가치통합 의사결정모델을 이용한 간호학생 대상 웹기반 환자권리교육 시뮬레이션 프로그램 개발 및 평가 (Development and Evaluation of a Web-based Simulation Program on Patient Rights Education using Integrated Decision Making Model for Nurse Students)

  • Purpose: This study was designed to develop and evaluate the a web-based simulation program on patient rights education using integrated decision making model into values clarification for nurse students. Methods: The program was designed based on the Aless & Trollip model and Ford, Trygstad-Durland & Nelms's decision model. Focus groups interviews, surveys on learning needs for patient rights, and specialist interviews were used to develop for simulation scenarios and decision making modules. The simulation program was evaluated between May, 2011 and April, 2012 by 30 student nurses using an application of the web-based program evaluation tools by Chung. Results: Simulation content was composed of two scenarios on patient rights: the rights of patients with HIV and the rights of psychiatric patients. It was composed of two decision making modules which were established for value clarifications, behavioral objective formations, problems identifications, option generations, alternatives analysis, and decision evaluations. The simulation program was composed of screens for teacher and learner. The program was positively evaluated with a mean score of $3.14{\pm}0.33$. Conclusion: These study results make an important contribution to the application of educational simulation programs for nurse students' behavior and their decision making ability in protecting the patient rights.

병원 간호사의 경피 상해 발생 실태 (Incidence of Cutaneous Injury in Clinical Nurses)

  • Purpose: To identify the incidence of cutaneous injury in clinical nurses. Method: From Feb.1 to 28, 2005, 276 clinical nurses were surveyed by questionnaire. Results: 1. Of the nurses, 53.6% had at least one incidence of cutaneous injury, and the mean number of injuries was 1.34. A higher incidence rate for cutaneous injury was found in nurses who were under the age of 25, unmarried and who had less than 3 years career experience. 2. The major causes of injury were syringe needles at 65.0%, and medical instrument were next followed by sharp objects or blades. The injuries occurred when the nurses were rearranging equipment after care (25.2%), taking blood samples (22.8%), separating syringes and needles (17.1%), during surgical operations (14.2%), and distribution of medications, treatments and recapping of needles (5.7% each). The hands were the most common body parts injured, and the most prevalent pathogens contaminating the instruments causing the injury were HBV syphilis, HCV and HIV in that order. 3. Of the injured nurses, 77.9% did not report the accident and 25.8% did not receive any treatment because there were no pathogens, it was a bother or there was difficulty reporting the incident. Conclusion: To reduce cutaneous injuries, intensive training and supervision may be needed for those of nurses under the age of 25, unmarried and with less than 3 years career experience.

토론식 AIDS 교육이 대학생의 AIDS 관련 지식, 편견, 사회적 상호작용 및 예방의도에 미치는 효과 (Effects of Discussion Centered AIDS Education on Knowledge, Prejudice, Social Interaction, and Intention for Prevention in College Students)

  • Purpose: The purposes of the study were to determine the effects of AIDS education which includes discussion on knowledge, prejudice, social interaction, and intention for AIDS prevention in college students. Method: A convenience sampling method was used to collect data, and 136 university students participated in the study. One group pretest-post test design was used and questionnaire for measuring knowledge about AIDS and attitude to AIDS were administered before and after 100 minutes of AIDS education, The education program consisted of a 50-minute lecture with video aids and 50 minutes of small group discussion. Means, SD, and paired t-test with the SPSS program were used to analyze the data. Results: There were significant increases in knowledge(p= .000) and social interaction(p= .002) after the education, but no significant changes in prejudice(p= .832) nor in intention for AIDS prevention(p= .074). Conclusion: AIDS education including discussion was found to be an effective method for improving knowledge and social interaction of college students. The results suggest that education which includes discussion sessions should be used with college students to improve social interaction with people who are HIV positive and enhance knowledge about AIDS.

한국의 건강검진 현황 (Current Status of Health Screening in Korea)

  • Along with a development of medical technology, a variety of tests, such as laboratory tests, x-ray and endoscopies are being used in health screening tests. As the tests determine the quality of health screening, test items and methods should be carefully selected. This study was to get hold of the test items of major health screening programs in Korea Most of the health screening programmes focused upon detection of risk factors and diagnosis of life - style related diseases (diabetes, hypertension, cardiovascular diseases, hypercholesterolemia, overweight, drinking, smoking, cerebrovascular diseases, osteoporosis) ,cancers (stomach, cervix, lung, breast, liver, colon, prostate, ovary, pancreas, thyroid, esophagus) , infectious diseases (hepatitis, tuberculosis, sexually-transmitted diseases, parasites) , chronic obstructive respiratory diseases, chronic renal diseases (bacteriuria hematuria, proteinuria) , anemia, glaucoma, hearing loss, Alzheimer disease, stress, early psychiatric diseases. The health screening tests were basic physical examination, basic laboratory tests (CBC, urinalysis, liver function tests, lipid tests, glucose, HbAlc, uric acid, electrolytes, serological tests (HBsAg, HBs-Ab, HCV-Ab, HIV-Ab, VDRL) EKG, x-ray (chest PA, CT) , endoscopy (gastroscopy, colonoscopy) , sonography(abdomen, thyroid, pelvis, breast) ,cytology (cervix) , bone density, tumor markets (NMP22, alpha-FP, CEA, CA-19-9, CA12S, PSA) and eye tests. Advanced technologies, like CT, PET, MRI, MRT/Angio, molecular testings) were widely used in hospital health screening programmes .In summary, a variety of tests were utilized in health screening in Korea. Those tests were utilized by stages or according to sex and age in most of health screening programmes, however a few program used tests to excess disregarding health screening subjects.

위치기반 상대빈도차 기반의 바이러스 염기서열 시그너쳐 추출 기법 (A Nucleotide Sequence Signature Extraction Method based on Position-Specific Relative Base Frequency Differences)

  • 동일한 집단에 속하는 개체를 다른 집단에 속하는 개체로부터 구별할 수 있는 염기의 특징을 해당 집단의 시그너쳐라고 한다. 학습 데이터는 두 집단에 속하는 염기서열들이고, 염기서열에 대한 시그너쳐는 개체를 다른 집단과 구별할 수 있는 위치의 염기들로 구성된 서열이다. 제안한 방법에서는 각 집단에 대해서 위치별로 염기의 발생빈도를 계산하고, 가장 발생빈도가 높은 염기를 결정한 다음, 다른 집단의 대응 위치에서 해당 염기의 빈도를 계산하여, 빈도차이가 지정한 분류임계값 이상이면, 해당 위치의 염기를 시그너쳐를 구성하는 특징으로 간주한다. 시그너쳐를 대한 임의의 염기서열에 대한 부합정도는 시그너쳐에 속하는 염기의 학습집단에서의 상대빈도값을 가중치로 하여 계산한다. 임의의 염기서열이 특정 집단에 속하는지 판단하기 위해서는 해당 집단의 시그너쳐에 대한 부합정도를 계산하게 되는데, 부합정도가 얼마이상이 되어야 해당 집단에 속하는 것으로 간주할지 기준이 되는 임계값을 엄밀도 임계값이라고 한다. 엄밀도 임계값은 학습 데이터 집합에 대해서 주어진 시그너쳐에 대한 엄밀도 임계값이 민감도와 특이도를 최대로 하는 것을 선택한다. 제안한 방법을 구현한 바이오인포매틱스 도구를 개발하여, 한국형 HIV-1 바이러스 시그너쳐 추출에 적용하여 분류특성이 우수한 시그너쳐를 추출할 수 있음을 확인하였다.

Swaziland의 물리치료 현황과 물리치료 교육프로그램 개발방향 (The Present State of Physiotherapy and the Development Direction of a Curriculum for Physiotherapy Educational Program in Swaziland)

  • Background: The purpose of this study was to search the present state of physiotherapy and the development direction of a curriculum for physiotherapy educational program, especially in field of orthopedic manual therapy, in Swaziland. Methods: The developed curriculum is as follows: 1. Disease and Health State in Swaziland. 2. Medical Human Resources State of Swaziland. 3. Physiotherapy Services State in Swaziland. 4. Higher Education State in Swaziland. 5. Development Direction of a Curriculum for Physiotherapy Educational Program in Swaziland. Conclusions: There is no a physiotherapist in Swaziland and Swaziland government which has not a physiotherapy education programme in any university of whole country. So we need to the development direction of a curriculum for physiotherapy educational program for the future of Swaziland, especially in field of orthopedic manual therapy. The reason is firstly, most of people are obesity in this country, so many people complained a lot of joint and muscle pain. And secondly, also there are many musculoskeletal disease patients now a day in Swaziland because of HIV/AIDS. Lastly, Swaziland country is in a poor and developing state, so physiotherapist it is hard to make a purchase and setting physiotherapy modalities apparatus at Swaziland country.

A DPL (DNA/peptide/liposomes) Tripartite Complex Effective for Transfection in Serum

  • A short peptide corresponding to the nuclear localization signal (NLS) of human immunodeficiency virus (HIV)-l Tat protein, Arg-Lys-Lys-Arg-Arg-Gln-Arg-Arg-Arg, was employed to improve the efficiency of cellular uptake of nucleic acids. The peptide was first mixed with a reporter plasmid and then with cationic liposomes to form a tripartite complex of DNA/peptide/liposomes (DPL). Transfection efficiency of the DPL complex was compared with that of the conventional DNA/liposomes (DL) complex. When the DPL complex was formed with various cationic liposomes, DOTAP/DOPE (DP) liposome exhibited superior transfection efficiency to other liposomes tested in vitro. With the inclusion of the peptide, the DPL complex showed much enhanced transfection in various cancer cell lines. Particularly, transfection of the DPL complex in serum increased cellular uptake of a transgene up to 2 fold when compared with that in a serum free condition. Further, when the DPL complex was infused through the ureteric route of a rat, transfection efficiency was shown to be better in reporter gene expression than that obtained with the DL complex. This study shows that the DPL complex that is easy to formulate can be employed for much enhanced cellular uptake of a trans gene.

전신질환과 관련된 음성장애의 치료 (The Management of Systemic Voice Disorders)

  • Variable systemic diseases affect larynx and vocal fold and result in voice change. Asthma and chronic obstructive pulmonary disease make increase of intra-abdomimal pressure followed by reflux of gastric acid, which stimulate vagal-bronchopulomary reflex aggravating cough and respiratory disturbance. Fungal laryngitis in the general population is extremely rare, but can occur in immunocompromised AIDS patients. Although, initially, empirical antifungal therapy for candidiasis is often given without biopsy, diagnostic direct laryngoscopy and biopsy is imperative if a substantial clinical response is not rapidly achieved. In the highly active anti-retroviral therapy era, HIV-positive patients are living longer and are at higher risk for developing non-AIDS-defining malignancies. The incidence of head and neck cancer (HNC) which is related with human papilloma virus infection has increased. The survival is significantly lower among the AIDS-HNC patients with CD4 counts ${\leq}200cells/{\mu}L$. Rheumatoid arthritis (RA) cause voice disturbance by developing cricoarytenoid joints fixation or nodule on vocal fold. Post-menopausal voice disorder (PMVD) is caused by decreased secretion of estrogen-progesterone resulting in decrease of fundamental frequency (F0). Hormonal replacement therapy is helpful to reduce F0 decrease. RA and PMVD result in slight voice change, but it could crucial in professional voice user.

Prevalence of Enteric Protozoan Oocysts with Special Reference to Sarcocystis cruzi among Fecal Samples of Diarrheic Immunodeficient Patients in Iran

  • The genus Sarcocystis is not usually considered as an important enteric pathogen in immune compromised patients. It might be expected that species for which humans are the final host (Sarcocystis hominis and Sarcocystis suihominis as well as possibly others) would be encountered increasingly often in immunodeficient persons. This study aimed to address how to detect and differentiate Sarcocystis oocysts and/or sporocysts from enteric protozoans in the diarrheal samples of immunodeficient patients in Shiraz, Iran. Diarrheal samples of 741 immunodeficient patients with recurrent persistent or chronic diarrhea were examined by microscopy and molecular biological analysis. Oocysts-positive samples were 68 Cryptosporidium spp., 9 Cystoisospora belli (syn. Isospora belli), 2 Cyclospora cayetanensis, and 15 microsporidia (Enterocytozoon bieneusi). Sarcocystis-like sporocysts found from a woman were identified as Sarcocystis cruzi through 18S rDNA amplification and phylogenetic analysis. To the best of our knowledge, this is the first report of S. cruzi from a human.
