의료용 서비스 로봇 개발에 관한 연구 (A Study on the Development of Medical Service Robot)

  • 본 연구에서는 RFID(Radio Frequency Identification)와 HL7(Health Level 7) 프로토콜을 이용하여 병원을 내원한 환자의 접수를 보다 신속하고 효율적으로 처리할 수 있는 시스템을 설계하였다. 본 논문에서 설계한 시스템을 사용하면 접수뿐만 아니라 병원 내에서의 환자 정보의 활용이 가능하며 이는 병원 내에서 중복적인 서류 처리나 환자 정보를 얻기 위한 과정을 축소시킬 수 있다. 또한 HL7 프로토콜을 이용하여 병원 진료 과정을 고려한 HL7 메시지 구조를 설계하고 적용하였다. 본 논문에서는 서비스 로봇을 이용하여 환자 접수 기능을 구현하였으며, 향후과제로 진료 시에 필요한 혈압 및 체온 등 생체 데이터를 로봇을 이용하여 취득할 수 있는 기능을 추가 하는 것이다.

병원의 보건교육 활성화 방안에 관한 고찰 -지역사회와 병원의 연계방안을 중심으로- (A Study on Improvement of Health Education in Hospitals)

  • The health education in hospitals has many problems including lack of the division specialized in health education, lack of implementation system of health education, insufficient professional health education and insufficient health education materials. Despite these restraints, hospitals should reinforce health education program as a part of active response centered on disease prevention and health promotion targeting healthy people who have potential health risk before the onset of disease, rather than passive approach emphasizing the treatment. Accordingly, health education team should be organized in a hospital, in which health educators can provide the following education services. First of all, hospitals themselves should provide active community services in collaboration with community organizations. In addition, multi-disciplinary approach in cooperation with schools, work places should be reinforced to establish the comprehensive health education system. The establishment of medical service referral system with other medical institutions and the linkage system for medical information exchange are also needed. The utilization of education materials obtained through these system should be open to community residents as well as patients. Finally, medical staff working in hospitals should try to provide the high-quality health education that is as high as the level of medical services. The high-quality health education is possible when its level is based on one required by community residents and the county rather than the international or arbitrary standard.

보건의료정보의 법적 보호와 열람.교부 (A Study on Legal Protection, Inspection and Delivery of the Copies of Health & Medical Data)

  • In a broad term, health and medical data means all patient information that has been generated or circulated in government health and medical policies, such as medical research and public health, and all sorts of health and medical fields as well as patients' personal data, referred as medical data (filled out as medical record forms) by medical institutions. The kinds of health and medical data in medical records are prescribed by Articles on required medical data and the terms of recordkeeping in the Enforcement Decree of the Medical Service Act. As EMR, OCS, LIS, telemedicine and u-health emerges, sharing and protecting digital health and medical data is at issue in these days. At medical institutions, health and medical data, such as medical records, is classified as "sensitive information" and thus is protected strictly. However, due to the circulative property of information, health and medical data can be public as well as being private. The legal grounds of health and medical data as such are based on the right to informational self-determination, which is one of the fundamental rights derived from the Constitution. In there, patients' rights to refuse the collection of information, to control recordkeeping (to demand access, correction or deletion) and to control using and sharing of information are rooted. In any processing of health and medical data, such as generating, recording, storing, using or disposing, privacy can be violated in many ways, including the leakage, forgery, falsification or abuse of information. That is why laws, such as the Medical Service Act and the Personal Data Protection Law, and the Guideline for Protection of Personal Data at Medical Institutions (by the Ministry of Health and Welfare) provide for technical, physical, administrative and legal safeguards on those who handle personal data (health and medical information-processing personnel and medical institutions). The Personal Data Protection Law provides for the collection, use and sharing of personal data, and the regulation thereon, the disposal of information, the means of receiving consent, and the regulation of processing of personal data. On the contrary, health and medical data can be inspected or delivered of the copies, based on the principle of restriction on fundamental rights prescribed by the Constitution. For instance, Article 21(Access to Record) of the Medical Service Act, and the Personal Data Protection Law prescribe self-disclosure, the release of information by family members or by laws, the exchange of medical data due to patient transfer, the secondary use of medical data, such as medical research, and the release of information and the release of information required by the Personal Data Protection Law.

  • LEX(Lifelong EHR based on XML)시스템은 이질적인 병원정보시스템을 가진 병원들이 HL7 메시지를 생성하고, 이를 통해 의료정보를 효율적으로 공유하고 교환할 수 있는 중앙의 임상데이터베이스(Central Clinical DataBase/CCDB)를 구축하는 XML 기반의 의료정보통합 프래임 워크이다. 본 논문에서는 기존의 병원정보시스템에 독립적인 HL7 메시지 서버 연동방식을 제안하여 HL7 메시지를 생성하고, 생성된 메시지를 효율적으로 저장하고 검색하기 위한 환자중심의 XML데이터베이스 스키마를 제안한다. 또한 HL7 메시지를 새로운 구조의 XML문서로 변환하기 위한 DTD를 제안함으로써 기존의 방식보다 저장공간을 절약하고, 효율적인 환자중심의 검색을 지원하는 방안을 제시한다.

순환기질환 감시체계 (Circulatory Disease Surveillance System in Korea)

  • The purpose of establishing the circulatory disease surveillance system in Korea is to ensure that the problems of circulatory disease importance are being monitored efficiently and effectively. The goals of circulatory disease surveillance system are to monitor the epidemiological trends of circulatory disease and to evaluate the outcome of health activity for controlling circulatory diseases. Surveillance system are being updated to achieve the needs for the integration of the surveillance and information system, the establishment of data standards, the electronic exchange of data, and changes in the goals of circulatory disease surveillance system to facilitate the response of this system to manage the national health problem effectively. This article provides the target diseases and determinant indicators to be monitored, structure of circulatory disease surveillance system, and many tasks and related activities that should be applied to this system.

  • Dr. Haruyuki Ogino, a member of the Japan Health Physics Society (JHPS), was awarded the 2021 Bo Lindell Medal by the International Commission on Radiological Protection (ICRP). To commemorate this, the "Joint JHPS-KARP-ARPS program for young radiation protection (RP) scientists to discuss the future of RP" was organized via a web meeting system. First, Dr. Ogino gave a lecture, and then young researchers selected from each academic society made presentations on the future of RP. After the presentations by the three researchers, a free talk was held-young researcher groups of each country being active is a great opportunity to collaborate and exchange information. It was emphasized that the low awareness of knowledge related to radiation has been common to all of the participating countries. Thus, it is necessary to utilize communication via web technology, as done for this program, effectively. One of the biggest advantages for Asia and Oceania is that we do not have a significant time difference. The round-table discussion was concluded by expressing the hope of active exchange and development of young researchers in the future.

재가노인의 포괄적 건강문제 평가와 관리방안 (Management Strategy by Evaluation on Comprehensive Health Problem in the Community-dwelling Elderly of Korea)

  • Purpose: The aim of this study was to define the health problem in the community-dwelling elderly of Korea and to compare differences of CAPs(Client Assessment Protocols) by characteristics. Method: Data was collected by visiting nurse from 556 elderly over 65 years in selected metropolitan areas. To evaluate the functional state of elderly in the community, I used "RAI-MDS HC; Residental Assessment Instrument Minimum Data Set-Home Care(2.0 version)" and established information exchange system among resources, by developing the data into a computer program. Results: The health problem of 'preventive health measures' was the largest(99.6%), and then 'health promotion(85.3%)', 'visual function(75.5%)', 'psychologic drug(68.9%)', 'pain(68.5%)', ‘social function(59.2%)', 'communication disorders(56.2%)', 'environmental assessment(53.2%)', 'depression & anxiety(46.9%)', 'oral health(43.4%)' followed. The number of health problems was average 10.16 in the community-dwelling elderly. Conclusion: The results suggest the need to emphasize the importance of assessment of the health problem of the elderly. We can apply it in the distribution of community resources and the development of service providing programs by figure out the health problem and resource in need for the elderly in the community.

FHIR를 이용한 심전도 자동 노티 연구 (Using FHIR, ECG Automatic notification Research)

  • HL7의 차세대 버전인 FHIR는 의료 환경에서 메시지 교환의 표준으로 이벤트 발생시점부터 데이터 교환을 손쉽게 할 수 있도록 지원하는 프로토콜이다. 응급실 등 심전도 기기로부터 환자의 자동 판독 결과를 FHIR 메시지로 생성하고, 생성된 메시지를 EMR 등 병원정보시스템에 전달하고 스마트 폰을 통해 해당 결과를 실시간으로 알려 줄 수 있음으로 다양한 의료기관에서 사용 될 것으로 생각한다. 또한, 향후 SMART on FHIR로 확대되어, 순환기내과, 흉부외과 병동에서 실시간 원격모니터링에도 활용 될 것으로 생각한다.

개인화된 건강 데이터 처리를 위한 2-Step 개방형 시스템의 설계 (Design of Two-Step Open System for Personalized Health Data Access)

  • ICT 힐링플랫폼의 목적은 만성질환 예방이며 생체신호 및 생활습관 등의 정보를 기반으로 한 질환조기 경보를 목표로 한다. 본 논문에서 제안하는 개인화된 건강 데이터 접근을 위한 2-Step 개방형 시스템(TOS)은 ICT 힐링플랫폼에서의 개인 건강관련 데이터 공급자와 서비스 공급자 사이를 연결하여 개인화된 건강 데이터를 중계하는 소프트웨어 엔진이다. 제안시스템은 개인건강 문서 저장부인 Inbound 모듈과 외부기관에 조회 서비스를 제공하기 위한 Outbound 모듈로 2step으로 분리하여 운용한다. 이를 위해 Step간의 데이터 교환 정의를 위해 개인화 편집 가능한 Manifest 개념을 제안한다. 설계된 시스템은 여러 건강 관련 서비스 기관들(병원, 피트니스센터, 건강검진센터, 개인건강장치 등)에 산재되어 있는 개인 건강 정보를 개인 주도하에 수집하고 개방화하는데 참조모델로 활용할 수 있다.

전통 약물의 국제 교류에 관한 소고(小考) (A Study on International Exchange of Traditional Herb Medicine)

  • Traditional medicine has been exchanged constantly from prehistoric times up to the present. As the global market trade on traditional medicine increases, people now emphasized the importance of traditional medicine. Previously, knowledge about herbal medicines are taught or learned indirectly. Most of it was learned through medical books. But in these contemporary times, herbal medicine's knowledge is shared through journals, congress and some other events in where traditional medicine's information are shared. In the international congress gathering; traditional medicine's experts from many countries shared some additional knowledge. First, "an attitude to medicine that emphasizes on Naturalism". Second, "respect for experienced in traditional medicine". Third, "respect for locality on traditional medicine". Fourth, "a protection for domestic traditional medicine industry" Fifth, "acceptance of traditional medicine from other countries according to domestic health care system".
