A pioneering version of an on-line management system for high-energy molecules (MS-HEMs) was developed by the ADD and BMDRC in Korea. The current system can manage the physicochemical and explosive properties of virtual and existing HEMs. The on-line MS-HEMs consist of three main routines: management, calculation, and search. The management routine contains a user-friendly interface to store and manage molecular structures and other properties of the new HEMs. The calculation routine automatically calculates a number of compositional and topological molecular descriptors when a new HEM is stored in the MS-HEMs. Physical properties, such as the heat of formation and density, can also be calculated using group additivity methods. In addition, the calculation routine for the impact sensitivity can be used to obtain the safety nature of new HEMs. The impact sensitivity was estimated in a knowledge-based manner using in-house neural network code. The search routine enables general users to find an exact HEM and its properties by sketching a 2D chemical structure, or to retrieve HEMs and their properties by giving a range of properties. These on-line MS-HEMs are expected be powerful tool for deriving novel promising HEMs.

Background: Whether there is a difference in outcomes for trauma patients transferring to the helicopter emergency medical service (HEMS) according to their previous team composition is controversial. The purpose of this study is to evaluate the effectiveness of trauma team-staffed-HEMS (TTS-HEMS) when transferring to a trauma center. Methods: A retrospective comparison was conducted on patients transported to a trauma center over a 6-year period by the TTS-HEMS and paramedic-staffed-HEMS (119-HEMS). Inclusion criteria were blunt trauma with age ${\geq}15years$. Patient outcomes were compared with the Trauma and Injury Severity Score (TRISS) (30-day mortality) and the Cox proportional hazard ratio of mortality (in hospital). Results: There were 321 patients of TTS-HEMS and 92 patients of 119-HEMS. The TTS-HEMS group had a higher Injury Severity Score and longer transport time but a significantly shorter time to emergency surgery. The prehospital data showed that the trauma team performed more aggressive interventions during transport. An additional 7.6 lives were saved per 100 TTS-HEMS deployments. However, the TRISS results in the 119-HEMS group were not significant. In addition, after adjusting for confounders, the hazard ratio of mortality in the 119-HEMS group was 2.83 times higher than that in the TTS-HEMS group. Conclusion: HEMS was likely to improve the survival rate of injured patients when physicians were involved in TTS-HEMS. Survival benefits in the TTS-HEMS group appeared to be related to the fact that the trauma team performed both more aggressive prehospital resuscitation and clinical decision making during transportation.

Smart Grid is considered as one of promising technology to alleviate severe environmental problem. In residential environment, Home Energy Management System (HEMS) can play a key role for green smart home. The HEMS can give several benefits like aslowering electric utility bill, improvement of efficiency of electric power consumption and integration of generator using renewable energy resources. However just limited functions of HEMS can be used for residential customer in real life because of lack of smart function in home appliances and optimal managing software for HEMS. This study provides comprehensive analysis for Home Energy Management System for residential customer. Simple HEMS system with real products on the market are explained and limitation of current HEMS are also discussed.

Recently, home energy management system (HEMS) for power consumption reduction has been widely used and studied. The HEMS performs electric power consumption control for the indoor electric device connected to the HEMS. However, a traditional HEMS is used for passive control method using some particular power saving devices. Disadvantages with this traditional HEMS is that these power saving devices should be newly installed to build HEMS environment instead of existing home appliances. Therefore, an HEMS, which performs with existing home appliances, is needed to prevent additional expenses due to the purchase of state-of-the-art devices. In this paper, an intelligent inference algorithm for EMS at home for non-power saving electronic equipment, called legacy devices, is proposed. The algorithm is based on the adaptive network fuzzy inference system (ANFIS) and has a subsystem that notifies retraining schedule to the ANFIS to increase the inference performance. This paper discusses the overview and the architecture of the system, especially in terms of the retraining schedule. In addition, the comparison results show that the proposed algorithm is more accurate than the classic ANFIS-based EMS system.

Purpose: We aimed to compare the transport time, the proportion of direct hospital visit and the emergency procedures between the current mountain rescue helicopter emergency medical service (HEMS) and physician-staffed mountain-rescue HEMS. Methods: During weekends from October 2, to November 21, 2010, 9 emergency physicians participated as HEMS staff in the mountain-rescue HEMS program of the Seoul fire department. Patient demographic data, transport time, proportion of direct hospital visits, and emergency procedures were recorded. We also collected data on HEMS mountain-rescued patients from June 1, to September 1, 2010, and we compared them to those for the study patients. After an eight-week trial of the HEMS, we performed a delphi survey to determine the attitude of the physician staff, as well as the feasibility of using a physician staff. Results: Twenty-four(24) patients were rescued from mountains by physician-staffed HEMS during the study period, and 35 patients were rescued during the pre-study period. Patient demographic findings were not statistically different between the two groups, but the transport time and the emergency procedures were. During the study period, the time from call to take-off was $6.1{\pm}4.1min$ (vs. $12.1{\pm}8.9min$ during the pre-study period, p-value=0.001), and the time from call to arrival at the scene was $15.0{\pm}4.8min$ (vs. $22.3{\pm}8.1min$ during the pre-study period, p-value=0.0001). The proportions of direct hospital visit were not different between the two groups, but more aggressive emergency procedures were implemented in the study group. The delphi survey showed positive agreement on indications for HEMS, rapidity of transport and overall satisfaction. Conclusion: A pilot trial of physician-staffed HEMS for mountain rescue showed rapid response and more aggressive performance of emergency procedures with high satisfaction among the attending physicians.
Purpose The healthcare services have drawn so much public attention, and many organizations such as schools and companies require the individuals to undergo the periodic health examination. In general, however, the mass health examination services are not managed in systematic way, and both examinees and medical staffs often experience much inconvenience while preparing, undergoing and managing the services. To address such problems, this paper aims to design the Health Examination Management System (HEMS), an integrated management system for mass health examination operations management, and implement its prototype. Design/methodology/approach First of all, HEMS enables the medical staffs to efficiently collect and manage the examination result data by supporting examination service management. Second, the users can efficiently analyze the cause-and-effect relationships among the examination items by using the visualization tool of HEMS based on the cluster heat map. Finally, the HEMS provides the operational supports for evaluating and managing the service performances. Findings The HEMS indicates that the conventional operations management approaches can be incorporated into the mass health examination services, and it is expected that the proposed system enables the examinees and the medical staffs to participate in such services in more efficient way.

Purpose: To improve outcome of severe trauma patient, the shortening of transport time is needed. Although helicopter emergency medical services (HEMS) is still a subject of debate, it must also be considered for trauma system. The aim of this study is to assess whether transport method (HEMS versus ground EMS) is associated with outcome among inter-hospital transport. Methods: All trauma patients transported to regional emergency center by either HEMS or ground EMS from September 2011 to September 2014. We have classified patients according to two groups by transport method. Age younger than 15 years and self-discharged patients were excluded. Results: A total of 427 patients were available for analysis during this period. 60 patients were transported by HEMS and 367 patients were transported by ground EMS. HEMS group had higher mortality than ground EMS group (23.3% vs 3.5%; p<0.001), and included more patients with excess mortality ratio adjusted injury severity score (EMR-ISS) above 25 (91.7% vs 48.8%; p<0.001). In the multivariable regression analysis, HEMS was not associated with improved outcome compared with ground EMS, but only EMR-ISS was associated with a mortality of patients (odds ratio, 1.06; 95% confidence interval, 1.04-1.09). Conclusion: In this study, helicopter emergency medical services transport was not associated with a decreased of mortality among the trauma patients who inter-hospital transported to the regional emergency center.

The purpose of this study is to conduct a questionnaire survey on the actual conditions of the working uniform and protective equipment put on by street cleaners, then to identify their inconvenience and problems and to draw solutions to improve them, and finally to design a spring-and-autumn street cleaner uniform which has enhanced functionalities such as motional flexibility, safety and soil proof. The questionnaire survey of this study was done by street cleaners who were in charge of street cleaning and collecting the recyclable waste. The result of this study, easily worn-out parts of the working clothing were the sleeve hems and the front part on the upper uniform, and the trouser hems and zipper on the lower uniform. Easily soiled parts were the sleeve hems, parts of arm on the upper uniform, and the trouser hems and knee parts on the lower uniform. Based on the results of the questionnaire survey, the sample uniforms were designed in consideration of material, design and pattern. A soil-proof material was used for easily soiled parts such as sleeve hems, knee parts and trouser hems. Reflective material was applied to the parts such as chest, back and the side lines of sleeves and trousers to raise visibility in working and to ensure safety. A crease was given to each side of the back. The height of sleeve cap was lowered to lift the motional flexibility of back and arm parts. Darts were applied to elbow and knee parts for a three-dimensional design. Gussets were inserted in the thigh part to give it activity. Then quantitative evaluation and a sensory evaluation, which was composed of outward appearance, motional flexibility and fitness for field operation, were conducted. According to these evaluations, the sample uniform was found to be more satisfactory than current uniform.

This paper addresses the load usage scheduling in the HEMS for residential power consumers. The HEMS would lead the residential users to change their power usage, so as to minimize the cost in response to external information such as a time-varying electricity price, the outside temperature. However, there may be a consumer's inconvenience in the change of the power usage. In order to improve this, it is required to understand the pattern of load usage according to the external information. Therefore, this paper suggests a methodology to model the load usage pattern, which classifies home appliances according to external information affecting the load usage and models the usage pattern for each appliance based on a copula function representing the correlation between variables. The modeled pattern would be reflected as a constraint condition for an optimal load usage scheduling problem in HEMS. To explain an application of the methodology, a case study is performed on an electrical water heater (EWH) and an optimal load usage scheduling for EHW is performed based on the branch-and-bound method. From the case study, it is shown that the load usage pattern can contribute to an efficient power consumption.
Purpose: This study was aimed to describe Helicopter Emergency Medical Services (HEMS) and analyze the flight nurses' activities in HEMS. Methods: Data were collected retrospectively from the air transportation reports that contained data of 168 patients transported by aircraft to G University medical center in Incheon since June, 2012 to March, 2013. Data were analyzed using descriptive statistics, and Mann-Whitney U test. Results: Average distance of flights was 44.0 km, duration of field treatment took 13.6 minutes, and duration of a flight from scene to hospital was 14.5 minutes. Nursing activities were categorized into 12 direct nursing activities and 5 nursing management activities, and a total number of 7806 nursing activities were occurred in HEMS. The most frequently performed nursing activity was measurement and monitoring (27.9%) followed by medication (11.5%) and respiratory management (8.7%). The most frequent nursing management were information management (11.0%). Nursing activities performed were significantly different depending on the patient's level of consciousness, cause of illness, crew configuration, and type of transportation. Conclusion: This study described HEMS nursing activities performed by flight nurses. Difference in nursing activities according to patient characteristics, crew configuration and type of transportation requires flight nurses to be prepared through educational programs to improve nursing activities and nursing management during air transportation.
