A DELAY-DIFFERENTIAL EQUATION MODEL OF HIV INFECTION OF CD4+ T-CELLS

  • In this paper, we introduce a discrete time to the model to describe the time between infection of a CD4$^{+}$ T-cells, and the emission of viral particles on a cellular level. We study the effect of the time delay on the stability of the endemically infected equilibrium, criteria are given to ensure that the infected equilibrium is asymptotically stable for all delay. We also obtain the condition for existence of an orbitally asymptotically stable periodic solution.

AIDS환자 치료를 위한 점진적 약물감소기법에 감염속도상수가 미치는 영향 (The influence of infection ratio on Gradual Reduction of Drug Dose for the treatment of AIDS patients)

  • In this paper, we study the influence of infection ratio on gradual reduction of drug dose for the five state HIV infection model that explicitly includes the population of the virus. We first compute all equilibrium points of the model and investigate the stabilities of them. As a result, a bifurcation diagram is obtained which shows a change in the equilibrium points, or in their stability properties, as the drug effect $\eta$ is varied from 0 to 1(alternatively, drug dose is changed from 1 to 0). Based on the bifurcation diagram, we show that the gradual reduction of drug dose can be applied for the treatment of AIDS patients. Moreover, we analyze the influence of the variation of infection ratio on the gradual reduction treatment. Computer simulation results are also presented to validate the proposed results.

  • This work concerns the stabilization of uninfected steady state of an ordinary differential equation system modeling the interaction of the HIV virus and the immune system of the human body. The control variable is the drug dose, which, in turn, affects the rate of infection of $CD4^{+}$ T cells by HIV virus. The feedback controller is constructed by a variant of the receding horizon control (RHC) method. Simulation results are discussed.

CD7-Specific Single Chain Antibody Mediated Delivery of siRNA to T Cells Inhibits HIV Replication in a Humanized Mouse Model

  • A major hurdle to the development of RNA interference as therapy for HIV infection is the delivery of siRNA to T lymphocytes which are difficult cells to transfect even in vitro. We have employed a single chain antibody to the pan T cell surface antigen CD7 was conjugated to an oligo-9-arginine peptide (scFvCD7-9R) for T cell-specific siRNA delivery in NOD/SCIDIL2${\gamma}$-/- mice reconstituted with human peripheral blood lymphocytes (Hu-PBL). Using a novel delivery, we first show that scFvCD7-9R efficiently delivered CD4 siRNA into human T cells in vitro. In vivo administration to Hu-PBL mice resulted in reduced levels of surface CD4 expression on T cells. Mice infected with HIV-1 and treated on a weekly basis with scFvCD7-9R-siRNA complexes targeting a combination of viral genes and the host coreceptor molecule CCR5 successfully maintained CD4/CD3 T cell ratios up to 4 weeks after infection in contrast to control mice that displayed a marked reduction in CD4 T cell numbers. p24 antigen levels were undetectable in 3 of the 4 protected mice. scFvCD7-9R/antiviral siRNA treatment also helped maintain CD4 T cell numbers with reduced plasma viral loads in Hu-PBL mice reconstituted with PBMC from donors seropositive for HIV, indicating that this method can contain viral replication even in established HIV infections. Our results show that scFvCD7-9R could be further developed as a potential therapeutic for HIV-1 infection.

Cost-Effectiveness of Voluntary HIV Testing Strategies in a Very Low-Prevalence Country, the Republic of Korea

  • Background: The Republic of Korea has a very low prevalence of human immunodeficiency virus (HIV) infection, but the number of new HIV diagnoses has steadily risen, strongly indicating a large number of undetected HIV infections. Thus, it is important for Korean public health authorities to adopt and encourage cost-effective HIV detection tools, such as rapid HIV screening tests. In this study, we aimed to evaluate the cost-effectiveness of enzyme-linked immunosorbent assays (ELISA) and rapid tests in a public health center (PHC) setting. Methods: We developed a decision analytic model to assess the per-examinee cost and the cost-effectiveness of identifying HIV patients in a PHC setting using two HIV testing strategies: conventional HIV screening by ELISA versus rapid HIV testing. Analysis was performed in two scenarios: HIV testing in an average-risk population and in a high-risk population. Results: Compared to the ELISA, the rapid test was cost-saving and cost-effective. The per-examinee cost was USD 1.61 with rapid testing versus USD 3.38 with ELISA in an average-risk population, and USD 4.77 with rapid testing versus USD 7.62 with ELISA in a high-risk population. The cost of identifying a previously undiagnosed HIV case was USD 26,974 with rapid testing versus USD 42,237 with ELISA in an average-risk population, and USD 153 with rapid testing versus USD 183 with ELISA in a high-risk population. Conclusion: Rapid testing would be more cost-effective than using conventional ELISA testing for identifying previously undiagnosed HIV-infected cases in Korea, a country with extremely low HIV prevalence.

  • The timings of two successive events of interest may not be measurable, instead it may be right censored or interval censored; this data structure is called doubly censored data. In the study of HIV, two such events are the infection with HIV and the onset of AIDS. These data have been analyzed by authors under the assumption that infection time and induction time are independent. This paper investigates the regression problem when two events arc modeled to allow the presence of a possible relation between two events as well as a subject-specific effect. We derive the estimation procedure based on Goetghebeur and Ryan's (2000) piecewise exponential model and Gauss-Hermite integration is applied in the EM algorithm. Simulation studies are performed to investigate the small-sample properties and the method is applied to a set of doubly censored data from an AIDS cohort study.

Prevention of Murine Acquired Immunodeficiency Syndrome (MAIDS) Development by Oriental Herb Extracts

  • Oriental medicinal herb extracts (OHE) showing anticancer activities were investigated for effectiveness as antiviral drugs. Infection of MuLV to cell line resulted in formation of giant syncytia. Number of giant syncytia in culture treated with OHE decreased by 40% compared to that of non-OHE-treated cell culture. To determine OHE effects on progeny release, RT-PCR was performed. In vivo animal studies demonstrated effectiveness of OHE as antiviral drug when administered orally. After OHE administration, viral cytopathic effects decreased. Infected mice showed splenomegaly and over-proliferation of lymphocytes with decreased CD4+ cell counts. These symptoms decreased in OHE-treated mice, indicating OHE maybe useful therapeutics against MuLV/MAIDS as Human Immunodeficiency Virus (HIV)/AIDS animal model. Results show XC plaque assay and in vivo MAIDS model using MuLV are suitable tools for screening anti-retroviral drug candidates.

이식을 위한 사람 양막의 소독 및 멸균공정에 의한 감염성 위해인자 불활화 효과 (Inactivation of Infectious Microorganisms by Disinfection and Sterilization Processes for Human Amniotic Membrane Grafts)

  • 이식을 위해 사용하는 사람 양막은 기증자로부터 수혜자에게 바이러스, 세균, 진균과 같은 감염성 위해인자를 전파할 위험이 있다. 따라서 적절한 소독 및 멸균 공정을 통해 이식용 양막 내재 또는 혼입 가능한 감염성 위해인자를 완벽하게 불활화하여야 한다. 본 연구에서는 인체조직은행에서 사용하고 있는 소독 공정과 멸균 공정의 바이러스 및 세균, 진균 불활화 효과를 검증하기 위해 국제적 가이드에 따라 5종의 바이러스[human immunodeficiency virus type 1 (HIV-1), bovine herpes virus (BHV), bovine viral diarrhoea virus (BVDV), hepatitis A virus (HAV), porcine parvovirus (PPV)]와 2종의 세균(Escherichia coli, Bacillus subtilis), 1종의 진균(Candida albicans)을 생물학적 지표로 사용하였다. 양막에 각 생물학적 지표를 첨가한 후 70% 에탄올 소독 공정, 감마선 조사 공정, 산화에틸렌 가스 멸균 공정을 실시한 다음 각 바이러스, 세균, 진균을 회수하여 정량한 후 불활화 정도를 비교하였다. 70% 에탄올 처리 공정에서 HIV-1, BHV, BVDV 같은 외피 바이러스는 처리 시간 2.5분 안에 불활화되었지만, HAV와 PPV 같은 비-외피 바이러스는 에탄올에 매우 큰 저항성을 나타내었다. 감마선 2.5 kGy 조사에 의해 HIV-1, BHV, BVDV는 검출한계 이하로 완벽하게 불활화되었다. HAV와 PPV는 각각 5 kGy와 25 kGy 조사에 의해 검출한계 이하로 불활화되었다. 산화에틸렌 가스 처리에 의해 본 연구에 사용한 모든 바이러스가 검출한계 이하로 불활화되었다. 70% 에탄올 처리 공정에서 E. coli와 C. albicans는 모두 5분 안에 완벽하게 사멸하였다. 하지만 B. subtilis는 큰 저항성을 나타내었다. 감마선 조사 공정과 산화에틸렌 가스 멸균 공정에서 E. coli, B. subtilis, C. albicans 모두 완벽하게 불활화되었다.

한방의료기관 감염관리에 대한 한의사의 인식도와 수행도 (Perception and Practice level of Korean Medical Doctors on Infection Control and Prevention in Korean Medicine Facilities)

  • Purpose : This study was designed to identify the perception and practice level of infection control among korean medical doctors and to identify factors that may influence the performance and practice level. Methods : Data were collected using the on­line survey method. Seven hundred and eighty four Korean medical doctors(KMD) participated the survey. The study was conducted from December 2018 to January 2019. Results : The results of this study are as follows. 1. Participants who experienced infection risk by needles or sharp instruments were 596(76%). and participants who had experienced blood or body fluid contact with the mucous membrane or skin of the patient during treatment were 226(28.8%) of them. 2. The degree of perception and practice of the infectious guideline was higher in the group over 50 years, in the doctor group, in the group with more than 6 years experience in clinic and in the group who work in the hospital. (p < 0.05) 3. In the performance of the infection control management related to the Korean medical treatment, the practice level of the article 'Discard the remaining needle that used for one patient' was the lowest at 4.02, 'Identify the patient and check the validity period of sterilization of medicines or instruments' was the second lowest in 4.16. 4. Among the contents of "Prevention of Nosocomial infection and Sanitary Safety Guidelines" issued by the Korean Medical Association in 2008, the guideline "Prevention of infection by pathogens such as HIV, MRSA, SARS" were lowest article in the perception and practice level of participants. 5. Regression analysis was performed to find out the factors affecting perception and performance of the participants. The regression model showed significant difference in the regression model of the working years. (p < 0.05) 6. In order to examine the effect of the variables on the perception and practice of the infectious guideline, the mediated effect of the knowledge and education level according to the years of working, age, education degree was found to be significant only in the education degree variable. (p < 0.05) In conclusion : in order to improve the perception and practice of infectious control of Korean medicine doctors, it is necessary to include the contents of infection management as essential education during the continuing education of Korean medicine association.

사회복지학 및 간호학 전공 대학생의 에이즈 감염인에 대한 인식, 지식, 편견 및 사회적 거리감 (University student's Perception, Knowledge, Attitude and Social distance for People living with HIV/AIDS)

  • 본 연구는 향후 사회적 취약계층을 돕는 전문직을 양성하는 분야인 사회복지학과 간호학 전공 대학생의 에이즈에 대한 인식, 지식, 에이즈 감염인 및 사회적 소수인에 대한 편견, 에이즈 감염인에 대한 사회적 거리감을 탐색하고, 사회적 거리감과 관련된 요인을 분석하는 것을 목적으로 한다. 645명의 자료를 분석하였으며, 분석결과, 첫째, 에이즈 감염인에 대한 인식은 성과 관련된 단어를 떠올리는 경우가 가장 많았으며, 둘째, 에이즈에 대한 지식의 경우, 예방과 관련된 문항, 감염경로에 대한 일부 문항은 전체적으로 지식이 높은 대상자라 하더라도 오답인 경우가 많았다. 셋째, 사회적 소수인에 대한 편견 및 에이즈 감염인에 대한 편견은 각각 상관관계가 통계적으로 유의하였으며, 마지막으로, 에이즈 감염인에 대한 사회적 거리감에 영향을 미치는 요인은 위계적 회귀분석 결과 에이즈 감염인에 대한 편견만 최종적으로 통계적으로 유의한 변인으로 남았다. 최종모델의 설명력은39%였다. 본 연구결과가 대학생 교육과 프로그램 개발의 방향 모색을 위한 기초자료로 활용되기를 기대한다.
