Implication of Polymorphisms in DNA Repair Genes in Prognosis of Hepatocellular Carcinoma

  • XRCC1 genetic polymorphisms could be associated with increased risk of various cancer, including hepatocellular carcinoma (HCC), the fifth most common cancer. We here conducted a study to explore the role of selective SNPs of the XRCC1 and XPD genes in the prognosis of HCC. A total of 231 cases were collected, and genotyping of XRCC1 Arg194Trp, XRCC1 Arg399Gln, XPD Lys751Gln and XPD Asp312Asn was performed by duplex polymerase-chain-reaction with the confronting-two-pair primer method. Our findings indicated XRCC1 399Gln/Gln genotype was associated with a significant difference in the median survival time compared with patients carrying Arg/Trp and Arg/Arg genotypes, and individuals with XPD 751 Gln/ Gln genotype had a significantly greater survival time than patients carrying Lys/Lys and Lys/Gln genotypes. The Cox's regression analysis showed individuals carrying XRCC1 399Trp/Trp genotype had 0.55 fold risk of death from HCC than Arg/Arg genotype. Similarly, XPD 751Gln/Gln had a strong decreasein comparison to XPD Lys/Lys carriers with an HR of 0.34. These results suggest that polymorphisms in XRCC1 and XPD may have functional significance in the prognosis of HCC.

세포분열에 관여하는 인간의 CIP29/Hcc1 유전자와 상동성을 가지는 분열형 효모의 새로운 유전자 mas1+의 특성분석 (Isolation and Characterization of mas1+ of Schizosaccharomyces pombe, a Homologue of Human CIP29/Hcc-1 Involved in the Regulation of Cell Division)

  • 세포주기조절에서 유전자 발현의 조절은 매우 중요한 부분이다. 본 연구에서는 인간의 유전자인 CIP29/Hcc1과 상동성을 가지는 분열형 효모의 새로운 유전자 mas1+을 분리하였다. 중합효소연쇄반응을 수행하여 cDNA를 얻고 이 cDNA의 염기서열을 분석한 결과 mas1+의 전체 염기서열은 735 bp로서, 245개의 아미노산을 암호화하고 있다. mas1+의 프로모터에서는 M-G1에 특이적인 전사를 보이는 유전자들에 보존되어 있는 PCB 서열이 발견되었다. 세포주기별 mas1+의 전사 수준을 분석한 결과 격막이 형성된 세포수의 빈도를 나타내는 격막 세포지표 의 양상과 유사하게 발현하는 것을 확인하였다. mas1+ 결손 돌연변이를 $25^{\circ}C$$36^{\circ}C$에서 배양한 결과, 세포질 분열과정이 늦어진 다중격막 세포의 빈도가 증가하였다. 이를 FACS로 분석하여 DNA 함량이 2C, 4C와 6C등이 형성됨을 확인하였다. mas1+결손 돌연변이 세포를 질소 결핍 배양액에서 배양한 결과 다중격막 세포의 형성이 확연히 증가하였는데 이는 질소 결핍에 따른 세포분열의 가속화 단계에서 mas1+의 결손이 특히 부정적 영향을 초래함을 시사한다. mas1+ 유전자 결손 돌연변이 세포에 mas1+을 포함한 plasmid를 형질전환한 후 mas1+의 발현을 유도한 결과 정상의 세포 형태로 전환됨을 확인하였다. Mas1 단백질에 EGFP를 융합시켜 발현을 유도한 결과 핵내에서 위치함을 분열형 효모와 인간 배양세포인 HeLa에서 확인하였다. 또한, mas1+ 결손 돌연변이에서 상동성을 가지는 인간 유전자 CIP29/Hcc1을 발현시킨 결과 multi-septate 세포가 줄어들었다. 한편, 생쥐의 배발달 단계에 따른 CIP29 유전자의 전사체 수준은 세포 분열이 활발한 시기에 증가하였다. 이상의 결과들은 Mas1은 인간의 핵단백질인 유전자 CIP29/Hcc1과 구조 기능적으로 상동성을 가지며, 세포주기 중 M-G1에 속하는 세포질 분열에 연관되어 있음을 시사한다.

Association Analysis of SERPINB5 Polymorphisms with HBV Clearance and HCC Occurrence in a Korean Population

  • Serpin peptidase inhibitor, Clade B (ovalbumin), Member 5 (SERPINB5), also known as maspin, is a potent tumor suppressor gene. It has correlations with many tumor cells, from pancreas cancer to breast cancer, so it is possible that it may also affect liver cancer. There has also been a report that SERPINB12, a gene placed right next to SERPINB5, is expressed in liver. For this study, 32 polymorphisms were identified in SERPINB5 by direct DNA sequencing, and 11 of them were selected to be tested with a larger scale subjects. The association of the 11 SERPINB5 polymorphisms with Hepatitis B virus (HBV) clearance, hepatocellular carcinoma (HCC) occurrence and the onset age of HCC were analyzed. There were no significant associations found between 11 SERPINB5 polymorphisms and HBV clearance. In the case of HCC occurrence, one of the haplotypes (ht) showed association with HCC occurrence (OR=2.26, p=0.005, $P^{Cor}=0.05$), albeit with a low statistical power (40.8%) and haplotype frequency (0.052). Further study with a bigger sample size will be needed to clearly verify the association between ht5 and HCC occurrence.

A formulated red ginseng extract inhibits autophagic flux and sensitizes to doxorubicin-induced cell death

  • Background: Ginseng is believed to have antitumor activity. Autophagy is largely a prosurvival cellular process that is activated in response to cellular stressors, including cytotoxic chemotherapy; therefore, agents that inhibit autophagy can be used as chemosensitizers in cancer treatment. We examined the ability of Korean Red Ginseng extract (RGE) to prevent autophagic flux and to make hepatocellular carcinoma (HCC) cells become more sensitive to doxorubicin. Methods: The cytotoxic effects of total RGE or its saponin fraction (RGS) on HCC cells were examined by the lactate dehydrogenase assay in a dose- or time-dependent manner. The effect of RGE or RGS on autophagy was measured by analyzing microtubule-associated protein 1A/1B-light chain (LC)3-II expression and LC3 puncta formation in HCC cells. Late-stage autophagy suppression was tested using tandem-labeled green fluorescent protein (GFP)-monomeric red fluorescent protein (mRFP)-LC3. Results: RGE markedly increased the amount of LC3-II, but green and red puncta in tandem-labeled GFP-mRFP-LC3 remained colocalized over time, indicating that RGE inhibited autophagy at a late stage. Suppression of autophagy through knockdown of key ATG genes increased doxorubicin-induced cell death, suggesting that autophagy induced by doxorubicin has a protective function in HCC. Finally, RGE and RGS markedly sensitized HCC cells, (but not normal liver cells), to doxorubicin-induced cell death. Conclusion: Our data suggest that inhibition of late-stage autophagic flux by RGE is important for its potentiation of doxorubicin-induced cancer cell death. Therapy combining RGE with doxorubicin could serve as an effective strategy in the treatment of HCC.

Targeting SHCBP1 Inhibits Cell Proliferation in Human Hepatocellular Carcinoma Cells

  • Src homology 2 domain containing (SHC) is a proto-oncogene which mediates cell proliferation and carcinogenesis in human carcinomas. Here, the SHC SH2-domain binding protein 1 (SHCBP1) was first established to be up-regulated in human hepatocellular carcinoma (HCC) tissues by array-base comparative genome hybridization (aCGH). Meanwhile, we examine and verify it by quantitative real-time PCR and western blot. Our current data show that SHCBP1 was up-regulated in HCC tissues. Overexpression of SHCBP1 could significantly promote HCC cell proliferation, survival and colony formation in HCC cell lines. Furthermore, knockdown of SHCBP1 induced cell cycle delay and suppressed cell proliferation. Furthermore, SHCBP1 could regulate the expression of activate extracellular signal-regulated kinase 1/2 (ERK1/2) and cyclin D1. Together, our findings indicate that SHCBP1 may contribute to human hepatocellular carcinoma by promoting cell proliferation and may serve as a molecular target of cancer therapy.

Hypermethylation of Suppressor of Cytokine Signaling 1 in Hepatocellular Carcinoma Patients

  • Hepatocellular carcinoma (HCC), the most common primary hepatic tumor, is highly prevalent in the Asia-Pacific region, including Thailand. Many genetic and epigenetic alterations in HCC have been elucidated. The aim of this study was to determine whether aberrant methylation of the suppressor of cytokine signaling 1 gene (SOCS1) occurs in HCCs. Methylation specific-PCR assays were performed to identify the methylation status of SOCS1 in 29 tumors and their corresponding normal liver tissues. An abnormal methylation status was detected in 17 (59%), with a higher prevalence of aberrant SOCS1 methylation significantly correlating with HCC treated without chemotherapy (OR=0.04, 95%CI=0.01-0.31; P=0.001). This study suggests that epigenetic aberrant SOCS1 methylation may be a predictive marker for HCC patients.

Differential Expression of IQGAP1/2 in Hepatocellular Carcinoma and its Relationship with Clinical Outcomes

  • Purpose: To investigate IQGAP1 and IQGAP2 expression in hepatocellular carcinoma (HCC) and itsassociation with HCC clinicopathological characteristics and survival outcomes. Methods: IQGAP1 and IQGAP2 mRNA and protein were measured in HCC tissues, para-tumor tissues and normal tissues by RT-PCR and Western blotting. We further examined 150 HCC samples with adjacent para-tumor tissues and 11 normal specimens by immunohistochemistry to evaluate the correlation of IQGAP1 and IQGAP2 with clinicopathological features and prognosis. Results: IQGAP1 mRNA and protein were up-regulated while IQGAP2 mRNA and protein were down-regulated in human HCC tissues compared with para-tumor and normal liver tissues (p<0.05). IQGAP1 expression was higher in primary HCC (122/150, 81.3%) than matched adjacent tissues (30/150, 20%, p<0.001), whereas IQGAP2 was lower (31/150, 20.7% as compared to 112/150, 74.7%, P<0.001). Positive IQGAP1 expression correlated with larger tumor size (p=0.002), advanced TNM stage (p=0.002) and tumor differentiation (III and IV, p=0.034). Negative IQGAP2 expression was significantly associated with larger tumor size (p=0.009), multicentric tumor occurrence (p=0.01), advanced TNM stage (0.009) and tumor differentiation (III and IV, p=0.020). Survival analysis revealed that patients with either IQGAP1+ or IQGAP2-tumors had significantly reduced disease-free survival (p<0.001 and 0.006 respectively) and overall survival (p<0.001 for both). Multivariate analysis showed that IQGAP1/2 switch was an independent prognosis factor for disease-free survival (HR=2.824) and overall survival (HR=2.189). Conclusion: Positive IQGAP1 and negative IQGAP2 expression were closely correlated with tumor progression and could be used as adjunctive biomarkers to improve prognostication for HCC patients.

Decreased Expression of LKB1 Correlates with Poor Prognosis in Hepatocellular Carcinoma Patients Undergoing Hepatectomy

  • Aim: To study any correlation of LKB1 expression with prognosis in hepatocellular carcinoma (HCC) cases. Methods: A total of 70 HCC patients and 20 primary intrahepatic stone patients in the first affiliated hospital of Wenzhou Medical College were enrolled in this study. LKB1 expression was detected by immunohistochemistry. Patients were followed-up and prognostic factors were evaluated. Result: LKB1 expression was decreased in the HCC samples. Loss of LKB1 expression in HCC was significantly related to histologic grade (P=0.010), vascular invasion (P=0.025) and TMN stage (P=0.011). Patients showing negative LKB1 expression had a significantly shorter disease-free and overall survival than those with positive expression (P = 0.001, P=0.000, respectively). Multivariate Cox regression analysis indicated that LKB1 expression level was an independent factor of survival (P = 0.033). Conclusion: HCC patients with decreased expression LKB1 have a poor prognosis. The loss of LKB1 expression is correlated with a lower survival rate.

전산화단층촬영 영상에서 통계적 특징을 이용한 질감특징분석 알고리즘의 적용: 간세포암 중심으로 (Application of Texture Feature Analysis Algorithm used the Statistical Characteristics in the Computed Tomography (CT): A base on the Hepatocellular Carcinoma (HCC))

  • 본 연구는 전산화단층촬영에서 간 질환의 자동 인식으로 질감특징분석(texture feature analysis. TFA) 알고리즘을 제안하고자 하였으며, 간세포암(Hepatocellular carcinoma. HCC)에 대한 컴퓨터보조진단(computer-aided diagnosis. CAD) 시스템을 설계하고, 제안하는 각 알고리즘의 성능을 평가하고자 하였다. HCC 영상에서 분석영역($40{\times}40$ 픽셀)을 설정하고 각 부분영상에 통계적 특징을 이용한 6가지 TFA 파라메터(평균 밝기, 평균 대조도, 평탄도, 왜곡도, 균일도, 엔트로피)비교하여 간세포암 인식률(recognition rate)을 구하였다. 결과적으로 TFA는 간세포암 인식률을 나타내는 척도로 유의함을 알 수 있었으며 6가지 파라메터에서 균일도가 가장 인식률이 높았으며 평균 대조도, 평탄도, 왜곡도가 비교적 높았고 평균 밝기와 엔트로피는 상대적으로 낮은 인식률을 나타내었다. 이와 관련하여 높은 인식률을 보인 알고리즘(최대 97.14%, 최소 82.86%)을 간세포암 영상의 병변을 판별하여 임상의 조기 진단을 보조하여 치료를 시행한다면 진단의 효율성이 높아 질 것으로 판단되었으며, 향후 효율적이고 정량적인 분석을 추가함으로써 질병인식의 일반화에 대한 기준 연구가 필요 할 것으로 사료되었다.

LI-RADS version 2018 in Patients with Prior History of Hepatocellular Carcinoma: Are LR4 Observations Enough for the Diagnosis of Recurrent HCC?

  • Purpose: We evaluated the diagnostic performance of LI-RADS version 2018 using gadoxetic acid enhanced MRI for recurrent but untreated HCC in patients with prior history of HCC. Materials and Methods: We enrolled 50 consecutive patients who 1) prior history of treatment of HCC, 2) underwent liver surgery for radiological/clinical diagnosis of new HCC between 2013 to 2018, 3) had gadoxetic acid enhanced MRI within one month before surgery, and 4) did not have more than five HCCs or infiltrative tumors only. Two radiologists reviewed MRI and determined the presence of LR3, LR4 and LR5 observations except previously treated tumors based on LI-RADS version 2018 in consensus. We sub-classified LR4 into LR4m (LR4 with major features only) and LR4u (LR4 upgraded from LR3 by ancillary features). LR4u were further sub-classified into LR4ua (with arterial phase hyperenhancement) and LR4un (without arterial phase hyperenhancement). Results: PPV for LR5, LR4 and LR3 observations for recurrent HCC were 100%, 61.5% and 25.0%, respectively. 100% (3/3) of LR4m were HCC. However, PPV of LR4u was 56.5%. PPV of LR4ua and LR4un were 73.3% and 25.0%, respectively. Sensitivity of LR5 and LR5+LR4 observations as a diagnostic threshold were 32.1% and 89.3%, respectively. Sensitivity for LR5+LR4m+LR4ua observations for diagnosis of HCC were 83.7% and significantly superior to that of LR5 without significant deterioration of specificity (75.0%). Conclusion: In patients with prior history of HCC, LR4 observations by major features or with APHE may be regarded as recurrent HCCs given high sensitivity and comparable specificity/PPV to LR5 observations.
