10 year follow up of a boy with Lesch Nyhan Syndrome

  • Lesch-Nyhan syndrome is a X-linked recessive disorder caused by a deficiency of the enzyme hypoxanthine-guanidine phosphoribosyltransferase (HPRT), enzyme to recycle purines. Case history: born induced vaginal delivery at 40 weeks complicated by premature membrane ruputure, body weight 2.820 gm. He showed failure to thrive showing severe protein aversion like milk products and pink daper. Developmental delay revealing rolling over at 10.5 month, followed by regression. Seizure at 2 months, His poor oral feeding was lifelong problem. Weak crying, spastic, choreoathetoid movement. Self mutilating behavior noted and diagnosed at age 3 years. No family history of consanguinity and neurological disorders. Method: Laboratory test, physical exam, imaging study and molecular. Clinical follow up Treat ment with allopurinol. Result: uric acid 10.5 mg/dL (N 3.5-7.9), APRT 151.1uM/ min/ml pro(25.7-101), HPRT 7.6 (N 233.5-701) and c.151C>T hemizygote (p,Arg51X). Abdominal sonogram showed staghorn calculi in both kidneys, brain MRI brain atrophy. Clinical follow up showed, seizure at 2 mo, developmental delay (head control and, rolling over at at 11mo, pointing body part at 2 yr 7 mo, eye hand coordination at 2 y 11mo,creeping at 3 y 7 mo, speaking words at 6 y 6 mo ),and developmental regression at 3 yr of age. Sleeping problem including insomnia and severe constipation. Self mutilating behavior (lip bite) started at 2.5 yr, neurologic sx including intermittent upward gaze accompanied by swallowing difficulty at 3 y 7 mo grand mal seizure at 4.5 yr and spastic extremity and trunchal hypotonia and choleoathetoid movement and ataxia at 6.5 yr. Scoliosis with severe spasticity at 9 yr 9 mo. Acute life threatening episode with irregular breathing at 9 yr and 9 mo, Emaciation and nephrolithiasis and recurrent pneumonia. Died suddenly at 10 yr 3 mo. Conclusion: life long feeding problem, chronic gut motility dysfunction, sleeping difficulty and progressing neurologic deterioration and nephrolithiasis despite normal serum uric acid maintence by allopurinol treatment.

Rapid Progression to Brainstem Encephalitis Caused by Enterovirus 71 Without Throat and Skin Lesions After a One-Day Fever

  • Infection with enterovirus (EV) 71 is usually associated with hand-foot-and-mouth disease and herpangina. The most frequent neurologic complication is brainstem encephalitis. A 30-month-old boy visited the pediatric emergency department with fever, lethargy, and abnormal eye contact. His mental status was slightly drowsy. On hospitalization day 2, the patient experienced respiratory arrest with apnea. Brain magnetic resonance imaging revealed bilateral symmetric T2-high signal lesions without enhancement in the posterior aspect of the brainstem and left medial temporal lobe. Electroencephalography was indicative of diffuse cerebral dysfunction with diffuse high amplitude and irregular delta activities. He underwent a gene study and was diagnosed with myoclonic epilepsy with ragged red fibers syndrome. We report a case of EV 71 brainstem encephalitis by polymerase chain reaction for nasopharyngeal aspirates and feces with rapid progression within one day of fever without the manifestation of throat and skin lesions because of his underlying mitochondrial disease.

Survey on Personal Medicines in Cheju Island (제주도 민간요법에 관한 조사연구)

  • The purpose is to inspect the personal medicines in Cheju island that are used traditionally and to get their characteristics. The subjectives are 39 Cheju people, men 10, women 29, who were horned, grown up in Cheju and agreed with this study. The ranges of age are 10 persons(m : 1, w: 9) over 40 less 50 years old, 14(m : 7, w : 7) over 50 less 60, 5(m : 1, w : 4) over 60 less 70, 6(m : 0, w : 6) over 70 less 80, and 4(m : 1, w : 3) over 80. The average age is 64.5 years old. The method to collect the data is 20 structured opening questionaires that are based on references. The duration to collect datas is 11days from 4th, Aprill 1997 to 14th, Aprill 1997. The workers who are trainned the interview methods went their villages and got answers after explaining the purpose and contents to them. Somtimes they used to record the answers. The analysis was identified the subjectives to four regions of Cheju, arranged answers with items, rearranged the same answers, counted with number and percentage. And classified the materials and characteristics. The results of this study are followed: The things that is used as personal medicines are the effects through experiences ans misteries that have hand down by word of mouth, even though they are not scientific. People used the materials arround their circumference. It is an accumulation of experiences. The ways used in eating, doing acupuncture or sting, exposing to smoke, wheedling, fixing after pounding. Almost materials are plants. Mugwort is effective in fever, gastritis, hemorrhoid, diarrhea, edematous hands or feet and dermatitis. Citron and Arrowroot in fever, gastritis. Seeds of Pumpkin in indigestive, hemorrhoid, edematous hands or feet. Gallic in fever, diarrhea, frostbite, dermatitis, and toothache. Motherwort in diarrhea, gastritis. Radish juice in indigestive, jaundice. Bean paste in burn, wound. Acupuncture in fever, gastritis, indigestive, back pain, edematous hands or feet. Sting as similar with it in fever, indigestive, edematous hands or feet. Cigarrette in hemorrhoid, wound, toothache. Cowstools in edematous hands or feet, wound. Sault is usded a lot in fever, gastritis, indigestive, hemorrhoid, uneffective voiding, edematous hands or feet, dermatitis, having a boil around the mouth, toothache, and eye disease. Japanese Parsley in fever, gastritis. Egg Apple in diarrhea, edematous hands or feet, frostbite. And Wild Chrysanthemum is effective in jaundice. In the conclusion, people used the things arround. A things is effective in several symptoms. If these are not effective, they would used the magic as god's anger. As locally, they used the grasses and fruits in the middle of Mt. Halla and seafood on the sea village.

THE COLOR COMPARISON OF VARIOUS RESIN CEMENTS WITH TRY-IN PASTES USED IN PORCELAIN LAMINATE VENEER (도재라미네이트에 사용되는 수종의 레진시멘트와 try-in paste의 색조 비교)

  • Statement of problem: The color change of final restoration resulting from resin cement is variable since the amount of the tooth is prepared is minimum. Purpose: The purpose of this study was to compare the color accuracy of try-in paste and their corresponding resin cements and the color stability of restoration intermediated resin cement according to two shades of three products(Rely-X. Variolink II. Choice). Material and method: Color was measured for porcelain disk sample without try-in paste, and porcelain disk sample intermediated by try-in paste and resin cement, then color differences were calculated. Results : 1. Color difference(${\Delta}E^*$) between try-in paste and resin cement showed the lowest value at Variolink II and increased in the order Choice and Rely-X statistically significant, and $?E^*$ of three resin cements was greater in A3 then A1. 2. The values of ${\Delta}L^*$ and ${\Delta}a^*$ between try-in paste and resin cement was in the increasing order of Variolink II, Choice and Rely-X. The ${\Delta}b^*$ value was increased in this order Choice, Rely-X and Variolink II in A1 on the other hand Variolink II, Rely-X and Choice in A3. 3. Color difference(${\Delta}E^*$) before and after resin cement showed the lowest value at Variolink II and increased in the order Rely-X and Choice. Conclusion : Above result revealed that the colors of try-in paste and resin cement were more or less accurate in the range of 0.35-1.95 of color difference which was unnoticeable with human eye, and Variolink II was superior to the other resin cements in color accuracy.

Morphological Variations in Relation to Geographical Distribution of Pond Smelt, Hypomesus olidus Pallas (한국산 빙어의 지리적 형태변이에 대하여)

  • The authors made an investigation concerning the geographical distribution and some major differentiations in morphological characteristics associated with the each habitat, of the pond smelt, Hypomesus olidus, which was introduced into Korea more than 50 years ago. Major characteristics utilized for the comparison study were upper jaw (maxillary), number of fin rays, scale structure, vertebral count, number of pyloric caeca, number of gill rakers, and the relative measurements of these and other internal characters. 1. The main areas of natural distribution are Paro Lake, Soyang Lake, Han River, Euirim Lake, and Ogjeong Lake (or Unam Lake) and the tributaries of these waters. 2. Morphological variations varied when the mean values of measurements from each area were compared with the standard measurement value by mosaic comparison, and in some instances a significant variation was found. 3. The most significant variations were found in the number of fin rays of pectoral and anal fins; the lengths from the tip of snout to the origin of pectoral fin, from snout to ventral fin, and from snout to anal fin, relative to standard body length; eye depth relative to head length; upper jaw: the number of pyloric caeca and the scale structure. Variations, on the other hand, by each habitat were not clear in the numbers of ventral and dorsal fin rays, gill rakers, vertebrae and lateral line scale : and the depth of caudal peduncle and distance from the tip of snout to the origin of dorsal fin relative to standard body length. 4. The scales of pond smelt showed some differences in the shape of scales, the shape and position of the focus and the number of ridges according to the scale positions on the body. No radii were present. The relations between longitudinal and transverse diameters of the scale suggest that the pond smelts of Soyang Lake and Un-am Lake are closely related, and those of Han River and Euirim Lake are also closely related. 5. The geographical variations in morphology of the pond smelt seem to have resulted from the variations in turbidity, water temperature, salinity (rather conductivity) and currents. 6. From the results obtained, it may be concluded that such factors as supramaxillary, relation between scale length and its breadth, number of caudal vertebrae and eye depth relative to head length may be used as the key characters for the classification of geographical varieties of Pond smelt.

Mathematician Taylor's Linear Perspective Theory and Painter Kirby's Handbook (수학자 테일러의 선 원근법과 화가 커비의 해설서)

  • In the development of linear perspective, Brook Taylor's theory has achieved a special position. With his method described in Linear Perspective(1715) and New Principles of Linear Perspective(1719), the subject of linear perspective became a generalized and abstract theory rather than a practical method for painters. He is known to be the first who used the term 'vanishing point'. Although a similar concept has been used form the early stage of Renaissance linear perspective, he developed a new method of British perspective technique of measure points based on the concept of 'vanishing points'. In the 15th and 16th century linear perspective, pictorial space is considered as independent space detached from the outer world. Albertian method of linear perspective is to construct a pavement on the picture in accordance with the centric point where the centric ray of the visual pyramid strikes the picture plane. Comparison to this traditional method, Taylor established the concent of a vanishing point (and a vanishing line), namely, the point (and the line) where a line (and a plane) through the eye point parallel to the considered line (and the plane) meets the picture plane. In the traditional situation like in Albertian method, the picture plane was assumed to be vertical and the center of the picture usually corresponded with the vanishing point. On the other hand, Taylor emphasized the role of vanishing points, and as a result, his method entered the domain of projective geometry rather than Euclidean geometry. For Taylor's theory was highly abstract and difficult to apply for the practitioners, there appeared many perspective treatises based on his theory in England since 1740s. Joshua Kirby's Dr. Brook Taylor's Method of Perspective Made Easy, Both in Theory and Practice(1754) was one of the most popular treatises among these posterior writings. As a well-known painter of the 18th century English society and perspective professor of the St. Martin's Lane Academy, Kirby tried to bridge the gap between the practice of the artists and the mathematical theory of Taylor. Trying to ease the common readers into Taylor's method, Kirby somehow abbreviated and even omitted several crucial parts of Taylor's ideas, especially concerning to the inverse problems of perspective projection. Taylor's theory and Kirby's handbook reveal us that the development of linear perspective in European society entered a transitional phase in the 18th century. In the European tradition, linear perspective means a representational system to indicated the three-dimensional nature of space and the image of objects on the two-dimensional surface, using the central projection method. However, Taylor and following scholars converted linear perspective as a complete mathematical and abstract theory. Such a development was also due to concern and interest of contemporary artists toward new visions of infinite space and kaleidoscopic phenomena of visual perception.

Associations between Insertion/deletion Polymorphisms in Galactose Mutarotaseand Carcass Traits in the Hanwoo Population of Jeju Island (제주도 한우 집단에서 Galactose mutarotase (GALM) 유전자의 삽입/결실 다형성과 도체형질의 연관성 분석)

  • This study examined the associations between the genotypes of the galactose mutarotase (GALM) gene and carcass traits in the Hanwoo population of Jeju Island, South Korea. The GALM genotypes were determined by the 14-bp (5'-GGTCTAATGACCAG-3') insertion/deletion (InDel) polymorphisms of the 3'-untranslated region (UTR). All three genotypes (LL, LS, and SS) were found in the Hanwoo steer population. The association analysis showed significant associations between genotypes and several carcass traits, including traits related to intramuscular fat content, such as meat quality, marbling score, and backfat thickness (p<0.05). Animals harboring the SS genotype showed not only higher levels of intramuscular fat content but also lower levels of backfat thickness than animals harboring the LL and LS genotypes. On the other hand, no significant associations were found between the GALM genotypes and carcass weight, eye muscle area, meat color, or fat color (p>0.05). Deleting the 14-bp segment in the 3'-UTR resulted in the modification of the secondary structure of RNA and appeared to affect gene expression by interfering with the binding ability of GALM mRNA with RNA-binding proteins and microRNAs. These results suggest that the 14-bp InDel polymorphism in the 3'-UTR region of the GALM gene affects cattle growth traits and carcass quality through galactose metabolism-mediated fat accumulation in muscle and backfat tissues.

Morphometric Characteristic between Diploid and Spontaneous Triploid Carp in Korea

  • We used flowcytometry to ploidy verification after that investigate difference between diploid and spontaneous triploid through the truss dimension and classical dimension at crucian carp, Carassius auratus, crucian carp C. cuvieri and common carp, Cyprinus carpio collected from Hangang river, Hantangang river, Imjingang river, Geumgang river, Yeongsangang river and Nakdonggang river, Korea. There were significant differences among the three species for the truss dimensions anterior origin of dorsal fin $(2){\times}$ anterior origin of anal fin (5), $2{\times}$ anterior origin of pelvic fin (6), $2{\times}$ origin of pectoral fin (7), posterior origin of dorsal fin $(3){\times}5$, $3{\times}6$, and $3{\times}7$ (P<0.05). There were no significant differences among the three species in the truss dimensions dorsal fin length ($2{\times}9$) and eye diameter (ED)(P>0.05). On the other hand, there were no significant differences in the several classical dimensions of each species (P>0.05). Three classical dimensions, most anterior extension of the head $(1){\times}2$, $1{\times}6$ and $2{\times}$ most posterior scale in lateral line (4) did not differ between the C. auratus diploid and spontaneous triploid (P>0.05). Two classical dimensions, $1{\times}6$ and longest length between most anterior extension of the head and gill cover ($1{\times}8$) did not differ between the C. cuvieri diploid and spontaneous triploid (P>0.05). One classical dimensions, $1{\times}2$ did not differ between the common carp diploid and spontaneous triploid (P>0.05). There were significant differences in the each diploid and triploid species (P<0.05). These results suggest that the classification of each species and classification between diploid and spontaneous triploid morphometrical parameters used in this study are useful indices of morphometrical status in the each species from major river of Korea.

Effects of Wearing Corrective Glasses for Presbyopia on Age-related Hyperopic Shifts and Reading Addition (노안보정용 안경 착용이 연령관련 원시화와 근용가입도 변화에 미치는 영향)

  • Purpose: This study was conducted to investigate the changes in distance refractive power and reading addition that occur after wearing corrective glasses for prebyopia. Methods: The subjects aged 42 to 75 years who had no ophthalmologic diseases and did not undergo cataract or retina surgery, had a corrected visual acuity of 20/20 and did not have a previous history of wearing corrective glasses for presbyopia. These subjects were divided into 3 groups: the control, reading spectacles and multifocals wearing group and they were measured for distance refractive power and reading addition at their visit. The maximum follow-up period was 73 months. Results: As for distance refractive power, the power tends to shift to hyperopia depending on age (r=0.486, p<.001) and 50.0% of the subjects increased plus power during this study period. Plus power Increments per year in distance refractive power in the reading spectacle wearing group were significantly different with control and multifocals wearing group (p<.05). On the other hand, the multifocals wearing group's increments were not different with control. Increments in reading addition were also increased in the reading spectacles group than in the control and multifocals wearing groups (p<.05). Conclusions: The age related hyperopic shift could be occurred in the elderly people, routine refraction is mandatory. And reading spectacles could induce an age related hyperopic shift and the additional need for reading addition that the prescription of multifocals may reduce changes in distance refractive power and reading addition.

The Effects of functional foot orthotics on the balance according to Foot Shape (기능성 발보조기의 족부형태별 균형유지에 미치는 영향)

  • The functional loot orthoses, when wearing shoes, are in the direct contact with bottom of foots to improve and recover the correctness of abnormal lower limb musculoskeletal imbalance and the primary role of foot and also, it can act to keep the balance and weight of body and support the weakened region, so that it is very helpful to keep body balance for the standing position. In this paper, it was researched that foot orthoses which is accommodable for the function of impact absorption including the gait stability affect on the balanced performances of body in according to the formation and the material of foot part. Taking into account the balanced performances by using the sway velocity, the estimation and comparison of the effects on the balanced performances by each formation and material for foot orthoses was evaluated into significant values(p<0.006) in only the eye-opening posture with Firm state, In this posture, the static process performed by each foot formation reveals in order of normal foot(p<0.010), flat foot(p<0.000) and hollow foot(p<0.003) and then, on the base of each formation of foot part, the result that analyze the effects of the materials of foot orthoses on the balance performance appeared showing that soft materials is more effective on the normal foot and, on the other hand, rigid materials is more effective in balancing on flat foot and hollow foot.
