원조 범람이 수원국의 보건부분 정부지출에 미친 영향분석: 아프리카 사하라 사막 이남 지역 국가들을 중심으로 (Analysis of Effect of Aid Fragmentation on Spending on Health by Recipients : Focus on the Sub-Sahara African Nations)

  • 이 연구의 목적은 원조 범람이 수원국의 정부지출에 미친 영향을 패널데이터를 이용해 실증 분석하고, 이를 통해 향후 공적개발원조 사업의 운영 방향에 대한 함의를 얻는 데 있다. 이 연구에서는 아프리카 사하라 사막 이남 지역 국가들의 보건 부문에 대한 원조범람지수를 산정하고, 원조범람이 보건 부문에 대한 정부지출에 어떤 영향을 미치는지를 실증분석하였다. 이 연구에서 수행한 실증분석 결과, 원조범람은 보건 부문에 대한 정부지출 수준을 유의미하게 감소시키는 효과를 발생시키는 것을 확인하였다. 그리고 이와 같은 현상이 발생한 것은 신규 사업을 추구하는 공적개발원조 사업의 특징(공여국의 니즈)과 가능하면 더 많은 공적개발원조 자금을 수혜하고자 하는 수원국 정부 간 상호 이익추구에 따른 것으로 추론된다. 분석결과를 토대로, 이 연구에서는 경쟁적이고 과도한 공적개발원조 사업지원이 수원국 정부의 예산운영 과정에 대한 왜곡을 유도해 결과적으로 공적개발원조 사업의 비효율을 초래하므로, 사업지원 과정에서 수원국의 정책 능력에 관한 종합적인 고려가 필요하며, 개발원조위원회(Development Assistance Committee)의 종합조정 기능의 강화가 필요함을 제안하였다.

제주지역내 재가암환자관리를 받은 환자와 보호자에게 제공되는 프로그램 만족도와 요구 (Satisfaction and Needs of Cancer Patients and Caregivers under Home-based Cancer Patients Management in Jeju)

  • 본 논문은 일개 지역암센터에서 제공되는 재가암환자관리프로그램의 만족도와 요구에 대한 조사를 통해 프로그램의 질 향상을 높이기 위해 시행하였다. 재가암환자관리에 등록된 된 환자와 보호자를 대상으로 2015년 3월부터 2015년 10월까지 자체적으로 개발된 설문지를 통해 면대면으로 조사를 시행하였다. 환자 59명과 보호자 41명, 총 101명을 대상으로 설문조사가 이루어졌다. 암종에서는 유방암이 주된 암이었으며 재가암환자의 방문회수에 대하여 51명(86%)의 환자와 36명(85%)의 환자 보호자가 만족하였다. 서비스 신청경로는 환자의 경우 암센터홍보지가 22명(37.29%), 보호자의 경우에는 주변사람의 권유가 11명(26.19%)으로 가장 많았다. 환자에게 직접 제공되는 처치를 제외하고 가장 선호하는 것은 심리상담이었으며, 나들이와 암종별 교육의 만족도도 높게 나타났다. 프로그램의 만족도는 5점 척도에서 $4.14{\pm}1.21$로 비교적 높게 나왔으며 환자가 보호자보다 높았으나 통계적으로 의미는 없었다($4.29{\pm}1.11$, $3.93{\pm}1.31$, p=0.141). 자아존중감은 보호자가 환자보다 높았으나 통계적으로 유의하지 않았다. 향후 재가암환자와 보호자들의 삶의 질을 높일 수 있도록 심리상담과 환자의 경제적인 상황을 고려한 맞춤형 프로그램개발이 필요할 것이다.

여성 베이비부머들의 식생활 태도와 미래 식생활 요구도 조사 (A Study on the Dietary Behaviors of Female Baby Boomers and the Needs for Future Perspectives of Dietary Life)

  • The purpose of this study is to examine female baby boomers' dietary habits and their attitudes together with their needs for future perspectives of dietary life. Our aim is to use these findings as a basic data when forecasting for food-related industries or policy making. A survey is being carried out for a total of 358 female baby boomers and analyzed by SPSS 12.0. The following is a summary of this study. The average age is 52.6 years old, most of them graduated from highschool (63.1%) and had a nuclear type of family (76.1%). Only 39.0% is composed of housewives, others had either full-time or part-time jobs. Self-assessment of stress is not so high and only 8.1% are dissatisfied with their lives. 38.2% are either overweight or obese in terms of BMI, and most of them are non-smokers (97.2%) or non-drinkers (63.0%). Their mean dietary habit scores are $70.6{\pm}11.8$, and the scores show significant relations with their education levels (p<0.01), monthly income (p<0.01), life satisfaction rates (p<0.001), stress levels (p<0.001), smoking habits (p<0.05), drinking habits (p<0.05), regular exercises (p<0.001) and regular health check-ups (p<0.05). The rate of skipping breakfast, lunch and dinner are 18.2%, 1.1%, 5.2% respectively. The main reason for skipping breakfast is the 'lack of time'. With regards to the frequency of grocery shopping, almost half of the subjects (55.7%) said '1~2 times per week' and bought mainly raw food sources such as vegetables, fruits, and meats. The majority of the subjects (91.3%) report that they cooked meals at homes, and took about 1 hour of time. The subjects also point out that cooking was a bothering task, and only 46.4% would prepare meals at home, while others would rather eat out or eat convenience foods. The main reasons for not wanting meal services in the elderly welfare facility are because they didn't want to live such places (48.4%) and the meals are tasteless (31.3%). As for delivery meal services, 60.1% are aware of it, and 39.9% would consider using it in the future. Factors to be considered when using the delivery meal service are sanitation (43.7%), nutrition (28.7%), taste (18.4%), price (6.3%), and brand name (2.9%). This study is expected to be used as useful information when developing food-related strategies for baby boomers in the future.

미국과 캐나다 노인밀집도시의 노인주거관련 사회적지원에 관한 연구 : 농촌지역 소도시를 중심으로 (A Study on Social Supports for the Elderly Housing in Senior Concentrated Cities in the United States and Canada : Focused on Small Cities along Rural Counties)

  • The purpose of this study is to explore social supports for elderly housing and their residential lives in small cities along rural counties of the United States and Canada, and suggest future implications for age-concentrated rural villages in Korea. In this study, five small and medium cities in non-metropolitan counties of California and Ontario province were visited and elderly residents and service experts were interviewed about their perceptions of community integrated social support networks for senior residences. The senior housing complexes were built due to influx of both metropolitan and rural residents seeking warm localities, traffic connections, business purposes in active production areas. and leisure attractions. There are five main social support networks for senior housing issues in these areas. First, the areas are claimed for senior zones and accordingly health industries are encouraged by local authorities. Second, the community is homogeneously constructed as a senior friendly environment and include features such as an RV park and mobile cottages. Third, senior-helping seniors are offered active work through golf-cluster active retirement communities. Fourth, traditional theme production camps are mobilized by the elderly workers. Lastly, an information system is maintained for screening volunteers and for senior abuse prevention. On the other hand, residential lives are occasionally negatively influenced by unbalanced concentrations of elderly facilities such as nursing stations and funeral homes. For the future of Korean rural elderly policies, suggestions are made as follows: first, an integrated urban and rural township that contains attractive places for early retiring people who seek a warm atmosphere in later life needs to be constructed. Second, an integrated model retirement village of urban and rural retirement life needs to be initiated as a measure of evaluating the adaptation process of movers in senior concentrated zones. Third, a cooperation system among governmental ministries needs to be formed with the long- term goal of establishing a traditional rural town of independent housing districts and medical facilities in rural areas. Fourth, productive and active lifestyles need to be maintained as the local community and government develop successful retirement rural villages, by limiting the expansion of nursing related facilities. Finally, generation integrated visiting welfare programs and services need to be further developed for the housing areas especially in the winter, when social integration and activity are relatively low.

When Disease Defines a Place: Batavia in British Diplomatic and Military Narratives, 1775-1850

  • The full impact of COVID-19 has yet to be felt: while it may not define the new decade, it is clear that its immediate significance was to test many of the basic operating assumptions and procedures of global civilization. Even as vaccines are developed and utilized and even as it is possible to see the beginning of the end of COVID-19 as a discrete historical event, it remains unclear as to its ultimate importance. That said, it is evident that the academic exploration of Southeast Asia will also be affected by both the global and regional experiences of the pandemic. "Breakthroughs of Area Studies and ASEAN in the Era of Homo Untact" promises to help reconceptualize the study of the region by highlighting the importance of redefined spatial relationships and new potentially depersonalized modes of communication. This paper acknowledges these issues by suggesting that the transformations caused by the pandemic should motivate scholars to raise new questions about how to understand humanity-particularly as it is defined by societies, nations and regions. Given that COVID-19 (and the response to it) has altered many of the fundamental rhythms of globalized regions, there is sufficient warrant for re-examining both the ways in which disease, health and their related spaces affect the perceptions of Southeast Asia. To achieve "breakthroughs" into the investigation of the region, it makes sense to have another glance at the ways in which the discourses about diseases and health may have helped to inscribe definitions of Southeast Asia-or, at the very least, the nations, societies and peoples who live within it. In order to at least consider these larger issues, the discussion will concentrate on a formative moment in the conceptualization of Southeast Asia-British engagement with the region in the late 18th and early 19th centuries. To that end three themes will be highlighted: (1) the role that British diplomatic and military narratives played in establishing the information priorities required for the construction of colonial knowledge; (2) the importance not only of "colonial knowledge" but information making in its own right; (3) in anticipation of the use of big data, the manner in which manufactured information (related to space and disease) could function in shaping early British perceptions of Southeast Asia-particularly in Batavia and Java. This discussion will suggest that rather than see social distancing or increased communication as the greatest outcome of COVID-19, instead it will be the use of data-that is, big, aggregated biometric data which have not only shaped responses to the pandemic, but remain likely to produce the reconceptualization of both information and knowledge about the region in a way that will be at least as great as that which took place to meet the needs of the "New Imperialism." Furthermore, the definition and articulation of Southeast Asia has often reflected political and security considerations. Yet, the experience of COVID-19 could prove that data and security are now fused into a set of interests critical to policy-makers. Given that the pandemic should accelerate many existing trends, it might be foreseen these developments will herald the triumph of homo indicina: an epistemic condition whereby the human subject has become a kind of index for its harvestable data. If so, the "breakthroughs" for those who study Southeast Asia will follow in due course.

Formation of Resilience in the Context of Volunteer Activities Using Information and Communications Technology

  • The article identifies and theoretically substantiates the trends of national resilience in the context of establishing the security of the country and its civilizational subjectivity. The strategy of development of the pedagogical university in the conditions of European integration into the European educational and scientific space based on certain characterological features of the personality of the volunteer in the context of allocation of personal resilience is developed. The analysis of both external and internal challenges and threats to the civilization of the country needs to be understood in the context of economic, socio-political, legal, military-political, spiritual-cultural, educational-scientific and network-information resilience. The concepts of "national resilience" and "national security" are quite close - at first glance, even identical. However, a deeper understanding clarifies the differences: national security is a state of protection of the country identity and its very existence, the realization of its national interests. In turn, resilience is a fairly effective strategy and a fundamental guarantee of national security. At the same time, it is extremely important to understand that both national security as a state and national resilience as a strategy are only means of achieving and developing a strong and humanistic civilizational subjectivity of the country. After all, such subjectivity opens for citizens the opportunity for development, dignified self-realization and a proper life. The restructuring of the volunteer's motivational sphere is due to the dominance of such leading motives, which are focused mainly on maintaining and restoring health, which leads to distorted meaningful life goals: isolation, alienation, passivity, inertia, reduced activity, limited communication, etc. The characteristics of relatively stable human behavior include several primary and secondary properties. The primary (relevant) properties include patience, trust, hope, faith, confidence, determination, perseverance, and love; the secondary - punctuality, neatness, obedience, honesty, loyalty, justice, diligence, thrift, accuracy, conscientiousness, obligation, etc. The restructuring of the volunteer's motivational sphere is due to the dominance of such leading motives, which are focused mainly on maintaining and restoring health, which leads to distorted meaningful life goals: isolation, alienation, passivity, inertia, reduced activity, limited communication, etc. The characteristics of relatively stable human behavior include several primary and secondary properties. Directly to the personal traits of resilience should be included methodological competencies, which include methodological knowledge, skills and abilities (ability to define ultimate and intermediate goals, plan, conduct and analyze knowledge, establish and implement interdisciplinary links with disciplines of medical-psychological-pedagogical cycles, etc.). All these competencies form the professional resilience of the volunteer.

지역사회 말기질환자 가족 부담감에 관한 연구 (A Study of Family Caregiver's Burden for the Terminally III Patients)

  • The purpose of this study was to describe the perceived burden of the terminally III patients's caregiver and to analyze relationship between the perceived burden and the various demographics, illness characteristics, family relationships, and economic factor of the family & patients. The sample of 132 caregivers who care for the terminally III patients Kyung-Gi province, Seoul, Korea. The period of this study was from August to September, 2002. The perceived burden of the family caregiver was measured by the burden scale(20 items, 4 point scale) developed by Montgomery et al. (1985). The Data was analyzed using SAS-program by t-test and ANOVA. The results were as follows; 1. The mean of the family caregiver's burden score was 3.02. The score showed that caregivers perceive severe the level of burden. The hight items of the family caregiver's burden were' I feel it is painful to watch patient's diseases'(3.77). 'I feel afraid for what the future holds for my patients'(3.66), 'I feel it reduced to amount of privacy time'(3.64). 2. The caregiver's burden was significantly related to patient's gender(F=3.17, p= 0.0020), patient's job(F=2.49, p=0.0476), caregiver's age(F=4.29, p=0.0030), and caregiver's job(F=2.49, p=0.0476). 3. The caregiver's burden according to illness characteristics showed no significant difference. 4. The caregiver's burden was significantly associated with patient's family relationship (F=4.05, p=0.0041), patient's care mean period in a day(F=47.18,

자료포락분석방법을 이용한 내원환자의 지역별 벤치마킹분석 : 일개 한방병원을 중심으로 (Benchmarking the Regional Patients Using DEA : Focused on A Oriental Medicine Hospital)

  • 본 논문의 목적은 지역을 대상으로 자료포락분석방법을 적용하여 한방병원에 내원한 환자의 지역적 특성 분석 및 성과개선규모를 파악하는 데 있다. 본 논문에서 사용된 자료는 대전광역시에서 운영 중인 일개 한방병원을 대상으로 수집되었다. 환자의 위치 정보는 행정구역의 가장 작은 단위인 '동'을 사용하였으며, 연구대상 병원의 의무기록 자료에서 환자 주소를 기준으로 수집하였다. 각 행정동의 인구사회학적 변수들은 통계청 자료를 사용하였다. 자료포락분석을 이용하여 행정동에서 내원한 환자의 규모를 비교 평가하였고, 그리고 Tobit 회귀분석을 이용하여 효율성 점수와 지역 특성 변수들 간의 관계를 분석하였다. DEA 분석결과 효율적인 동은 6개였으며, Tobit 회귀분석에서는 각 동별 기초생활수급자수와 총인구수 변수에서 통계적으로 유의하였다. 본 논문에서는 분석단위를 행정구역의 최소단위인 행정동 별로 세분화하여 자료포락분석방법이 가지는 벤치마킹의 개념을 이용하여 분석하였다는 점에서 의의가 있다. 그리고, 병원의 성과 개선을 위해 필요로 하는 각 지역별 환자수를 구체적으로 파악하여다는 점에서 의의가 있다.

말기암 환자와 가족의 의료 및 간호 서비스 요구 (The Study on the Medical and Nursing Service Needs of the Terminal Cancer Patients and Their Caregivers)

  • In this study, we attempted to investigate the needs and problems of the terminal cancer patients and their family caregivers to provide them with nursing information to improve their quality of life and prepare for a peaceful death. Data was collected from August 1, 1995 to July 31, 1996 at the internal medicine unit of S hospital in Seoul area with the two groups of participants who were family members of terminal cancer patients seventy four of them were in-patients and 34 were out-patients who were discharged from the same hospital for home care. The research tool used in this study has been developed by selecting the questionnaires from various references, modifying them for our purpose and refining them based on the results of preliminary study. While general background information about the patients was obtained by reviewing their medical records, all other information was collected by interviewing the primary family caregivers of the patients using the questionnaire. The data collected were analyzed with the SPSS PC/sup +/ program. The results of this study are summarized as follows ; 1) Most frequently complained symptoms of the terminal cancer patients were in the order of pain(87%), weakness(86.1%), anorexia(83.3%) and fatigue (80.6%). 2) Main therapies for the terminal cancer patients were pain control (58.3%), hyperalimentation(47.2%) and antibiotics(21.3%). 3) Special medical devices that terminal cancer patients used most were oxygen device (11.1%), and feeding tube(5.6%). Other devices were used by less than 5% of the patients. 4) The mobility of 70.4% of the patients was worse than ECOG 3 level, they had to stay in bed more than 50% of a day. 5) Patients wanted their medical staffs to help relieve pain(45.4%), various physical symptoms(29.6%), and problems associated with their emotion(11.1%). 6) 16.7% of the family caregivers hoped for full recovery of the patients, refusing to admit the status of the patients. Also, 37% wished for the extension of the patient's life at least for 6 months. 7) Only 38.9% of the family members was preparing for the patient's funeral. 8) 45.4% of family caregivers prefer hospital as the place for the patient's death, 39.8% their own home, and 14.8% undetermined. 9) Caregivers of the patients were mostly close family members, i.e., spouse(62%), and sons and daughters or daughter-in-laws(21.3%). 10) 43.5% of the family caregivers were aware of hospice care. 46.8% of them learned about the hospice care from the mass media, 27.7% from health professionals, and the rest from books and other sources. 11) Caregivers were asked about the most difficult problems they encounter in home care, 41 of them pointed out the lack of health professionals they can contact, counsel and get help from in case of emergency, 17 identified the difficulty of finding appropriate transportation to hospital, and 13 stated the difficulty of admission in hospital as needed. 12) 93.6% of family caregivers demanded 24-hour hot line, 80% the visiting nurses and doctors, and 69.4% the volunteer's help. The above results indicate that terminal patients and their family caregivers demand help from qualified health professionals whenever necessary. Hospice care system led by well-trained medical and nursing staffs is one of the viable answers for such demands.

프라이버시 보호 상황인식 시스템 개발을 위한 쌍방향 P3P 방법론 (A Mutual P3P Methodology for Privacy Preserving Context-Aware Systems Development)

  • One of the big concerns in e-society is privacy issue. In special, in developing robust ubiquitous smart space and corresponding services, user profile and preference are collected by the service providers. Privacy issue would be more critical in context-aware services simply because most of the context data themselves are private information: user's current location, current schedule, friends nearby and even her/his health data. To realize the potential of ubiquitous smart space, the systems embedded in the space should corporate personal privacy preferences. When the users invoke a set of services, they are asked to allow the service providers or smart space to make use of personal information which is related to privacy concerns. For this reason, the users unhappily provide the personal information or even deny to get served. On the other side, service provider needs personal information as rich as possible with minimal personal information to discern royal and trustworthy customers and those who are not. It would be desirable to enlarge the allowable personal information complying with the service provider's request, whereas minimizing service provider's requiring personal information which is not allowed to be submitted and user's submitting information which is of no value to the service provider. In special, if any personal information required by the service provider is not allowed, service will not be provided to the user. P3P (Platform for Privacy Preferences) has been regarded as one of the promising alternatives to preserve the personal information in the course of electronic transactions. However, P3P mainly focuses on preserving the buyers' personal information. From time to time, the service provider's business data should be protected from the unintended usage from the buyers. Moreover, even though the user's privacy preference could depend on the context happened to the user, legacy P3P does not handle the contextual change of privacy preferences. Hence, the purpose of this paper is to propose a mutual P3P-based negotiation mechanism. To do so, service provider's privacy concern is considered as well as the users'. User's privacy policy on the service provider's information also should be informed to the service providers before the service begins. Second, privacy policy is contextually designed according to the user's current context because the nomadic user's privacy concern structure may be altered contextually. Hence, the methodology includes mutual privacy policy and personalization. Overall framework of the mechanism and new code of ethics is described in section 2. Pervasive platform for mutual P3P considers user type and context field, which involves current activity, location, social context, objects nearby and physical environments. Our mutual P3P includes the privacy preference not only for the buyers but also the sellers, that is, service providers. Negotiation methodology for mutual P3P is proposed in section 3. Based on the fact that privacy concern occurs when there are needs for information access and at the same time those for information hiding. Our mechanism was implemented based on an actual shopping mall to increase the feasibility of the idea proposed in this paper. A shopping service is assumed as a context-aware service, and data groups for the service are enumerated. The privacy policy for each data group is represented as APPEL format. To examine the performance of the example service, in section 4, simulation approach is adopted in this paper. For the simulation, five data elements are considered: $\cdot$ UserID $\cdot$ User preference $\cdot$ Phone number $\cdot$ Home address $\cdot$ Product information $\cdot$ Service profile. For the negotiation, reputation is selected as a strategic value. Then the following cases are compared: $\cdot$ Legacy P3P is considered $\cdot$ Mutual P3P is considered without strategic value $\cdot$ Mutual P3P is considered with strategic value. The simulation results show that mutual P3P outperforms legacy P3P. Moreover, we could conclude that when mutual P3P is considered with strategic value, performance was better than that of mutual P3P is considered without strategic value in terms of service safety.
