C-ANCA-positive glomerulonephritis associated with subacute infective endocarditis caused by Bartonella infection

  • Glomerulonephritis (GN) is sometimes associated with infective endocarditis (IE). Bartonella endocarditis is difficult to diagnose because it is rare and cannot be detected by blood culture. This is the first report of cytoplasmic anti-neutrophil cytoplasmic antibody-positive subacute endocarditis-associated GN caused by Bartonella infection in South Korea. A 67-year-old man was hospitalized due to azotemia. He complained of weight loss and anorexia for 6 months. A diagnosis of IE was made based upon echocardiographic detection of vegetations on the mitral and aortic valves and a Bartonella antibody titer of 1:2,048. Renal histology identified focal crescentic GN. Azotemia and proteinuria improved after doxycycline and rifampin treatment combining with steroid therapy.

Development of inactivated Akabane and bovine ephemeral fever vaccine for cattle

  • Akabane and bovine ephemeral fever (BEF) viruses cause vector-borne diseases. In this study, inactivated Akabane virus (AKAV)+Bovine ephemeral fever virus (BEFV) vaccines with or without recombinant vibrio flagellin (revibFlaB) protein were expressed in a baculovirus expression system to measure their safety and immunogenicity. Blood was collected from mice, guinea pigs, sows, and cattle that had been inoculated with the vaccine twice. Inactivated AKAV+BEFV vaccine induced high virus neutralizing antibody (VNA) titer against AKAV and BEFV in mice and guinea pigs. VNA titers against AKAV were higher in mice and guinea pigs immunized with the inactivated AKAV+BEFV vaccine than in animals inoculated with vaccine containing revibFlaB protein. Inactivated AKAV+BEFV vaccine elicited slightly higher VNA titers against AKAV and BEFV than the live AKAV and live BEFV vaccines in mice and guinea pigs. In addition, the inactivated AKAV+BEFV vaccine was safe, and induced high VNA titers, ranging from 1 : 64 to 1 : 512, against both AKAV and BEFV in sows and cattle. Moreover, there were no side effects observed in any treated animals. These results indicate that the inactivated AKAV+BEFV vaccine could be used in cattle with high immunogenicity and good safety.

사물별갑청피탕(四物鼈甲靑皮湯)과 사물별갑청피탕가미방(四物鼈甲靑皮湯加味方)의 항암작용(抗癌作用)과 면역반응(免疫反應)에 미치는 영향(影響) (Experimental studies on anti-tumor and immunomodulatory effects of Samulbyulgapchoengpitang and one herb added)

  • In order to investigate the effect of Samulbyulgapchoengpitang(SB) and one herb added(SBA) on anti-tumor and immune response, the author performed this experimental study, Tumor weight(TW), mean survival days(MSD) and body weight(BW) in vivo, natural killer cell activity(NKCA). rosette forming cells(RFC), phagoctic activity in recticuloendomethrial system(PA), delayed type hypersensitivity(DTH), hemoagglutinin titer(HA) and hemolysine(HL) in vitro were measured in mice. 1. MSD was prolonged in both of treated groups(SB and SBA) as compared with control group. 2. TW was decreased in both of treated groups with statistical significance as compared with control group. 3. BW was increased in both of treated groups and just only in SB with statistical significance as compared with control group. 4. DTH was increased in both of treated groups with statistical significance as compared with control group. 5. HA was increased in both of treated groups with statistical significance as compared with control group. 6. HL was increased in both of treated groups with statistical significance as compared with control group. 7. RFC was increased in both of treated groups and just only in SB with statistical significance as compared with control group. 8. NKCA was increased in both of treated groups with statistical significance as compared with control group. 9. PA was increased in both of treated groups with statistical significance as compared with control group. According to the above experimental results, it is suggested that SB and SBA will have anti-tumor substance and enhance the effect of immune response. But we have to consider the longtime prescription of SBA because there have been no experiments in its side effect or accumulation in body.

닭에 있어서 닭와포자충 감염이 sRBC에 대한 항체반응에 미치는 영향 (Effect of Cryptosporidium baileyi infection on antibody response to sRBC in chickens)

  • 닭에 있어서 닭와포자충 감염이 면역억제에 미치는 영항을 규명하기 위한 일환으로 150마리의 2 일령 SPF 병아리에 $5{\;}{\times}{\;}10^5$의 닭와포자충 오오시스트를 경구 투여하였다 경구투여 후 3일부터 95일까지 4일 간격으로 5마리씩의 병아리 정맥 내에 $1{\;}{\times}{\;}10^7$의 SRBC를 주입한 다음 각각 5일 후에 혈액을 채혈하여 SRBC의 응집소가를 측정하였다. 감염군에 있어서 분변 속의 오오시스트 배설은 정상적인 양상이었으며. 응집소가는 시일이 경과함에 따라 감염, 비감염 양군 모두 점점 높아져 52일에 최고에 이른 다음 점점 낮아졌다 한편. 실험기간을 통하여 응집소가는 오오시스트를 투여하지 않은 비감염군에 비하여 오오시스트를 투여한 감염군이 의의있게 낮았다. (P<0.05) 결과로 미루어 보아 닭와포자충 감염닭은 비감염닭베 비하여 항체반응이 저하되는 경향이 있다.

  • 3년령의 중성화된 암컷 시츄 견이 급성의 구토, 설사 및 전신 쇠약으로 인하여 내원하였다. 환자는 전신 쇠약, 호흡수 증가 및 노력성 호흡을 나타냈으며 신경 검사 상에서는 사지 근육 쇠약과 항문 조임근의 긴장도가 감소하였다. 방사선 검사를 통하여 거대식도가 확인되었다. 좌측 뒷다리 근육의 생검에 대한 조직병리 검사 상에서는 2형섬유 위축을 제외하고는 특이적인 이상 소견이 발견되지 않았다. 혈청의 아세틸콜린 수용체 항체가가 상승되어 있어 (0.78 nmol/L; 정상 범위, 0.6 nmol/L 이하) 급성 전격 중증 근육무력증으로 확진되어졌다. 환자는 pyridostigmine bromide에 대하여 극적으로 반응하여, 근육 강도, 거대식도, 그리고 호흡 기능이 현저하게 호전되었으며 현재 7개월 동안 환경은 성공적으로 관리되어져 오고 있다.

COVID-19 Vaccination Alters NK Cell Dynamics and Transiently Reduces HBsAg Titers Among Patients With Chronic Hepatitis B

  • Coronavirus disease 2019 (COVID-19) vaccination may non-specifically alter the host immune system. This study aimed to evaluate the effect of COVID-19 vaccination on hepatitis B surface Ag (HBsAg) titer and host immunity in chronic hepatitis B (CHB) patients. Consecutive 2,797 CHB patients who had serial HBsAg measurements during antiviral treatment were included in this study. Changes in the HBsAg levels after COVID-19 vaccination were analyzed. The dynamics of NK cells following COVID-19 vaccination were also examined using serial blood samples collected prospectively from 25 healthy volunteers. Vaccinated CHB patients (n=2,329) had significantly lower HBsAg levels 1-30 days post-vaccination compared to baseline (median, -21.4 IU/ml from baseline), but the levels reverted to baseline by 91-180 days (median, -3.8 IU/ml). The velocity of the HBsAg decline was transiently accelerated within 30 days after vaccination (median velocity: -0.06, -0.39, and -0.04 log10 IU/ml/year in pre-vaccination period, days 1-30, and days 31-90, respectively). In contrast, unvaccinated patients (n=468) had no change in HBsAg levels. Flow cytometric analysis showed that the frequency of NK cells expressing NKG2A, an NK inhibitory receptor, significantly decreased within 7 days after the first dose of COVID-19 vaccine (median, -13.1% from baseline; p<0.001). The decrease in the frequency of NKG2A+ NK cells was observed in the CD56dimCD16+ NK cell population regardless of type of COVID-19 vaccine. COVID-19 vaccination leads to a rapid, transient decline in HBsAg titer and a decrease in the frequency of NKG2A+ NK cells.

약병원성 조류인플루엔자 사독백신개발을 위한 후보주(ADL0401)의 면역 원성 및 안전성 평가 (Evaluation on Immunogenicity and Safety of Avian Influenza Isolate(ADL0401) as a Candidate for the Killed Vaccine against tow-Pathogenic Avian Influenza)

  • 본 연구는 약병원성 조류인플루엔자 사독백신 개발을 위하여 백신 후보주(ADL0401)로서의 생물학적 특성 및 개발 백신에 대한 면역원성 및 안전성 평가를 실시하였다. 백신 후보주인 ADL0401의 병원성을 조사한 결과 폐사는 없었으며, 임상증상 및 바이러스 재분리율 양상이 국내 표준 야외주인 MS96과 상당히 유사한 생물학적 특성을 나타내어 약병원성 조류인플루엔자의 특징을 갖고 있음을 알 수 있었다. 표준야외주인 MS96과 백신후보주인 ADL0401간의 이종항원의 중화능 실험을 한 결과 r값 = 0.71로 동종항원(r값 = 1)에 비하여 다소 낮은 중화능을 나타내었지만, 두 바이러스간의 항원성엔 큰 차이가 없음을 알 수 있었다. 불활화 능력실험으로 $0.1\%$ Formalin을 $37^{\circ}C$ 조건에서 처리하였을 때 가장 효과적인 결과를 가져왔으며, MS96은 2시간, ADL0401은 1시간만에 다소의 역가 저하는 있었으나 빠르게 불활화가 이루어짐을 확인하였다. 개발 백신의 면역원성 및 안전성 실험 결과 ISA 70 adjuvant 백신 접종시 백신 접종 전후로 폐사율 및 임상증상은 관찰되지 않았으며, 높은 수준의 항체 역가를 형성함으로써 가장 면역원성이 우수한 것으로 확인되었다.

Genetic and biological characteristics of recent Korean isolates of avian influenza virus subtype H9N2

  • The worldwide distribution and continuing genetic mutation of avian influenza virus (AIV) has been posed a great threat to human and animal health. A comparison of 3 isolates of AIV H9N2, A/chicken/Korea/KBNP-0028/00 (H9N2) (KBNP-0028), A/chicken/Korea/SNU8011/08 (H9N2) (SNU 8011) and an inactivated oil vaccine strain A/chicken/Korea/01310/01 (H9N2) (01310), was performed. The former 2 AIVs were isolated from field cases before and after the application of an inactivated H9N2 vaccine in 2007, respectively. The antigenic relationship, viral shedding, tissue tropism and genetic analysis were examined. The comparison of virus shedding from the cloaca and the oropharynx revealed that both isolates were more frequently isolated from the upper respiratory tract (90~100%) 1 day post inoculation (DPI) compared with isolation 5 DPI from gastrointestinal tracts (10~60%). Moreover, the isolate KBNP-0028 were recovered from all organs including bone marrow, brain and kidneys, indicating higher ability for broad tissue dissemination than that of SNU 8011. KBNP-0028 replicated earlier than other strains and with a higher titer than SNU 8011. In full-length nucleotide sequences of the NA gene and a partial sequence of the HA gene of SNU 8011, we found that there might be significant changes in tissue tropism, virus replication and genetic mutation in AIV H9N2 isolates.

십전대보탕(十全大補湯) 와송(瓦松) 및 십전대보탕가와송(十全大補湯加瓦松)의 항암효과(抗癌效果)와 면역반응(免疫反應)에 관(關)한 연구(硏究)

  • In order to research the effects of Shipjundaebotang(SDT) and Orostachys Herba(OH) on anti-tumor and immune response, the author performed this experimental study. Experimental groups are divided into five groups, which are solid extract of Orostachys Herba by water(OHW), solid extract of Orostachys Herba by ethanol(OHE), solid extract of Shipjundaebotang (SDT), solid extract of Shipjundaebotang added by solid extract of Orostachys Herba by water(SDT+OHW) and solid extract of Shipjundaebotang added by solid extract of Orostachys Herba by ethanol (SDT+OHE). In these experimental studies, extension of survival days for anti-tumor effect was observed, and de layed type hypersensitivity and rosette forming cell for cell-mediated immune response, hemagglutinin titers and hemolysin titers for humeral immune response, spleenic natural killer cell activity and carbon clearance (K-index) in vitro were measured with mice. The result were summerized as follows: I. SDT, SDT+OHW and SDT+OHE treated groups were significantly recognized to extend the survival days of tumor bearing mice as compared with the control group. 2. Delayed type hypersensitivity was significantly increased in SDT, SDT+OHW and SDT+OHE treated groups as compared with control group. 3. Hemagglutinin titer was increasred in all sample groups as compared with control group, but not significantly. 4. Hemolysin titers was significantly increased in SDT, SDT+OHW and SDT+OHE treated groups as compared with control group, and SDT+OHE treated group showed the increasing effect with significance as compared with the other sample groups. 5. For the effect of roselle forming cell quantitation, SDT, SDT+OHW and SDT+OHE treated groups showed the increasing effect with significance as compared with control group. 6. Natural killer cell activity was significantly increased in SDT+OHW as compared with comrol group, but the other groups, except OHW and SDT+OHW treated groups, revealed the increasing effect as compared with control group, but the significance was not admitted. 7. For the effect of K-index(Carbon clearance), SDT, SDT+OHW and SDT+OHE treated groups showed the increas ing effect with significance as compared with control group. 8. The study didn't show that Orostachys herba had any significance with survival days, anti-tumor effect and immune re sponse.

Systemic Analysis of a Novel Coxsackievirus Gene Delivery System in a Mouse Model

  • In order to systemically investigate the possibility of using coxsackievirus B3 (CVB3) to deliver foreign genes in vivo, a recombinant strain of CVB3 encoding the renilla gene (CVB3-renilla) was constructed. The recombinant CVB3 resulted in extensive and transient expression of the renilla protein within mouse organs, especially the pancreas. The level of expression was generally dependent upon the viral titer present. Moreover, the CVB3-renilla strain was completely attenuated. Interestingly, the recombinant CVB3 vector was expressed much more strongly in mouse organs than was a comparable adenoviral vector. The CVB3-renilla strain did not express the renilla gene in mice with pre-existing coxsackievirus-specific neutralizing antibodies, but direct organ-specific administration of the virus during open-peritoneum surgery was able to circumvent this immunity. This coxsackievirus vector may represent a useful means for delivering and expressing foreign genes in mouse models in an acute and extensive fashion.
