A Cross-sectional Study of Deficiency-Excess Pattern Identification with Blood Cytokines and Characteristics of Patients with Asthma (천식환자 허실변증별 혈액 싸이토카인 및 임상적 특성에 관한 단면적 연구)

  • Objective: The aims of this study were to analyze deficiency-excess pattern identification and to compare the blood cytokines in patients with asthma. Methods: A total of 112 patients with asthma who met the inclusion and exclusion criteria were divided into deficiency syndrome and excess syndrome groups. Blood was examined for eotaxin, interleukin (IL)-1β, IL-4, IL-5, IL-6, IL-13, and tumor necrosis factor (TNF)-α. The Quality of Life Questionnaire for Adult Korean Asthmatics (QLQAKA), a Visual Analogue Scale (VAS), and heart rate variability (HRV) tests were administered to both groups. Results: Pattern identification divided the 112 patients into two categories: a deficiency syndrome group (N=52) and an excess syndrome group (N=60). Analysis of blood cytokines showed higher levels of IL-4, IL-5, and IL-13 in the deficient pattern than in the excess pattern group, but the difference was not statistically significant. Analysis of the HRV revealed a significantly higher mean value for the very-low-frequency (VLF) and high-frequency (HF) bands in the deficiency than in the excess syndrome group. The morbidity duration was longer in the deficiency than in the excess syndrome group, but the difference was not statistically significant. Analysis of the QLQAKA and VAS scores showed a negative correlation, whereas BMI and VAS showed a positive correlation. Conclusions: Levels of blood cytokines, including eotaxin, IL-1β, IL-4, IL-5, IL-6, IL-13, and TNF-α, did not differ statistically between the deficiency and excess syndrome groups. The development of a more accurate asthma-specific pattern identification tool would be useful in asthma control.

Change of Heart Rate Variability in Depressive Disorder after Physical or Psychological Stress (우울장애 환자에서 육체적 및 정신적 스트레스 시 심박변이도의 변화)

  • Objectives: This study was designed to assess the change of heart rate variability (HRV) at resting, upright, and psychological stress states in depressive disorder patients. Methods: HRV was measured at resting, upright, and psychological stress states in 62 depressive disorder patients. We used visual analogue scale (VAS) score to assess tension and stress severity. Beck depression inventory (BDI) and state trait anxiety inventories I and II (STAI-I and II) were used to assess depression and anxiety severity, respectively. Differences between HRV indices and VAS score were evaluated using paired t-tests. Gender difference analysis was conducted with ANCOVA. Results: SDNN (standard deviation of normal to normal intervals), LF/HF (low frequency/high frequency), and VLF (very low frequency) were significantly increased, while NN50 and pNN50 were significantly decreased in the upright position compared to resting state. SDNN, RMSSD (root mean square of the differences of successive normal to normal intervals), and VLF were significantly increased, while pNN50 was significantly decreased in the psychological stress state compared to resting state. SDNN, NN50, and pNN50 were significantly lower in an upright position compared to a state of psychological stress, and LF, HF, and LF/HF showed no significant differences Conclusion: The LF/HF ratio was significantly increased after physical stress in depressive disorder. However, the LF/HF ratio was not significantly increased after psychological stress, and the change in LF/HF ratio after physical stress and psychological stress did not significantly differ from each other. Significant increase in SDNN, NN50, and pNN50 in an upright posture compared to psychological stress suggests that depressive patients react more sensitively to physical stress than psychological stress.

  • 본 연구에서는 긴장 스트레스를 평가하기 위하여 작업 중에 긴장 조건을 부가하고 행동량, 주관량, 생리량을 계측하여 통계적으로 긴장 상태 유무와 작업 난이도에 따른 변화를 분석하였다. 긴장 스트레스를 유발시키기 위하여 20대 남자 대학생 3D명에게 세 가지 레벨의 트랙킹 작업을 수행하게 하면서 긴장상태를 유발하여 행동량, 주관신고와 생리신호를 측정하였다. 행동량과 주관신고의 결과를 사용하여 본 연구에서 상정된 작업난이도와 긴장조건이 적절한 가를 살펴보고 생리신호를 사용하여 작업난이도와 긴장감을 분석하였다. 난이도가 낮을 때 HRV의 회귀도와 GSR의 평균값은 긴장 조건이 있을 때가 없을 때에 비해서 통계적으로 유의하게 높게 나타났으며 난이도가 높을 때에는 피부온도의 값이 긴장 조건이 있을 때가 없을 때에 비해서 통계적으로 유의하게 높게 나타났다. 또한 긴장 조건이 있는 경우 작업의 난이도가 높을 때에 낮을 때에 비해서 호흡수의 값이 높게 나타났다. 따라서, 본 연구에서 상정한 긴장감은 양호하게 부가되었고 긴장감은 생리신호를 사용하여 측정할 수 있을 것으로 분석되었다.

  • There is substantial evidence that anatomical connections and functional interactions exist between vestibular and autonomic systems. Heart rate variability (HRV) including mean, standard deviation, coefficient of variation (CV), power spectrum was analyzed for evaluation of the physiological role of the vestibular system on control of heart rate in rabbits. In anesthetized rabbits, electrical stimulation of the vagus nerve decreased heart rate and decreased LF/HF by increasing HF. On the cervical sympathetic nerve increased heart rate and increased LF/HF by increasing LF. Atropine, cholinergic blocker, increased heart rate and increased LF/HF by reducing HF, and propranolol, ${\beta}$-adrenergic blocker, decreased heart rate and decreased LF/HF by reducing LF> In unanesthetized rabbits, stimulation of the vestibular system induced by rotation or caloric increased heart rate and increased LF/HF by increasing LF> Also electrical stimulation of the vestibular nerve produced the same of effects as rotation or caloric in anesthetized rabbits. These results suggest that Stimulation of the vestibular system increased heart rate not by inhibiting the parasympathetic nerve but by activating the sympathetic nerve.

  • Medications or conitive-behavior methods have been mainly used as a treatment of alcoholism. lately the virtualy technology has been applied to the kink of alcoholic disorders. A virtual environment makes him having avility to over come the drink. In this study, we were implemented by making panorama images and 3D object modules using 3D MAX, VRML, JAVA. And the BAR stimulator that composed with a position sensor, head mount display, and audio system, is suggested. To illustrate the physiological difference between a person who has a alcoholism and without a liquor bottle, heart rate was measured during experiment, and also measured a person's HR after the virtual reality training. The system measures the Physiological signals such as ECG, Temperature, analyzes those data automatically. The system has two parts, one is physiological data acquisition part which gets the body signal, and the other one is mobile nuit which includes signal processing and transmission functions, And Bluetooth allows two parts to communicate with each other. we demonstrated the subjective effectiveness of virtual reality psychotherapy through the clinical experiment.

The Influence of Transcutaneous Electrical Stimulation on Autonomic Function (경피신경전기자극이 자율신경계에 미치는 영향)

  • Purpose:The purpose of this study is to investigate the effect of transcutaneous electrical stimulation on autonomic nervous system using heart rate variability analysis. Methods:31 subjects were evaluated with HRV before and after a single high-frequency TENS for 15minutes treatment. The standard deviation of all the normal RR-interval(SDNN) and lower frequency/high frequency ratio(LF/HF ratio) were recorded with TAS-9. Results:After single TENS treatment, autonomic adaptation as analyzed by SDNN was increased from $42.47\pm12.96$ to $4.43\pm16.76$(p>0.05) and autonomic balance as analyzed by LF/HF ratio was increased from $1.4\pm0.45$ to $1.45\pm0.41$(p>0.05). Conclusion:In this study, SDNN and LF/HF ratio changed in health young students as a result of single TENS treatment. But there is no significant difference between pre-treatment and post-treatment of SDNN and LF/HF ratio.

Analysis of Factors Affecting Stress in the Elderly Women

  • The purpose of this study is to identify the factors influencing the stress of the elderly women by various tools and to provide basic data for adaptation to the aging women. For this purpose, stress was measured by stress measuring equipment (HRV) and stress measurement questionnaire (SRI) in 31 elderly women over 60 years old. In summary, the results of this study suggest that the worse the subjective health, and when there is a spouse, it predicts a stress. In order to reduce the stress of elderly women based on these results, physical health management and environmental factors that can enhance the intimacy between husband and wife are also necessary.

Verification of Physiological and Psychological Effects of Vertical Indoor Garden (수직형 실내정원이 주는 생리적·심리적 효과 검증)

  • The purpose of this study is to verify the physiological and psychological benefits of indoor garden to humans and to analyze the comparison according to Type A behavior pattern. The subjects included 18 male university students. Heart rate variation was used as a parameter of physiological assessment, and Semantic Differential (SD), Profile of Mood States-Brief (POMS), and State-Trait Anxiety Inventory (STAI) were used as tools for psychological evaluation. The participants were subjected to a psychological evaluation when they were present in a garden. As a result, the parasympathetic nervous activity was increased in the indoor garden. In the analysis of psychological evaluation, indoor garden improved the positive mood states and decreased negative feelings with significant changes only in Type A group. This study supported that indoor garden can have physiological and psychological relaxing effects, which could be more significant in Type A group than Type B.

Correlational Analysis of Supine Position Time and Sleep-related Variables in Obstructive Sleep Apnea Syndrome (폐쇄성 수면무호흡 증후군에서 앙와위 자세시간과 수면관련변인 간 상관관계 분석)

  • Objectives: A supine sleep position increases sleep apneas compared to non-supine positions in obstructive sleep apnea syndrome (OSAS). However, supine position time (SPT) is not highly associated with apnea-hypopnea index (AHI) in OSAS. We evaluated the correlation among sleep-related variables and SPT in OSAS. Methods: A total of 365 men with OSAS were enrolled in this study. We analyzed how SPT was correlated with demographic data, sleep structure-related variables, OSAS-related variables and heart rate variability (HRV). Multiple linear regression analysis was conducted to investigate the factors that affected SPT. Results: SPT had the most significant correlation with total sleep time (TST ; r = 0.443, p < 0.001), followed by sleep efficiency (SE ; r = 0.300, p < 0.001). Snoring time (r = 0.238, p < 0.001), time at < 90% SpO2 (r = 0.188, p < 0.001), apnea-hypopnea index (AHI ; r = 0.180, p = 0.001) and oxygen desaturation index (ODI ; r = 0.149, p = 0.004) were significantly correlated with SPT. Multiple regression analysis revealed that TST (t = 7.781, p < 0.001), snoring time (t = 3.794, p < 0.001), AHI (t = 3.768, p < 0.001) and NN50 count (t = 1.993, p = 0.047) were associated with SPT. Conclusion: SPT was more highly associated with sleep structure-related parameters than OSAS-related variables. SPT was correlated with TST, SE, AHI, snoring time and NN50 count. This suggests that SPT is likely to be determined by sleep structure, HRV and the severity of OSAS.

Study on the Radial Pulse Wave Variables and Heart Rate Variability after Acupuncture Stimulation (침자극이 좌관맥상과 심박수 미세변화에 미치는 영향)

  • We investigated the effects of acupuncture stimulation on pulse wave variables and heart rate variability (HRV) in healthy adults. To evaluate twenty healthy volunteers (10 men, 10 women) had acupuncture into both Hapkok (Ll4) and Taechung (Liv3) for 15 minutes. Radial pulse, Heart Rate Variability, body temperature and blood pressure were checked before and after acupuncture to evaluate Pulse Wave Variability and Autonomous Function. The results were as follows; Heart rate was significantly increased while systolic blood pressure (mmHg) and diastolic blood pressure (mmHg) were significantly increased after acupuncture treatment. Sixteen subjects didn't change representative pulse wave variables after acupuncture treatment. Energy, height of main peak (H1), height of pre-dicrotic valley (H2) and height of descending valley (H4) were decreased while height of dicrotic peak (H5) was significantly decreased after acupuncture treatment. Time to main peak (T1), time to pre-dicrotic valley (T2), time to dicrotic peak (T5), total time (T) and T-T4 were decreased while time to descending valley (T4) was increased after acupuncture treatment. Total area (At), area of main peak (Aw) and ratio of diastolic period area (Ad) were decreased while ratio of systolic period area (As) and angle of main peak (MPA) were increased after acupuncture treatment. The standard deviation of all normal RR intervals (SDNN) was increased while the root mean square of successive differences between the normal heart beats (RMSSD) was significantly increased after acupuncture treatment by time domain analysis. Low frequency power (LF) and LF/HF ratio were decreased while high frequency power (HF) was significantly increased after acupuncture treatment by frequency domain analysis. This study suggests that acupuncture treatment changes pulse wave variability and heart rate variability. Further study on various acupuncture treatment for pulse wave variability and heart rate variability is required.
