달리기 시 인솔의 굽힘 강성 증가에 따른 발목과 중족골 관절의 운동학적 변인 및 관절 협응에 미치는 영향 (The Effects on Kinematics and Joint Coordination of Ankle and MTP Joint as Bending Stiffness Increase of Shoes during Running)

  • Objective: The aim of this study was to analyze body stability Joint coordination pattern though as bending stiffness of shoes during stance phase of running. Method: 47 male subjects (Age: 26.33 ± 2.11 years, Height: 177.32 ± 4.31 cm, Weight: 65.8 ± 3.87 kg) participated in this study. All subjects tested wearing the same type of running shoes by classifying bending stiffness (A shoes: 3.2~4.1 N, B shoes: 9.25~10.53 N, C shoes: 20.22~21.59 N). They ran 10 m at 3.3 m/s (SD ±3%) speed, and the speed was monitored by installing a speedometer at 3 m intervals between force plate, and the measured data were analyzed five times. During running, ankle joint, MTP joint, coupling angle, inclination angle (anterior-posterior, medial-lateral) was collected and analyzed. Vector coding methods were used to calculate vector angle of 2 joint couples during running: MTP-Ankle joint frontal plane. All analyses were performed with SPSS 21.0 and for repeated measured ANOVA and Post-hoc was Bonferroni. Results: Results indicated that there was an interaction between three shoes and phases for MTP (Metatarsalphalangeal) joint angle (p = .045), the phases in the three shoes showed difference with heel strike~impact peak (p1) (p = .000), impact peak~active peak (p2) (p = .002), from active peak to half the distance to take-off until take-off (p4) (p = .032) except for active peak~from active peak to half the distance to take-off (p3) (p = .155). ML IA (medial-lateral inclination angle) for C shoes was increased than other shoes. The coupling angle of ankle angle and MTP joint showed that there was significantly difference of p2 (p = .005), p4 (p = .045), and the characteristics of C shoes were that single-joint pattern (ankle-phase, MTP-phase) was shown in each phase. Conclusion: In conclusion, by wearing high bending stiffness shoes, their body instability was increased during running.

기능적 전기자극 치료기를 이용한 중풍구두가 편마비 환자의 보행에 미치는 영향 (The Effects Where the Stroke Shoes Which Use Functional Electric Stimulation Goes Mad to Walking of the Hemiplegia)

  • Purpose: An objective analysis and observations were to be done on hemiplegia patients that are wearing a walking support device, Stroke shoes. Their improvements in walking pace, the reduction of distance between the two knee joint, the increase of curve angle of the knee joint and their steps and the reduction of ankle joint upon swing phase were analyzed using a 20 walking analyzer. Methods: An examination was carried out to see the patients' communication skill and independent walking and then let them walk with the Stroke shoes on to get results before and after wearing it. Simi Reality Motion Systems GmbH (Germany, 2007) was used to analyze the results regarding knee joint and ankle joint angle changes of sagitta plane and coronal plane, stepping distances, distances between the knees and walking pace. Results: 1. The articulation angle of ankle joint during swing phase decreased and knee joint has shown a statistically significant increase in such value(p<0.05). 2. Only knee joint showed a significant increase in articulation angle during heel strike(p<0.05). 3. Knee joint showed a significant increase in articulation angle during toe off(p<0.05). 4. The distance between the two knees as well as their foot steps significantly decreased compared with when Stroke shoes were not worn(p<0.05). 5. Stroke shoes with FES have shown positive effects on the patients in improving their walking styles overall. (p<0.05). Conclusion: There was an improvement in rotation walking pattern by a reduction in the distance between the knees after wearing Stroke shoes with FES. Plantar flexion reduced that occurred in ankle joint during walking and flexion angle increased in knee joint, both of which improved foot drop which was a major problem in hemiplegia patients. Also it is believed that the device will have some positive influences on knee joint stiffening paralysis to aid in improving inefficient walking phases.

보행 시 Wedged 인솔과 보행진행 각이 슬관절 외측 밀림(Lateral Thrust)에 미치는 영향 (The Influence of Wedged Insole and Foot Progression Angle on Lateral Thrust of Knee During Walking)

  • The purpose of this study was to identify the influence of wedged insole and foot progression angle (FPG) on lateral thrust of knee in healthy subjects. Fifteen healthy male subjects were recruited from Suncheon First College, in Suncheon. The subjects randomly walked at the comfortable velocity under five conditions: bare footed, medio-lateral $10^{\circ}$ wedged insoles, toe-in and toe-out gait. The lateral thrust was measured by a accelerometer with telemeter during walking. Data was collected while each subject walked for about 10 gait cycle on a flat, level walkway at their normal speed. The middle three gait cycle were used for averaged peak value of lateral acceleration. The three averaged peak value of lateral acceleration were collected under each condition at heel strike. The results showed that averaged peak value of lateral acceleration increased significantly in medial wedged insole and toe-in gait and decreased significantly in lateral wedged insole and toe-out gait as compared with bare footed (p<.05). These results suggest that wedged insole as well as walking strategy, such as foot progression angle, may prevent progression of degenerative knee osteoarthritis.

보행 시 생체신호분석을 통한 신발 착용 유무에 따른 마찰 특성 비교 (A Comparative Study on the Characteristics of Friction with/without shoes by Analyzing Bio-signals during walking)

  • 보행 시 지면에 대한 수직력에 대한 전단력의 비율, 즉 보행자가 사용한 마찰계수(UCOF)는 미끄러짐 발생 가능성 있는 시점을 식별하는데 사용된다. 신발보행은 신발창 두께와 경도, 뒤꿈치 모양, 밑창 문양 등의 신발 디자인이 사용마찰계수를 변화시킬 것이다. 본 연구에서는 보행 시 신발 착용 유무에 따른 사용마찰계수(UCOF) 차이를 분석하기 위해 성인남녀 21명(여자 10명, 남자 11명, 나이: $25.2{\pm}2.3yrs$, 키: $165.6{\pm}7.2cm$, 몸무게: $62.2{\pm}7.8kg$)을 대상으로 보행속도, 지면반력, 사용마찰계수(UCOF) 최대 시점, CoP-CoM-수직선 각도의 차이를 알아보고, 사용마찰계수(UCOF)와의 상관관계를 분석하였다. 그 결과, 첫째, 보행 시 신발 착용으로 인해 체중수용기(제동기)의 더 이른 시점에 사용마찰계수가 최대치에 도달하고, 또 그 크기도 증가한다. 둘째, CoP-CoM의 Tangent 값과 사용마찰계수(UCOF1)와의 상관관계는 오른발 제동 초기시점(UCOF1_h)보다 추진 후기에 발생하는 왼발(다음발) 착지시점(UCOF2_h)에서 더 높은 상관관계를 보여, 제동기 보다는 추진기(다음 발 제동기)와의 연관성을 시사한다.

장애물 높이에 따른 보행의 운동형상학적 변화에 대한 연구 (The Kinematic Patterns of Walking according to Obstacle's Height)

  • Background : The Purposes of this study were to understand difference between free walking and obstacle over walking through the naked eye and motion analysis device, and to review merits of obstacle walking training as item of functional assessment in clinical situations. Methods : All participants were male and performed 3 types of walking methods: free walking, obstacle over walking with low block(height=10cm, width=8cm), and obstacle over walking with high block(height=20cm, width=8cm). All walking were performed 3 trials respectively. Results : In the naked eye, initial contact with toes occurred more than heel strike in obstacle over walking, and the flexion angle of hip and knee were increased in obstacle over walking. On interpretations though motion analysis device, cadence, gait speed and weight accept were significant statistically(p<.05). Cadence and gait speed were decreased, and weight accept duration was increased in obstacle over walking. Rotation among three pelvic motions was significant statistically(p<.05), flexion among three hip motions was significant statistically(p<.05) and flexion among three ankle motions was significant statistically(p<.05). Rotation and flexion among three ankle motions was significant statistically(p<.05). Conclusion : Both the naked eye and interpretations of the device presented many difference between free walking and obstacle over walking. In overcrossing obstacles, many participants appeared walking strategy by perform initial contact with toes. Knee flexion was most significant statistically(p<.05) in obstacle over walking with 20cm block.

Effects of Hallux Valgus Orthoses on Ground Reaction Force Using 3D Motion Analysis in Individuals With Hallux Valgus Deformity

  • Background: Hallux valgus (HV) is a foot deformity developed by mediolateral deviation of the first metatarsophalangeal joint. Although various foot-toe orthoses were used to correct the HV angle, verification of the effects of kinetics variables such as ground reaction force (GRF) through three-dimensional (3D) gait analysis according to the various type of orthoses for HV is insufficient. Objects: This study aimed to investigate the effect of soft and hard types of foot and toe orthoses to correct HV deformity on the GRF in individuals with HV using 3D motion analysis system during walking. Methods: Twenty-six subjects participated in the experiment. Participants had HV angle of more than 15° in both feet. Two force platforms were used to obtain 3D GRF data for both feet and a 3D motion capture system with six infrared cameras was used to measure exact stance phase point such as heel strike or toe off period. Total walk trials of each participant were 8 to 10, the walkway length was 6 m. Two-way repeated measures ANOVA was used to determine the effects of each orthosis condition on the various GRF values. Results: The late anteroposterior maximal force and a first vertical peak force of the GRF showed that the hard type orthosis condition significantly increased GRF compared to the other orthosis conditions (p < 0.05). Conclusion: There were significant effects in GRF values when wearing the hard type foot orthosis. However, the hard type foot orthosis was uncomfortable to wear during walking. Therefore, it is necessary to develop a new foot-toe orthosis that can compensate for these disadvantages.
