신의료기술평가제도 운영의 개선현황과 발전방향 (Improvement Status and Development Direction of New Health Technology Assessment)

  • Since the introduction of new health technology assessment in 2007, benefit coverage process of health insurance related to new health technology has become an upgraded system through the evidence-based decisions. As a result of enforcing this system for 10 years, however, there have been several rising concerns. It needs to support the insufficient evidence of medical technologies, introduce reassessment system for post management of market entry technologies, and improve evaluation methods and process. In addition, there is the possibility of emerging an unheard-of medical technology, fused various categories like artificial intelligence, robot, information technology, physics and life science in the fourth industrial revolution. Now, new updated system introduced to improve new technology assessment, such as 'limited health technology assessment system,' 'system for postponement of new health technology assessment,' 'one-stop service system,' and 'integrated operation of approval for medical devices and new health technology assessment.' Therefore it needs to prepare the improvement plan for new health technology assessment to be established more advanced system, and we have to resolve concerns by communication with various healthcare experts and patients now and for ever.

지역사회 건강증진 협력 개발 연구의 지향 과제 (Health Promotion Research on Community Collaboration and Partnership Building: Current Emphases and Directions)

  • Objectives: As collaboration for community health promotion is much emphasized, the concept and process of community collaboration for health needs to be discussed. This paper discusses varying types of collaboration and collaboration building processes and suggests directions for enhancing community collaboration research and practice. Methods: Leading literature on community partnership building and community health development and current community partnership research program information were reviewed. Results: Although the term collaboration is used interchangeably with cooperation, partnership, network, or coalition, conceptual differences need to be acknowledged in order to develop and reinforce the processes of collaboration. Collaboration building goes in hand with community-based participatory research and systems thinking, which should be supported by long-term, systematic planning and evaluation research. Conclusion: Collaboration for community health promotion needs to be defined and agreed conceptually, thus collaboration process can be developed. Effective collaboration building will be facilitated by systematic thinking and participatory research. Research support system should appreciate the time-intensive, process-oriented nature of collaboration building by providing long-term research funding and emphasizing process and long-term evaluation.

가정.방문물리치료 및 학교물리치료의 필요성 및 유형실태에 대한 조사연구 (A Survey of Needs and Types of Home Physical Therapy, Visiting Physical Therapy and School Physical Therapy)

  • The purpose of this survey was to give data and information about type and needs of Home Physical Therapy, Visiting Physical Therapy and School Physical Therapy for physical and nurse. The subjects were 154(99 physical therapists and 55 nurses) who were working at geriatric rehabilitation hospitals and children hospitals. The period of questionary collection was from the 15 of August to the 15th of September 2011. And data was analysis from 99 articles such as journals related to physical therapy, and searched with keyword 'home and visiting physical therapy' by web site and Korea National Assembly Library from 1991 to 2011. The data was analysis with percentage, mean, standard deviation and ANOVA by SPSS PC 12.0. The results were as follows; 1. The definition of 'Home Physical Therapy' has been community based on physical therapy service for the patient who had diagnosis by medical doctor, has been based on medical law. The definition of 'Visiting Physical Therapy' has been community based on physical therapy service at home for the patient who had diagnosis by medical doctor, for the national basic living security, and senior citizen over 65 years who lives alone, has been based on law for community health and law of long term health insurance. The definition of 'School Physical Therapy' has been school based on physical therapy service at school after class for the disabled children who are studying at school, has been based on special education law article 28. 2. As for the knowledge of the Home and Visiting and School Physical Therapy, both groups PT and nurse were 'I do not know'125(81.3%) of the difference the concept of 3 definitions, so it means to need education and information about the different concept of three physical therapy. As for the needs of home and visiting physical therapy, both groups of PT and Nurse were 'needs' 151(98.1%). Physical therapist showed of 'Needs' on visiting physical therapy 35(35.4%), home physical therapy 32(32.3%), and schole physical therapy 32(32.3%). Nurse showed of 'Needs' on home physical therapy 23(41.8%). visiting physical therapy 19(34.5%), school physical therapy 13(23.6%). Therefore it is necessary to have home and visiting physical therapy as for the elderly and disabled person. 3. As for the qualification of Home and Visiting physical therapist, both PT and nurse groups showed as follows; take post graduation education program for home and visiting therapy after became PT : home physical therapist 108(70.1%), visiting physical therapist 106(68.8%). So it means education center or university can be developed post graduation program for home and visiting physical therapist. 4. As for the 'Needs' of school physical therapy, both groups of PT and nurse showed as follows; 'Needs' 142(92.2%), 'Needs superviser education program' 148(96.1%), in PT group showed 'I will participate of education program' 92(92.9%). 5. As for the present states of research papers or report of home, visiting, and school physical therapy was as follows; the 103 papers for 8 fields about' the needs of home and visiting physical therapy' from 1991 to 2011, the 13 papers for 2 fields about school physical therapy from 2001 to 2011, so total papers were 114 articles.

결혼이주여성 대상 건강정보 웹사이트 분석 (A Study on Health Information Websites for Migrant Marriage Women)

  • 이 연구에서는 결혼이주여성을 위한 건강정보 웹사이트를 구축하는데 있어서 효과적인 방안을 모색하고자 하였다. 이를 위하여 이들에게 유용한 건강정보의 구조와 내용이 어떠한지 파악하고자 하였으며, 국내에서 결혼이주여성을 위해 정보를 제공하고 있는 웹사이트의 구조적 특성을 분석하고, 해당 웹사이트에서 제공하고 있는 건강정보를 대상으로 그 내용과 특징을 파악하고자 하였다. 그리고 이에 따른 문제점 도출과 분석을 통해, 결혼이주여성의 정보요구를 충족시키는, 결혼이주여성을 위한 인터넷 건강정보 제공의 개선방향을 모색하고자 하였다.

난임 여성의 간호 요구 측정 도구 개발 및 타당도 검정 (Nursing needs assessment scale for women with infertility: development and validation)

  • Purpose: High-quality nursing care must be provided for women with infertility, and their nursing needs must be identified. Although scales have been developed to assess infertility-related stress, quality of life, and psychosocial status, there is a lack of scales that assess the nursing needs of women with infertility. The purpose of this study was to develop a needs assessment scale for nursing care in women with infertility and to verify its reliability and validity. Methods: The 250 subjects in this study were women with infertility recruited from four hospitals. The scale was developed following the framework of DeVellis, through a literature review, in-depth interviews, development of preliminary items, verification of content validity, development of secondary items, verification of construct validity, and extraction of the final items. Date were analyzed using item analysis, factor analysis, confirmatory factor analysis, Pearson correlation coefficients, and Cronbach's alpha. Reliability was tested using Cronbach's alpha, and validity was evaluated using item analysis, exploratory factor analysis, and criterion validity. Results: The final version of the nursing needs assessment scale for woman with infertility consisted of 18 items. Four factors (physical and psychological nursing needs, needs for information regarding treatment, needs for infertility-related understanding and concern, and supportive needs) explained 66.0% of the total variance. Cronbach's alpha was .92 for the overall instrument and ranged from .88 to .91 for the subscales. Conclusion: These results suggest that this needs assessment scale for nursing care in women with infertility demonstrated acceptable validity and reliability and contained items suitable for assessing the level of nursing care needed by women with infertility.

Prioritizing the Preferences of Iranian Cancer Patients Regarding Acquisition of Health Information: Strategy for Patient Education

  • Recognizing cancer patients' preferences to obtain health information can help improve and reform the methods of communicating and providing proper services and consequently lead to effective patient education. The present cross-sectional study to prioritize the preferences of cancer patients regarding the acquisition of health informationwas conducted on cancer patients referred to hospitals affiliated to Semnan University of Medical Sciences in 2015. An anonymous self-administered questionnaire was developed. In the field of side effects of medications, 50 (46.7%) reported knowing about weight change, in the area of achieving relative health, 62(57.9%) announced awareness about diet, and 45 (42.1%) reported physical complications as a first regarding information needs. In the area of obtaining information, 50 (46.7%) tended to take their information through means outside of the hospital setting. These results can help with design of clinical information systems, as they inform the most relevant and useful coverage designed for cancer patients. Providing useful information through healthcare providers, the media and clinical information systems can act as a major source of social support for cancer patients.

문헌분석을 통해서 도출한 공공도서관 소비자건강정보(CHI) 서비스 연구 (A Critical Analysis of Literature for Consumer Health Information Services in Public Libraries)

  • 인터넷사용량의 증가, 건강한 삶에 대한 관심이 증가함에 따라, 공공도서관에서의 소비자건강정보(Consumer Health Information: CHI) 요구가 증가하고 있다. 그럼에도 불구하고, 국내의 경우 공공도서관에서 제공할 수 있는 CHI 서비스에 대한 연구는 미비하다. 본 연구에서는 CHI와 관련한 공공도서관의 역할, 공공도서관 사서(소비자건강사서)의 역할, 그리고 공공도서관에서 제공하는 CHI 서비스 내용 등을 규명하고자 문헌분석을 실시하였다. 본 연구에서 도출된 공공도서관에서 제공해야 할 서비스로는 1) CHI 장서 및 정보원 개발, 2) CHI 참고서비스, 3) CHI 이용자 교육, 4) CHI 취약 계층에 대한 서비스, 5) CHI 아웃 리치 프로그램 개발, 6) CHI 협력체계구축 등이며, 각각의 서비스 개발에 대한 현황과 관련 사례를 제시했다. 비록 본 연구에서는 미국 및 국외 나라들의 서비스를 위주로 조사했지만, 연구 결과는 국내 공공도서관의 CHI 서비스 제공의 중요성을 강조하고, 실제 서비스를 개발하는 중요한 가이드라인이 될 수 있을 것이다.

중년기 여성의 폐경에 대한 교육요구도, 지식과 태도에 대한 연구 (A Study of Educational Need, Attitude and Knowledge toward Menopause in Middle-aged Women)

  • Purpose: The purpose of this study was to investigate educational needs, knowledge and attitude towards menopause in middle-aged women Method: The research design was a descriptive study. The subjects of this study were 231 middle-aged women. Data was collected from a structured questionnaire from October 2 to November 30, 2007. Result: The score of educational needs towards menopause was relatively high at 3.83/5. The score of educational needs related to 'Osteoporosis'(3.99 score), 'Cardiovascular disease'(3.91 score), and 'Hormone replacement therapy'(3.87 score) was relatively high. The score of know ledge towards menopause was relatively low at 17.28/30. The score of attitude towards menopause was neutral at 2.3/4. The relationship between knowledge and attitude towards menopause revealed a negative significant correlation(r=-.194, p=.003). The relationship between knowledge and educational needs towards menopause revealed a positive significant correlation(r=0.275, p<.0001). The degree of educational needs of women before menopause is higher than in menopausal women. Conclusion: There is a need to develop a continuing educational program according to age and menopause condition. Specialists(Nurses) must give information about menopause for a positive attitude toward menopause.

척추질환자의 의료기관 정보탐색행태와 탐색성과 (Information Searching Behavior for Medical Institutions of Spine Patients and Searching Outcomes)

  • Background: Information asymmetry between physicians and patients is one of the most unique characteristics of health care. But as consumerism spreads in health care sector, health care consumers are searching comparative information about quality and cost of providers from many information sources. Providing comparative information to health care consumers not only makes consumers choose hospital rationally, but also invigorates the health care market by providers' competition. However there are few studies regarding information searching behavior of health care consumers, then this study is carried out. Methods: The purpose of this study is to understand the information searching behavior of health care consumers based on their characteristics and the types of medical institutions. For this purpose, 313 spinal patients' data of 11 medical institutions (university hospital, spine specialized hospital, clinic) located in Seoul were collected by self-administered surveys. Results: The results of this study are as follows: 1) t-test/analysis of variance analysis showed that according to various characteristics of health care consumers and the types of medical institutions, the level of information searching of each source and the amount of information searching and searching outcomes are statistically different. 2) Regression analysis showed that influence on searching outcomes are statistically different according to the level of information searching of each source and searching content and the amount of information searching has positive effects on searching outcomes. Conclusion: The significance of this study is to provide empirical basis for establishment of health care policy reflecting information needs and preference of health care consumers.

고령층의 자가건강관리 역량강화를 위한 인터넷 건강정보 활용 실태 (The Use of the Internet Health Information for the Elderly)

  • Purpose: The number of people using the internet to meet their health information needs is growing. However, little is known about the characteristics of the senior internet health information seeker. The purpose of this study was to identify the socio-demographic and health-related characteristics, as well as the internet health information seeking behaviors and attitudes in the elderly. Method: Data was collected from 385 elderly subjects(aged 55 years or older) and living in Seoul and Gyeonggi. We used questionnaires developed by researchers in this study. Data was analyzed using the following methods: descriptive statistics, t-test, $X^2$-test and Fisher's exact test. All statistical tests were performed using the SAS version 9.13. Results: The elderly using the internet to seek health information had relatively good subjective health and were living with a spouse. In general, the purpose of internet use was to search for information, and mainly used at home. The search in the disease management area was higher than the frequency of searches for health promotion. The elderly showed a generally positive attitude regarding health information. Conclusion: It is needed to increase the use of the Internet for finding health information and promote accessibility for internet health information in the elderly.
