Deciding not to Operate in Head Injuries and Legal Considerations

  • It is not the best way to treat a hopeless patient with life-sustaining medical devices until the heart beats stop. Advanced medical technology may prolong the life for a significant period without recovery from the disease. However, it would give an unbearable economic burden to the family and the society. In 2006, we decided not to operate 9 patients with traumatic intracranial hematomas. We examined those patients with special references to possible legal and ethical problems. It is reasonable to withhold a treatment after documentation that the family never wants any life sustaining treatment when the treatment does not guarantee the meaningful life.

  • People with epilepsy are believed to be at a higher risk of incurring accidental injury than people who do not have seizures. The incidence of injury, either due to seizure or accident as a consequent of seizure is also high and varies from 0.03% to 3%. The most common injuries are head contusions, lacerations, burns and fractures. In this article, we present a case of quadriplegia after a generalized epileptic seizure.

Anti-dandruff Effect of Extract Mixture of Lophatheri Herba, Patriniae Radix, Allii tuberosi Herba, and Menthae Herba (담죽엽, 패장, 구채 및 박하 추출 혼합물의 두풍백설 치료효과)

  • Lophatheri Herba, Patriniae Radix, Alli tuberosi Herba, and Menthae Herba have long been used externally for treatment of injuries or inflammations in Korea. This study was carried out to examine the anti-dandruff effect of the mixture made with equal parts of 70%-ethanol extracts of Lophatheri Herba, Patriniae Radix, Allii tuberosi Herba, and Menthae Herba (LPAM) and of each one above, compared with 1%-zinc pyrithione (Standard) used commercially as anti-dandruff. The results showed the mixture was more effective than each crude drug. The 10%-LPAM produced no toxicity in skin reaction test and eye irritation test, and showed significant antimicrobial activity against Pityrosporum ovale, the major cause of dandruff, and also showed significant anti-dandruff and itch-improvement effects on head skin.

Nonpenetrating Chest Trauma 410 Cases (비관통성 흉부손상 410예 보고)

  • From Jan. 1985 to Sep. 1989, four hundred and ten patients were admitted to the department of Thoracic and Cardiovascular Surgery, Yonsei University College of Medicine with a diagnosis of nonpenetrating chest trauma. The most common cause of injury were motor vehicle accidents(75.1%) with the prevalent age group being their forties. The four most common findings were rib fracture(83.4%), hemothorax(50.7%), pneumothorax(24.6%) and clavicle fracture(12.0%), Fifty piratory insufficiency and the ventilation time was correlated with 6 fators : 1) shock 2) endotracheal intubation or tracheostomy 3) hemothorax 4) clavicle fracture 5) flail chest 6) more than three combined injuries or combined head injury. Ventilatory care was also related with mortality and complications such as pneumonia sepsis and empyema thoracis(p<0.05). The most common organism of post-traumatic infection were Pseudomonas and Staphylococcus.

The Accident and Injury Characteristics of Elderly Drivers on Lateral Impact (고령 운전자 측면충돌 사고 및 상해특성)

  • Domestic insurance claims were statistically investigated to analyze the elderly driver's accident patterns and injury types in side impact crashes. Medical treatment records and accident vehicle damage photos have been surveyed for 5,419 cases. The results of our statistical analysis showed that the thorax injury risk of the elderly drive group is 8.8 and 4.0 times higher than that of the young and middle age group respectively. Diagnosis showed that most thorax injuries were caused by rib fracture. The head injury risk of the elderly female driver group seemed to be higher than that of the younger female driver group, however, statistical test has not been conducted because of the lack of number of samples for elderly female accident.

The Clinical Analysis of Flail Chest (동요흉 환자의 임상적 고찰)

  • From 1980 through 1993, sixty one patients having traumatic flail chest were analysised retrospectively at the Department of Thoracic and Cardivascular Surgery, Chonbuk National University Hospital. There were 47 men and 14 women, mean age, 49.3 years, age range 4 to 82 years. The most common mode of trauma was automobile accident, common combined other organ injuries were skeletal injury [ 36 patients and neurologic injury [ 20 patients . In the mode of treatment, ventilator therapy was done in 34 cases and operative stabilization was done in 18 cases [ Kirschner or steel wire: 9 cases, Judet`s strut: 9 cases . Sixteen patients died [26 % . The main factors associated with fatal outcome were shock [ p < 0.002 , head injury [ p < 0.005 , and more than 50 years of age [ p < 0.05 . In fatal cases, 14 patients died during in ventilator therapy [ 14/34, 41 % and 2 patients died following operative stabilization of chest wall [ 2/18, 11 % .The overall cause of death was septicemia, ARDS, ARF, hypovolemic shock and hypoxic brain damage.

  • Although a number of neck injuries are generated, the data which quantify the kinematic response of the human head and cervical spine in low-speed rear-end automobile collisions is very limited. On this problem, just few in vitro experimental research or some experimental research using dummy on neck injury by rear-end collision was conducted, thus systematic research is requested on full scale injury mechanism. An occupant model for the response of the occupant subject to rear-end collision using commercial dynamics package DADS was developed. Developed model shows more close agreement with the experimental data compared with the MADYMO simulation results for the cases of ${\delta}V=16$ kph in sled test. For the case of ${\delta}V=8$ kph and 33.5 kph with production seat, model also shows its reliable response compared with experimental results using Hybrid III and Hybird III with RID.

  • Crashworthy design of a train is a systematic approach to ensure the safety of passengers and crews in railway transportation for the prescribed accident scenarios. This approach needs new structural arrangements and designs to absorb higher levels of impact energy in a controlled manner and interior designs to minimize passenger injuries. In this paper, crashworthy design approach is applied to the tilting train express (TIX) design which is newly being developed. Based on a head-on collision and a level crossing collision scenarios, the crash behaviors of a TTX design candidate arc evaluated numerically using the finite element method. Finally, design alternatives which show better crashworthy performances are proposed and verified through the full-rake collision simulations.

  • The two main forms of myositis ossificans are congenital and acquired. Either form is rare in the head and neck region. The acquired form is often due to trauma, with bullying as a fairly common cause. This report of myositis ossificans of the platysma in an 11-year-old female patient emphasizes the need for a high index of suspicion in unexplainable facial swellings in children and the benefit of modern investigative modalities in their management.

Effects of Yukmijihwangwon in Rat Models of intracerebral hemorrhage (뇌출혈 동물모델에서 육미지황원의 효과 연구)

  • Intracerebral hemorrhage (ICH) is associated with a considerable proportion of strokes and head injuries. The mechanism of brain cell injury associated with hemorrhage may be different from that due to pure ischemia. Therefore, it is essential that models of intracerebral hemorrhage be developed and well characterized in animal model. Yukmijihwangwon (YM) has been known to reinforce the vital essence and have antioxidant activities. In this study, the protective effects of YM was investigated against ICH in animal models. Adult rats had 2 microliters saline containing 0.5 and 5 unit bacterial collagenase infused into the right caudate nucleus. It was found out that YM was effective in protecting brain against ICH.
