Insulin/GLP-1 Treatment for Patients with DM

  • Combining basal insulin therapy with a glucagon-like peptide-1 receptor agonist (GLP-1 RA) has clear clinical advantages, and is supported by the latest EASD/ADA position statement (1). IDegLira is a once-daily combination of the basal insulin, degludec, and the GLP-1RA, liraglutide, in one pen. The DUAL phase 3 clinical trial program provides important evidence about the efficacy and safety of IDegLira in three different populations of patients with type 2 diabetes (T2D): insulin naïve subjects uncontrolled on oral antidiabetic drugs (OADs), subjects uncontrolled on OAD(s) and a GLP-1 RA, and subjects uncontrolled on OAD(s) and basal insulin. Treatment with IDegLira reduced mean HbA1c to below the EASD/ADA treatment target of 7.0% in all five trials. The mean reduction of HbA1c from baseline ranged from 1.3% and 1.9%. IDegLira resulted in weight loss for subjects uncontrolled on basal insulin, was weight neutral for subjects on OADs and weight gain was minimal (2 kg) for subjects previously treated with a GLP-1 RA. Rates of hypoglycaemia were low across all the trials, particularly considering the level of glycaemic control achieved.

The Antidiabetic Effects of Black Ginseng Extract(BGE)and Geumsan Black Ginseng 05-FF(GBG05-FF) on In Vitro and In Vivo Assay (흑삼추출물(BGE)과 금산흑삼표준화소재(GBG05-FF)의 In Vitro와 In Vivo상에서의 항당뇨효과)

  • The prevalence of diabetes mellitus continues to rise alarmingly because of industrialization of society. The aim of this study was to investigate the antidiabetic effects of black ginseng extract (BGE) which was performed panax ginseng. First we examined the inhibitory effects of BGE on ${\alpha}$-glucosidase. But there was no practical effects in our observations. We, also, investigate the effects of BGE on glucose uptake of skeletal muscle using 2-NBDG in $C_2C_{12}$ myotube. BGE significantly improved the glucose uptake considered as a lowered blood glucose level. Effects of GBG05-FF on fasting blood glucose and glycated hemoglobin (HbA1c) were investigated in streptozotocin (STZ)-induced diabetic mouse. After injection of STZ, fasting blood glucose and glycated hemoglobin rapidly increased. But STZ-induced diabetic mouse treated with GBG05-FF significantly reduced the level of fasting blood glucose and HbA1c. This results showed that supplementation of BGE improve the diabetic parameters and BGE have a potentiality as a functional food for Diabetes mellitus.

Improvement of Lipid Metabolism and Antihyperglycemic by Lentinus edodes in High Fat-fed and Streptozotocin-treated Rats (고지방과 streptozotocin으로 유도한 제 2형 당뇨에서 표고버섯이 지질대사와 항당뇨 효능에 미치는 영향)

  • Abnormal regulation of glucose and impaired lipid metabolism that result from a defective or deficient insulin are the key etiological factor in type 2 diabetes mellitus (T2DM). The our study evaluated the beneficial effect of diet supplementation with Lentinus edodes on hyperglycemia and lipid metabolism in normal and type 2 diabetic rats. The animals were divided into 4 groups: group I(control) rats were fed standard diet (12% of calories as fat); group II (T2DM) rats were fed HFD (40% of calories as fat) for 2 weeks and then injected with STZ (50 mg/kg); group III and group IV rats were continually fed a diet containing 1% and 10% Lentinus edodes for 4 weeks after T2DM induction, respectively. After 4 weeks we determined biochemical parameters such as glucose, insulin concentration, serum total cholesterol (TC), triglycerides (TG), high-density lipoprotein (HDL), and glycosylated hemoglobin (HbA1c) concentration were also measured. There was a significant reduction in serum TC and TG in the Lentinus edodes supplement groups. The Lentinus edodes diet supplementation were found to have a potent lipid metabolism improvement as well as LDL concentration decreased and HDL concentration was increased. Concentrations of blood glucose and HbA1c in the experimental groups II were significantly decreased after 4 weeks compared with the control group. The Lentinus edodes diet supplementation is useful in regulating the glucose level, improves the insulin, HbA1c, serum lipid metabolism in experimental diabetic rats. We suggest that Lentinus edodes supplementation may have the control effects of diabetes mellitus by improving blood glucose control and lipid metabolism.

  • Objects: This study is a descriptive study to identify the factors influencing the glycated hemoglobin control in type 2 diabetes patients. Methods: This study used raw data from the third year of the National Health and Nutrition Examination Survey performed from 2015. The subjects were 285 people who were classified as type 2 diabetes patients among the 7,380 participants of the National Health and Nutrition Examination Survey. Findings: The factors influencing the glycated hemoglobin control in type 2 diabetes patients were found to be age, education level, prevalence period and sleeping time. Improvements: In type 2 diabetes patients, it is very important to have appropriate education on diabetes when they discovered the disease morbidity in the middle age rather than in the aged period and to have enough sleeping time. A practical program to help this education and training should be developed.

Effect and Safety of Replacement Therapy with Insumon in Type 2 Diabetes Mellitus (제2형 당뇨병 환자에 대한 대체 요법 [인슈몬]의 효과 및 안전성)

  • This research was concentrated to Investigate the effects of Insumon as a newly developed dietary supplement in patients with type 2 diabetes. The average glycated hemoglobin level was dropped from 7.0${\pm}$1.1% to 6.7${\pm}$0.9% after 8 weeks with statistic significance (p=0.0202). For patients of HbA1c over 7%, the average HbA1c significantly decreased from 8.1${\pm}$0.8% to 7.5${\pm}$0.8% (p=0.0171). The average fasting blood glucose level was decreased significantly from 9.5${\pm}$1.2 mmol/L (171.6${\pm}$22.3 mg/dL) to 8.5${\pm}$1.5 mmol/L (152.3${\pm}$26.3 mg/dL) (p=0.0262) after 4 weeks and to 8.8${\pm}$1.5 mmol/L (158.3${\pm}$24.1 mg/dL) (p=0.0445) after 8 weeks in the group of patients of FBG of over 7.8 mmol/L (140 mg/dL). The average HDL cholesterol level was increased significantly after 8 weeks from 1.2${\pm}$0.2 mmol/L (46.5${\pm}$8.6 mg/dL) to 1.3${\pm}$0.3 mmol/L (51.6${\pm}$10.9 mg/dL) (p=0.0007). Thus, these results suggested that patients with type 2 diabetes might benefit by Insumon as a herbal dietary supplement without any serious side effects.

Dietary Quality and Self-Management Status according to the Glycemic Control in the Elderly with Type 2 Diabetes (노인 당뇨환자에서 혈당조절에 따른 식사의 질과 당뇨자가관리상태)

  • Evidence for the effects of different health behaviors, including diet, in elderly diabetes is currently limited. The aim of this study was to compare the quality of diet and health behaviors in Korean elderly T2DM patients, using a glycemic control. T2DM elders(>65 yr, n=48) were recruited and categorized by the concentration of glycated-hemoglobin HbA1c; subjects with HbA1c<7% were the good control(GC) group, and subjects with $HbA1c{\geq}8%$ constituted the poor control(PC) group. General characteristics, self-management behavior questionnaires, and 3-d diet records were all collected and assessed. No significant differences in general characteristics between GC and PC were detected, with the exception of a higher level of education in GC(p<0.05). A twofold longer duration of diabetes was observed in PC as compared to GC(p<0.01). The GC group did exercise for a longer time(p<0.001), and had an earlier beginning of diabetes self-management education (DSME) by healthcare practitioners using a team teach as compared with the PC group(p<0.05). The total dietary quality index(p<0.001) and individual index for carbohydrate(p<0.001) or vegetables and fruit(p<0.05) were better in GC than in PC. Therefore, the earlier DSME including intensive exercise and balanced diet selection should be expected to improve glycemic control in diabetic Korean elders.

Dapagliflozin's Effects on Glycemia and Cardiovascular Risk Factors and Incidence of Adverse Events in Patients with Type 2 Diabetes (제2형 당뇨병 환자에서 Dapagliflozin이 혈당과 심혈관계질환 위험인자에 미치는 영향 및 안전성)

  • Background: Dapagliflozin is an oral selective inhibitor of sodium-glucose cotransporter 2(SGLT2), the kidney transporter chiefly responsible for glucose reabsorption from the glomerular filtrate. Because this mechanism does not require the action of insulin, dapagliflozin rarely causes hypoglycemia. Dapagliflozin may affect blood glucose control as well as blood pressure and the body weight which are one of the cardiovascular disease risk factors. However, dehydration and ketoacidosis are reported as the side effects of the dapagliflozin treatment and the safety issues have been occurred. The aim of this study is to analyze the effectiveness and adverse events of dapagliflozin in Korean patients. Methods: From December 2014 to August 2015, we retrospectively reviewed the electronic medical records of type 2 diabetes patients who were prescribed dapagliflozin at Severance Hospital. Results: A total of 202 Korean patients were enrolled in this study. The effectiveness in the reduction of blood glucose was statistically significant(p<0.001). Dapagliflozin decreased 0.74% of HbA1c after 24 weeks. Significantly more participants achieved the target HbA1c level(HbA1c<7%) after 24 weeks(n=42, 35.3%) than before taking dapagliflozin(n=21, 17.6%). Blood pressure decreased 5.7 mmHg systolic blood pressure(SBP), 1.9 mmHg diastolic blood pressure(DBP) after 24 weeks. More than one quarter of participants(n=35, 29.4%) experienced weight loss. Most common adverse event was genitourinary symptoms. Conclusion: In this study, the effectiveness of dapagliflozin in improving glycemic control, blood pressure control, and weight loss was statistically significant. However, elderly and female patients, who have higher incidence of adverse events, should use dapagliflozin cautiously.

Diabetes Mellitus and HbA1c Levels Associated with High Grade Prostate Cancer

  • Pirpose: The aim of this study to analyze the association between history of diabetes mellitus (DM) with risk of prostate cancer (PCa) and cancer grade among men undergoing radical prostatectomy for PCa. Materials and Methods: 50 patients with DM and 50 patients without DM who undervent radical prostatectomy (RP) were included in the study. Age at biopsy, height, weight, digital rectal examination (DRE), pre-biopsy PSA levels, prostate volume, histopathologic diagnosis after surgery and gleason scores were collected data from all patients. Histologic material obtained at biopsy was given a Gleason score; tumours with a Gleason score ${\geq}7$ were considered high grade and <7 were considered low grade. Results: The mean age at the time of biopsy was 63.7 in patients with DM and 61.6 in patients without DM. Diabetic men had significantly lower PSA levels (p=0.01). Mean PSA level $7.04{\pm}2.85$ in patients with DM and $8.7{\pm}2.86$ in patients without DM, respectively. Also, diabetic men had higher RP tumor grade than men without DM (p=0.04). We found that HbA1c levels were higher in patients who have high grade prostate cancer (p<0.05). Conclusions: Diabetic men undergoing RP have lower PSA levels and have significantly higher grade PCa. We must be careful for screening PCa in patients with DM. Although the patients had lower PSA levels, they might have high grade disease.

  • Korea augmentation satellite system (KASS) is the Korean satellite based augmentation system (SBAS) developed by ministry of land, infrastructure, and transport (MOLIT) since 2014. Since KASS is the safety critical system that can affect to the safety of airplane, the software of KASS is developed according to the DO178B software level induced from safety analysis. In case of KASS control station (KCS), most of the software of KCS get assigned software level E in DO178B. In that case, ECSS-Q-ST-80C category D is assigned as a software product assurance standard. In this paper, the software related standard ECSS-E-ST-40C, ECSS-Q-HB-80-04A are analyzed to satisfy ECSS-Q-ST-80C and as a result the software product assurance activities regarding software life cycle and the software quality model, metric is proposed for the product assurance of the KCS software.

Influence of periodontal treatment on the glucose and lipid metabolism in Korean type 2 diabetic patients (한국인 제2형 당뇨병환자에서 치주치료가 당질 및 지질대사에 미치는 영향)

  • The aim of this study was to appraise the influence of conventional periodontal treatment on metabolic control in Korean type 2 diabetic patients. In addition, their periodontal change was compared with non-diabetic patients. Before and after treatment, it was performed to measure periodontal and metabolic indices in thirteen type 2 diabetic patients. Periodontal indices included plaque index, gingival index, bleeding on probing, probing pocket depth, gingival recession, and clinical attachment level. Metabolic indices included glycated hemoglobin(HbA1c), fasting plasma glucose, fasting plasma insulin, total cholesterol, triglyceride, and HDL-cholesterol. Plaque index, gingival index, bleeding on probing, probing pocket depth, and gingival recession showed significant improvements in the statistics. Diabetic patients showed no statistically significant differences in the changes of periodontal indices compared with non-diabetic patients. HbA1c values decreased in five of the thirteen subjects and fasting plasma glucose levels were reduced in four of the seven subjects after periodontal treatment. All five subjects whom HOMA values were calculated in showed the increases of insulin secretions. The results of this study ascertained the possibility of the better glycemic contol after conventional periodontal treatment in Korean type 2 diabetic patients and diabetes were well healed of their periodontal diseases after the treatment.
