Effect of the size of the bony access window and the collagen barrier over the window in sinus floor elevation: a preclinical investigation in a rabbit sinus model

  • Purpose: The aim of this study was to investigate the effect of (1) the size of the bony access window and (2) collagen membrane coverage over the window in sinus floor elevation in a rabbit sinus model. Methods: Small bony access windows (SW; ø 2.8 mm) were made in 6 rabbits and large windows (LW; ø 6 mm) in 6 other rabbits. Both sinuses in each rabbit were allocated to groups with or without coverage of a collagen membrane (CM) on the window, resulting in 4 groups: SW, LW, SW+CM, and LW+CM. After 4 weeks of healing, micro-computed tomographic, histologic, and histomorphometric analyses were performed. Results: Bony healing in the window area was incomplete in all groups, but most bone graft particles were well confined in the augmented cavity. Histologically, the pattern of new bone formation was similar in all groups. Histomorphometrically, the percentage of newly formed bone was greater in the groups with CM than in the groups without CM, and in the groups with SW than in the groups with LW (12.92%±6.40% in the SW+CM group, 4.21%±7.73% in the SW group, 10.45%±4.81% in the LW+CM group, 11.77%±3.83% in the LW group). The above differences were not statistically significant (P>0.05). Conclusions: The combination of a small bony access window and the use of a collagen membrane over the window favored new bone formation compared to other groups, but this result should be further investigated due to the limitations of the present animal model.

Value of Fecal Calprotectin Measurement During the Initial Period of Therapeutic Anti-Tubercular Trial

  • Background/Aims: The diagnosis of intestinal tuberculosis (ITB) is often challenging. Therapeutic anti-tubercular trial (TATT) is sometimes used for the diagnosis of ITB. We aimed to evaluate the changing pattern of fecal calprotectin (FC) levels during TATT in patients with ITB. Methods: A retrospective review was performed on the data of 39 patients who underwent TATT between September 2015 and November 2018 in five university hospitals in Daegu, South Korea. The analysis was performed for 33 patients with serial FC measurement reports. Results: The mean age of the participants was 48.8 years. The final diagnosis of ITB was confirmed in 30 patients based on complete mucosal healing on follow-up colonoscopy performed after 2 months of TATT. Before starting TATT, the mean FC level of the ITB patients was 170.2 ㎍/g (range, 11.5-646.5). It dropped to 25.4 ㎍/g (range, 11.5-75.3) and then 23.3 ㎍/g (range, 11.5-172.2) after one and two months of TATT, respectively. The difference in mean FC before and one month after TATT was statistically significant (p<0.001), and FC levels decreased to below 100 ㎍/g in all patients after one month of TATT. Conclusions: All ITB patients showed FC decline after only 1 month of TATT, and this finding correlated with complete mucosal healing in the follow-up colonoscopy after 2 months of TATT.

치근단 병소를 가진 영구치의 보존적 치수 치료 (CONSERVATIVE ENDODONTIC TREATMENT OF PERMANENT TEETH WITH PERIAPICAL LESIONS : CASE REPORTS)

  • 최근 미성숙 영구치의 치근단 유두에서 존재하는 미분화 줄기세포가 새로 발견되었고 이 줄기세포들은 치근 상아질의 형성에 관여하는 법랑모세포의 근원으로 보여진다. 미성숙 영구치를 다룰때 이 줄기세포들의 보존은 치근의 지속적인 형성을 완성시키게 한다. 따라서 치근단 염증을 동반한 미성숙 영구치에서 치근단의 최소한의 침습으로 임상 및 방사선적 치유 양상을 관찰할 수 있었기에 보고하고자 한다. 본 증례는 조선대학교 치과대학병원 소아치과에 내원한 만 10세 남아(하악 우측 소구치), 만 8세 여아(상악 좌측 소구치)에서 임상 및 방사선 검사결과 치수괴사와 치근단 농양으로 진단된 영구치에서 치근단부위의 기구조작을 제한하고 MTA(Mineral Trioxide Aggregate)를 치수강내에 적용하였다. 정기적으로 관찰한 결과 치근단 농양의 치유와 치근 발육 및 성장의 양상이 관찰되었다. 치수괴사와 치근단 농양을 가진 영구치에서 치근단부위의 치수 및 치유두를 보호함으로써 관찰기간동안 양호한 예후를 보였으며, 이는 어린환자의 미성숙영구치에서 보존적 치수치료의 시도에 대한 보다 많은 임상적 검증 및 장기간의 고찰이 필요할 것이다.

성견의 실험적 상피하 결합조직 이식시의 치주조직의 치유 (Periodontal Wound Healing of the Experimental Subepithelial Connective Tissue Graft in Dogs)

  • Several methods have been used for regeneration of tissue lost by periodontal disease. Subepithelial connective tissue graft technique, one of the technniques of mucogingival surgery, is used for the regeneration in esthetic problems such as recession, and denuded root coverage. This study is performed to evaluate the healing process and the regeneration and reattachment of periodontal tissue, including the reconstruction of junctional epithelium, and connective tissue. Alveolar defects in five adult dogs were treated with periodontal surgery and were attained by removing the marginal alveolar bone by $4{\time}3mm$ from CEJ in the labial side of incisors, and root surfaces were planed. The experimental sites were divided into two groups as follows. 1. root planing alone(control group) 2. with connective tissue graft (Experimental Group) In the two groups flaps were positioned and sutured tightly, the healing processes were observed and were histologically compared with each other after 2days, 4days, 1week, 2weeks, 4weeks. The results were obtained as follows : 1. In the two groups blood clots were observed as early as 2 and 4 days, and were resorbed at 1 week. 2. In the two groups moderate inflammation was observed as early as 2 and 4 days, decreased at 1 and 2 weeks, and disappeared at 4 weeks. 3. Junctional Epithelium migration was more significant in the control group, and was restrained by graft materials in the experimental group. 4. Features of connective tissue fiber attachment partially showed the parallel pattern in the two groups from 2 weeks, and entirely from 4weeks. 5. Anastomosis, between graft and connective tissue, appeared from 4 days in the experimental group and the border between them was not discriminated at 4weeks.

치아재식시 골형태형성단백이 치주조직 치유에 미치는 영향 (EFFECTS OF BONE MORPHOGENETIC PROTEIN ON THE HEALING OF PERIODONTIUM AFTER TOOTH REPLANTATION OF THE RAT)

  • The purpose of this study is to evaluate the influences of the bone morphogenetic protein (BMP) on the healing of periodontal ligament and alveolar bone after replantation of tooth, and to examine the possibility of its clinical application. 45 Sprague Dawley rats weighted about 100 gram were divided into 3 experimental groups by different dose of BMP. All the upper right and left 1st molar were extracted after 5 days feeding of 0.4% ${\beta}$-aminopropionitrile, and right molar were used as experimental group and left molar were used as control group. The root surface of experimental molar were treated with 25,50 and l00ng/ml of human recombinant Bone morphogenetic protein-4 (rh-BMP-4) with micropipet, and 1M Sodium hypochloride were used on control root surface. All the experimental animals were sacrificed as 1, 2, 4, 7 and 14 days after autoreplantation of upper 1st molar into their own position. The maxilla were disected included both side of 1st molar. The collected tissue were processed from demineralization to paraffin embeding as usual procedure, and the specimens were prepared with Hematoxylin-Eosin stain for the light microscopic evaluation. The results of this study were as follows : 1. There was no significant differences between control and experimental site on 1 and 2 days after replantation of tooth. In the case of 4th days, the evidence of tissue regeneration were observed on experimental site to compare the controls. New osteoid were revealed on high concentration of BMP at 7 days after replantation, and it became more obvious at 14 days, 2. The effect of the rh-BMP-4 coated on root surface was revealed slight influences for the prolifertion of cells of periodontium and tissue regeneration as dose-dependent pattern. 3. Bony ankylosis was observed between alveolar bone and root surface due to the remarkable amount of osteoid formation on the 14 days after replantation of root. In the conclusion, it was suggested that topical application of the rhBMP-4 on the root surface has influence on the periodontal ligament and alveolar bone. The application method of BMP on the root should be designed with calculation of proper concentration.

개인화된 건강 데이터의 대량 처리 모니터링을 위한 메시지 모델 및 동적 버퍼 할당 설계 (Design of Dynamic Buffer Assignment and Message model for Large-scale Process Monitoring of Personalized Health Data)

  • ICT 힐링플랫폼은 만성질환 예방을 목적으로 하며 개인의 생체신호 및 생황습관 등의 정보에 기반을 둔 질환 조기 경보를 목표로 한다. 이를 위한 2-step 개방형 시스템(TOS)에는 힐링플랫폼과 개인건강데이터 저장소간의 중계가 설계되었으며 데이터 처리과정을 실시간으로 전송(모니터링)하기 위한 대량 커넥션 기반의 publish/subscribe(pub/sub) 서비스가 고려되었다. 그러나 TOS pub/sub의 초기 설계에서는 커넥션 메시지를 deflate 알고리즘으로 인코딩하기 위해, 커넥션의 유휴(idle) 여부 및 메시지의 종류에 상관없이 동일한 버퍼를 할당한다. 본 논문의 동적 버퍼 할당은 다음과 수행된다. 우선 각 커넥션의 메시지 전송 유형을 큐잉하고, 각 큐는 tf-idf를 통해 특징(feature)추출 연산 후 벡터로 변환하여 k-means 클러스터에 입력하여 군집을 생성한다. 특정 군집으로 분류된 커넥션은 해당 군집의 자원 테이블에 따라 자원을 재할당 한다. 이때 각 군집의 센트로이드(centroid)는 해당 군집을 대표하는 큐잉 패턴을 사전에 선택하여 자원참조 테이블(버퍼 크기별 인코딩 효율)로 도출한다. 제안된 설계는 TOS의 인코딩 버퍼 자원을 네트워크 커넥션에 효율적으로 배분하기 위해, 군집 및 특징 연산을 위한 연산 자원과 네트워크 대역폭 간의 trade-off를 수행함으로써 TOS의 tps(단위 시간당 실시간 데이터 처리 모니터링 연결수)를 높이는데 활용할 수 있다.

Streptococcus mutans로부터 분리한 Mutan이 흰쥐의 골결손부 치유에 미치는 영향 (Effect of Mutan Isolated from Streptococcus mutans on the Healing of Bone Defect in Rat)

  • 본 연구는 외과적으로 형성된 흰쥐 하악골 결손부에 S.mutan에서 분리 정제된 mutan을 주사하고, ${\mu}CT$로 촬영하여 골결손부의 치유에 미치는 영향을 3차원 골 미세구조 지표로 평가하기 위하여 실시하였다. 흰쥐 치조골에 임계 크기 결손부인 1.5 mm 지름의 원형 결손부를 형성하여 대조군에는 동일량의 saline을 투여하고, 실험군으로 LPS와, mutan을 주사하여 골치유에 미치는 영향을 관찰하였다. 수술 후 4주에 희생하고 치유 결과를 3차원 미세구조의 형태 계측학적 지표값을 비교 분석하여 다음과 같은 결론을 얻었다. 실험동물의 체중변화는 총 12회 투여 기간 동안 일정한 증가를 나타냈다. 8회차부터 mutan 처리군은 대조군과 비교하여 유의한 차이를 보였으며(p<0.05), 9회차에는 mutan 처리군이 대조군과 LPS 처리군과 비교하여(p<0.05), 10~12회차에는 LPS 처리군과 비교하여 유의한 체중증가를 나타냈다(p<0.01). Mutan이 생체 내에서 하악의 골치유 효과를 알아보기 위해 ${\mu}CT$를 통해 형태계측학적 지표 분석 결과, 대조군에 비해 LPS 처리군에서 뚜렷한 골결손부 치유 지연 및 골흡수 양상을 보였으며, mutan 처리군 은 LPS의 작용보다 미약하지만 골치유를 지연시키는 것으로 확인되었다. ${\mu}CT$를 통해 골밀도 분석 결과, 대조군과 비교하여 LPS와 muatn 처리군에서 낮은 골밀도치를 나타냈고, mutan은 LPS 처리군보다 유의하게 높은 골밀도치를 보였다(p<0.01).

가토에서 관절원판 후조직 절단 및 교합고경 감소가 악관절에 미치는 영향 (THE EFFECTS OF INCISON OF RETRODISCAL TISSUE AND OCCLUSAL REDUCTION ON TEMPOROMANDIBULAR JOINT OF RABBIT)

  • In this study, effects of incision of retrodiscal tissue and unilateral occlusal reduction on temporomandibular joint of rabbit were investigated. Twenty-seven adult New Zealand White Rabbits, weighing over 3.5kg, were utilized in this study. Temporomandibular joint surgery was performed in left temporomandibular joint of 24 rabbits to displace disc anteriorly through incising the retrodiscal tissue 1-2mm posterior to the disc. They were divided into two groups : twelve were left untreated after surgery, occlusal reduction was performed on left posterior teeth every 2 weeks in the other twelve rabbits, The remaining three served as the control group. The sample were sacrificed by 8, 12, and 16 weeks after surgery. Histologic examinations were performed after sacrificing them. The results were as follows : 1. Histologic findings which were manifested by flat articular fossa, broad articular surface, generalized recession of articular cartilage and sclerosis of subchondral bone were observed. These findings were similar to internal derangement. 2. In the rabbits untreated after surgery, thin cartilagenous layer and necrotic tissue were observed in 8 weeks group, calcifying cartilagenous layer was observed in 12 weeks group, and cartilagenous layer on anterior portion was observed in 16 weeks group. So, it showed gradual healing pattern into the normal tissue except displaced disc. 3. Occlusal trauma after surgery resulted in generalized recession of upper and lower articular surface. Necrosis and vertical split on condylar process of mandible were observed in 8 weeks group. Osteoclasts, exposure of subchondral bone due to erosion on upper and lower articular surface, and degenerative changes on retrodiscal tissue were observed in 16 weeks group. So, it showed continuous prowess pattern of osteoarthrosis.

뮤지컬 배우의 배역 투사 양태와 치유 시스템 (Musical Actor's Projection Pattern and Treatment System)

  • 본 연구는 뮤지컬 배우의 공연 후 배역 투사와 그 후유증 양태를 파악하여 제도적인 치유 시스템을 모색하기 위해 실시되었다. 우리나라 정상급 남녀 뮤지컬 배우 22명을 상대로 심층 인터뷰를 하여 분석한 결과, 배우들은 대부분 공연 후 1개월 이상 장기간 배역 투사에 시달리며 우울증 등의 후유증으로 고통 받고 있으며, 치유도 전문가 상담을 회피한 채 자기진단에 따라 기분전환에 의존하고 있는 것으로 나타났다. 투사는 극중 주인공, 부정적 현실적 캐릭터 연기, 격한 감정을 표출할 때 특히 두드러졌다. 연구 결과는 배우들의 생생한 제언처럼 케이컬처 시대의 수준에 맞게 배우가 출연 전후 배우 친화적인 전문가의 상담 치료를 받도록 입법을 하고 재원을 조성해 민간에 배우전문 케어센터를 구축해야 한다는 점을 암시한다.
