  • The purpose of this study was to investigate muscle activity of the lower limbs when walking in jeans in order to obtain basic information for development of new jeans patterns with excellent movement adaptability. Using three types of jeans (basic, medium, and slim) with different ease on hip, knee circumference, and crotch length, and two different types of shoes, Electromyogram (EMG) of the lower limbs muscle was measured for four healthy subjects walking on treadmills and stairs. EMG of vastus lateralis, semitendinous, tibialis anterior and medial head of gastrocnemius muscles was measured. The muscle activity was assessed in RMS (Root Mean Square) value of the EMG. On the treadmill in sneakers, only the vastus lateralis muscle showed a significant difference in RMS value depending on patterns. Basic and medium jeans allowed higher muscle activity than trunks of slim jeans did. On the treadmill in high heels and slim jeans, the RMS values of all muscles were significantly smaller than in basic jeans, whereas no significant differences were shown while in trunks or medium jeans. On the stairs either in sneakers or in high heels, no significant differences were shown between all muscle activities for all types of jeans. On the treadmill, greater fatigue was induced in all muscles from walking in high heels than in sneakers for all jeans patterns. When walking on the stairs wearing either type of shoes, however, the effect of jeans pattern on muscle activities was different from muscle to muscle.

  • Objective : To investigate the effect of surgery time on prognosis of newborns with meningomyelocele. Methods : The records of neonates with meningomyelocele were retrospectively analyzed. Demographic and clinical characteristics as well as information, timing of surgery, and durations of hospital stay and antibiotic therapy were recorded. Results : The records of 30 babies were included in the final analysis. Overall, the mean gestational age was $37.7{\pm}2.7$ weeks, with a mean birth weight of $2967{\pm}755$ g and head circumference of $35.8{\pm}3.8$ cm. In terms of localization, 46.6% of the meningomyeloceles were lumbosacral, 40% were lumbar, 10% were thoracolumbar and 3.3% were thoracal. The mean size of the meningomyelocele sacs was $4.33{\pm}1.2$ cm. Newborns underwent surgery on average of $8.2{\pm}5.9$ days after birth, with an overall mean duration of hospital stay of $30{\pm}25.1$ days. Patients were divided into two groups based on timing of surgery (group 1, ${\leq}5$ days; group 2, >5 days), and comparisons between groups revealed that earlier surgery was associated with significantly shorter durations of hospital stay (p<0.001) and antibiotic therapy (p<0.05). Conclusion : Early surgical intervention (${\leq}5$ days) was associated with a shorter duration of hospital stay and antibiotic therapy as well as a lower complication rate. We recommend that corrective surgery be undertaken as soon as reasonably possible.

  • As social changes, there are growing number of instituted infants. Most of them were poor growth and developmental outcome due to situational and environmental vulnerability. Developmental interventions for this infants have been the subjects of debate for many years. The purpose of this study was to evaluate the effectiveness of massage intervention on the physical growth of orphaned infants during the first 6 month of life. This study utilized equivalent control group with repeated measuring by quasi experimental design. Fifty eight orphaned infants were randomly assigned to control(n=28), or an experimental(n=30) group. In addition to routine orphanage care, infants in the experimental group received 15 minutes of massage intervention twice a day, 5 days a week, for 4 weeks. Data has been collected from April 1st, 2000 to August 31st, 2002 and were analyzed using SPSS/PC(Version 10.0) with chi-square test, t-test, and repeated measures ANOVA to determine the effectiveness of massage intervention and to test the homogeneity of general characteristics between experimental and control group. The results were as follows; Compared to the control group, the experimental group has gained significantly more weight(F=33.52, p < .0001) and had larger increases in length(F=10.04, p < .0001) and head circumference(F=49.78, p < .000) after the 4-week intervention period and at 6 months of age. These data demonstrate that massage intervention may be effective in facilitating growth for newborn infants placed in orphanages during first 6 months of life.

  • This study were conducted to assess the physical growth and developmental status of infants in orphanage in order to provide an empirical data. The subjects for this study were 104 infants and toddlers who were reared in an orphanage in D Metropolitan city. The instrument used for this study were anthropometric assessment and DDST for normative data of development. Data has been collected from September 1st, 1998 to August 31st, 2000 and were analyzed using SPSS/PC(Version 10.0) with frequency, mean, standard deviation, ANOVA and Chi-square test. The results of this study were as follows; 1. 30.8% of infants in orphanage had abnormal weight, 26.9% had abnormal length, and 22.1% had abnormal head circumference and most of them were distributed below 50 percentile of growth chart. 2. 53.8% of infants in orphanage had normal, 27.9% had qustionable, and 18.3% had abnormal developmental screening test results, especially, 31.5% of infants in orphanage ages 3 to 5 years had abnormal developmental screening test results, according to the Denver Developmental Screening Test(DDST). There was a significant developmental delay noted in the language and fine motor-adaptive sector. 3. It is anticipated that developmental delays would increase in severity by older the mean age of orphanage infants and longer the time being raised in orphanage. It would be concluded that the physical growth and developmental status of orphaned infants were very vulnerable and serious and it is suggested that there needed an effective intervention strategies to promote growth and development of infants in orphanage.

  • Purpose: This study was conducted to describe health in optimal fitness (HOF) in young children born prematurely and to analyze factors affecting HOF in health status, investment resources, and anthropological values, based on HOF theory. Methods: A case-control study of 76 children with preterm births (PTB) was conducted at 24 to 42 months of corrected age. Their HOF status was evaluated based on height, weight, head circumference, and the Korean-Bayley Scale of Infant Development-II and classified as either HOF-achieved or HOF-uncertain in the domain of growth, development, and all together. Results: For growth, development, and all, 26.3%, 27.6%, and 47.4% of children, respectively, belonged to the HOF-uncertain group. Logistic regression analysis showed that longer length of hospital stay (${\geq}21days$; OR=7.8; 95% CI [1.5, 40.5]), worse scores on the Home Observation for Measurement of the Environment (HOME) (${\geq}38$; OR=0.1; 95% CI [0.0, 0.4]), having a working mother, (OR=5.7; 95% CI [1.2, 27.6]), and an older mother (${\geq}35years$; OR=8.8; 95% CI [2.1, 37.3]) were statistically significant contributors of HOF-uncertain in the domain of all. Conclusion: Findings show that young children born prematurely with prolonged stays in a neonatal intensive care unit and insufficient socioeconomic resources at home are more likely to exhibit delayed growth and development.

  • In recent years, the interests and needs of the Pedestrian Protection System (PPS), which is mounted on the vehicle for the purpose of traffic safety improvement is increasing. In this paper, we propose a pedestrian candidate window extraction and unit cell histogram based HOG descriptor calculation methods. At pedestrian detection candidate windows extraction stage, the bright ratio of pedestrian and its circumference region, vertical edge projection, edge factor, and PCA reconstruction image are used. Dalal's HOG requires pixel based histogram calculation by Gaussian weights and trilinear interpolation on overlapping blocks, But our method performs Gaussian down-weight and computes histogram on a per-cell basis, and then the histogram is combined with the adjacent cell, so our method can be calculated faster than Dalal's method. Our PCA reconstruction error based pedestrian detection candidate window extraction method efficiently classifies background based on the difference between pedestrian's head and shoulder area. The proposed method improves detection speed compared to the conventional HOG just using image without any prior information from camera calibration or depth map obtained from stereo cameras.

  • The most detrimental pressure pulsations in high-head pump-turbines is caused by the rotor-stator interaction (RSI) between the guide vanes and runner blades. When the pump-turbine operates in the S-shaped region of the characteristic curves, the deteriorative flow structures may significantly strengthen RSI, causing larger pressure pulsations and stronger vibration with an increased risk of mechanical failure. CFD simulations were carried out to analyze the impacts of flow evolution on the pressure pulsations in the S-shaped region of a model pump-turbine. The results show that the reverse flow vortex structures (RFVS) at the runner inlet have regular development and transition patterns when discharge reduces from the best efficiency point (BEP). The RFVS first occur at the hub side, and then shift to the mid-span near the no-load point, which cause the strongest pressure pulsations. The locally distributed RFVS at hub side enhance the local RSI and makes the pressure fluctuations at the corresponding sections stronger than those at the rest sections along the spanwise direction. Under the condition of RFVS at the mid-span, the smaller flow rate make the smaller difference of pressure pulsation amplitudes in the spanwise direction. Moreover, the rotating stall, rotating at 35.7%-62.5% of the runner rotational frequency, make the low frequency components of pressure pulsations distribute unevenly along the circumference in the vaneless space. However, it have little influence on the distributions of high components.

  • This report describes the long-term follow-up of a 10-year-old female patient with Joubert syndrome with short stature and brachydactyly. She presented with hyperpnea alternated with hypopnea, uncontrolled jerking eye movements, and hypotonia during early infancy. She was diagnosed with Joubert syndrome based on clinical symptoms and typical MRI findings at 5 months of age. Abnormal ventilation and eye movements disappeared at around 4 years of age. Head circumference kept within normal range for her age, but her height and weight growth were markedly retarded. Simple X-ray showed an enlarged skull with increased digital markings, hypoplasia of facial bones, and abnormal enchondral bone formations in hands and feet. This article is the first report of Joubert syndrome with peripheral dysostosis.
