Clinicopathologic Factors in Selection of Surgical Procedure in Parotid Tumor Surgery - A Retrospective Review of 245 Cases - (이하선 종양 수술술식 선택에 있어 임상병리학적 요인 - 245예의 후향적 분석 -)

  • Introduction: A routine superficial parotidectomy with facial nerve dissection in parotid tumor surgery often results in facial dysfunction, Frey syndrome and defect in operation site. Formal facial nerve dissection has been a recommended procedure, because pleomorphic adenoma is a commonly recurrent tumor in case of inadequate surgical management, however it can not be always reasonable in aspect of postoperative sequelae. Patients and Methods: Through retrospective review of 245 cases parotidectomies and follow up for more than three years, clinicophathologic factors influencing to the selection of surgical procedure were considered to be age, sex, and preoperative pathology confirmed by preoperative MRI and FNA. Results: Five categories were established as follow for surgical decision in parotid tumor surgery. Category 1. Superficial lobe adenoma -- Superficial parotidectomy -- 124 Category 2. Deep lobe adenoma -- Deep parotidectomy -- 39 Category 3. Non pleomorphic adenoma -- Tumorectomy 1.5cm adenoma in young female -- Tumorectomy -- 25 Category 4. Recurrent multicentric tumor -- Parotidectomy+RT -- 9 Category 5. Parotid cancer; Parotidectomy + UND (RND) + RT -- 48 ; CORE (Composite Regional Ear Resection) -- 2 Conclusion: Surgical morbidity and recurrence rate could be minimized by individualizing the surgical procedure according to the category principle based on the clincopathologic features.

Structural Components of Leadership Perceived by Head Nurses (수간호사가 인지하는 리더십의 구성요소)

  • Purpose: The purpose of this study was to understand the components of successful leadership and organizational structural barrier to apply leadership perceived by head nurses. Method: We collected the data using interviews from August 1 to August 31 in 2003. The participants of this study were 19 head nurses from two university hospitals in Pusan. Result: Three categories of successful leadership in nursing organization were identified; leadership, opportunity, authority. Leadership category included accepting nurses' opinions, providing comfortable work environment, and interpersonal harmony. Opportunity category included verbal convincing for professional development and emotional support. Authority category included accomplishment and competence. Head nurses included autonomy, incentives, and equal opportunity across to different discipline as power category. Barriers identified by head nurses were lack of opportunity and power from hospital level. They seem to be frustrated when they don't have enough support from hospital. Conclusion: We think that head nurses have to apply effective leadership to achieve organizational goal and harmonious nursing environment. Based on the barriers identified in the study, interdisciplinary managemental intervention can lead better quality of care and nurses' work satisfaction which could further enhance hospital productivity and quality of care.

Effects of Interpretation Strategies and Consumers' Goals on Consumers' Response to Hybrid Products (해석 전략과 소비자 목표가 융합제품에 대한 소비자 반응에 미치는 영향)

  • Extending the findings of Rajagopal and Burnkrant (2009), this research examines the moderating role of consumers' goals (i.e., head category-relevant goal vs. modifier category-relevant goal) in the effects of two different interpretation strategies (i.e., relational interpretation vs. property interpretation) on product beliefs and attitudes toward hybrid products. In the current research, we make two predictions. First, we predict that both head category and modifier category beliefs will be higher under property interpretations than under relational interpretations in the modifier category-relevant goal priming conditions, whereas there will be no significant differences between each product category beliefs across the two interpretation conditions in the head category-relevant goal priming conditions. Second, we predict that attitudes toward hybrid products will be higher under property interpretations than under relational interpretations in the modifier category-relevant goal priming conditions, whereas there will be no significant differences between the attitudes toward hybrid products across the two interpretation conditions in the head category-relevant goal priming conditions. These predictions are tested and confirmed in two experiments. Finally, we discuss theoretical and practical implications of our findings and develop directions for future research.

  • In this paper, we propose a method to extract isa/instanceOf relation from category structure. Existing researches use lexical patterns to get isa/instanceOf relation from the category structure, e.g. head word matching, to determine whether the given category link is isa/instanceOf relation or not. In this paper, we propose a new approach which analyzes other category links related to the given category link to determine whether the given category link is isa/instanceOf relation or not. The experimental result shows that our algorithm can cover many cases which the existing algorithms were not able to deal with.

Intra-Rater and Inter-Rater Reliability of Various Forward Head Posture Measurements

  • The purpose of this study was to determine the intra-rater and inter-rater reliability of various forward head posture measurements. Ten healthy adults (age, $20.4{\pm}2.2$ yrs; height, $164.0{\pm}5.5$ cm; weight, $58.7{\pm}7.3$ kg) participated in the study. They were free of injury and neurologic deficits in the upper extremities and neck at the time of testing. The subjects were asked to perform head forward posture by under the guidance of physical therapists. Markers were placed on the C7 spinous process, mastoid process, tragus of the ear, outer canthus, and forehead. Measurement 1 for forward head posture assessment was measured as the angle between the horizontal line through C7 and the line connecting the C7 spinous process with the tragus of the ear. Measurement 2 was measured as the angle between the C7 spinous process, the mastoid process and the outer canthus. Measurement 3 was measured as two kinds of angles the HT (head tilt) angle is between the line from the midpoint of forehead to the tragus line and Y-axis at the tragus point. The NF (neck flexion) angle is between the line from the tragus to the C7 line and the Y-axis at the C7. Intra-rater, inter-rater reliability and coefficient of variation was assessed by comparing the measured values from three kinds of measurements of forward head posture. The intra-rater reliability was indicated by intraclass correlation coefficients [ICC(1,1)] and inter-rater reliability was shown by intraclass correlation coefficients [ICC(3,k)]. The results of study were as follows: ICC(1,1) values for intra-rater reliability of three measurements were in the 'excellent' category. ICC(3,k) values for inter-rater reliability of three measurements were also in the 'excellent' category. The coefficient of variation of method 2 had a lower value than method 1 and method 3. This data means that the measured value of method 2 was less scattered. Further research is needed to determine whether the validity of all measurements is revealed in the 'excellent' category.

An Analysis of Household Expenditures for Wedding, Funeral, Ancestor Worship, and 60th Birthday Anniversary (도시가계의 가정의례비 지출에 대한 관련요인 분석)

  • The purpose of this study was to investigate the effect of household characteristics on household expenditures on wedding, funeral, ancestor worship, and 60th birthday anniversary. Data for this study were from the 1996 Annual Report on the Family Income and Expenditure Survey. OLS regression analysis was used to identify the effect of household characteristics on money spent on family ceremony expenditures. The household income, age of head, education of head, and family size were all significant factors for understanding family ceremony expenditures. The results also indicated that additional income from wife's labor force participation was associated with lesser expenditures on family ceremony, while business income, asset income, and transfer income had no effects on this category. The effect of the third quarter of 1996 was significant on family ceremony expenditures, indication that households spent significantly less for this category in the summer than did in the winter, holding other factors constant.

The Role Adaptation Process of Head Nurses in the General Hospitals

  • Purpose. The purpose of this study is to identify the role adaptation process experienced by head nurses. Methods. Data were collected from 10 head nurses with in-depth interviews about their actual experiences. All the interviews were recorded and transcribed. The collected data were analyzed by the grounded theory methodology of Strauss and Corbin. Results. The core category was identified as 'weighing and balancing,' and the role adaptation process was divided into six stages by time sequence: inquiring, approaching to others, bringing people into their fold, working with conviction, avoiding conflicts, and settling. Conclusion. The results of this study yield useful information for top mangers of nursing to identify, by stages, the demands of the head nurses in their role adaptation process. The findings of this study contributes to developing programs that facilitates the head nurses' role adaptation.

A Study on Design Preference about Traditional Feminine Head Ornament for Development of Fashion Cultural Products (패션문화상품 개발을 위한 전통 여성 수식의 디자인 선호도 연구)

  • This research aims at the contribution to globalize and modernize the traditional Korean image by comprehending the taste of design that domestic college students have for traditional feminine head ornaments and subsequently elaborate the development of cultural products that are related to these decorative objects. In regards to this research method, the examination on the traditional feminine ornaments was followed through a review of literature and precedent studies and a survey was conducted on the preference about them. After the adoption of final valid responses, an analytical method, PASW 18.0, was used for frequency analysis, technical analysis, reliability, and regression analysis. The results were as follows. First, in the category of tendency analysis for the application of traditional feminine headpiece in fashion cultural products, it was revealed that a taste for the design that meet the satisfaction for both trend and practicability was prominently prevalent. Also, the design that express the individual characteristic was taken as a preferred option. Second, in the preference for the design of traditional feminine headpieces in fashion cultural products, the result indicated that the modern type was preferred in the form of re-creation as long as those products deform the tradition. As for the selective taste for patterns, their preference came in the order of plant, animal, and geometry-abstract types. Especially, for the case of plant and animal patterns, the reinterpreted design of modernized shapes were opted rather than a simply recopied format of the conventional type of the feminist head ornament. Third, for the category of item selection to apply the feminine head ornament in order to design the fashion cultural products, it turned out that people preferred the application to accessory rather than clothing. Lastly, it was found that rarity, harmony with other fashion goods, pattern, and design should be considered when the traditional motif was used for cultural products.

  • The Bethesda System for Reporting Thyroid Cytopathology (TBSRCT) is crucial for cytopathologists to use a standardized, category-based reporting system for thyroid fine needle aspirations and is effective for clear communication with the referring physicians. The new Bethesda System for Reporting Thyroid Cytopathology, the third edition in 2023, provides several key updates. The most important update is the assignment of only single name for each of the six diagnostic categories: (I) nondiagnostic; (II) benign; (III) atypia of undetermined significance; (IV) follicular neoplasm; (V) suspicious for malignancy; and (VI) malignant. An implied risk of malignancy (ROM) for each of six categories has been updated based on extensively published data since the second edition of TBSRTC in 2017 and offers both an average ROM for each category and the expected range of cancer risk. Estimated final ROM after excluding "Noninvasive Follicular Thyroid Neoplasm with Papillary Like Nuclear Features (NIFTP)" for each of six categories has been updated based on the reported mean decreases in the ROM if excluding NIFTP. For atypia of undetermined significance (AUS) category, the subcategorization is simplified and more formalized into 2 subgroups, AUS-nuclear atypia or AUS-other, based on the implied ROM and molecular profiling. For the pediatric thyroid disease, pediatric ROMs and management algorithms are newly added for the same six reporting categories for this age group. New or revised disease nomenclatures including high-grade follicular-derived carcinoma has been updated according to the recently published 2022 World Health Organization Classification of Thyroid Neoplasms. Brand new two chapters are added including clinical perspectives and imaging studies (Chap. 13) and the use of molecular and other ancillary tests (Chap. 14). The atlas is updated with new images to illustrate more effectively for new disease entity and diagnostic criteria.

Grounded Theoretical Analysis on the Hospital Accreditation Experience of Head Nurses in General Hospitals (종합병원 수간호사의 의료기관평가 경험에 대한 근거이론적 접근)

  • Purpose: The purpose of this study was to conduct a grounded theoretical analysis on the hospital accreditation experience of head nurses in order to understand their behavior on the adaption of this new system. Methods: The participants were 8 head nurses with more than 3 years of experience. The data were collected through in-depth interviews using audiotape recording analyzed by the constant comparative method described in Strauss and Corbin's methodology. Results: There were 113 concepts, 26 subcategories and 12 categories identified through the open coding process. In the axial coding, the following paradigm model was proposed: 1) the causal conditions were 'hardware problem' and 'software problem', 2) the contextual conditions were 'vertical relationship', 'individual preference', and 'family support', 3) the intervening conditions were 'passive conditions' and 'active conditions', 4) the action/interaction strategies were 'leading role' and 'dependent role', 5) the consequences were 'positive acceptance' and 'negative acceptance', 6) the central phenomenon was 'difficult situation' and 7) the core category was 'leading in harmony'. Conclusion: The new system led head nurses having difficulties as the middleman between the hospital administration and general nurses, but they made a continuous effort to overcome and adapt to it through a number of strategies.
