The Influence of the Risk Factors and Nutritional Status on the Development of Pressure Sores for the Risk Patients in ICU (욕창발생위험요인과 영양상태가 중환자실 입원환자의 욕창발생에 미치는 영향 - 욕창발생위험집단을 중심으로 -)

  • Pressure sores are a serious concerns in that respect to increasing risk of medical complications and medical costs. Prevention and care of pressure sores is an essential area of nursing practice. The nurse at ICU should be more careful of maintaining the skin integrity of patients especially than at any other place. This study was conducted to determine if the risk facotrs of pressure sores and nutritional status of the patients at risk for pressure sores is related the occurance of pressure sore. The risk group refers the patients having the below 14 scores of the braden scale. The 100 subjects were recruited from the ICU ward at an university hospital in Choongnam. The parameters for nutritional status are the blood chemistry including plasma protein, albumin, hemoglobin and the anthropometric measurements consisting of weight, BMI, LBM, the proportion of body fat, body fluid and triceps skin fold using bioimpedence analizer and caliper. The results are as follows : 1. The subjects were 55 years and stayed 8 days on average. Of the 100 subjects, males were 61%, neurologic/neurosurgical diseases were 68% and the incidence of pressure sores was 17% mainly occuring within 3days after the admission. 2. The present paralysis(or paraplegia) and edema(arm, leg, trunk) were showed more significantly the subjects with pressure sores than those without pressure sores. 3. Regarding with the nutritional status, the subjects with pressure sores had significantly lower the weight, BMI, LBM, body fluid, albumin than the ones without pressure sores. This results were supported the reports of previous studies that the decreased weight and albumin could be the important predictors of pressure sores. Thereafter we should encourage these factors to be utilized in predicting pressure sores for a comprehensive assessment. Nurse should identify patients at risk of the development of pressure sores, assess their nutritional status and dietary intake at regular intervals.

Nonverbal Communication between Patients with Dementia and Their Nurses in an Elderly Care Institution (일개 노인요양기관에 입소한 치매노인과 간호사의 비언어적 커뮤니케이션 연구)

  • Purpose: The purpose of the study was to describe nonverbal behaviors of nurses in communication with patients with dementia in an elderly care institution in Korea. Methods: Conversation analysis was utilized to analyze the data which were collected using video camera to capture non-verbal as well as verbal behaviors. A total of 66 episodes of everyday conversations were analyzed using seven nonverbal categories: affirmative head nodding; illustrative gestures; patient-directed eye gaze; smiling and/or laughing; forward leaning; affective touch; and instrumental touch. Results: Among seven categories, patient-directed eye gaze(94%) was the most frequently utilized among nurses followed by affirmative head nodding(67%) and forward leaning(67%), while smiling was the least used(32%). Affective touch was identified in 39 episodes(59%). Among them, the most frequently touched area was hand or handshaking(59%) followed by shoulder or back, arm, and face. There were wide differences among nurses in terms of using affective touch, ranging from 0% to 98%. Nonverbal behaviors were more frequently identified in effective episodes than in ineffective episodes. Conclusion: Actively utilizing effective nonverbal behaviors may help geriatric nurses in promoting communication and in establishing rapport with patients with dementia.

The Influence of Human Rights Sensitivity and the Nursing Work Environment on Workplace Bullying Victimization among Nurses in Small- and Medium-Sized Hospitals (중소병원 간호사의 인권감수성과 간호근무환경이 직장 내 괴롭힘 피해에 미치는 영향)

  • Objectives: The purpose of this study was to examine the influence of human rights sensitivity and nursing work environment on workplace bullying victimization among nurses in small- and medium-sized hospitals. Methods: The participants were 255 nurses from 5 general hospitals in Busan. A dataset was collected using a structured self-reporting questionnaire during the month of July 2018. The data were analyzed with SPSS WIN 23.0 software (IBM Corp., Armonk, NY, USA) using independent t-test, one-way analysis of variance, Pearson's correlation coefficient, and multiple regression analysis. Results: Nursing work environment showed negative correlation with workplace bullying victimization. Being a witness to bullying, organizational support, head nurse's leadership, and relationships with peers were found to influence workplace bullying victimization, and these 4 variables explained 37% of workplace bullying victimization. Conclusions: Formal procedures for cases of bullying and improvements in the leadership of head nurses and peer relationships are crucial to reducing workplace bullying victimization among nurses in small- and medium-sized hospitals. Hospital executives' efforts to provide sufficient physical and human resources for nursing services and to improve the welfare of nurses are also needed.

  • A 3-day-old Appaloosa colt was examined because of acute onset of ataxia, left-sided head tilt, fever and heart murmur. The foal continued to nurse but became increasingly depressed and mild colic. On plain radiological examination, the foal was normal. Uroperitoneum was diagnosed following abdominocentesis. Postmortem examination revealed bladder rupture. The lesion was in the dorsomedial part of the bladder wall. We suggested that the cause of bladder rupture was parturient trauma. Even though the rupture was associated with trauma, the margin of rupture of bladder was clearly smooth and regular.

Development and Evaluation of the e-learning Orientation Program for Nursing Student's Adapting to Clinical Practicum (임상실습 적응을 위한 e-learning 오리엔테이션 프로그램 개발 및 평가)

  • Purpose: The purpose of this study was to develop an e-learning orientation program for nursing students(junior), to evaluate the program, and to find out its effects on adapting to clinical practicum. Methods: The e-learning orientation program was developed according to ADDIE model(analysis, design, develop, implement, evaluate) using an Active Tutor Program. Twelve nursing professionals and 31 nursing students(learners) evaluated the e-learning orientation program. Thirty one nursing students participated in the e-learning orientation program both on-line and off-line. Results: The e-learning orientation program consisted of 5 parts including 'Taking a lesson in gastrointestinal tract', 'Fundamental nursing skill and reading a medical chart', 'Taking a ward orientation by head nurse', 'Clinical practice with preceptors', and 'Writing for case report'. Adapting scores to clinical practicum were significantly different between pre-test and post-test(t=-5.76, p=.000). Conclusion: The e-learning orientation program of on-line and off-line has an effect on adapting to clinical practicum for nursing students.

A Study on relationship between the personality and the work achievement of nurse (간호원의 성격특성과 근무성적과의 관계에 관한 연구)

  • The main purpose of this study was to investigate the desiarable selection of nursing. The subjects for this study were made up of 144 R.N. in active service at H. University hospital in Seoul. This study was “Choice of nursing seale”as a tool which was MMPI questionare developed by Beaver(1953), and was consist of the choice of nursing scals made up to 66 items. The finding of this study related to their personality and their work achievement showed very significant differences: the results of this study are: 1. There was a significance of the difference between the responses of 64 student nurses and 64 College Women in Educations Curricular to Beaver score. (p <.001) 2. There was a significance in Beaver's score between high group(above 82) and low group(below 53) according to head nurses' work achievement score (p <.05) 3. This study had relationship between the personality and the work achievement of nurses. For the reason, I suggest that we had to apply to select the appropriate nurses in Health Field as Hospitals.

Influence of Emotional Intelligence, Communication, and Organizational Commitment on Nursing Productivity among Korean Nurses

  • Purpose: Improving productivity in nursing practice is an important issue. This study investigated factors affecting nursing productivity of Korean clinical nurses. Methods: A structured survey tool was used in a cross-sectional design with a convenience sample of 239 nurses working in university hospitals. Stepwise multiple regressions were done to identify influential factors. Results: The level of nursing productivity was at a moderate level (3.3 out of 5). Those nurses who were over 36, married, over master-graduated, regularly employed, on day duty, and with experiences as a charge or head nurse reported better achievements in nursing productivity than the other groups of nurses. All three independent variables, age, and employment status explained 55.4% of the variance in nursing productivity. Conclusion: The leaders and managers of nursing organizations should develop educational programs aimed at increasing nurses' competencies in relation to emotion controls and communication skills, which consequently should improve nursing productivity.

A Study on Perceived Discomfort in Patients Following Cardiac Catheterization (심도자 검사 환자의 불편감에 관한 연구)

  • Cardiac cathererization has become a routine diagnostic procedure indicated for evaluation of a wide varity of cardiac conditions. Patients are admitted to the coronary care unit after cardiac catheterization. These conscious patients report feeling uncomfortable in the CCU, but no studies have been done on the effects of bed rest, sand bags on the femoral puncture site and restricted mobility for 4 to 12 hours or longer postprocedure. The main objective of this thesis is to provide basic data to nursing on interventions which de-crease the uncomfortableness experienced by patients in the CCU following cardiac catheterization. In this phenomenological study, the various discomforts felt by the patients were collected and classified. The study subjects were a convenience sample of 29 patients who were admitted to the CCU of a general hospital in Inchon following cardiac catheterization. They were conscious, so they were able to communicate without difficulty. The data were collected over an U days period from July 21, to October 14, 1994. The subjects were interviewed using unstructured open questions and the interviews were tape recorded with the patient's permission The data were analyzed using the Van Kaams phenomenological method. Reliability and validity were exammed by two professor of nursing science, one head nurse, one staff nurse and one cardiologist. The results of the study are summarized as follows ; 1. The 129 descriptive statements by the postcardiac catheterization patients of discomfort were organized into 19 themes. 2. The 19 themes were divided into 3 categories ; physical, psychological, and environmental aspects. 3. The problems concerning the physical aspect were the discomfort of restriction of movement, dysuria, medical devices, pain in the puncture site, symptom is related to the procedure of cardiac catheterization, headache and dizziness, leg painand tingling sensation, and chest pain. The problems concerning the psychological aspect were regret resulting from dependency, economic burden, dissatisfaction with medical personnel, dissatisfaction with medical service system, anxiety about the result of the procedure, concern about the prognosis, loneliness, and concern over treatment procedure. The problems concerning the environmental aspect were influence from neighboring patients, noise, and maladaptation to environmental change. The necessity for holistic care which satisfies physical, psychological, and environmental need must be emphasized in order to solve these discomforts.

A Development Study of Common Clinical Document Forms for Traditional Korean Medicine Information Standardization (한의 정보 표준화를 위한 공통 임상 기록 서식 개발 연구)

  • Objectives: The clinical document forms, a format for collecting clinical data, is the most fundamental object of standardization. Doctors must have a mutual understanding of the clinical chart. Methods: Clinical document forms were developed by investigating existing conditions in hospitals and conducting demand surveys, doing literature research, and seeking expert advice for the improvement of version 1.0. In addition, an organization of a network of 19 Oriental medical doctors and nurses, 190 patients, and users of collected and assessed data was formed to come up with version 2.0. Results: The overall format was divided into different portions that the patient, nurse, and doctor must fill out, respectively. The patient's section consists of demographic data, lifestyle details, history, and symptoms. The data to be supplied by the nurse include the patient's vital signs and anthropometric parameters. As for the doctors, they are to supply data regarding the patient's palpitation, the detailed symptoms of the patient's head, ophthalmological and otorhinolaryngological symptoms (mouth), respiration, circulatory organ and chest conditions, digestive-organ conditions (thirst), neuropsychiatric conditions, reproductive system, musculoskeletal system, skin (depilation), etc. Conclusions: Common clinical chart development is the prior question to Traditional Korean Medicine standardization. A web-based clinical document format should be developed to support diagnosis and treatment, and furthermore EMR (electronic medical record system) and EHR (electronic health record) developed. Clinical information could be shared through a network of medical institutions and be useful Traditional Korean Medicine for evidence-based medicine.

A Study of Factors Influencing Morale of Hospital based Home Care Nurses (병원중심 가정전문간호사의 직무관련 사기(士氣) 정도)

  • Purpose: The purpose of this study was to examine morale and related factors of hospital based home care nurses. Method : The subjects were 159 home care nurses from 94 hospitals and clinics which were operating home care programs around the country. Data were collected for 40days from March 14, to April 24, 2005. The questionnaire consisted of 51 items including 12 general variables and 39 items of nurses' morale. The reliability of the questionnaire by Cronbach's ${\alpha}$, was .88. Result : The level of the morale was found as mean score 2.69 in 4 point scale The high ranks of morale were self actualization($3.05{\pm}0.43$) and job satisfaction($3.03{\pm}0.43$), the factors which showed lower points were evaluation of work ($2.47{\pm}0.53$), welfare($2.42{\pm}0.42$), promotion system ($2.35{\pm}0.45$) and wages($2.23{\pm}0.54$). The level of morale according to the general variables were significantly different in such variables ; home care nursing antecedent(p = .000), motivation for job selection(p= .030), intention to quit the job(P= .000). Variables of intention to quit the job(15.7%) and home care nursing antecedent; 6.7%(p=.001) showed 22.4% of explanatory persuasion effect on level of morale. Conclusion : To improve a morale of home care nurses, the arrangement of nursing department should be consider nurse's aptitude and interest and allow them to have longer period of work in that part. Also wages, promotion system and welfare should be reformed as relevant as their career.
