital for profit" were located in the center because this study reviewed the editorials published on major newspapers for the recent three years regarding the privatization of health care or hospital for profit. Next important keywords (words) were "people," "health"and "health insurance." It shows that privatization of health care was not simply seen as the opening of medical service market but as an important issue related to health of people and health Insurance. Next words with high centrality were "objection" and "allowance." Through the contents analysis of editorials for the last three years, it was found that the opinions for and against the privatization were equally matched according to the centrality analysis result. On the other hand, there is one noticeable result in centrality analysis, which is the keywords such as "US," "Korea US" and "FTA" showed centrality to some extent. It shows privatization is handled relating US and Korea US FTA by editorials.

  • In this paper, based on the reference, we try to review the second issues about opening medical market and health care privatization by each topic and propose the measures and alternatives. Currently, in Korea, connection with launch of the WTO system and force of the FTA, the medical industries getting liberalization and globalization. Thus, it is expected to plunge to full-free competition system, and Korean medical institutions started the global competition which completely different dimension. It means that according to the liberalization of the healthcare market the real problem can be caused and also, the incessant discussion and effort for the implementation of international community are needed. Regard to attracting foreign patients and opening medical markets, the government also spreading the continued advancement strategy politically until now. However, generating problems with implication is inevitable and measures and alternatives to it are also needed. In accordance with the opening, the accompanying suggestions is medical privatization, that is, whether the health care pursue the profit not the not-for-profit and the current hospitals in Korea they are leaved as non-profit hospitals and let the make the subsidiary as general commercial enterprises, it seems indirect. However, it is like a healthcare privatization virtually thus, implication seem be large. Of course, through the public opinion and legal reservation, the liberalization and privatization of medical market can be delayed or not forced. It would be not fit in the flow of the inevitable globalization, it can be inhibited national interest and economic development also, and it can be the critical implications which shake the health system and collapse of the domestic health care market.

A Study on the Problems and Improvement of Long-Term Care Insurance for the Elderly (노인장기요양보험제도의 문제점 및 개선방안에 관한 고찰)

  • Korea is facing one of the fastest aging population problems in the world due to the extension of life expectancy and low birth rates due to economic development and the development of health care. It has become difficult to take care of elderly people in need of long-term care at home, and social problems such as dementia and the increase in stroke cases are serious problems. The government is enacting and implementing the Long-Term Care Insurance Act for the purpose of improving the quality of life for senior citizens subject to long-term care and alleviating the burden of family support in order to solve such social structure and problems for senior citizens. However, as a result of the implementation of the long-term care insurance system for the elderly, the privatization of the facilities and the disorder are causing widespread requests for public accountability of the services and the enhancement of the quality of the facilities. In addition, various arrangements are needed for the production of low-wage medical care workers, tasks for quality improvement, problems regarding financing methods, and so on. In order to maintain and develop the long-term care insurance system for the elderly, the government will consider seeking ways to improve the legal and institutional aspects according to changes in the social environment of the age, as well as seeking mental and psychological measures for the stabilization of old life as well as physical health.
