군 입영자에서의 B형 간염 발생 규모와 B형 간염 바이러스 보유자가 이에 미치는 영향 (The Incidence of Hepatitis B in Military Service and the Effect of Asymptomatic HBsAg Carriers on the Incidence)

  • The purpose of this study was to investigate the incidence rate of hepatitis B in the military service and to examine the effect of the asymptomatic hepatitis B surface antigen(HBsAG) carries on the incidence of hepatitis B. The subject were 223,270 men who were conscripted to the Korean Army from 1991 to 1994. According to the conscripted year, four conscription cohort were constructed. At the screening examination for military service no test for hepatitis B were performed in 1991 and 1992. In 1993, a screening test for hepatitis B were performed and those who were confirmed as HBsAg positive o. showed high titers $(\geqq100IU)$ of nm glutamic-pyruvic transaminase(SGPT) were excluded from conscription. In 1994, the criteria for conscription was changed and those who were HBsAg positive were not excluded from conscription. Only those who showed $\geq$ SGPT 100IU were excluded. The main results were as follows ; 1. The positive rate of HBsAg is 5.5% in the conscripted men. 2. The incidence rates of the hepatitis B in 1991 and 1992 conscription cohort were 9.96 and 8.10 per ten thousand person-year, respectively. The incidence rate of the hepatitis B was 1.34 per ten thousand person-year in 1993 conscription cohort which was confirmed as HBsAg negative at the screning test, and 7.41 per ten thousand person-year in 1994 conscription cohort which included the HBsAg positive. 3. The incidence rate of hepatitis B was 99.98 per ten thousand person-year in HBsAg positive group and 2.25 per ten thousand person-year in HBsAg negative group. The incidence rate of the group with high SGPT and HBsAg positive was 255 times higher than that of normal population. 4. The incidence of hepatitis B in HBsAg negative group did not increase even though the probability of personal contact with HBsAg positive had been increased. from the above results, the men who have high SGPT with HBsAg positive should be excluded from military service, and it can not be said that asymptomatic HBsAg carriers influence on the hepatitis B incidence among the HBsAg negative through personal contact.

직장(職場) 채용(採用) 신체검사(身體檢査)에서 나타난 B형(型) 바이러스 간염(肝炎) 검사성적(檢査成績)에 관(關)한 고찰(考察) (Study on the prevalence of HBV Ag and HBV markers revealed through preemployment examination in asymptomatic healthy persons)

  • Primary screening test for serum HBsAg by RPHA from 4,805 persons who were clinically well through preemployment examination for the period of one calendar year of 1983 revealed 476 (9.9%) positive individual carriers. There were no significant differences in distribution of positives of serum HBsAg by age group, profession, or province area. Among positives of serum HBsAg, 356 (74.8%) showed normal findings and 120 (25.2%) showed abnormal findings in liver function test, respectively. Radioimmunoassay was done in 169 positives of HBsAg and RIA detected 10 negative persons who were positive by RPHA revealing 5.9% of false positive rate and 94.1% of sensitivity of RPHA. In RIA profile of HBV markers, pattern I (HBsAg+, Anti-HBe+) was 46.6%, pattern II (HBsAg+, HBeAg+) was 33.3%, pattern III (HBsAg+only) was 18.3%, pattern IV (HBsAg+, HBeAg+, Anti-HBs+) was 1.3%, pattern V (HBsAg+, HBeAg+, Anti-HBe+) was 0.6%, respectively. There were no positives of HBsAg among 10 persons who were negatives of HBsAg by RIA.

  • Objectives : The purpose of this study was to investigate the relationship of subjective recognition for oral health in HBsAg positive carriers based on the data of the $5^{th}$ National Health and Nutrition Examination Survey. Methods : A statistical package SPSS 21.0 was used to analyze the relationship between oral health status and subjective oral health awareness based on data of the $5^{th}$ National Health and Nutrition Examination Survey. Results : The subjective poor oral health was found in those who had mastication and speaking discomfort and those having dental prosthesis. Conclusions : The respondents considered themselves to be in poor oral health when the subjective systemic health status and objective oral health status were worse.

만성 B형 간질환에서 간염 B virus 표식자 발현에 따른 DNA의 검출 (Detection of Serum Hepatitis B Virus DNA According to HBV Markers in Chronic Hepatitis B Liver Disease)

  • 본 연구는, HBV의 증식 및 감염력을 파악하는데 가장 중요한 표지자로 알려진 HBV DNA를 최근 임상에서 많이 이용되고 있는 PCR법을 사용해 검출함으로써 기존의 dot blot법과 PCR법을 비교하고, 만성 간질환 환자에서 HBV의 e항원, 항체체계에 따른 HBV DNA의 검출율을 PCR법을 이용하여 알아봄으로써 HBV의 증식 및 감염력을 파악하고자 무증상 HBsAg 보유자 17명(HBeAg 양성 9명, anti-HBe 양성 8명), 만성 B형 간염 환자 91명(HBeAg 양성 50명, anti-HBe 양성 41명), 간경변증 한자 57명(HBeAg 양성 21명, anti-HBe 양성 36명), 원발성 간세포암 환자 27명(HBeAg 양성 10명, anti­HBe 양성 17명)을 대상으로 HBV DNA의 검출빈도를 조사하여 다음과 같은 결과를 얻었다. Dot blot법의 경우 HBeAg 양성인 환자군, anti-HBe 양성인 환자군에서의 HBV DNA의 검출율은 각각 58.9%, 34.3% 이었고, PCR법에 의한 검출율은 상기군에서 각각 72.2%, 53.9%였다. HBeAg 음성인군에서의 PCR법에 의한 HBV DNA 검출율은 무증상 HBsAg 보유자의 경우 25.0%, 만성 B형 간염 환자군의 경우 61.0%, 간병변증 환자군의 경우 52.8%, 원발성 간세포암 환자군의 경우 52.9%였다. 무증상 HBsAg 보유자에서 HBeAg 양성인 군과 음성인 군간의 HBV DNA 검출율은 각각 77.8%, 25.0%로 유의한 차이가 있었으나, 나머지 군에서는 HBeAg 양성인 군과 음성인 군간의 HBV DNA 검출율의 유의한 차이는 없었다. 이상의 결과로 PCR을 이용할 경우 기존의 dot blot보다 훨씬 예민하게 HBV DNA를 검출할 수 있고, HBsAg 양성인 만성 간질환 환자의 대부분에서는 HBeAg 및 anti-HBe 표식자 체계와는 무관하게 바이러스의 증식이 지속되고 있음을 알 수 있었으며, PCR법이 무증상 HBsAg 보유자의 예후를 알 수 있는 지표로서 유용성에 대한 가능성을 의미하나 확실한 의의를 알기 위하여 전향적인 연구가 필요할 것으로 사료된다.

A Study on the Positive Rates of HBsAg and Anti-HBs from Old People's Home and Casual Ward in Daegu, Korea

  • Serum hepatitis is still recognized as a major public health problem in many countries. The most common etiologies are acute viral hepatitis A and B, and hepatitis B virus (HBV) was known as the pathogen of serum hepatitis. since the HBV causes chronic hepatitis. HBV infection is hyperendemic in Korea. It is known that 8% and 1% of the Korean population is chronic carriers of HBV. The total number of 487 serum specimen (old people's home 190, causal ward-Daegu hope village 297) were collected at the laboratory of Department of Clinical Pathology of Daegu medical center. The laboratory tested the specimen to detect for hepatitis B surface antigen (HBsAg) and anti-HBs using reverse passive hemag-glutination (RPHA) and passive hemagglutination (PHA) method between April, 2011 and June, 2011. In one-step test, HBsAg and anti-HBs of Humasis were used. The positive rate for HBsAg was 6.80% (33/487), and the rate in male was 6.93% (19/274) higher than that of female 6.57% (14/213). In the positive rate of HBsAg by age group, the highest positive rate group was in the group of 40-49 years in male (12.0%), and 50-59 years in female (17.65 %). The overall positive rate for anti-HBs was 42.70% (208/487), showing the higher positive rate of 43.80% in male (120/274) than that of 41.30% (88/213) in female. The highest positive rate of anti-HBs in age group was the 40-49 years group in male (52.00%) and the group of under 29 in female (66.67%).

일부 농촌지역 초.중학생의 B형 간염 바이러스 감염 발생률에 대한 조사 연구 (Incidence of Hepatitis B Virus Infection of the School Children in a Rural Area of Korea)

  • Infection by hepatitis B virus is one of the major health problems of this nation. HBsAg positive rates of general population and school children were known to be as about 8 percent and 3.9 to 5.9 percent respectively. To study the incidence rate of hepatitis B infection in school children of rural area, author had examined 475 school children of relatively isolated agricultural area for baseline prevalence of hepatitis B virus serologic markers and followed up 415 school children during 10 months to determined the frequency of serologic conversion. The major results are summarized as followings: 1) Among the 278 susceptible children who were followed up, 26 had seroconversion for HBsAg or Anti-HBs. Therefore, the cumulative incidence rate during 10 months is estimated 9.4%. 2) The incidence rate of hepatitis B infection tends to increase with age (6-9yrs: 3.2%, 10-14yrs: 9.5%, 15-17yrs: 18.9%), and the incidence rate in male (13.0%) was higher than in female (5.7%). 3) The incidence rates of hepatitis B virus infection were not different statistically among three economic classes (The rates of lower, middle and higher class were 11.8%, 7.1% and 10.5%.). 4) The incidence rates of hepatitis B virus infection were not different statistically between visitors and non-visitors of clinic or hospital, dental clinic, persons received IV and not received IV, and persons with familial history and without familial history of liver diseases. Therefore all of these factors were not identified as risk factor of hepatitis B virus infection. And the transmissibility within the class of school was not recognized, too. 5) Among the 25 children who were HBsAg positive when enrolled, 15 (60%) were still HBsAg positive, who were identified as chronic carrier 15 of 415 school children were chronic carriers, then chronic carrier rate was estimated 3.6%, and there was no difference between sexes. 6) Of 38 children who had been Anti-HBs positive when enrolled, 5 (13.2%) lost Anti-HBs. Therefore, the loss rate of Anti-HBs per year is estimated to be 15.8%.

Expression of Hepatitis B Virus S Gene in Pichia pastoris and Application of the Product for Detection of Anti-HBs Antibody

  • Antibody to hepatitis B surface antigen (HBsAb) is the important serological marker of the hepatitis B virus (HBV) infection. Conventionally, the hepatitis B surface antigen (HBsAg) obtained from the plasma of HBV carriers is used as the diagnostic antigen for detection of HBsAb. This blood-origin antigen has some disadvantages involved in high cost, over-elaborate preparation, risk of infection, et al. In an attempt to explore the suitable recombinant HBsAg for the diagnostic purpose, the HBV S gene was expressed in Pichia pastoris and the product was applied for detection of HBsAb. Hepatitis B virus S gene was inserted into the yeast vector and the expressed product was analyzed by sodium dodecyl sulphate polyacrolamide gel electrophoresis (SDS-PAGE), immunoblot, electronic microscope and enzyme linked immunosorbent assay (ELISA). The preparations of synthesized S protein were applied to detect HBsAb by sandwich ELISA. The S gene encoding the 226 amino acid of HBsAg carrying ahexa-histidine tag at C terminus was successfully expressed in Pichia pastoris. The His-Tagged S protein in this strain was expressed at a level of about 14.5% of total cell protein. Immunoblot showed the recombinant HBsAg recognized by monoclonal HBsAb and there was no cross reaction between all proteins from the host and normal sera. HBsAb detection indicated that the sensitivity reached 10 mIu (micro international unit)/ml and the specificity was 100% with HBsAb standard of National Center for Clinical Laboratories. A total of 293 random sera were assayed using recombinant S protein and a commercial HBsAb ELISA kit (produced by blood-origin HBsAg), 35 HBsAb positive sera and 258 HBsAb negative sera were examined. The same results were obtained with two different reagents and there was no significant difference in the value of S/CO between the two reagents. The recombinant HBV S protein with good immunoreactivity and specificity was successfully expressed in Pichia pastoris. The reagent for HBsAb detection prepared by Pichia pastoris-derived S protein showed high sensitivity and specificity for detection of HBsAb standard. And a good correlation was obtained between the reagent produced by recombinant S protein and commercial kit produced by blood-origin HBsAg in random samples.

간기능 검사상 이상을 보인 환자에서의 HBV 표식자 발현 양상 (A Clinical Study of HBV Markers in Various Liver Diseases Carriers and Controls)

  • 저자들은 급성간염 39예, 만성간염 79예, 간경변증 30예, 원발성간암 16예, 보유자 14예 및 비교군 129예의 모두 307명을 대상으로 하여 SGOT, SGPT 및 HBV 표식자를 조사하여 다음의 결과를 얻었다. 1. HBsAg의 발현율은 급성간염의 66.7%, 만성간염의 63.3%, 간경변증의 36.7%, 원발성간암의 81.3% 및 비교군에서 27.1%인 것으로 나타났다. 2. AntiHBc의 발현율은 급성간염의 0%, 만성간염의 21.5%, 간경변증의 36.7%, 원발성간암의 31.3%, 보유자의 0% 및 비교군에서 44.2%로 나타났다. 3. HBeAg은 만성간질환중 만성간염에서 45.6%로 가장 높게 나타났고 AntiHBe는 원발성간암에서 56.3%로 가장 높았다. 4. AntiHBc는 각종 간질환, 보유자 및 비교군에서 높게 나타났으며 이는 AntiHBc자체는 간질환에 직접 관계가 없는 감염의 흔적으로 생각된다. 5. HBsAg 보유자 14명중 6명(42.3%)에서 혈청 GOT, SPT 치의 이상 상승이 있었다. 6. HBV에 감염된 혈청학적 증거가 있으면서 간기능에 이상이 있을 때, HBV 이외에 non-A, non-B 바이러스, Delta agent 또는 타 바이러스나 관련된 다른 요인과의 관계가 있을 수 있는 점을 고려하여야 할 것으로 생각된다. HBV의 5 가지 표식 자가 모두 음성으로 나타난 환자는 급성간염의 5.1%, 만성간염의 5.1%에 달했으며, 이것은 HBV 이외의 다른 원인이 병인이 될 수 있음을 시사하며 비교군 중 SGOT, SGPT치의 증가를 보인 군에서는 17.6%, 정상 SGOT, SGPT치를 나타낸 군에서는 12.8%에서 모든 HBV 표식자가 음성으로 나타났다.

  • Many investigators have been pursuing various attempts so far to produce hepatitis B surface antigen(HBsAg) vaccines using the techniques such as isolation from plasma of chronic HBsAg carrier, recombinant DNA technique or preparation of synthetic peptides specific for immunogenic determinants. Hepatitis B virus can not grow on any cell lines by the tissue culture technique at the present time. The plasma of chronic HBsAg carrier is expensive and its source is limited. The HBsAg from the recombinant DNA technique gave still very low yield. Another approach, therefore, has been initiated to develop a synthetic hepatitis B virus vaccine. The possible use of several distinct synthetic vaccines in prophylaxis can be facilitated by availability of full synthetic immunogens. Peptides synthesized for potential application as antiviral vaccines have been mostly tested in the form of conjugates with carrier proteins, although the free synthetic peptide can be immunogenic. To understand basic knowledges on the antigenicity and immunogenicity of a synthetic peptide specific for major immunogenic determinant of HBsAg, a nonapeptide, $H_2N^{139}Cys-Thr-Lys-Pro-Thr-Asp-Gly-^{146}Asn-Aba$ COOH, which corresponds to HBsAg amino acid residues 139 to 147, was synthesized by the Merrifield's solid-phase method with a slight modification. The antigenicity and immunogenicity of this specific synthetic peptide were examined comparing with purified plasma-derived natural HBsAg. The results obtained are as follows; 1. The peptide synthesized showed the identical amino acid composition to the theoretical value. The degree of purification and molecular weight were acertained by methods of high performance liquid chromatography and mass spectrometry. 2. Using m-maleimidobenzoyl-N-hydroxysuccinimide ester as a conjugating agent, the synthetic peptide was conjugated to rabbit albumin and ${\gamma}$-globulin, tetanus and diphtheria toxoids, and keyhole limpet hemocyanin. Their conjugation yields were 8.3, 9.5, 15.8, 13.5, and 11.2%, respectively. 3. The natural HBsAg was purified from plasma of chronic HBsAg carrier. By the electron microscopic observation of the purified natural HBsAg preparation, no Dane particles were observed and the preparation showed negative DNA polymerase activity. 4. Antigenicity of the synthetic peptide and the plasma-derived natural HBsAg was determined by competition radioimmunoassay using $^{125}I$-natural HBsAg. Their 50% inhibitions appeared as $90{\mu}g/ml$ and $0.12{\mu}g/ml$ for the synthetic peptide and the natural HBsAg, respectively. This indicates that the former was about 750-fold less antigenic than the latter. 5. Immunogenicity of the synthetic peptide was determined by administering the peptide-carrier conjugates into rabbits with and without Freund's complete adjuvant. Regardless the carrier proteins and adjuvant, positive immune responses to the synthetic peptide were observed. The higher antibody titers, however, were shown in the groups administered with Freund's complete adjuvant. 6. Immunizing dose 50% in mice of the various peptide-carrier conjugates was 5.47, 6.00, 65.16, 31.25 and $13.03{\mu}g/dose$ for rabbit albumin and ${\gamma}$-globulin, tetanus and diphtheria toxoids, and keyhole limpet hemocyanin, respectively, while the natural HBsAg showed $0.65{\mu}g/dose$. 7. It was postulated that homologous proteins prefer to heterologous ones as the carriers.

정상 성인에 있어서의 B형 간염 바이러스 감염에 관한 혈청역학적 연구 (Seroepidemiology of Hepatitis B Virus Infection in Healthy Korean Adults in Seoul)

  • While there have been not a few reports on the seroepidemiological characteristics of hepatitis B virus (HBV) infection in Korea, most of them, however, have had several limitations; operational definition of HBV infection, validity of detection methods of HBV serologic markers, size of the study population, and confirmation of the vaccination history against HBV, etc. In order to avoid such limitations, authors randomly selected 1,495 healthy adults among the 217,511 insured (target population) of Korean Medical Insurance Corporation, living in seoul, and tested HBV serologic markers by RIA method and conducted direct interview to them. Although HBV serologic markers (HBsAg, anti-HBs and anti-HBc) of all the subjects were tested, 392(26.2%) of interview failure cases and 361 vaccinee were excluded from the actual population. Finally, the serologic markers tested of 742 nonvaccinee (study population) only were analysed for the seroepidemiologic observation of the natural infection of HBV. The seroepidemiological characteristics of HBV infection in Korea were as follows ; 1. Point prevalence of HBs antigenemia was 11.7(9.1{\sim}14.3)% in male, which was slightly higher than that of female, 9.5($3.7{\sim}15.3$)%. This level was one of the highest among those of Asian-Pacific countries. Decreasing tendency of HBsAg prevalence alter the age of 50 was observed, which seems to be due to selective attrition of HBV chronic carriers among the healthy adults and/or to the limited-lasting duration of the HBs antigenemia, in part. 2. Point prevalence of anti-HBc(78.8% in male,50.9% in female) was higher than that of anti-HBs(65.2% in male,46.6% in female), respectively. And both of them were higher in male than in female. Increasing tendency of the prevalence of both antibodies was observed by age, which seems to be largely due to recurrent infection in adults and to some cumulative effect, in part, of their relatively longer-lasting duration. 3. The level of HBV infection defined by positive for at least one of the 3 serologic markers of HBV by RIA method was 84.7($81.8{\sim}87.6$)% in male and 61.2($51.9{\sim}70.5$)% in female, which was also one of the highest among those of Asian-Pacific countries. The proportion of susceptible population to HBV infection among healthy adults was 15.3% in male and 38.8% in female. 4. The relative frequency of current or past infection and chronic carrier among HBV infected person was estimated. The currently or past infected was estimated 75.7% in male and 71.8% in female, and chronic carrier state, 13.8% in male and 14.1% in female. The analysis of the geometric mean of the antibody titer in anti-HBs positive sera indicated also to be compatible with the above findings, suggesting that active, even though inapparent, infection of HBV occur so frequently among healthy adults in Korea.
