노인의 인지기능과 상지기능의 관련성에 관한 체계적 고찰 (A Systematic Review on the Association between Cognitive Function and Upper Extremity Function in the Elderly)

  • 목적: 본 연구는 노인의 인지기능과 상지기능의 관련성을 알아보고자 하며, 근거 자료들을 P.I.C.O. 방법에 따라 제시하였다. 연구방법: 2015년 11월부터 2016년 1월까지 연구를 진행하였으며 2000년 1월부터 2015년 10월까지 국외 학술지에 개재된 논문을 Medline과 PubMed를 통하여 검색하였다. 검색용어로는 elderly, dementia, Alzheimer's disease, Mild cognitive impairment, age-related, aging, cognitive, upper extremity function, hand function, hand-grip strength, grip force, complex motor function, bimanual, dexterity, UE performance, coordination을 사용하였다. 연구결과: 본 고찰에 포함된 총 8개의 연구 중 7개의 연구에서 상지기능이 노화에 따라 영향을 받는다는 것으로 나타났고, 1개의 연구에서는 노화에는 영향을 받지 않으나, 쥐기 힘 및 성별에 의해 물건조작능력에 영향을 받는 것으로 나타났다. 연구 분석 결과 노인 및 치매환자의 인지기능이 상지기능에 영향을 미치며 상호 관련성을 보였다. 결론: 본 연구는 노인을 대상으로 인지기능이 상지기능에 미치는 영향 및 관련성을 알아보고자 하였으며, 인지기능 정도에 따른 작업치료 적용 및 중재기법을 선택하는데 근거 자료로 활용될 수 있을 것으로 사료된다. 향후 국내 연구에서는 다양한 평가도구를 사용하고 질적 수준이 높은 연구들이 체계적으로 이루어져야 할 것이다.

근막변형모델이 최대악력, 힘 감각 그리고 관절가동범위에 미치는 효과 (The Effect of Fascial Distortion Model on Maximal Grip Strength, Force Sense and Range of Motion)

  • Purpose : The fascia is the body's largest sensory organ affected by mechanical stimuli such as pressure. Fascial distortion model (FDM) is one of the fascia treatment techniques, and it is based that most musculoskeletal problems are caused by three-dimensional distortion of the fascia. Until now, some studies have been conducted to investigate the effect of FDM, but it is still not enough. In this study, among the six techniques of FDM, trigger band (TB) and cylinder distortion (CyD) were applied to the forearm to investigate the immediate effect on the maximal grip strength, force sense and range of motion. Methods : 22 healthy adults in their 20s and 30s participated. Before and after the application of FDM, maximal grip strength, force sense and range of motion were measured. For analysis, a paired t-test was performed, and the significance level was set to p<.05. Results : After FDM, there were a significant differences that increased the force sense and the range of motion (p<.05). But the maximal grip strength did not show a significant difference (p>.05). Conclusion : The results of this study, the FDM did not affect maximal grip strength, but it was shown to increase the sense of force and range of motion. We assumed that it is the result of the changes in the states of the fascia and changes in the activity of mechanical receptors induced by pressure and stretch from FDM application.

Relationship between Fat-Free Mass and Muscle Strength, Nutrient Intakes, Exercise Habits in Male Aged

  • This study was performed fir the purpose of finding the relationship between fat-free mass (FFM) which mainly represents muscle mass change, and muscle strength. The correlation between diet and exercise behaviors and FFM in healthy elderly men over age 55 was also investigated. FFM and corresponding hand grip strength declined significantly with increasing age. However the concentration of serum albumin, the indicator of protein nutritional status, showed no change with increases in age. The subjects were categorized into three groups according to FFM textiles. Anthropometry such as height, BMI, circumferences of waist and hips, body fat mass percent and hand grip strength decreased significantly in the lowest FFM group. But albumin level showed no change relative to FFM level. FFM showed a significant relationship to nutrient intake such as energy and fat. However, no association was found with exercise behavior probably because there were no instances of resistance exercise habits among the subjects studied. The variance in FFM could be attributed 86.2% to height and age. The variance of height-adjusted FFM could be attributed only 40.6% to age and fat intake. In conclusion decreases in FFM may cause reductions in muscle strength in elderly men. Increasing energy and fat intake were associated with increased FFM and may protect elderly men from the risk of sarcopenia. FFM had no association with endurance exercise habits.

GFRP 보강근의 인장강도 분석을 위한 시험방법 비교 연구 (A Comparative Study on the Testing Methods for the Analysis of Tensile Strength of GERP Rebars)

  • 본 연구에서는 GFRP 보강근의 인장특성치 시험을 위한 그립 시스템의 적합성을 검증하기 위하여 캐나다 규준에서 제안하는 그립(CSA 그립), ASTM에서 제안하는 그립(ASTM 그립) 및 프리스트레싱 강연선의 정착에 일반적으로 사용되는 쐐기형 그립 등을 사용하여 GFRP 보강근에 대한 인장특성치 시험을 실시하였다. 또한, 현재 외국에서 상용화되고 있는 대표적인 2종류의 GFRP 보강근(나선형 GFRP 보강근, 모래분사형 GFRP 보강근) 및 국내에서 자체 제작한 원형 GFRP 보강근을 대상으로 하여 인장특성치 분석을 위한 시험을 실시하고 각각의 제안된 그립의 적용성 여부를 검토하였다. 본 시험에 사용된 시험편의 제작, 가력 및 측정장치의 설치 등은 CSA S806-02에서 제안하는 권고사항에 따라 실시하였다. 외국의 상용화된 GFRP 보강근에 대하여 그립의 종류를 달리하여 실시된 본 시험결과에 의하면, CSA 그립을 사용하여 시험된 GFRP 보강근의 인장강도가 가장 높은 값을 보이는 것으로 나타났다. 그러나 ASTM 그립을 사용한 시험편에서 관측되는 강도저하 현상은 CSA 그립을 사용한 시험체에 비하여 약 10% 미만인 것으로 관측되었다. 한편, CSA 그립은 제작공정이 까다로울 뿐만 아니라 재사용이 불가능하여 경제성 측면에서도 불리한 것으로 파악되었다. 따라서, 실용적인 측면에서 판단하면 GFRF 보강근의 인장시험에는 ASTM 그립이 적절할 것으로 판단된다.

Relationship among Stress, Anxiety-depression, Muscle Tone, and Hand Strength in Patients with Chronic Stroke: Partial Correlation

  • PURPOSE: This study was conducted to identify the relationships among stress response inventory, hospital anxiety and depression, muscle tone and stiffness, and hand strength in chronic stroke patients. METHODS: A total of 14 chronic stroke patients voluntarily agreed to this experiment and were included in this study. All measurements were performed in one day and in a room without noise. The tests conducted in this study were as follows: muscle tone and stiffness of the upper trapezius hand grip measurement. Subjects were also asked to complete surveys describing the following: stress response inventory and hospital anxiety and depression scale. RESULTS: There were significant correlations among stress response inventory and hospital anxiety and depression, stress response inventory and hand strength, and hospital anxiety and depression and hand strength (P<.05). There were high positive correlations between stress response inventory and hospital anxiety and depression (r=.979), while there were moderate negative correlations between stress response inventory and hand strength (r=-.415) and between hospital anxiety and depression and hand strength (r=-.420). CONCLUSION: The results of the present study indicate that there is a relationship among stress response inventory, hospital anxiety and depression, and hand strength in patients with chronic stroke.

수부기능의 재건을 위한 기능성 유리 박근 근피판 이식 (Functioning Gracilis Musculocutaneous Free Flap Transplantation for the Reconstruction of Hand Function)

  • Purpose : We performed this study in order to analyze the clinical results of functioning gracilis musculocutaneous free flap transplantation for reconstructon of hand function in cases of Volkmann's ischemic contracture and massive soft tissue injury of forearm. Materials and methods : 18 cases were reviewed with 5 yeas of mean follow up period. We evaluated total active motion of the finger joints and wrist, pinch and grip strength. Results : The flap were survived in 17 cases and 1 case was failed due to infection. The sum of active motion of finger joints(TAM) was improved from 0 to $173.8^{\circ}$. The average grip and pinch strength was improved from 0 kg to $2.7{\pm}2.1kg$ and from 0 kg to $2.4{\pm}1.6kg$. Conclusion : The results in most cases were acceptable in relatively long term follow-up. It may be an option for reconstruction of hand function in cases of Volkmann's ischemic contracture and traumatic upper extremity injury.

Evaluation of the Contributions of Individual Finger Forces in Various Submaximal Grip Force Exertion Levels

  • Objective:The aim of this study is to evaluate contributions of individual finger forces associated with various levels of submaximal voluntary contraction tasks. Background: Although many researches for individual finger force have been conducted, most of the studies mainly focus on the maximal voluntary contraction. However, Information concerning individual finger forces during submaximal voluntary contraction is also very important for developing biomechanical models and for designing hand tools, work equipment, hand prostheses and robotic hands. Due to these reasons, studies on the contribution of individual finger force in submaximal grip force exertions should be fully considered. Method: A total of 60 healthy adults without any musculoskeletal disorders in the upper arms participated in this study. The young group (mean: 23.7 yrs) consisted of 30 healthy adults (15 males and 15 females), and the elderly group (mean: 75.2 yrs) was also composed of 30 participants (15 males and 15 females). A multi-Finger Force Measurement (MFFM) System developed by Kim and Kong (2008) was applied in order to measure total grip strength and individual finger forces. The participants were asked to exert a grip force attempting to minimize the difference between the target force and their exerted force for eight different target forces (5, 15, 25, 35, 45, 55, 65, and 75% MVCs). These target forces based on the maximum voluntary contraction, which were obtained from each participant, were randomly assigned in this study. Results: The contributions of middle and ring fingers to the total grip force represented an increasing trend as the target force level increased. On the other hand, the contributions of index and little fingers showed a decreasing trend as the target force level increased. In particular, Index finger exerted the largest contribution to the total grip force, followed by middle, ring and little fingers in the case of the smallest target force level (5% MVC), whereas middle finger showed the largest contribution, followed by ring, index and little fingers at the largest target force levels (65 and 75% MVCs). Conclusion: Each individual finger showed a different contribution pattern to the grip force exertion. As the target force level increase from 5 to 75% MVC, the contributions of middle and ring fingers showed an increasing trend, whereas the contributions of index and little fingers represented a decreasing trend in this study. Application: The results of this study can be useful information when designing robotic hands, hand tools and work equipment. Such information would be also useful when abnormal hand functions are evaluated.

우세측에 따른 상지와 하지의 기능 비교 (Comparison of Upper and Lower Limb Functional Ability Depends on Dominants)

  • Purpose. The purpose of this study was compared of left dominant and the right dominant hands about coordination. Methods. The participants were 30 occupational therapy students in M university in Changwon, from April 28th 2015 to May 1. The test tools were Jebson-Taylor Hand Function Test, Hand Strength & Pinch, Grooved Pegboard Test, Box & Block and Balance Test. Results. Right dominant hand was higher than left dominant in hand grip and dexterity, hand manipulation skills. And one leg standing and toe standing were higher left foot than right foot dominant. Conclusion. Most of all evaluation items showed a better result right dominant hand than left dominant. But shows that there is no functional difference between left-hand dominant than right-hand. So left-hand dominant coordination was evaluated by a better than right-handed.

요양병원 노인에서 수면의 질의 따른 손 기능과 수면 장애 요인 간 관련성 연구 (The Relation between Hand Function and Sleep Disturbance Factors According to Quality of Sleep among Elderly Patients in Geriatric Hospitals)

  • 목적 본 연구는 요양병원 노인에서 수면의 질에 따른 손 기능 변화와 수면의 질과 손 기능, 수면 장애 요인간의 상관관계를 알아보고자 하였다. 연구방법 2017년 11월부터 2018년 1월까지 6개월 이상 입원중인 23명의 환자를 대상으로 수면 측정도구 A를 사용하여 수면의 질, 악력계를 이용하여 악력을 평가 하였고 BBT, 9-Hole Peg test를 사용하여 협응력을 평가 하였다. 수집된 자료는 SPSS 21.0 통계 프로그램을 사용하여 대응 표본 t-검정, 피어슨 상관분석을 실시하였다. 연구결과 수면 점수 '상' 집단에서는 수면의 질과 BBT 간에, 악력과 BBT 간에 상관관계를 보였고 '중' 집단에서는 악력과 BBT 간에 상관관계를 보였다. 두군 모두에서 수면의 질과 수면 양상 간의 유의미한 높은 상관관계를 보였다. 결론 요양병원에 입소한 노인 중 수면 장애의 질적 차이를 보일 때 BBT와 악력 향상을 위한 치료의 병행은 수면의 질을 향상시킬 수 있으며 수면 양상 중재는 수면의 질을 개선하는데 효과적일 것으로 생각 된다.

뇌졸증 환자의 상지기능에 영향을 미치는 요인 (Factors Affecting the Upper Limb Function in Stroke Patients)

  • 본 연구는 뇌졸중 후 상지기능의 관련 요인과 각 요인의 상대적 영향력을 알아보기 위해 시행되었다. MMSE-K(Mini Mental State Examination Korean version) 24점 이상인 100명의 뇌졸중 환자를 대상으로 하였다. 상지기능은 성별, 우세손, 뇌손상 원인, 뇌반구 손상부위, 신체마비 부위, 언어장애 유무와는 유의한 차이가 없었으나(p>0.05), 아탈구 유무는 상지기능에 유의한 차이를 보였다(p<0.05). 상지기능은 상지근력(Manual Muscle Testing), 고유수용성감각(Proprioception), 근 긴장도 (Modified Ashworth Scale), 장악력(Grip strength), 통증(Visual Analog Scale)과 상관관계를 보였다(p<0.05). 위의 변수를 모두 포함하여 분석하면 77.6%의 상지기능 설명력을 보였고 이 중 상지근력이 가장 높은 설명력을 보였다. 뇌졸중 환자의 상지기능 재활을 위해서는 특히 상지근력 강화와 더불어 고유수용성감각, 근 긴장도, 장악력, 통증 그리고 아탈구 유무의 관련 요인들을 포괄적으로 중재에 포함시켜야 할 것으로 사료된다.
