Contact Stress Analysis of Artificial Hip Joints Using Finite Element Method (유한요소법을 이용한 인공 고관절의 접촉응력 해석에 관한 연구)

  • The modern orthopaedics frequently uses the total hip replacement in the artificial hip joint. The wear in this joint requires a re-replacement of hip joints because it is under the severe load and friction conditions. To solve these problems the previous studies have been mainly focussed on the development of new materials. The research of new materials, however, needs much time and effort since it should be experimented for its bio-compatibility, friction, and wear characteristics. To reduce the work, in this study, the finite element analysis is applied to find new combinations of bio-materials in the total hip replacement which has the excellent contact characteristics. A non-linear FEM program MARC with 5-node axisymmetric element was used for analyzing the contact stresses between the hip joints. The computed results show that in case of acetabulum UHMWP has good characteristics, in femoral head, $Al_2O_3$, and in stem, Ti6Al4V.

Correlation of Hip Joint Range of Motion and Femur Head Anterior Glide Mobility with Gait Ability in Stroke Patients (뇌졸중 환자의 고관절 관절가동범위와 대퇴골두 전방가동성, 보행 능력간에 상관관계)

  • PURPOSE: The aim of the study was to investigate the relationships among the hip joint passive range of motion (ROM) and femur head anterior glide (FHAG) mobility on the gait ability in patients with post-stroke hemiparesis. METHODS: The participants were 37 patients (30 male, 7 female) living in Daejeon. The ROM of the hip joint was measured by using goniometry and the FHAG mobility was measured by using the Prone Figure-4 test. The walking ability was assessed by using the 10m walk test (10MWT), and the 6-min walk test (6MWT). RESULTS: The FHAG was negatively correlated with hip extension (r=-.554, p<.05) and flexion (r=-.337) on the affected side as well as with hip extension (r=-.480), abduction (r=-.361), and adduction (r=-.426) on the non-affected side (p<.05). The gait ability was correlated with the hip joint external rotation on the non-affected side (p<.05), but showed no significant correlation with the hip ROM on the affected side (p>.05). CONCLUSION: This study provides evidence that in patients with post-stroke hemiparesis, the FHAG mobility might be correlated with hip extension. Based on these results, the FHAG mobility may be used to determine the hip extension in patients with post-stroke hemiparesis.

Biomechanical Analysis of the Shelf Operation for Dysplastic Hip Joint by Finite Element Analysis (유한요소해석을 이용한 이형성 고관절의 선반형성술에 대한 생체역학 해석)

  • The aim of this study was biomechanical analysis of shelf operation in patients with dysplastic hip joint by finite element contact analysis. Two dimensional CT images were used to construct the finite element models to analyze the contact pressure, and the 3D expansion of the Ninomiya's method was used in the calculation of the resultant force in the hip joint. The surgery recovered the center-edge angles to the normal anatomical range and increased the contact areas in two patients. The maximum contact pressures and von-mises stresses were decreased. The present study provides the biomechanical guideline of optimal surgical parameters to maximize the surgical efficiency and the clinical outcomes in dysplastic hip joint using the shelf operation.

Effects of Hip Joint Mobilization on Pain, Balance, and Gait in Patients with Patellofemoral Pain Syndrome (고관절 가동술이 슬개대퇴통증증후군 환자의 통증, 균형 및 보행에 미치는 영향)

  • Background: Patellofemoral pain syndrome (PFPS) is common knee disorder encountered in clinical: notably, altered hip biomechanic may contribute to PFPS. In this study, We investigated the effects of hip joint mobilization on pain, balance, and gait in patients with PFPS. Methods: Patients were randomly assigned to a control group (n=18) or an experimental group (n=20). Both groups received exercise therapy thrice a week for 4 weeks. The experimental group performed additional hip joint mobilization thrice a week for 4 weeks. Measurement were obtained in each patient pre-intervention and post-intervention (after 4 weeks). Results: The assessed items included the visual analog scale (VAS), one leg standing test (OLS), timed up and go test (TUG), and the 10m walk test (10MWT). Post-intervention assessment showed significantly improved results in both groups (p<.01). A significant intergroup difference was observed only in the results of the 10MWT (p<.05). Conclusion: Our results indicate that hip joint mobilization with exercise therapy may be useful to improve PFPS.

Changes in Muscle Acvity of the Quadriceps Femoris with Changes in the Ankle and Hip Joint Angles During a Sit-to-Stand Movement (앉은 자세에서 일어서기 동작 시 발목관절과 엉덩관절 각도 변화에 따른 넙다리네갈래근 근활성도 변화)

  • Purpose : The purpose of this study was to confirm changes in the muscle activity of the quadriceps femoris with changes in the ankle and hip joint angles during the transition from sitting to standing. Method : Twenty-five healthy 20-30-year-old women participated in the study. The subjects performed standing-up movements in four positions: standard posture (hip = $90^{\circ}$/ankle = $0^{\circ}$); posture on a decline board (hip > $90^{\circ}$/ankle < $0^{\circ}$), posture on a footboard (hip > $90^{\circ}$/ankle = $0^{\circ}$); and posture on a decline board with a higher chair (hip = $90^{\circ}$/ankle < $0^{\circ}$). Then, the muscle activities of the rectus femoris, vastus medialis and vastus lateralis were measured using surface electromyography. Result : The muscle activities of the rectus femoris, vastus medialis and vastus lateralis in the footboard application were statistically significantly higher than in the application of the decline board with a higher chair. Conclusion : This study confirms that the flexion of the hip joint has a greater effect than the plantar flexion of the ankle joint on the muscle activity of the quadriceps during a sit-to-stand movement.

Effects of Prosthetic Mass Distribution on Musculoskeletal System during Amputee Gait (의지 보행시 의지 무게 분포가 근골격계에 미치는 영향)

  • The optimized prosthetic mass distribution was a controversial problem in the previous studies because they are not supported by empirical evidence. The purpose of the present study was to evaluate the effect of prosthetic mass properties by modeling musculoskeletal system, based on the gait analysis data from two above-knee amputees. The joint torque at hip joint was calculated using inverse dynamic analysis as the mass was changed in knee and foot prosthetic components with the same joint kinematics. The results showed that the peak flexion and abduction torque at the hip joint were 5 Nm and 15 Nm when the mass of the knee component was increased, greater than the peak flexion and abduction torque of the control group at the hip joint, respectively. On the other hand, when the mass of the foot component was increased, the peak flexion and abduction torque at the hip joint were 20 Nm and 15 Nm, greater than the peak flexion and abduction torque of the control, respectively. The hip flexion torque was 4.71-fold greater and 7.92-fold greater than the hip abduction torque for the knee mass increase and the foot mass increase on the average, respectively. Therefore, we could conclude that the effect of foot mass increase was more sensitive than that of knee mass increase for the hip flexion torque. On the contrary, the mass properties of the knee and foot components were not sensitive for the hip abduction torque. In addition, optimized prosthetic mass and appropriate mass distributions were needed to promote efficiency of rehabilitation therapy with consideration of musculoskeletal systems of amputees.

A Clinical Study of Hip Joint Pain Improved with Gihwangbaekho-tang (지황백호탕(地黃白虎湯)으로 호전된 고관절 통증 환자 치험례)

  • 1. Objectives The purpose of this study is to evaluate the effects of Gihwangbaekho-tang which is based on the Sasang Constitutional Medicine for the patient who has suffered from hip joint pain. 2. Methods The subject is about 86-year-old patient suffering from hip joint pain who was diagnosed as Soyangin interior over heated disease based on his Nature & Emotion, physical characteristics, symptoms, and we have prescribed Gihwangbaekho-tang. We used Visual analogue scale(VAS) for the assessment of Hip joint pain. 3. Results and Conclusions The patient was treated with Gihwangbaekho-tang and hip joint pain, thirst, redness of the face, constipation and general condition were improved. This study describes tile improvement of Hip joint pain and the change of symptoms by using Gihwangbaekho-tang. In this study, we applied Gihwangbaekho-tang to 86-year-old patient for about forty days, and Gihwangbaekho-tang didn't cause any serious side effect on the patient.

Measurement on range of two degrees of freedom motion for analytic generation of workspace (작업영역의 해석적 생성을 위한 2자유도 동작의 동작범위 측정)

  • To generate workspace analytically using the robot kinematics, data on range of human joints motion, especially range of two degrees of freedom motion, are needed. However, these data have not been investigated up to now. Therefore, in this research, we are to investigate an interaction effect of motions with two degrees of freedom occurred simultaneously at the shoulder, virtual hip(L5/S1) and hip joints, respectively, for 47 young male students. When motion with two degrees of freedom occurred at a joint such as shoulder, virtual hip and hip joints, it was found from the results of ANOVA that the action of a degree of freedom motion may either decrease or increase the effective functioning of the other degree of freedom motion. In other words, the shoulder flexion was decreased as the shoulder was adducted or abducted to $60^{\circ}C$TEX>or abducted from $60^{\circ}C$TEX>to maximum degree of abduction, while the shoulder flexion increased as the joint was abducted from $60^{\circ}C$TEX> to $60^{\circ}C$TEX> The flexion was decreased as the virtual hip was bent laterally at the virtual hip joint, and also did as the hip was adducted or abducted from the neutral position. It is expected that workspace can be generated more precisely based the data on the range of two degrees of joint motion measured in this study.

Radiofrequency Lesion Generation of the Articular Branches of the Obturator and Femoral Nerve for Hip Joint Pain -A case report- (고관절통을 위한 폐쇄신경 및 대퇴신경 관절지의 고주파열응고술 -증례보고-)

  • Percutaneous radiofrequency thermocoagulation has been applied in patients with various forms of chronic pain, such as facet joint pain, cancer pain and trigeminal neuralgia. A major portion of the hip joint is innervated by the articular branches of the femoral and obturator nerves. Radiofrequency thermocoagulation of the articular branches of the obturator and femoral nerves can be a good alternative treatment for patients with hip joint pain, especially in those where surgery is not applicable. A patient suffering hip joint pain due to metastatic cancer underwent multiple radiofrequency lesioning of the femoral and obturator nerves at $80^{\circ}C$ for 120 seconds, using a Racz-Finch Kit. The patient experienced about a 50% reduction in the pain, without any numbness or other side effects.

A Biomechanical Study on the Hip Joint Flexibility Based on the Proprioceptive Neuromuscular Facilitation (고관절 가동성 증진을 위한 고유수용성 신경근 촉진 스트레칭의 생체역학적 고찰)

  • Joint flexibility is an important factor which affect the process and duration of the therapeutic methods in the filed of occupational therapy. Hip joint flexibility and electromyography (EMG) of major flexor and extensor for the hip joint were examined to understand the biomechanical characteristics of Proprioceptive Neuromuscular Facilitation (PNF). Hip joint flexibility increased $18.9^{\circ}$ on average after PNF was performed by a designated assistant on 10 college students. EMGs of quadriceps femoris muscle and hamstring muscles agreed with biomechanical characteristics of proprioceptive organs in muscles.
