Central Anticholinergic Syndrome by the Use of Scopolamine Patch -A case report- (수술후 통증관리 환자에서 Scopolamine Patch에 의해 발생한 중추항콜린성증후군 -증례 보고-)

  • A 77-year-old male patient was admitted to the hospital with acute appendicitis and underwent emergency appendectomy under inhalation anesthesia without any complications. After the operation, the patient was placed on epidural analgesia with the mixture of fentanyl-bupivacaine and scopolamine patch for nausea and vomiting. The patient experienced central anticholinergic syndrome (CAS) with hallucination, confusion, somnolence, dysarthria and delirium at 20 hours after patch placement and recovered from all symptoms of CAS in 24 hours after the removal of the patch. The CAS occurs when central cholinergic sites are occupied by specific drugs and also as a result of an insufficient release of acetylcholine. The diagnosis of CAS is often determined by a process of exclusion and not actually made until a positive therapeutic response to physostigmine, a centrally active anticholiesterase agent, has taken place. Treatment of CAS includes prompt removal of the patch, cleansing of the area, and physostigmine administration. The scopolamine patch should be used with extreme caution in elderly patient.

Refeeding Syndrome Associated with Delusion in a Schizophrenic Patient (망상과 관련되어 발생한 정신분열병 환자의 Refeeding Syndrome 1예)

  • Refeeding syndrome is a life-threatening but treatable condition occurring in any patients with starvation. Electrolyte imbalance, especially hypophosphatemia and fluid shift result in metabolic changes in various systems. The authors reported a case of refeeding syndrome associated with prolonged starvation due to delusion and hallucination in a 38-year-old schizophrenia patient. Hypophosphatemia, hypokalemia, edema and elevation of liver enzyme was occurred despite careful treatment. The symptoms were improved after consistent electrolyte supplement. Close observation and repetitive monitoring are important to prevent refeeding syndrome.

  • Central anticholinergic syndrome occurs when an anticholinergic substance works in vivo or as a result of the insufficient release of acetylcholine. Its symptoms include confusion, agitation, behavioral change, hallucination, blurred vision, and dysarthria. Occasionally, these symptoms occur with the use of a scopolamine patch. A 54-year-old female complained of behavioral change and confused mentality. She attached a scopolamine patch at the postauricular area in the morning of the day before her hospital visit. Neurological examination revealed bilateral symmetric mydriasis without light reflex. The brain MRI was normal, and electroencephalography showed nonspecific abnormalities. The patient recovered completely after the removal of the scopolamine patch.

  • Cyclosporine is an immunosuppressant usually used to prevent renal transplantation rejection. Nephrotoxicity and hypertension are considered as the most frequent side effects of cyclosporine treatment. The neurotoxic effects of cyclosporine such as agitation, anxiety, delirium, depression and psychosis have recently been found. Methylprednisolone may increase as well plasma concentration of cyclosporine, which leads to side effects. Here we report a $Henoch-Sch\"{o}nlein$ nephritis patient treated with cyclosporine and methylprednisolone who has experienced psychosis including visual and auditory hallucination and convulsion.

  • Objectives : The purpose of this study was to investigate the clinical application of Korean Traditional treatment to Jeon-zeong(癲證) with Chronic hypertrophic rhinitis. Methods : In this case, a 25 years old male patient has suffered by schizophrenia for 2 years and by Chronic rhinitis for 9 months. During 171 days, we treated the patient with Korean Traditional treatment. Specially blood-Letting therapy helped the patient to decrease the auditory hallucination, catalepsy and retardation as well as the snuffles. Results : After treatment. the chief complain and accompanying symptoms were subsided and improved. And Personal and social performance scale was increased. Conclusions : According to this study, Korean Traditional treatment such as Blood-Letting therapy, acupuncture and heb-medication is effective for the cure of Jeon-zeong(癲證) with Chronic hypertrophic rhinitis.

A Case of Acute Psychosis after Sibutramine Ingestion (시부트라민(sibutramine) 음독 후 발생된 급성 정신병증 1례)

  • A 38-year-old woman was admitted to our hospital due to agitation, nausea, chest discomfort, tachycardia and hypertension 6h after ingesting approximately 60 capsules of sibutramine. The woman developed the clinical features of acute psychosis, including auditory hallucination, agitation and paranoid ideation, on day 2. No relevant changes were detected on the laboratory examinations or on the electrocardiogram throughout the period of hospitalization. She was treated with risperidone and benzodiazepine. The symptoms subsequently resolved completely with cessation of the provoking agent. The patient was discharged on day 7 and the follow-up revealed no sequelae for 5 months.

Steroid-induced Psychosis in Adolescent Patient with Crohn's Disease

  • Crohn's disease is a chronic inflammatory disease that involves the upper and lower gastrointestinal tracts, including the oral cavity. For the conventional treatment of inflammatory bowel diseases, steroids are usually used as induction therapy, and azathioprine is used as maintenance therapy. Steroids are associated with various side effects. Under rare circumstances, they can even cause psychotic symptoms, and reports on steroid-induced psychosis in the pediatric population are few. Here, we report the first case of steroid-induced psychosis in a 16-year-old female with Crohn's disease administered with steroids as induction therapy in South Korea. She showed psychotic symptoms, including auditory hallucination, loosening of association, and idea of reference, after 2 weeks of administering prednisolone 60 mg/day. One week after steroid discontinuation, the patient gradually recovered and was treated with quetiapine. Therefore, pediatricians should be aware of serious psychiatric adverse effects when administering corticosteroids for various indications.

  • Narcolepsy is a central neurologic system disease. It begins early in life with disabling symptoms including excessive daytime sleepiness, cataplexy, sleep paralysis, hypnagogic hallucination and nocturnal sleep fragmentation. Patient with typical symptoms of narcolepsy is diagnosed by objective data from nocturnal polysomnography and multiple sleep latency tests. Narcolepsy is controlled with various medications. Nowadays, modafinil with favorable side effects profiles compared with traditional stimulant is mainly used. Gamma hydroxyl butyrate is effective in cataplexy. Cataplexy is also controlled with antidepressant such as Venlafaxine, SSRI, and TCA. As the knowledge of pathophysiology of narcolepsy expands, new treatment including immunological method, application of hypocretin and histamine systems have been tried.

  • Delirium is a neuropsychiatric disorder accompanying symptoms of hallucination, drowsiness, and tremors. It has high occurrence rates among elders, heart disease patients, and burn patients. It is a medical emergency associated with increased morbidity and mortality rates. That s why early detection and prevention of delirium ar significantly important. And This mental illness like delirium occurred by complex interaction between risk factors. In this paper, we identify risk factors and interactions between these factors for delirium using multi-factor dimensionality reduction (MDR) method.

  • 'Fantasy' in this study is Not something fantastic in the used to be-way we can Imagine easily. This study is to figure out 'fantasy' and 'the fantastic' as a significant measure to interpret the contemporary space-time. In particular, it analyzed that fantasy have influenced on some kind of specific trend what is expressed a tendency to contemporary interior designs of informal, indeterminacy and unpredictability. Fantasy is represented as 'the fantastic space' through spatial revealing, concealing, distorting, and connoting. Representation becomes a major issue in almost many kinds of knowledge-system that start from the mind to go out to the world in a realistic or idealistic way. Therefore, the fantastic space reconstructs the reality while making itself through to representational creation-process. It is Important for fantasy and the fantastic space to help detect behind beneath the reality more than what we know.
