엉치엉덩관절 기능부전에 대한 복합 도수치료 프로그램이 여성의 생리통, 관절 통증, 엉덩관절 가동 범위에 미치는 영향: 단일 사례 보고 (Effects of a Complex Manual Therapy Program for Sacroiliac Joint Dysfunction on Women's Menstrual Pain, Joint Pain, and Hip Range of Motion: A Single Case Study)

  • Background: This study aimed to investigate the effects of a complex manual therapy program on hip and knee pain occurring together with menstrual pain and iliotibial band tightness in a woman with sacroiliac joint dysfunction. Methods: This study was carried out over a period of 3 months by dividing the treatment process into three sequential stages. Intensities of menstrual pain, hip pain, and knee pain, and hip range of motion (ROM) in the participant were assessed at baseline and at the end of each stage, and a modified intervention was applied according to results of assessments after each stage. Results: There was a decrease in the intensity of menstrual pain after the first and third stages of treatment, and there were reductions in the intensities of hip and knee pain after the third stage. Hip ROM was rather decreased after the second stage, but it was increased after the third stage. Conclusion: The findings of this study indicate that treatment of the instability and positional fault of sacroiliac joint can relieve hip and knee pain, improve hip ROM, and alleviate primary dysmenorrhea in patients with sacroiliac joint dysfunction.

노인의 고관절 ROM에 관한 연구 (A Study on Hip Joint ROM of the Elderly)

  • The purpose of this study is to know the average of hip joint range of motion and difference according to the aging for the elderly. This study consisted of elder male(n=75) and elder female(n=109). The result of assessment and analysis in hip pint range of motion are as follows : 1) The average hip flexion(knee flexed) joint range of motion in 60-69(from sixty to sixty-nine)years old are $104.26^{\circ}$(Left-Male), $101.00^{\circ}$(Right-Male), $107.05^{\circ}$(Left-Female), $107.05^{\circ}$(Right-Female). 70-79years old are $104.59^{\circ}$(L-M), $102.05^{\circ}$(R-M), $105.73^{\circ}$(L-F), $108.75^{\circ}$(R-F). 80-89years old are $101.53^{\circ}$(L-M), $101.13^{\circ}$(R-M), $96.83^{\circ}$(L-F), $97.67^{\circ}$(R-F). There was significant difference in hip flexion(knee flexed) among female group(p<.01). The average hip flexion(knee extended) joint range of motion in 60-69(from sixty to sixty-nine)years old are $73.13^{\circ}$(Left-Male), $72.04^{\circ}$(Right-Male), $77.29^{\circ}$(Left-Female), $75.97^{\circ}$(Right-Female). 70-79years old are $74.95^{\circ}$(L-M), $72.19^{\circ}$(R-M), $76.73^{\circ}$(L-F), $76.65^{\circ}$(R-F). 80-89years old are $70.83^{\circ}$(L-M), $70.37^{\circ}$(R-M), $69.00^{\circ}$(L-F), $69.00^{\circ}$(R-F). There was significant difference in left hip flexion(knee extended) among female group(p<.05). 2) The average hip extension joint range of motion in 60-69years old are $13.09^{\circ}$(L-M), $12.78^{\circ}$(R-M), $10.97^{\circ}$(L-F), $10.68^{\circ}$(R-F). 70-79years old are $8.95^{\circ}$(L-M), $8.48^{\circ}$(R-M), $11.24^{\circ}$(L-F), $10.90^{\circ}$(R-F). 80-89 years old are $8.40^{\circ}$(L-M), $8.23^{\circ}$(R-M), $7.33^{\circ}$(L-F), $7.33^{\circ}$(R-F). There was significant difference in left(p<.01) and right(p<.05) hip extension among male group(p<.05). 3) The average hip abduction joint range of motion in 60-69 years old are $33.04^{\circ}$(L-M), $33.17^{\circ}$(R-M), $33.16^{\circ}$(L-F), $33.37^{\circ}$(R-F). 70-79 years old are $31.00^{\circ}$(L-M), $30.05^{\circ}$(R-M), $32.44^{\circ}$(L-F), $32.68^{\circ}$(R-F). 80-89 years old are $29.07^{\circ}$(L-M), $27.90^{\circ}$(R-M), $28.17^{\circ}$(L-F), $28.67^{\circ}$(R-F). There was no significant difference among group. 4) The average hip adduction pint range, of motion in 60-69years old are $29.57^{\circ}$(L-M), $29.35^{\circ}$(R-M), $31.87^{\circ}$(L-F), $31.89^{\circ}$(R-F). 70-79, years old are $27.41^{\circ}$(L-M), 27.00(R-M) $30.85^{\circ}$(L-F), $31.28^{\circ}$(R-F). 80-89 years old are $26.87^{\circ}$(L-M), $26.63^{\circ}$(R-M), $24.67^{\circ}$(L-F), $24.83^{\circ}$(R-F). There was significant difference in hip abduction among female group(p<01). 5) The average hip external rotation pint range of motion in 60-69years old are $32.26^{\circ}$(L-M), $31.17^{\circ}$(R-M), $33.53^{\circ}$(L-F), $34.42^{\circ}$(R-F). 70-79 years old are $31.64^{\circ}$(L-M), $28.62^{\circ}$(R-M) $31.29^{\circ}$(L-F), $31.45^{\circ}$(R-F). 80-89 years old are $26.40^{\circ}$(L-M), $26.07^{\circ}$(R-M), $24.77^{\circ}$(L-F), $24.27^{\circ}$(R-F). There was significant difference in left(male, female p<.01) and right(female p<.0l) hip external rotation among group. 6) The average hip internal rotation joint range of motion in 60-69years old are $30.30^{\circ}$(L-M), $28.13^{\circ}$(R-M), $34.27^{\circ}$(L-F), $36.03^{\circ}$(R-F). 70-79years old are $31.24^{\circ}$(L-M), $29.57^{\circ}$(R-M), $28.51^{\circ}$(L-F), $29.10^{\circ}$(R-F). 80-89 years old are $24.63^{\circ}$(L-M), $24.40^{\circ}$(R-M), $24.27^{\circ}$(L-F), $24.27^{\circ}$(R-F). There was significant difference in left(male p<.05, female p<.01) and right(female p<.01) hip internal rotation among group.

힙업기능이 적용된 기립보조의자의 생체역학적 평가 (Biomechanical Assessment of Lifting Chair with Hip-up Function)

  • Lifting chair is a typical assistive product to aid for standing up and sitting down for persons with disability. It is particularly useful for the elderly persons whose muscular system is weakened by degenerative joint disease. This paper describes biomechanical assessments of lifting chair with hip-up function. In experiments we measured 3D motion and electromyographic(EMG) signal on the femoral muscle when subject performs the standing motion on the predetermined seat height. Based on assessment of the standing motion, we select the hip-up angle as 15 degrees to cover the relative femoral angle when EMG peak value is appeared. Using an implemented prototype lifting chair with hip-up function we perform standing experiments with five subjects. The experimental results show that the lifting and hip-up functions are effective to assist for standing up motion with less muscle force.

몸통 운동의 제약이 최대 수직점프의 수행에 미치는 영향 (The Effects of Restricted Trunk Motion on the Performance of Maximum Vertical Jump)

  • 본 연구에서는 수직점프시 몸통운동의 제약이 수직점프의 수행에 미치는 영향을 알아보기 위해 10명의 성인 남성을 대상으로 몸통을 자유롭게 사용한 일반적인 형태와 몸통동작을 제한한 형태의 수직점프를 비교, 분석하였다. 분석 결과 몸통동작을 제한한 경우 비 제약점프에 비해 10% 정도의 수행 손실이 있었는데, 이는 이지 순간의 중심 높이보다는 이 지속도의 차이에 의해 발생하였다. 몸통의 운동은 엉덩관절의 모멘트 파워를 증가시켜 추진의 초기 시점부터 지면반력의 증가에 기여하였는데 이러한 요인이 수행력 증가의 주요한 요인으로 작용하였다. 반면 몸통운동이 제한된 점프에서는 엉덩관절에서의 역학적 출력의 감소를 보상하는 측면에서 무릎관절의 역할이 증가하였으나 충분치 못하였다. 또한 몸통동작의 제한으로 엉덩관절 무릎관절 발목관절의 순차적인 신전패턴과는 상이하게 추진시점 직후 엉덩관절과 무릎관절이 동시에 신전하는 형태의 점프가 이루어져 협응패턴의 변화가 나타났다. 결국 본 연구의 결과 몸통의 적절한 사용은 수직점프의 수행 향상에 효과적으로 기여하는 것으로 나타났다.

태권도 주춤서기 후 앞꼬아서기 옆차기의 운동학적 분석 (A Kinematic Analysis of Taekwondo Juchumseogi hu Apkkoaseogi yeopchagi)

  • The purpose of this study was to analyze the movements of the lower extremity joints during a taekwondo kick motion called 'Juchumseogi hu Apkkoaseogi yeopchagi', which was administered to players to improve their balance, stability, and range of motion for the prevention of injuries. Eight professional players and amateur players were recruited as the subjects. Kinematic data were collected by four real-time infrared cameras. The hip joint, knee joint, and ankle joint angles were measured using instruments. During the 'Juchumseogi hu Apkkoaseogi yeopchagi' kick motion, there were small and inconsistent effects on each joint. This study processed the data using the Windows SPSS Ver. 18.0 to get an independent t-test, with the setting, p< .05. Results indicated that hip joint, knee joint, and ankle joint angles were almost significantly different between professional and amateur player during 'Juchumseogi hu apgeule Apkkoaseogi' kick motion.

증강현실 기반의 단기간 무릎운동이 관절가동범위와 균형에 미치는 영향 - 예비연구 (The Influence of Augmented Reality based Knee Exercise in Short Period on Range of Motion and Balance - Pilot study)

  • Purpose: This study investigated the effect of the augmented reality (AR)-based knee joint short period exercise program and used a motion analyzer with a 3D camera to determine the range of motion and dynamic balance and further investigate the effects of therapeutic exercise on patients. Methods: This study used AR-based motion analysis and a Y-balance test to measure the range of motion (ROM) of each joint: the hip joint and the knee joint. After the measurements, an exercise program was applied to the subjects, using the knee motion program function, and the muscles of the quadriceps femoris and the hamstring were stretched or strengthened. Results: Our results showed knee joint extension at the dominant hip joint flexion position. While there was no significant difference (p>.05) at this position, there were significant differences in the non-dominant hips, unbalanced knee joint flexion, and superior knee joint flexion (p<.05). The Y-balance test using the non-dominant leg supported by the dominant legs showed that the absolute reach was $69.70{\pm}7.06cm$ before the exercise, and the absolute reach after the exercise was $77.56{\pm}6.09cm$ (p<.05). Conclusions: There was a significant difference when the movement of the lower limbs supported the superior limbs, and a significant difference was found in the ROM when the non-dominant side supported the dominant side. Therefore, the AR-based exercise program improves the balance of the human body and the range of motion of the joints, and research that aims to improve patients abilities should continue.

Effect of Proprioceptive Neuromuscular Facilitation Stretching on Pain, Hip Joint Range of Motion, and Functional Disability in Patients with Chronic Low Back Pain

  • Objective: We aimed to identify the effects of proprioceptive neuromuscular facilitation (PNF) stretching on pain, hip range of motion, and functional disability in patients with chronic low back pain. Design: Randomized controlled trial Methods: In total, 45 patients with chronic low back pain were randomly divided into a conventional stretching group (n=22) and a PNF stretching group (n=23). Both interventions were performed three times per week for 6 weeks. Assessments were made using the visual analog scale, Flexion-Abduction-External Rotation test, modified Thomas test, prone hip extension test, and Oswestry disability index before and after the 6-week intervention period. We conducted a paired t-test to compare the within-group findings before and after the intervention. An independent t-test was used to compare the between-group differences. The statistical significance level was set at α=0.05, for all variables. Results: Both groups showed significant improvements in pain, hip range of motion, and functional disability after the intervention (p<0.05). A significant difference was observed in pain, hip range of motion, and functional disability in patients belonging to the PNF stretching group (p<0.05). Conclusions: This study provides evidence that the application of PNF stretching can effectively reduce pain and improve hip range of motion and functional disability in patients with chronic low back pain.

사이클 페달링 시 안장높이에 따른 하지관절 각도와 근육활성화의 상관관계 (Relationship between Lower -Limb Joint Angle and Muscle Activity due to Saddle Height during Cycle Pedaling)

  • The purpose of this study was to investigate the effects of different saddle heights on lower-limb joint angle and muscle activity. Six elite cyclists(age: $32.2{\pm}5.2years$, height: $171.0{\pm}3.5cm$, weight: $79.7{\pm}5.6kg$, cycle career: $13{\pm}6.2years$) participated in three min. submaximal(90 rpm) pedaling tests with the same load and cadence based on saddle heights where subject's saddle height was determined by his knee flexion angle when the pedal crank was at the 6 o'clock position. Joint angles(hip, knee, ankle joints) and the activity of lower limb muscles(biceps femoris(BF), vastus lateralis(VL), tibialis anterior(TA) and gastrocnemius medial(GM)) were compared by measuring 3D motion and electromyography(EMG) data. Results showed that there were significant differences in minimum hip & knee joint angle and range of motion of hip and knee joint between saddle heights. Onset timing and integrated EMG of only BF among 4 muscles were significantly different between saddle heights. Especially there was a negative relationship between minimum hip joint angle and onset timing of BF in most subject, which means that onset timing of BF became fast as the degree of bending of the hip joint became larger by saddle height. Optimal pedaling will be possible through increased amount of muscle activation due to the appropriate burst onset timing by proper pedaling posture with adjusted saddle height.

A Statistical Model for Marker Position in Biomechanics

  • Objective: The purpose of this study was to apply a general linear model in statistics to marker position vectors used to study human joint rotational motion in biomechanics. Method: For this purpose, a linear model that represents the effect of the center of hip joint rotation and the rotation of the marker position on the response was formulated. Five male subjects performed hip joint functional motions, and the positions of nine markers attached on the thigh with respect to the pelvic coordinate system were acquired at the same time. With the nine marker positions, the center of hip joint rotation and marker positions on the thigh were estimated as parameters in the general linear model. Results: After examining the fitted model, this model did not fit the data appropriately. Conclusion: A refined model is required to take into account specific characteristics of longitudinal data and other covariates such as soft tissue artefacts.
