방사선 치료 중인 두경부 종양 환자를 대상으로 메게이스 내복 현탁액 경구투여 후 유효성 및 안전성에 관한 다기관, 공개, 관측연구 (Multicenter, Open-Labeled, Observational Clinical Trial to Investigate the Efficacy and Safety of Oral Administration of Megace Suspension in Patients with Head and Neck Cancer During Radiotherapy)

  • Background and Purpose:Nausea, vomiting and weight loss are common problems that are encountered in the course of cancer patient treatment who are receiving radiotherapy. In this study, we are aiming to analyze the effect of megestrol acetate on quality of life of head and neck cancer patients receiving radiotherapy, resulting from improvement of weight loss, appetite and nutritional status via multicenter, open-labeled, observational clinical trial. Material and Methods:A total of 270 patients from 10 medical institutes who are receiving radiotherapy or who have completed radiotherapy within 3 months, between February 2007 and February 2008, were selected as candidates for the study. Megestrol acetate suspension(megace) was given to the subjectives once a week for 4 weeks with the amount of 20ml(megestrol 800mg). Measurement of weight and questionnaire surveys were carried out three times: at the start of the study, 4 weeks after the start of the medication, and 4 weeks after the end of the medication, respectively. Results:The group who has received megace had a total number of 199, and control group was 70. The group who have received megace showed mean weight loss of 1kg in 8 weeks, compared with the weight loss of 5.5kg in control group, which showed that the medication was effective in reducing the amount of weight loss(P=0.027). The group who received megace had a tendency to report a reduced rate of decrease in the score of appetite, nausea and vomiting, and QOL score, but it did not have statistical significance(P>0.05). Conclusion:Megestrol acetate have reduced the degree of weight loss significantly, and it has a tendency to reduce the rate of decrease in appetite, aggravation of nausea and vomiting, and quality of life.

Survival in Head and Neck Cancers - Results of A Multi-Institution Study

  • Background: The prime output of Hospital Based Cancer Registries is stage and treatment based survival to evaluate patient care, but because of challenges of obtaining follow-up details a separate study on Patterns of Care and Survival for selected sites was initiated under the National Cancer Registry Programme of India. The results of stage and treatment based survival for head and neck cancers by individual organ sites are presented. Materials and Methods: A standardized Patient Information Form recorded the details and entered on-line at www.hbccrindia.org to a central repository - National Centre for Disease Informatics and Research. Cases from 12 institutions diagnosed between 1 January 2006 and 31 December 2008 comprised the study subjects. The patterns of treatment were examined for 14053 and survival for 4773 patients from five institutions who reported at least 70% follow-up as of 31 December 2012. Results: Surgical treatment with radiation for cancer tongue and mouth showed five year cumulative survival (FCS) of 67.5% and 60.4% respectively for locally advanced stage. Chemo-radiation compared to radiation alone showed better survival benefit of around 15% in both oro and hypo-pharyngeal cancers and their FCS was 40.0%; Hazard Ratio (HR):1.5;CI=1.2-1.9) and 38.7%; (HR):1.7; CI=1.3-2.2). Conclusions: The awareness about the requirement of concurrent chemo-radiation in specifically cancers of the oro and hypopharynx has to be promoted in developing countries. The annual (2014) estimate number of new Head and Neck cancers with locally advanced disease in India is around 140,000 and 91,000 (65%) patients do not receive the benefit of optimal treatment with ensuing poorer survival.

후두전적출술 후 장기적인 삶의 질에 대한 평가 (Long Term Assessment of Quality of Life in Laryngectomee)

  • Objectives: Despite various attempts to preserve the normal voice in advanced laryngeal cancer, it is inevitable for many advanced laryngeal cancer patients to undergo total laryngectomy and thus making a trade off between quality and quantity of life. Laryngectomees are faced with voice loss, change in physical appearance and health deterioration which hinder their efforts to rehabilitate back into their family and employment leading to poor quality of life. The objectives of this study were to evaluate the quality of life in long term surviving laryngectomees to, define the factors which are most important in determining their present quality of life and to propose a theoretical model for quality of life after laryngectomy in Korea. Materials and Methods: From 1986 to 1995, 120 laryngectomees with no evidence of disease were followed up for at least 3years were evaluated. Each of 15 quality of life domains with a total of ten points were given for a quality of life score. Results: There was no significant difference in the physical-mental adaptation index such as general health and mental health between the laryngectomees and control group(p>0.05). However, social adaptation index such as social activity, occupational status and economic status for laryngectomees were significantly lower with 2.3, 3.5, and 4.4 points compared to control group with 8.5, 7.6, and 7.1 points respectively(p<0.05). Conclusion : Vocational and social rehabilitation should be emphasized with highest priority for improving their economic status and thus improving their quality of life.

두경부암 환자에서 수술 및 방사선치료 후 갑상선 기능 저하 (Hypothyroidism Following Surgery and Radiation Therapy for Head and Neck Cancer)

  • 목적 : 두경부암 환자에서 수술 및 수술 후 방사선치료가 갑상선 기능에 미치는 영향을 평가하기 위하여 전향적 연구를 시행하였다. 대상 및 방법 : 1986년 9월부터 1994년 12월까지 수술후 방사선치료를 받은 두경부암 환자 중 추적 관찰이 가능했던 71예를 대상으로 하였다. 환자의 연령분포는 32세부터 73세였고, 중간값은 58세였으며, 여자 12예, 남자 59예였다. 원발 병소부위는 후두가 34예로 가장 많았고, 구강 12예, 구인두 1예, 하인두 13예, 상악동 2예, 타액선 3예, 근원 불명 전이성 경부암 6예였다. 전후두절 제술 및 경부곽청수술을 시행한 환자는 45예였고, 경부곽청수술만 시행한 환자는 26예였다. 모든 환자에 대하여 방사선치료 전 및 방사선치료 후에 정기적으로 임상검사 및 갑상선 기능검사 (T3, T4, free T4, TSH, antithyroglovulin antibody, antimicrosomal antibody)를 시행하였다. 방사선량은 40.6Gy에서 60Gy였고 중간값은 50Gy였으며 추적관찰 기간은 3개월에서 80개월이었다. 결과 : 갑상선 기능 이상의 빈도는 $56.3\%(40/71)$였다. 31예 $(43.7\%)$는 갑상선 기능이 정상이었고 7예 $(9.9\%)$는 임상적 갑상선 기능저하증이었으며 33예 $(46.5\%)$는 준임상적 갑상선 기능저하증이었다. 갑상선 기능항진증, 갑상선 결절, 악성종양은 발견되지 않았다. 경부곽청수술군보다 전후두절제술 및 경부곽청수술군에서 갑상선 기능 이상이 일찍 발생하였다(p<0.05). 방사선치료 후 갑상선 기능 이상의 발생 빈도에 영향을 준 위험인자는 전후두절제술 및 경부곽청수술과 방사선치료의 병용요법 (P=0.0000)이었다. 전후두절제술 및 경부곽청수술군 45예 중 36예 $(80\%)$에서 갑상선 기능 이상이 발생하였으며 경부곽청수술군 26예 중 4예 $(15.4\%)$에서만이 갑상선 기능이상이 발생하였다. 결론 : 두경부암 환자에서 수술 및 방사선치료 후 갑상선 기능 이상은 비교적 흔한 합병증이며, 갑상선 기능 이상의 발생 빈도에 영향을 미치는 인자는 전후두절제술 및 경부곽청수술과 방사선치료의 병용요법이었다. 갑상선 기능 이상을 조기에 발견하여 치료하기 위하여 방사선치료 전 및 방사선치료 후에 정기적인 갑상선 기능검사가 필요할 것으로 사료된다.

Cancer Stem Cells in Head and Neck Squamous Cell Carcinoma: A Review

  • Research indicates that a small population of cancer cells is highly tumorigenic, endowed with the capacity for self-renewal, and has the ability to differentiate into cells that constitute the bulk of tumors. These cells are considered the "drivers" of the tumorigenic process in some tumor types, and have been named cancer stem cells (CSC). Epithelial-mesenchymal transition (EMT) appears to be involved in the process leading to the acquisition of stemness by epithelial tumor cells. Through this process, cells acquire an invasive phenotype that may contribute to tumor recurrence and metastasis. CSC have been identified in human head and neck squamous cell carcinomas (HNSCC) using markers such as CD133 and CD44 expression, and aldehyde dehydrogenase (ALDH) activity. Head and neck cancer stem cells reside primarily in perivascular niches in the invasive fronts where endothelial-cell initiated events contribute to their survival and function. Clinically, CSC enrichment has been shown to be enhanced in recurrent disease, treatment failure and metastasis. CSC represent a novel target of study given their slow growth and innate mechanisms conferring treatment resistance. Further understanding of their unique phenotype may reveal potential molecular targets to improve therapeutic and survival outcomes in patients with HNSCC. Here, we discuss the state-of-the-knowledge on the pathobiology of cancer stem cells, with a focus on the impact of these cells on head and neck tumor progression, metastasis and recurrence due to treatment failure.

경부청소술 후 어깨 및 척수 부신경의 기능평가 (Functional Evaluation of Spinal Accessory Nerve and Shoulder after Neck Dissection)

  • Objective: The aim of this study is to evaluate shoulder function and preoperative and postoperative electrophysiological changes related to the spinal accessory nerve with reference to neck dissection technique. Materials and Methods: We evaluated shoulder function by pain, strength and range of motion in a total of 35 neck dissection cases of 29 patients with head and neck cancer or thyroid papillary cancer. Electrophysiologic studies were performed before surgery, after third postoperative weeks and 6 months respectively. The results of each test according to the types of neck dissection were compared. Results: Clinical parameters of shoulder function and electrophysiologic study showed deterioration in early postoperative periods and improvements in late postoperative periods when the spinal accessory nerve was spared and permanent nerve damage was observed in radical neck dissection. There were correlations between the clinical parameters and electrophysiologic studies. Conclusion: The shoulder function after spinal accessory nerve sparing procedure is better than the function after nerve sacrificing procedure.

  • 원발미상의 경부 전이암의 경우에 있어 동시성 양측 편도암은 드물며, 일부에서는 일측 편도 절제술을 선호하나, 본 증례와 같이 양측 편도암이 발견되는 경우가 있고 또한, 양측 편도절제술이 일측 편도절제술보다 이환율이 크지 않기 때문에 양측 편도 절제술을 시행하는 것이 타당하다고 생각된다.

Concordant Surgical Treatment: Non-melanocytic Skin Cancer of the Head and Neck

  • Background: Skin cancer is the most common type of cancer. Of the 4 million skin lesions excised annually worldwide, approximately 2 million are considered cancerous. In this study, we aimed to describe a regional experience with skin cancers treated by a single senior surgeon and to provide a treatment algorithm. Methods: The medical records of 176 patients with head and neck non-melanocytic skin cancer (NMSC) who were treated by a single surgeon at our institution between January 2010 and May 2016 were retrospectively reviewed, and their data (age, sex, pathological type, tumor location/size, treatment modality) were analyzed. Patients with cutaneous squamous cell carcinoma (cSCC) who were classified as a high-risk group for nodal metastasis underwent sentinel node mapping according to the National Comprehensive Cancer Network guidelines. Results: Among the patients with NMSC who were treated during this period, basal cell carcinoma (BCC; n=102, 57.9%) was the most common pathological type, followed by cSCC (n=66, 37.5%). Most lesions were treated by complete excision, with tumor-free surgical margins determined via frozen section pathology. Thirty-one patients with high-metastasis-risk cSCC underwent sentinel node mapping, and 17 (54.8%) exhibited radiologically positive sentinel nodes. Although these nodes were pathologically negative for metastasis, 2 patients (6.5%) later developed lymph node metastases. Conclusion: In our experience, BCC treatment should comprise wide excision with tumor-free surgical margins and proper reconstruction. In contrast, patients with cSCC should undergo lymphoscintigraphy, as nodal metastases are a possibility. Proper diagnosis and treatment could reduce the undesirably high morbidity and mortality rates.

방사선 치료 유무 및 수술 범위에 따른 후두전절제술 후 인두피부누공 발생률 분석 (Pharyngocutaneous Fistula Formation Rate after Total Laryngectomy, Related with Previous Radiotherapy and Extent of Surgery)

  • Background and Objectives : Pharyngocutaneous fistula is the most common complication after total laryngectomy. The objective of this study was to determine the incidence of pharyngocutaneous fistula after total laryngectomy in patients operated on in our department and to establish whether specific factors predispose to fistula formation. Materials and Methods : For 10 years(2003-2014), 49 cases of patients who were diagnosed with laryngeal cancer and underwent total laryngectomy in our department. Patients who underwent radiotherapy before the surgery was 24 cases(48.9%) of all total laryngectomy patients. And those who were needed flap reconstruction because of extensive tumor involvement to hypopharynx were also 24 cases(48.9%), whereas primary closure were performed in 25 cases(51%). Results : The postoperative pharyngocutaneous fistula was found in 12 of the 49 patients(24.5%). The radiotherapy before surgery was a statistically significant factor that increase the incidence of postoperative fistulas(p=0.037). Large extent of surgery including flap reconstruction was almost statistically significant factor of raising postoperative fistula rates(p=0.051). Conclusion : According to this study, the presence of postoperative fistula seems to be influenced by previous radiotherapy and the extent of surgery. These could be the risk factors of pharyngocutaneous fistula after total laryngectomy.

Role of PET/CT in Treatment Planning for Head and Neck Cancer Patients Undergoing Definitive Radiotherapy

  • Background: In this study, we aimed to investigate the benefits of 18F-deoxyglucose positron emission tomography/computed tomography (FGD-PET/CT) imaging for staging and radiotherapy planning in patients with head and neck cancer undergoing definitive radiotherapy. Materials and Methods: Thirty-seven head and neck cancer patients who had undergone definitive radiotherapy and PET/CT at the Uludag University Medical Faculty Department of Radiation Oncology were investigated in order to determine the role of PET/CT in staging and radiotherapy planning. Results: The median age of this patient group of 32 males and 5 females was 57 years (13-84years). The stage remained the same in 18 cases, decreased in 5 cases and increased in 14 cases with PET/CT imaging. Total gross tumor volume (GTV) determined by CT (GTVCT-Total) was increased in 32 cases (86.5%) when compared to total GTV determined by PET/CT (GTVPET/CT-Total). The GTV of the primary tumor determined by PET/CT (GTVPET/CT) was larger in 3 cases and smaller in 34 cases compared to that determined by CT (GTVCT). The GTV of lymph nodes determined by PET/CT (GTVLNPET/CT) was larger in 20 cases (54%) and smaller in 12 cases (32.5%) when compared to GTV values determined by CT (GTVLNCT). No pathological lymph nodes were observed in the remaining five cases with both CT and PET/CT. Conclusions: We can conclude that PET/CT can significantly affect both pretreatment staging and assessed target tumor volume in patients with head and neck cancer. We therefore recommend examining such cases with PEC/CT before treatment.
