Usefulness of Narrow Band Imaging Endoscopy in the Diagnosis of Head and Neck Cancer : A Prospective Study (두경부암 진단의 새로운 내시경 진단방법 협대역 영상(Narrow Band Imaging, NBI)의 유용성-전향적 연구)

  • Background and Objectives : Narrow Band Imaging(NBI) is a novel optical technique that enhances the visualization of superficial microvascular architecture which is commonly increased and founded as an irregular shape in a neoplastic lesion. The aim of this study is the evaluation of the usefulness of NBI in the diagnosis of Head and Neck Cancer. Subjects and Methods : From December 2009 to January 2011, 31 consecutive patients who were diagnosed with head and neck malignancy were enrolled in this prospective study. The malignant findings of NBI were demarcated brownish lesion or increased intraepithelial papillary capillary loops(IPCLs) with or without irregularity. Results : There were 29 cases(93.5%) of well demarcated brownish lesion, 26 cases(83.9%) of increased IPCLs and 4 cases(12.9%) of satellite lesions. Diagnostic accuracy of endoscopic examination was increased from 83.9% to 93.5%, when NBI was applied to the conventional endoscopy(p>0.05). Conclusion : NBI is a powerful and safe screening test, which can be performed in out patient clinic without any supplementary procedure.

A Study on the Correlation between Signs/Symptoms of the Craniomandibular Disorder and Head/Neck Posture (두개하악장애의 증후발현과 두경부자세 사이의 상관성에 관한 연구)

  • To evaluate the contribution of head/neck posture to signs/symptoms of craniomandibular disorder(CMD), the author clinically, cephalo-metrically, analyzed craniomandibular index (CMI), clinical dysfunction index(CDI), occlusal index(OI) and head posture, cervical spine, cervicovertebral anatomy, craniofacial variables from 30 CMD patients and control. And the author analyzed difference between patients and control, and the correlation between CMI, CDI and head posture, cervical spine, cervicovertebral anatomy, craniofacial variables in CMD patients and control. The obtained results were as follows : 1. There was very high significant correlation between CMI and CDI(r=0.8969, p<0.01). 2. There was significant difference between patients and control in head/neck posture, head/neck anatomy(p<0.05). 3. There was significant correlation between CMI, CDI and head posture, cervical spine, cervicovertebral anatomy, craniofacial variables(p<0.05). 4. There was more significant correlation between DI and head/neck posture, head/neck anatomy than between PI and head/neck posture, head/neck anatomy in CMD patients(p<0.05)

A Phase n Study on UFT Therapy for Head and Neck Squamous Cell Carcinoma (두경부 편평세포암종에 대한 UFT 투여 후 항종양효과에 관한 제2상 임상 연구)

  • A Phase II study of UFT which is a mixture of Tegafur and Uracil was conducted in two institutions during past two years. Ninty-four patients of head and neck squamous cell carcinoma entered this trial, of which sixty-eight were evaluated. Among those, thirty-six cases were previously untreated and thirty-two cases were recurrent UFT was administrated orally at a daily dose of $400mg/m^2$ for eight weeks. The results were as following: 1) Overall response was 30.88%, but for 38.36% for 36 cases of the untreated cases, 21.88% for 32 cases of recurrent cases. 2) UFT was more effective in early stage and well differentiated squamous cell carcinoma and UFT tended to reduce the tumor size maximally at fourth or fifth week 3) There was no serious side effects except mild gastrointestinal disturbances such as nausea and vomiting, which were recovered immediately after stop or reducing a daily dose. Therefore, UFT therapy is clinically effective for head and neck squamous cell carcinoma and also may be useful for combination or palliative chemotherapy because of mild side effects.

The Effect of Neutron Therapy on Head and Neck Cancer (두경부암(頭頸部癌)에서 중성자선(中性子線) 치료(治療)의 효과(?果))

  • The result of neutron therapy on head and neck cancer using KCCH -Cyclotron neutron which had been using from October 1986 to September 1989 in the Korea Cancer Center Hospital. Among the total of 27 patients the cases of malignant salivary gland tumor were 14 and the cases of advanced head and neck cancer of AJCC stage IV were 13. The local control rate was 80% in malignant salivary gland tumor and 46.2% in advanced head and neck cancer. The 2 year survival rate was 60% in malignant salivary gland tumor and 38.5% in advanced head and neck cancer. Although there was no significant difference in prognosis according to the pathologic types, squamous cell carcinoma revealed a pattern of poor prognosis. The major complication from the neutron therapy had developed 7.1% in malignant salivary gland tumor and 23.1% in advanced head and neck cancer. In conclusion, neutron therapy is superior in the treatment of malignant salivary gland tumor and also effective in the treatment of advanced head and neck cancer when it can avoid to treat some site of low tolerance.

  • Schwannoma is a benign well-encapsulated tumors arising from the sheath of Schwann cell of the peripheral motors, sensory, and cranial nerves, but not from the optic and olfactory nerves. Since it is relatively common in the head and neck region, it should be included in the differential diagnosis of head and neck tumor. However, reports of laryngeal involvement have rarely appeared in the literature. We have experienced a 50-year old woman and 39-year old woman with history of progressive voice change. We recognized a benign mass at the false cord area with the telelaryngoscope and CT. The tumors were successfully removed by transoral CO2 laser resection without tracheotomy.

  • Complete cricotracheal separation, which is the most severe type of laryngeal trauma, is an uncommonly seen injury that clinicians have limited experience in managing. However, it is potentially fatal. Due to limited exposure to this condition, mismanagement can occur, which may further aggravate the patient's condition. The most crucial part of managing this injury is to establish a secure airway. Tracheostomy under local anesthesia is the preferred method of airway stabilization, in order to avoid further injuries to the airway caused by endotracheal intubation. Here, we discuss the management of complete cricotracheal separation based on a case experienced in the east coast region of Malaysia, where this type of injury is rarely encountered.

Distribution of Neck Node Metastasis in Head and Neck Cancer (두경부종양 환자에서 경부림프절 전이의 분포에 관한 고찰)

  • Objectives: The records of 936 patients with previously untreated carcinoma of the head and neck were reviewed in order to define the incidence and topographical distribution of neck node metastasis on admission. Materials and Methods: The common regions of metastasis are presented for each of the 12th individual head and neck sites selected for study. Results and Conclusion: Knowledge of the preferred areas of spread and those that are almost never involved allows the design of more adequate radiation therapy plans to manage the individual tumors.

A Case of Cystic Lymph Node Metastasis of Tonsil Cancer Mimicking 2nd Branchial Cleft Cyst (제2열 새열 낭종으로 오인되었던 편도암의 낭성 경부 임파절 전이 1예)

  • Cystic lymph node metastasis of head and neck squamous cell carcinoma(HNSCC) which presumed to be mainly originated from oropharynx including Waldeyer's ring may present as a benign cystic mass on lateral neck such as branchial cleft cyst. Branchial cleft cyst is one of the most common lateral neck cystic mass which may result in regional infection or lymph adenopathy. Many of previously reported literatures showed the incidence of cystic lymph node metastasis from oropharynx including Waldeyer's ring. Preoperative imaging studies and fine needle aspiration cytology cannot provide the accurate results until excision of cystic mass for the diagnostic or therapeutic purpose. Recently, we experienced the rare case of cystic lymph node metastasis from ipsilateral tonsil, which mimicked infected 2nd branchial cleft cyst. Thus, we reported our experience with presentation of case and review of literatures.

The Relationship between the Causative Allergens of Allergic Diseases and Environments in Korea Over a 8-Year-Period: Based on Skin Prick Test from 2006 to 2015

  • Background and Objectives: The present study evaluated the results of skin prick test using 55 allergens at 20 centers in the Republic of Korea in 2006, 2010, and 2014-2015. The aim was to assess changes in the positive rate of allergens according to temporal, regional, and environmental factors. Materials and Method: In total, 20 hospitals were selected based on the population distribution in the Republic of Korea. A skin prick test panel comprising 55 aeroallergens was distributed to 18 hospitals for this prospective study. The 2006 and 2010 skin prick test results were collected and analyzed retrospectively from 20 hospitals, while the 2014/2015 skin prick test results (from June 2014 to May 2015) were collected prospectively from 18 hospitals. Results: A total of 14,897 SPT test results were analyzed: 4,319 in 2006, 7,431 in 2010, and 1,852 in 2014/2015. The overall rate of skin prick test positivity to more than two allergens was significantly higher in males than females. The positive rates of alder pollens and birch, oak and ragweed pollen positivity were increased in older patients. Several positive rates were increased according to the temperature in spring. The positive rates for beech pollen, birch pollen, hazel pollen, oak pollen, Tyrophagus putrescentiae, mugwort, cat, Acarus siro, Lepidoglyphus destructor and Tyrophagus putrescentiae were significantly increased, while those of Cult rye pollen and dandelion were significantly decreased over the three test periods. The overall positive rate for allergens in Jeju province varied significantly from Seoul and other cities. Conclusion: Change in the positive rate of multiple aeroallergens was evaluated in the Republic of Korea over time. Our findings can be used to recommend aeroallergens suitable for inclusion in skin prick test panels in the Republic of Korea and will facilitate further investigation of changes in the patterns of allergic diseases.
