Follow-up of Children with Chronic Hepatitis B Virus Infection (B형 간염 바이러스 만성 감염 소아의 추적 관찰)

  • Purpose : The serial clinical findings, biochemical results, and serological hepatitis B virus(HBV) markers in Korean children with chronic HBV infection were analyzed to determine the relationships among these factors. Methods : Ninety children have been chosen from those who have visited to the Department of Pediatrics at St. Vincent's Hospital in The Catholic University of Korea from July 1st, 1995 to June 30th, 2000. The sample patients were followed up for over six months. HBV markers and liver function tests were all performed. Results : All children were asymptomatic at presentation. Eighty-three percent of the children had a history of chronic HBV infection in their families. Eighty-one percent were HBeAg positive, 16% were anti-HBe positive, while 3% were all HBeAg and anti-HBe negative. The prevalence of HBeAg among three age groups : 0~5; 6~10; and 11~15 year-old was 90%, 96% and 61% respectively. The prevalence of HBeAg in less than 10 year-old group was significantly higher than 11~15 year-old group(P=0.001). Serum ALT levels were within 40 IU/L in 64% children, 41~80 IU/L in 17%, 81~200 IU/L in 10%, and beyond 201 IU/L in 9%. The percentage of abnormality of ALT levels in HBeAg positive patients was significantly higher than that of HBeAg negative(P=0.036). Eleven of the 73 HBeAg positive children lost their HBeAg and seroconverted to anti-HBe. In these cases, all had transient elevations in ALT levels before HBeAg seroconversions. The annual rates of spontaneous seroconversion of HBeAg and HBsAg were 9.7% and 0.6%, respectively. Conclusion : Recognition of the dynamics of these changes in viral markers and biochemical findings is needed in the selection and evaluation of therapeutic regimens, establishment of treatment, and calling for controlled trials with adequate follow-up. The hepatitis B carrier state may be asymptomatic in children however, continued surveillance of carriers is important to determine the individual adverse prognostic factors of chronic HBV infections.

Comparision of Hepatitis B Virus Markers in the Serum and the Cerebrospinal Fluid (혈청과 뇌척수액에서 B형 간염 바이러스 표식자 존재의 비교)

  • We investigated HBV markers in serum and cerebrospinal fluid of 50 subjects with neurologic disorders or other disorders, who visited Dept. of neurology, college of medicine, Yeungnam University, from April-1 to August-31 1994 and were performed cerebrospinal fluid analysis to investigate the detection rate of HBV markers in cerebrospinal fluid and the possibility of neurologic disorders associated with HBV infection. The results were as follows. The positivity of HBsAg and HBV prevalence rate in serum were 6(12.0%) and 37(74.0%). The number of patient with HBsAg, only anti-HBV and no markers were 6(12.0%), 31(62.0%) and 13(26.0%), respectively. The positivity of HBsAg and HBV prevalence rate in cerebrospinal fluid were 3(6%) and 18(36.0%). The number of patient with HBsAg, only anti-HBV and no markers were 6(100.0%), 12(38.7%) and 0(0.0%) respectively. The number of patient with virus associated diseases(VAD) and non virus associated diseases(NVAD) were 26(52%) and 24(48%). The HBV prevalence rate in serum of VAD and NVAD groups were 88.5% and 58.3% (p<0.05). The HBV prevalence rate in CSF of VAD and NVAD groups were 53.8% and 16.7%(P<0.05). The HBV prevalence rate in serum and CSF of VAD and NVAD groups were 60.9% and 28.6%. Aa a conclusion, the HBV markers in the CSF were partially detected at the presence of the HBV markers in serum. The prevalance rate of HBV in the CSF was increased at the HBsAg positive in the serum and the serum and the CSF was significantly increased at the VAD group than the NVAD group.

Survey of Hepatitis B Surface Antigen in the Sera of Healthy Adults in Seoul and Gyunggi-Do (서울 및 경기도 지역 주민에서의 B형간염항원에 대한 조사(제 4 보))

  • To investigate the annual distribution of hepatitis B surface antigen(HBs Ag) in healthy adults, this study was performed by means of counter-immunoelectrophoresis with sera obtained from 4,856 healthy adults who reside in Seoul and Gyunggi-Do area from January to December, 1981. The experimental procedures were same with the previous reports. The subjectives were consisted of two groups: one is mass life group(3.853 adults) and the other, home life group(1.003 adults). The results were as follows: 1. Among the 4.856 cases tested, 326 cases(6.71%) showed HBs Ag positive. HBs Ag positive rates in the group of mass life and home life were 6.51% and 7.48%, respectively. 2. It showed slightly high positive rates in the home life and long term duration group, and upper class group showed slightly high positive rate. 3. In seasonal variation, it showed relatively high positive rate(7.38%) in winter, and A blood group showed higher rate(7.60%) than the other blood groups. And then, boundary area of Seoul city showed slightly high positive rate(7.36%) in areal distribution.

The Correlation between ALT (Alanine Aminotransferase) Value and Prevalence of Anti-HEV (ALT (Alanine Aminotransferase) 검사성적과 E형 간염항체 양성률간의 관련성)

  • In order to elucidate the prevalence of anti-HEV, anti-HCV and HBsAg in ALT (Alanine Aminotransferase)-elevated patient group and healthy person control group, we examined anti-HEV, anti-HCV and HBsAg in 92 persons, respectively. The prevalence of anti-HEV in ALT-elevated patient group (4.3%) was not significantly higher than that (2.2%) of healthy person control group(P>0.05). While in healthy person control group the prevalence of anti-HEV in female (2.4%) was higher than that (2.0%) in male, and in ALT-elevated group male (4.8%) was higher than female (3.3%), there were no significances. The positive rate of anti-HEV was not significantly increased with age between healthy person control and ALT-elevated patient group. The odds ratio's of HEV, HCV and HBV with ALT-elevated patient group were 2.05 (95% CI: 0.31-16.54), 8.67 (95% CI: 1.06-188.63) and 7.23 (95% CI: 2.47-22.71), respectively, but had no significance. It was turned out that HCV and HBV were significantly correlated with ALT-elevated patient group, but HEV had no significance with it in this paper.

Production of Recombinant Humanized Anti-HBsAg Fab Fragment from Pichia pastoris by Fermentation

  • In this report, we describe the high-yield secretory expression of the recombinant human anti-HBsAg Fab fragment from Pichia pastoris that was achieved by co-integration of the genes encoding the heavy and light chains (both under the control of alcohol oxidase promoter) into the genome of the yeast cells. The fed-batch fermentations were carried out in a 5 L scale. Both chains of the Fab were successfully expressed upon methanol induction. The absorbance ($OD_{600}$) of the broth can reach 350~500 at the end of fed-batch phase. After the induction, the expression level of the recombinant Fab (soluble) reached 420~458 mg/L. The recombinant Fab fragment was purified from the crude culture supernatant by ion exchange chromatography and the purity of the recombinant Fab fragment was over 95%. The affinity activities of the crude fermentation supernatant and the purified Fab were analyzed by indirect ELISA, which showed that the purified recombinant Fab fragment had high affinity activity with hepatitis B surface antigen.

Prevalence of Anti-HCV among the Health-checkup Adults in Jeonbuk Province (전북 지역 건강 검진자들의 Anti-HCV 양성률 조사)

  • The author was performed to investigation of current status of prevalence for anti-hepatitis C virus (HCV) among the health-checkup adults in Jeonbuk province. A toal of 1,553 (male 1,046, female 507) serum samples were diagnosed by 3rd generation enzyme immunoassay (EIA) for anti-HCV. Total prevalence of anti-HCV was 0.9%, and prevalence of male and female were 0.8% and 1.2%, respectively. The prevalence of female was higher than male. According to ages group, prevalence of anti-HCV was highest in 60 age group, but it was not found in 20 age group. 14 samples with anti-HCV positive were diagnosed by EIA for hepatitis B virus surface antigen (HBs Ag), by chemiluminescence immunoassay (CLIA) for serum albumin, alanine transaminase (ALT) and asparagine transaminase (AST). Positive for HBs Ag was not found. The mean of serum albumin levels was 4.5 g/dL, and mean of ALT and AST were 34.3 IU and 31.9 IU, respectively. Through this study, I know that the prevalence of anti-HCV among adults in Jeonbuk, and suggest that the positive of anti-HCV persons who have lower serum albumin, normal to mild elevations in serum enzymes are chronic hepatitis.

Expression of recombinant protein from Oenanthe javanica DC. leaf tissues as a biofactory (Biofactory로서의 미나리 엽조직을 이용한 재조합단백질 발현)

  • Fresh Oenanthe javanica DC. leaves still attached to stem architecture were immersed in NaOH solution for 3 min before agroinfiltration and co-cultivation. MTT assay revealed that NaOH solution containing up to 0.7% was still safe for the leaf viability. Fluorometric GUS enzyme analysis showed that 0.5% NaOH-treated leaf tissues were efficiently transformed by vacuum infiltration for 20 min with Agrobacterium cells at a density of $OD_{600}=0.5$ to 1.0. These conditions worked well for the expression of HBsAg, which was confirmed by western blotting and ELISA.

Long Term Follow Up of Interferon-alpha Treatment in Children with Chronic Hepatitis B (만성 B형간염 환아에 대한 Interferon-alpha 치료결과의 장기 추적관찰)

  • Purpose: We tried to evaluate the long term efficacy and positive predictive factors of interferon-alpha treatment in children with chronic hepatitis B. Methods: The study population included 113 children who received interferon therapy between May 1982 and July 2002 (20 years) for chronic hepatitis B in Department of Pediatrics, Yonsei University College of Medicine. Male to female ratio was 2.3 : 1 and the mean age at diagnosis was $11.1{\pm}4.1$ years old. Response to treatment was defined as normalization of alanine aminotransferase (ALT), disappearance of HBeAg and HBV-DNA Eighty two children responded while 32 did not. Interferon-alpha was given intramuscularly for 6 months at a dosage of $3{\times}10^6$ unit, 3 times weekly. In relapsed cases, lamivudine or interferon retreatment was done. Results: Seroconversion rate was 77.0% in terms of HBeAg, 74.3% in terms of HBV-DNA, and 80.5% in terms of ALT normalization after treatment. Seroconversion rate of both HBeAg and HBV-DNA was 72.6%. Analyzed by life table method, the effect of the treatment had been maintained over 10 years after cessation of therapy. Pre-treatment ALT level was the only significant positive predictive factor of response. Eleven cases (13.4%) relapsed, and 2 out of 3 showed response when treated with lamivudine and 1 out of 3 with interferon retreatment. Conclusion: Interferon-alpha showed significant efficacy in the treatment of chronic hepatitis B in our study. Further studies about the effect of interferon therapy on complications of hepatitis such as hepatocarcinoma, cirrhosis are warranted.
