Haloperidol의 약물속도론적 연구 (III) -정신분열증 환자에 있어서 Haloperidol의 일회 주사 및 경구투여시의 Pharmacokinetics- (Pharmacokinetics of Haloperidol (III) -The Pharmacokinetics of Haloperidol after single oral and intravenous doses in schizophrenic patients-)

  • The pharmacokinetics of haloperidol were determined after single oral and intravenous doses in 13 male schizophrenic patients. Plasma concentrations of haloperidol(HP) and reduced haloperidol(RH) were measured by high performance liquid chromatography. Plasma concentration data obtained were analyzed by obth model dependent (one-or two exponential decay models using nonlinear regression) and model independent (AUC and first moment curve) approaches. The two methods were found to be in close results. After intravenous injections of HP in 8 patients (10 mg/man), the mean central and peripheral volume of distributions were $2.85\;{\pm}\;1.70$ and $8.09\;{\pm}\;2.10\;l/kg$, respectively, and mean steady state volume of distribution was $11.87\;{\pm}\;3.21\;l/kg$. Mean clearance, MRT and elimination half life were $12.39\;{\pm}\;3.25\;ml/min/kg$, $925.10\;{\pm}\;166.79\;min$ and $676.35\;{\pm}\;126.45\;min$, respectively. After oral administrations of HP in 5 patients, mean peak time and peak concentration were $217.63\;{\pm}\;61.60\;min$ and $9.77\;{\pm}\;2.92\;ng/ml$, respectively. Mean MRT and elimination half life were $1112.23\;{\pm}\;131.73\;min$ and $724.02\;{\pm}\;120.03\;min$, respectively, and these parameters were not significantly different from those of intravenous injection of HP. Absolute bioavailability of HP oral product was found to be about 44%. The profiles of plasma RH concentration-time curves after oral or intravenous doses of HP were similar. Also it was found that the elimination rate of RH was solwer than that of HP by comparing the slopes of plasma concentration-time curves of HP and RH.

  • 한국특산 매자나무(Berberis Koreana Palibin)의 뿌리에서 분리한 majarine은 isoquinoline 알카로이드로서 본 교실에서는 중추신경계에 대한 약리작용을 검토하고 있다. Majarine을 마우스 복강내로 투여하여 직장온도 변화와 haloperidol, cyproheptadine과 reserpine 등에 대한 약물 상호작용을 관찰하여 다음과 같은 성적을 얻었다. Majarine은 마우스에 있어서 용량의존적으로 현저한 체온감소를 나타내었으나, 0.1 mg/kg투여시 체온증가의 유의성을 보였다. 체온감소는 haloperidol과 cyproheptadine으로 억제되었다. Reserpine처치 마우스에 있어서 ${\alpha}$-methyl-p-tyrosine으로 전처치한 다음 majarine 2.0mg/kg 투여시 체온감소를 나타내었다. 이러한 결과로 보아 majarine의 체온변화는 dopamine과 serotonin수용체에 관련성이 있다고 사려되고, 체온감소는 dopamine수용체에 직접적으로 작용한다고 생각되는 바이다.

  • To investigate characteristic drug effects on the genetic basis, the authors administered haloperidol- the $D_2$ antagonist- and clozapine -the atypical antipsychotics with few extra- pyramidal side effects- to the rats. Then, we edobtain brain specimen from the striatum, prefrontal cortex, and cortical region and compared the degree of c-fos expression. The results are 1) haloperidol was found to produce a rapid and transient induction of dos mRNA expression in striatum as compared with cortex and prefrontal area. 2) clozapine was found to produce rapid induction of c-fos mRNA in striatum and prefrontal area. From these data, we can concluded that the mechanism of action of haloperidol is different from the mechanism of clozapine in gene expression.

Haloperidol Induces Calcium Ion Influx Via L-Type Calcium Channels in Hippocampal HN33 Cells and Renders the Neurons More Susceptible to Oxidative Stress

  • Haloperidol is a classical neuroleptic drug that is still in clinical use and can lead to abnormal motor activity following repeated administration. However, there is little knowledge of how it triggers neuronal impairment. In this study, we report that it induced calcium ion influx via L-type calcium channels and that the elevation of calcium ions induced by haloperidol appeared to render hippocampal cells more susceptible to oxidative stress. Indeed, the level of cytotoxic reactive oxygen species (ROS) and the expression of pro-apoptotic Bax increased in response to oxidative stress in haloperidol-treated cells, and these effects were inhibited by verapamil, a specific L-type calcium channel blocker, but not by the T-type calcium channel blocker, mibefradil. These findings indicate that haloperidol induces calcium ion influx via L-type calcium channels and that this calcium influx influences neuronal fate.

Haloperidol 투여후 금단기간에 따른 백서 선조체의 [$^3H$]Spiperone 결합 및 Dopamine 대사물질의 변화 (Time-Course of [$^3H$]Spiperone Binding and Dopamine Metabolism in the Rat Striatum after Withdrawal from Haloperidol Ttreatment)

  • The effects of 3 week treatment with haloperidol(2mg/kg/day, i.p.) on dopamine(DA) D2 receptor and DA metabolism in rat striata were studied at various time points after withdrawal from the drug treatment. Striatal DA D2 receptors were characterized with the radioligand 0.5nM [$^3H$]Spiperone. Dopamine(DA), homovanillic acid(HVA), 3,4-dihydroxyphenyl acetic acid(DOPAC) in rat striatum were measured with the high performance liquid chromatography. Drug withdrawal for 1 week induced significant increase in the number of D2 receptor in striatum after withdrawal for 1 week(p<0.05), and then this change was restored to control level during the withdrawal time of 2 and 4 weeks. There was no difference in striatal concentrations of DA and its metabolites among the groups. In conclusion, one-week withdrawal from chronic haloperidol treatment induced DA D2 receptor supersensitivity in the striatum, and that was normalized rapidly. Though this adaptive change in DA receptors, it may not affect the metabolism of DA in striatum.

정신분열증 환자에서의 Haloperidol 및 Reduced Haloperidol의 혈장농도와 임상반응과의 상관성 (Relationship between Plasma Concentrations of Haloperidol and Its Metabolite, Reduced Haloperidol, and Clinical Response in Schizophrenia)

  • The relationship between the plasma haloperidol (HP) concentration and clinical response, and the effects of its active metabolite, reduced haloperidol (RH) on clinical response of HP were investigated in schizophrenic patients. In clinical study I, with 17 schizophrenic patients (male 8, fermale 9) who were administered with three different fixed doses of HP (15, 30 and 50 mg/day) for 3 weeks, the concentrations of HP and RH in plasma and blood and clinical response had been checked before and every week during the study. The clinical response was evaluated by the method of brief psychiatric rating scale (BPRS), and relative improvement of clinical response based on baseline BPRS (before drug treatment) was calculated. The concentrations of HP and RH in plasma and blood were assayed by HPLC. In clinical study II, the plasma RH/HP concentration ratios were checked in 11 patients who were administered with high doses of HP, over 60 mg a day, because of the poor clinical response at usual doses of HP. Plasma HP concentration and relative improvement of BPRS at 3 week in schizophrenic patients showed a 'curvilinear' relationship, and the clinical response was improved relatively over 50% based on the baseline BPRS in the range of $5{\sim}57\;ng/ml $ of HP in plasma. Also, the plasma RH concentrations were increased nonlinearly as the plasma HP concentration increased, and in high plasma HP concentration, over 30 ng/mI, clinical response gradually decreased, while the plasma RH/HP concentration ratio increased nonlinearly. Blood partition coefficients of HP and RH were not changed according to daily HP dose and duration of drug therapy. From these results, it is noted that the higher plasma RH/HP concentration ratio, resulted from the accumulation of RH as HP concentration increased, might explain the 'curvilinear' decrease of HP clinical response.

노인 기질성 정신장애 환자의 급성 정신증상에 대한 리스페달 사셰와 할로페리돌 근주의 효과 비교 (Risperdal Sachet and Oral Lorazepam versus Intramuscular Haloperidol and Lorazepam Injection for Acute Psychotic Symptom in the Elderly Patients with Organic Mental Disorder)

  • Objectives : The purpose of present study was to investigate the effect, safety and tolerability of risperdal sachet(oral solution) with lorazepam tablet versus intramuscular haloperidol and lorazepam injection for management of acute psychotic symptom in the elderly with organic mental disorder. Methods : Total 37 patients who have dementia, medical or physical diseases, associated with acute psychotic symptom were randomly assigned to oral treatment with 1mg of risperdal sachet(oral solution) plus 1mg of lorazepam(N=17) or to intramuscular treatment with 2.5mg of haloperidol plus 2mg of lorazepam (N=20). The change of CGI scores was used for the evaluation of efficacy. Results : Mean score improvements at 15, 30, 60, and 120 minutes after treatment were statistically significant at each time point in both groups(p<0.001) and were similar in both groups(p=0.189). Conclusion : A single oral dose of risperdal sachet(oral solution) plus lorazepam was as effective and tolerable as parenterally administered haloperidol plus lorazepam for the rapid control of acute psychotic symptom in the elderly with organic mental disorder.

Effects of Dopamine and Haloperidol on Morphine-induced CREB and AP-1 DNA Binding Activities in Differentiated SH-SY5Y Human Neuroblastoma Cells

  • In the present study, we first examined whether the changes in the DNA binding activities of the transcription factors, cAMP response element binding protein (CREB) and activator protein-1 (AP-1) mediate the long-term effects of morphine in differentiated SH-SY5Y human neuroblastoma cells. The increases in CREB and AP-1 DNA binding activities were time-dependent up to 6 days of morphine treatment (1, 4, and 6 days). However, the significant reduction in the DNA binding activities of CREB and AP-1 was observed after 10 days of chronic morphine $(10\;{\mu}M)$ administration. Secondly, we examined whether the changes of CREB and AP-1 DNA binding activities could be modulated by dopamine and haloperidol. Dopamine cotreatment moderately increased the levels of the CREB and AP-1 DNA binding activities induced by 10 days of chronic morphine treatment, and haloperidol cotreatment also resulted in a moderate increase of the CREB and AP-1 DNA binding activities. However, dopamine or haloperidol only treatment showed a significant increase or decrease of the CREB and AP-1 DNA binding activities, respectively. In the case of acute morphine treatment, the CREB and AP-1 DNA binding activities were shown to decrease in a time-dependent manner (30, 60, 90, and 120 min). Taken these together, in differentiated SH-SY5Y cells, morphine tolerance seems to involve simultaneous changes of the CREB and AP-1 DNA binding activities. Our data also suggest the possible involvement of haloperidol in prevention or reversal of morphine tolerance at the transcriptional level.

흰쥐에서 할로페리돌에 의해서 유발된 강경증에 미치는 후구 적출의 영향 (Effects of Olfactory Bulbectomy on Catalepsy Induced by Haloperidol in Rats)

  • We attempted to clarify the effect of bilateral olfactory bulbectomy on catalepsy induced by haloperidol in rats. The incidence of catalepsy induced by haloperidol remarkably increased after lesion of olfactory bulb, which was significantly inhibited by L-5-hydroxytryptophan, L-DOPA, and ginseng's total saponin but reserpine and ${\alpha}-methyl-p-tyrosine$ were ineffective. The dopamine content of brain was significantly decreased by olfactory bulbectomy, but this result was reversed by ginseng's total saponin.

Effect of ethanolic extract of some anti-asthmatic herbs on clonidine and haloperidol-induced catalepsy in mice

  • The ethanolic extract of some medicinal plants having anti-asthmatic activity such as Solanum xanthocarpum, Curcuma longa, Glycyrrhiza glabra, Piper longum, A. vasica, A. lebbeck, and Tinospora cordifolia was evaluated for antihistaminic and anti-cataleptic activity. The aqueous solution of ethanolic extract of S. xanthocarpum and G. glabra potentiated histamine-induced tracheal chain contractions. Whereas, C. longa, P. longum, and T. cordifolia, and A. lebbeck were without any significant effect on histamine. Only A. vasica inhibited histamine-induced tracheal chain contraction. G. glabra per se produced contraction of the tracheal chain, which was blocked by pretreatment with atropine. Single dose of S. xanthocarpum potentiated clonidine-induced catalepsy but on repeated doses (once in a day for 3 days) inhibited catalepsy. Pretreatment with ethanolic extract of C. longa, P. longum, T. cordifolia inhibited catalepsy whereas G. glabra and A. lebbeck significantly potentiated clonidine-induced catalepsy. None of the extracts inhibited haloperidol-induced catalepsy. Thus the extracts having antihistaminic activity or mast cell stabilizing activity inhibited clonidineinduced catalepsy.
