Severity of Obstructive Sleep Apnea and Heart Rate Variability : Detrended Fluctuation Analysis (폐쇄성 수면 무호흡증의 심각도와 심박동 변이율 : 탈경향변동분석)

  • Objectives : The detrended fluctuation analysis is one of the nonlinear methods for the investigation of biological time series. It quantifies the fractal scaling properties and is known to be useful in the evaluation of long-range correlations in time series. The heart rate variability(HRV) of obstructive sleep apnea syndrome (OSAS) patients during nighttime was analyzed by detrended fluctuation analysis to assess its relationship with the severity of the symptoms. Methods : Fifty nine untreated male OSAS patients with moderate to severe symptoms(mean age=45.4${\pm}$11.7 years, apnea-hypopnea index, AHI${\geq}$15) underwent nocturnal polysomnography. Moderate(AHI=15-30, N=22) and severe(AHI>30, N=37) OSAS patients were compared for the indices derived from detrended fluctuation analysis and frequency domain analysis of HRV. Results : In the detrended fluctuation analysis, the alpha values were 0.75${\pm}$0.11 and 0.82${\pm}$0.07 for the severe and the moderate OSAS groups respectively. The difference was significant(p<.01). The alpha value had negative correlation with AHI(r=-.425, p=.001). Negative correlation coefficients were also found in the relationships between the alpha values and very low frequency(VLF)(r=-.425, p=.001), low frequency(LF)(r=-.633, p= <.001) and the LF/HF ratio(r=-.305, p=.019) respectively. LF/HF ratio(p=.005) was higher in the severe OSAS group compared to that of the moderate OSAS group. Conclusion : In this study, the detrended fluctuation analysis showed the significant difference between the two OSAS groups classified according to their severity of symptoms. The scaling exponent showed the negative correlation with AHI and indicies of frequency domain analysis. This result suggests that detrended fluctuation analysis can be helpful to estimate the severity of OSAS.

Observational Study on the Effectiveness of Korean Medical Treatment on Stress Caused by Traffic Accidents (한의 치료가 교통사고 환자의 스트레스 및 심박변이도 변화에 미치는 영향)

  • Objectives : The purpose of this study is to find the effectiveness of Korean medical treatment on stress caused by traffic accidents. HRV analysis and IES-R K research were used to evaluate it. Methods : We selected patients who entered the KangNam KyungHee Hospital between 28 Sep 2013 and 14 Apr 2014 and met inclusion/exclusion criteria. Before taking treatment, we took HRV and IES-R-K first to know the exact mental conditions of patients. For a week, we had taken acupuncture treatment, herb medicine treatment and physical therapy. After that, we compared patients' changes between before-treatment and after-treatment with the same examination. Results : In a period of one week, Korean medical treatment makes LF(Norm) stay within the normal range and HF(Norm) increase. But, we could not find statistical significance between two. The normal range of LF/HF ratio increased, and there was statistical significance. After treatment, the IES-R-K score tended to decrease. Statistically, korean medical treatment is useful to alleviate subjective stress by traffic accidents. Conclusions : Korean medical treatment makes the sympathetic system relax and parasympathetic system rise so it balances an autonomic nervous system. But, there is not statistical significance. Also, it gives positive effect on car accident patients' treatment in terms of easing their subjective stress. Prospective and Larger study will be necessary.

An Analysis of Comparison between Autogenes Training and Imaginary Meditation by Heart Rate Variability and SCL-90-R (심박변이도 및 SCL-90-R 측정을 통한 자율훈련법과 과일 명상의 효과 비교 분석)

  • Objective: There are many meditation methods used in clinical field. But the effect of meditation may be different to people's mind, and meditation itself. Now we investigate this subject with two different meditation methods which has been analyzed by heart rate variability. Method: We tried to check people's heart rate who educated meditation, before educated and during the meditation. The meditation was educated differently for each group. One is the auto genes traning, and the other is the imaginary meditation by thinking about eating and smelling favorite fruits. All of them are used to oriental psychiatry clinic patients. And we simultaneously conduct people's emotional state by SCL-90-R. Result: The result in Autogene traning group has significant difference in total power and HRV-index score between before and during the training but in imaginary meditation group has no significance: Autogene traning group has a significance in low frequency score and has increased tendency with increasing tendency of Heart rate index and Total power. Through the SCL-90-R score, the higher depression t-score, the anxiety t-score, and the hostility score is, the HRV scores such as total power, low frequency, and HRV-index much increase. and so on. Conclusion: We can guess that the Autogenes training has more significant effect than the imaginary meditation. And we can also guess that as the emotional problem is getting more severe, the effect of meditation would be much better. And we finally suggest that we should make a standard guideline of imaginary meditation, cause the imaginary meditation has so many variation.

Analysis of Heart Rate Variability in Cold Hypersensitivity Females Visiting Gangnam Kyung-Hee Korean Hospital (강남경희한방병원에 내원한 냉증을 호소하는 여성의 Heart Rate Variability 특성 연구)

  • Objectives: To characterize autonomic dysfunction in patients with Cold hypersensitivity, their heart rate variability(HRV) were measured and analyzed with cold hypersensitivity patients, compared with those of normal population. Methods: We studied 56 patients visiting Gangnam Kyung-hee korean hospital from 1st January 2010 to 31th December 2010. Heart rate variability were obtained from 28 female patients who suffer from cold hypersensitivity and 28 healthy female controls in resting state. We studied the difference of Heart rate variability between two groups by Independent T-test using SPSS for windows(version 17.0). Results: Standard deviation of NN interval(SDNN), total power(TP), Very low frequency(VLF) in patients with cold hypersensitivity were significantly different with those of controls. While no significant differences were observed in square root of mean squared differences of successive NN intervals (RMSSD), low frequency (LF) and high frequency(HF). Conclusions: The decreased values of heart rate variability(HRV) study means that they may have some kinds of imbalance in autonomic nervous system in cold hypersensitivity patients. heart rate variabiliry(HRV) study might be a tool of diagnosis and predictors in cold hypersensitivity patients.

Relationships between depression, anxiety, 'exterior-interior pattern and cold-heat pattern' and Heart Rate Variability in healthy Subjects (건강인의 표리 한열 변증, HRV, 우울, 불안 지표의 상관성 분석)

  • The identification of the exterior-interior pattern and the cold-heat pattern is one of the most frequently used diagnostic methods in Oriental medicine. No systematic studies, however, have yet been conducted to determine the emotional and autonomic factors involved in the exterior-interior and cold-heat. In this study, the relationships between depression, anxiety, and the exterior-interior and cold-heat patterns in 100 healthy female volunteers with a mean age of 42.77 were also investigated. The autonomic nervous system's control of human temperature is a well known fact. Thus, this paper also aimed to investigate the correlationof the autonomic nervous system and patterns of the exterior-interior and cold-heat. The analysis of heart rate variability (HRV), which has become a popular non-invasive tool for assessing the activities of the autonomic nervous system, was conducted in this study. No relationship was found between the exterior-interior and cold-heat pattern scores and the degree-of-depression scores. But there was a significant difference between the exterior-interior and cold-heat pattern scores of the different anxiety types, and between those of the different anxiety levels. The depression and anxiety levels also had an effect on the HRV indices.

Postprandial Hypotension and Heart Rate Variability in Older Adults (노인의 식후 저혈압과 심박동변이)

  • Purpose: A study was designed to compare heart rate variability (HVR) of older adults with postprandial hypotension (PPH) with older adults with no PPH. Methods: The samples were community-dwelling older adults (${\geq}$ 60 years old) with PPH (n=35) and without PPH (n=35). HRV was measured by 10 minutes of electrocardiography using Physiolab 400 and analyzed in time domain (SDNN) and with power spectral analysis in frequency domain (TP, LF, HF, LF/HF). Blood pressure was measured every 15 minutes before and until 120 minutes after the meal. Results: All HRV parameters were lower in the PPH group compared to those with no PPH. Specifically the measures of SDNN, TP, LF, and LF/HF of the PPH group were significantly lower (U=385.0, $p$=.008; U=361.0, $p$=.003; U=375.0, $p$=.005; U=368.0, $p$=.004, respectively). Further, the LF and LF/HF of SBP (${\geq}140$ mmHg) group and the LF/HF of DBP (${\geq}$ 90 mmHg) group were significantly lower ($x^2$=6.33, $p$=.042; $x^2$=33.10, $p$ <.001; $x^2$=11.03, $p$=.004, respectively). Conclusion: These findings indicate that the autonomic nervous functions in persons with PPH are less activated than those persons with no PPH. The findings contribute to an understanding of HRV and the association of autonomic nervous function with the mechanism of PPH.

Molecular Cloning and Nucleotide Sequence of the G protein of a Korean Isolate of Infectious Hematopoietic Necrosis Virus (한국에서 분리된 IHNV-PRT의 G protein의 유전자 클로닝과 염기서열 분석)

  • To characterize the Korean isolate of infeciious hematopoietic necrosis virus (IHNV-PRT), a partial DNA fragment G gene of the MNV-PRT was amplified by RT-PCR. cloned inlo pGEM-T easy vector and analyzed for nucleotlde sequences. The size of the PCR pmduct was about 442 bp. The nucleotlde sequence homologies ofthe G gene of IHNV-PRT were 95%, 94%, 94% 94%, 93%, 53%. respectively. with those of foreign isolates of IHNV, IHNV-RB-76. IHNV-LR-73, MNV-K, IHNV-WRAC, Im-SRCV, IHNV-Col-85. However, it showed 81% homology with that of other fish rhabdovirus, hisame rhabdovirus (HRV). Frou~ the rcsults of deduced amino acid sequence homology analysis. G protein of IHNV-PRT showed 96% hornologies with those of foreign isolates of IHNV but 89% homology with that of HRV These results indicaled that, even though G gene of IHIW-PRT showed low homology with that of HRY it was highly conserved among different strains of THNV.

  • Stress at work has become a serious problem affecting many people of different professions, life situations, and age groups. Stress management should start far before the stress start causing illnesses. In this study, studies were conducted to evaluate stress by measuring the Galvanic skin Response(GRS), Electrocardiograph(ECG), and Electroencephalogram(EEG) generated during images and noise stimuli. The GRS amplitude showed that the stress situation was 27.9 % higher than the baseline. And after the stimulus period, the response time of baseline was longer than 71.6 % than the stress situation. The stress response characteristics of the HRV showed that the rate of change in RMSSD was 16.4 %, and the rate of change of the HF Power was 29.7 %. EEG showed that the frequency band was gradually changed to the ${\theta}$ wave band during stress stimulation. We will be able to utilize image stimuli and noise stimuli as an objective indicator of stress and correlation.

Analysis on Heart Rate Variability (HRV) Characteristics of Patients with Insomnia during Perimenopause and Postmenopause: A Retrospective Chart Review (불면증이 동반된 여성 갱년기 환자의 심박변이도 특성 분석 : 후향적 차트리뷰)

  • Objectives: The purpose of this study is to demonstrate Heart Rate Variability characteristics of menopausal patients with insomnia. Methods: From March 1, 2014 to June 20, 2017, Heart Rate Variability was measured in 102 menopausal patients who visited Cheonan Korean Medicine Hospital of Daejeon University. We compared accompanying symptoms and Heart Rate Variability values depending on sleep quality in menopausal women. Results: The accompanying symptoms of menopausal patients were as follows: hot flushes (45.1%), tiredness (25.49%), chest discomfort and palpitations (23.53%), headache (17.65%), arthralgia and muscular pain (17.65%), cold sensitivity of hands and feet (15.69%), urinary frequency (14.71%) and anxiety (10.78%). The frequency of chest discomfort and palpitation was significantly higher in the menopausal insomnia group than in normal sleep group. Comparing Heart Rate Variability between two groups, Standard deviation of the NN interval (SDNN), Total Power (TP), and Low Frequency (LF) values were significantly lower in insomnia group. Conclusions: Chest discomfort and palpitations were more frequent in insomnia patients in menopausal women than normal sleep group, and Standard deviation of the NN interval (SDNN), Total Power (TP), Low Frequency (LF) were significantly lower in HRV values.

Effect of Acupuncture at the Field of the Auricular Branch of the Vagus Nerve on Autonomic Nervous System Change (미주신경 감각분지 분포영역의 자침이 자율신경 변화에 미치는 영향)

  • Objectives This study was designed to identify the changes of autonomic nervous system (ANS) which was induced by acupuncture at the field of the auricular branch of the vagus nerve. Methods 30 healthy adults were selected and classified into two groups; experimental group, control group. After providing mental stress, acupuncture was applied at external ear in experimental group and no treatment executed in control group. The evaluation of ANS function was measured by heart rate variability (HRV). We statically analyzed the difference of HRV parameters which include mean heart rate (MHRT), standard deviation of all N-N intervals (SDNN), square root of the mean of the sum of the squares of differences between adjacent N-N intervals (RMSSD), total power (TP), low frequency power (LF), high frequency power (HF), LF/HF ratio. Results All subjects showed significant increase in SDNN, LF after stress stimulation (p<0.05). Immediately after intervention, MHRT was significantly decreased (p<0.001) and RMSSD, HF were significantly increased in experimental group (p<0.05). After the end of intervention, SDNN, HF, RMSSD, TP, LF were significantly increased in experimental group (p<0.01, p<0.05). And when comparing baseline HRV, SDNN, LF were significantly increased in control group (p<0.01) and SDNN, RMSSD, TP, LF were significantly increased in experimental group (p<0.05). In the subgroup analysis, normal balance of ANS group showed significant increase in TP, LF, SDNN, HF (p<0.01, p<0.05). Conclusions We suggested that acupuncture at external ear, region of the vagus nerve distribution could increase parasympathetic activity and cause changes and reregulation of the ANS.
