A Comparison of Oral Health Behavior and Oral Health Outcomes between Cooperative and Non-Cooperative Groups following Implementation of an Oral Health Care Program (치위생 과정에 근거한 구강예방프로그램 적용 후 협조군과 비협조군 간 구강건강상태 및 행동 비교)

  • The purpose of this study was to reveal analyze the relationship between status of participation in an oral health care program and oral health outcomes among patients in Korea, and to evaluate the results to provide evidence regarding the feasibility of widespread implementation of the program. Patients were designated as either cooperative or non-cooperative with the oral health care program and were assigned to each group accordingly. Modified dental hygiene process (M-DHP) of the oral healthcare program was modified to form the dental hygiene process. The study included 48 patients at a dental clinic in Busan, Korea. Questionnaires were used to collect information on oral health behavior (OHB), clinical examination was used to record bleeding on probing (BOP) and O'Leary index, and phase microscopy was used to identify microorganisms. Differences between groups were evaluated using repeated measures ANOVA. Our results showed that the group cooperative with the oral health care program showed greater improvement in OHB, BOP, and O'Leary index than the non-cooperative group. Second, patient satisfaction with the M-DHP was very high, particularly for content and the friendly nature of the staff. The cooperative group showed greater improvement in oral health than the non-cooperative group for all metrics. Our results suggest that this low-coste program, if implemented, would be actively accepted and utilized in dental clinics.

Effectiveness of oral health promotion on the oral health education in some high school students (일부 고등학교 학생들의 구강보건교육에 대한 구강건강증진 효과)

  • Objectives : This study is to examine the effectiveness in the oral health promotion through the oral health education for high school students. In adolescence, there seems to be the high incidences of dental caries, gingivitis and periodontal diseases. Concerning preventing all such outbreaks, this study focuses on applying the oral health education to their health education, and on analyzing its effectiveness. Methods : To verify its effectiveness, this research administered 85 students of both genders of a certain high school in Seoul during May-October 2010, followed by the evaluation of DMFT, S-PHP, and CPITN. Results : First, the oral health promotion through the education was statistically significant in the permanent effects of index (FT index). Second, the effect of S-PHP index gradually decreased in statistics from the first to the fifth session, but, after five months ever since the education, it seemed relatively higher in the next sessions. Third, the changing in the level of CPITN decreased continuously as the students received the education repeatedly, but there is no significant difference in statistics for each session. Conclusions : For the oral health promotion in the high school students, the education ought to be focused on the behavior-change objectives rather than on the knowledge-based objectives. The repeated education for tooth-brushing instruction should be stressed for preventing from returning to the old bad behaviors.

  • Objectives: This study is prepared to understand the past of the KJHSM (Korean Journal of Health Service Management), to analyze the papers in the KJHSM, to identify the research trends during 2007~2018, and to help the future development of research. Methods: The data for the objectives was collected major KCI index and key words of the articles in KCI web site. Results: The KJHSM has good performaces that KCI IF(2 year) is 0.98, Centrality Index(3 Year) is 1.188, Immediacy index is 0.19, and Self Citation Rate(2 Year) is 25.81. Conclusions: In order to improve the academic quantity and quality in the KSHSM, we will need the vision to corporate and participate through more good research results and performance with desirable ethic value.

Regional Variation of EQ-5D Index and Related Factors in Community Health Survey: Major Role of Psychosocial Factors in Korea (지역사회건강조사에서 EQ-5D index의 지역간 변이와 관련 요인: 사회심리적 요인의 중요성)

  • Objectives: As an ecological study, this study was performed to identify the community-level variation of health related quality-of-life (HRQOL), and to explore the factors that explain the variation, using 2017 Korean Community Health Survey (KCHS) data. Methods: Community health indicators of KCHS, which are correlated with the EQ-5D index of Si-gun-gu districts, were selected as independent variables. Multiple linear regression model was used to derive factors that explain regional variations in the EQ-5D index. Results: The EQ-5D index variation in 229 districts nationwide was 1.1 times for extremal quotient (EQ) and 1.0 for coefficient of variance (CV). The Si-gun-gu districts with the EQ-5D index in the lower 25% were more distributed in the province (27.7%) than in the metropolitan area (20.3%). As a result of multiple linear regression analysis, the depressed mood experience rate, perceived stress rate, suicide ideation rate, and physician diagnosed arthritis rate were derived as major factors of the variation. Conclusions: In order to reduce the gap in HRQOL between the districts, the priority of local health policies should be placed on the above factors including psychosocial factors.

Development of a Cardiovascular Index That Estimates Cardiovascular Health Degree by Survey and Noninvasive Examination (설문과 비침습적 검사를 통하여 심혈관계 건강 정도를 알려주는 심혈관지수의 개발)

  • An index was developed that estimates cardiovascular health degree with easily available physiological information such as survey and noninvasive measurement. The survey score was calculated by utilizing questions related to personal disease history, self-feeling, and management state. The measurement score was calculated using physiological parameters such as blood pressure, accelerated plethysmograph(APG), and heart rate variability(HRV), and augmentation index(AI). In order to evaluate effectiveness of the cardiovascular index and modify weighting factors used in each item, a clinical trial was done in a general hospital. The cardiovascular index showed a clear correlation of 0.685 with the doctor's score on the cardiovascular health degree. The correlation between the self-estimated score and doctor's score was as low as 0.217. The large gap between these two scores demonstrated necessity of more objective tools like the cardiovascular index. The cardiovascular score showed a significant difference between normal persons and patients suffering hypertension or diabetes. (p=0.000).

Comparison Study of Body Weight Perception and Physiological Index by Body Mass Index Level in Young Adult Women (초기 성인기 여성의 체중에 따른 주관적 체격인식과 생리지표 비교)

  • Objectives: The purpose of this study was to assess the characteristics of body weight perception and physiological index in young adult women. Methods: Subjects were 283 Korea women. Data was from the 2005 Korean National Health and Survey. Three groups-low normal and overweight-were based on Body mass index(BMI). General characteristics, health status perception, weight control behavior and physiological index such as BMI, total cholesterol, high-density cholesterol (HDL), low-density cholesterol (LDL), triglyceride, systolic and diastolic blood pressure, and waist circumference were compared. Data were analyzed by t-test, $x^2$-test and ANOVA using SPSS program. Results: Of the normal weight group, 28.7% of subjects perceived themselves as overweight. Of the low weight group, 30.3% regarded their weight as normal weight. Of the overweight subjects, weight control was attempted by exercise (50.1%) and food reduction (77.1%). Significant group-related differences were evident in HDL, LDL, triglyceride, systolic blood pressure and waist circumference among three groups. Conclusions: Perception and attitude regarding body weight can be inaccurate. A weight control program should consider the relation of physiological index and weight based on BMI.

Imbalance in Cardiovascular Surgery Medical Service Use Between Regions

  • Background: This study uses the relevance index to understand the condition of regional medical service use for cardiovascular surgery and to identify the medical service use imbalance between regions. Methods: This study calculated the relevance index of 16 metropolitan cities and provinces using resident registration address data from the Ministry of Government Administration and Home Affairs and the 2010-2014 health insurance, medical care assistance, and medical benefits claims data from the Health Insurance Review and Assessment Service. We identified developments over the 5-year time period and analyzed the level of regional imbalance regarding cardiovascular surgery through the relative comparison of relevance indexes between cardiovascular and other types of surgery. Results: The relevance index was high in large cities such as Seoul, Daegu, and Gwangju, but low in regions that were geographically far from the capital area, such as the Gangwon and Jeju areas. Relevance indexes also fell as the years passed. Cardiovascular surgery has a relatively low relevance index compared to key types of surgery of other fields, such as neurosurgery and colorectal surgery. Conclusion: This study identified medical service use imbalance between regions for cardiovascular surgery. Results of this study demonstrate the need for political intervention to enhance the accessibility of necessary special treatment, such as cardiovascular surgery.

Association between Oral Health Status and Perceived General Health (EuroQol-5D) (구강건강상태와 감지 건강상태(EuroQol-5D)와의 연관성)

  • The purpose of this study was to examine association between oral health status and perceived general health symptom. We analyzed 14,231 subjects who participated in Korea National Health and Nutrition Examination Survey (2007~2009). All individuals were examined by a questionnaire about socioeconomic history, smoking and drinking habit, the frequency of daily tooth brushing, the presence of regular dental visit, and EuroQol-5D. Dental survey was conducted to find the decayed, missing, and filled teeth (DMFT) index and community periodontal index. Subjects with perceived problem with mobility had higher DMFT index (odds ratio, 1.18, p<0.001). Subjects with with perceived problem with pain/discomfort had higher DMFT index (odds ratio, 1.16, p<0.001). Self-rating general health symptom was not associated with periodontitis (p>0.05). Perceived general health was associated with DMFT index. It is recommendable that we can use the perceived general health to predict oral health status.

The Relationship Between Dietary Pattern and the Community Periodontal Index in Elderly People Living Alone in Rural Areas (농촌지역 독거노인의 식이패턴과 지역사회치주지수와의 관련성)

  • Objectives : The purpose of this study was to investigate the relationship between dietary pattern and the Community Periodontal Index in elderly people who live alone in rural areas and to provide necessary data to strengthen nutrition education about the oral health of elderly people who live alone. Methods : Dental examinations and questionnaires were conducted with 380 elderly people who live alone in rural areas of Gyeonggi-do. Results : The Community Periodontal Index was higher when the elderly people who live alone had a low intake of vegetables and fruits, a high intake of sugars, a low number of breakfasts, a high frequency of overeating and a high frequency of instant ingestion. Conclusions : It is necessary to provide nutritional management services for the elderly people who live alone in rural areas and to provide preventive centered comprehensive oral care.

Effects of Oral Health Knowledge and Practices on Decayed, Missing and Filled Teeth Index and Quality of Life -Path Analysis Using Structural Equation Modeling- (구강보건 지식과 실천이 우식경험치아와 삶의 질에 미치는 영향 -구조방정식을 이용한 경로분석을 중심으로-)

  • This study aims to measure oral health knowledge and practice level among dental clinic patients, and examine their relationships with decayed, missing and filled teeth (DMF) index and the oral health-related quality of life (QOL). Empirical data have been collected from 311 patients through structured questionnaires and dental examinations conducted in Busan area. According to the analysis of causal relationship, oral health knowledge turned out to have a direct effect on oral health practices, which exerted both direct and indirect effects on DMF index and oral health-related QOL. While DMF index directly affected the oral health-related QOL, oral health knowledge proved to have only an indirect effect on DMF index and oral health-related QOL, mediated by oral health practices. Considering all these findings, effective oral health education and other intervention programs should focus on motivating people to participate in the active practices of desirable oral health behaviors, which will lead to the prevention of oral diseases and the subsequent improvement in the QOL.
