Effect of the pulmonary recruitment maneuver on pain after laparoscopic gynecological oncologic surgery: a prospective randomized trial

  • Objective: To evaluate the effectiveness of the pulmonary recruitment maneuver (PRM) at the end of the operation to decrease laparoscopy-induced abdominal or shoulder pain after gynecological oncologic surgery. Methods: In total, 113 women undergoing laparoscopic surgery for malignant or premalignant gynecological lesions were assigned randomly to two groups: the PRM group (the patient was placed in the Trendelenburg position ($30^{\circ}$) and the PRM, consisting of two manual pulmonary inflations to a maximum pressure of $40cmH_2O$) (n=54) and the control group (n=52). Postoperative shoulder and abdominal pain was assessed 12, 24, and 48 hours later using a visual analog scale (0-10). In addition, the incidence of post-discharge nausea and vomiting was recorded until 48 hours after discharge. Results: Postoperative shoulder pain at 12 and 24 hours was significantly less severe in the PRM group ($2.2{\pm}0.5$ and $2.0{\pm}0.4$) than in the control group ($4.0{\pm}0.5$ and $3.9{\pm}0.4$; both p<0.001). The PRM significantly reduced the severity of upper abdominal pain at 12 and 24 h compared with the control group ($3.1{\pm}0.4$ and $2.9{\pm}0.4$ vs. $2.9{\pm}0.5$ and $4.9{\pm}0.5$; both p<0.001). The analgesic requirement during the postoperative period was similar in the two groups (control group, 78.8%; PRM group, 75.9%; p=0.719). Conclusion: The PRM effectively and safely reduced postoperative shoulder and upper abdominal pain levels in patients undergoing laparoscopic gynecological oncologic surgery. The impact of peripheral neuropathy symptoms, self-care ability, and disturbances to daily life on quality of life among gynecological cancer patients undergoing chemotherapy: a cross-sectional survey

  • Purpose: This study investigated the effects of peripheral neuropathy symptoms, self-care ability, and disturbances to daily life on quality of life (QoL) among gynecological cancer patients undergoing chemotherapy. Methods: The participants included 144 patients with gynecological cancer undergoing anticancer chemotherapy at a tertiary hospital in Seoul, South Korea, from December 1, 2021 to January 28, 2022. Convenience sampling was used to recruit patients who had received 4 or more cycles of chemotherapy using a paclitaxel-platinum regimen, and a self-reported questionnaire was used to collect data. Descriptive statistics, the t-test, analysis of variance, Scheffé test, Pearson correlation coefficients, and multiple regression analysis were performed. Results: Most of the participants had ovarian cancer (70.1%) or endometrial cancer (14.6%), and the most common number of treatment cycles was 6 to 10 (29.2%). The mean QoL (60.83±19.89) was greater than the midpoint. The regression model analyzing the patients' QoL was statistically significant (F=15.38, p<.001) with an explanatory power of 56.7%. Self-care ability (β=.39, p<.001), disturbances to daily life (β=-.38, p<.001), the duration of peripheral neuropathy symptoms (β=2.14, p=.034), and regular exercise (β=-2.12, p=.036) were found to significantly affect QoL. Conclusion: Efforts to improve the self-care ability of gynecological cancer patients who have experienced peripheral neuropathy after receiving chemotherapy and mitigate disturbances to their daily life can improve their QoL. Healthcare professionals should identify peripheral neuropathy symptoms and examine the effects of the symptoms on patients' daily lives. Improving the self-care ability of patients and alleviating their limitations in daily life may improve QoL.

  • Immunotherapy is an effective treatment option for gynecological malignancies. Radiologists dealing with gynecological patients undergoing treatment with immune checkpoint inhibitors should be aware of unconventional immune-related imaging features for the evaluation of tumor response and immune-related adverse events. In this paper, immune checkpoint inhibitors used for gynecological malignancies and their mechanisms of action are briefly presented. In the second part, patterns of pseudoprogression are illustrated, and different forms of immune-related adverse events are discussed.

여성생식기암 환자 배우자의 스트레스와 적응 (The Stress and Adaptation of the Spouses of Patients with Gynecological Cancer)

  • Purpose: This study was undertaken to obtain the basic data on the spouses‘ stress and adaptation caring their wives affected by gynecological cancer. In addition, moderating variables for adaptation, family resource and situational definition, were also investigated. Method: This study was descriptive survey research work by questionnaire on the 71 spouses hospitalized in an university hospital located on Gwangju from Sep. 2001 to Oct. 2002. Results: The mean score of stress was 2.27, and the mean score of adaptation was 3.12. The correlation between stress and adaptation was significant (r = -.692, p = .000). The adaptation score of high stress group with high family resource was higher (p = .041) than the group with low family resource. The adaptation score of high stress group with high situational definition was higher (p = .033) than the group with low situational definition. Conclusion: This study showed that the spouses experienced a similar level of stress compared with the level of families with other type of cancer. This study also showed that the stress and adaptation levels were different depending upon the family resource and situational definition. This aspect should be considered for the development of nursing intervention program for the spouse of patients with gynecological cancer.

간호중재분류(NIC)에 근거한 간호중재수행분석 I -병원 간호사를 중심으로- (Analysis of the Nursing Interventions Performed by Hospital Nurses Using NIC)

  • The purpose of this research was to identify nursing interventions performed by hospital nurses in Korea. The sample consisted of 311 nurses working in three hospitals. The Nursing Interventions Use Questionnaire developed by the Iowa Intervention Project team was used for data collection. The instrument was translated to Korean using the method of back-translation. Eighteen interventions were performed at least daily. Interventions in the Physiological : Basic domain were most frequently used at least daily. No interventions in the Family and Behavioral domains were used by nurses at least once a day. The most frequently used interventions was Documentation, followed by the interventions Medication : Parenteral, Intravenous(IV) Insertion, Temperature Control, and Shift Report. The intervention performed least often was Reproductive Technology Management. Nurses working in intensive care units on the whole performed interventions most often, while nurses working in obstetric, gynecological, and pediatric units performed them least often. The nurses working in intensive care unit, medical and surgical care units performed the interventions in the Physiological : Basic domain more often than the nurses working in obstetric, gynecological, and pediatric units. The nurses working in obstetric, gynecological, and pediatric units used the interventions in the Family domain more often than the nurses working in the other three units. This study contributes to the documentation of nursrs' work in Korea. Further study will be needed to validate nursing activities of each NIC intervention.

통증조절 개별교육이 부인과 수술환자의 통증조절 방해, 수술 후 통증 및 통증조절 만족도에 미치는 효과 (Effects of Pain Control Education on Pain Control Barrier, Postoperative Pain and Pain Control Satisfaction in Gynecological Patients)

  • Purpose: The purpose of this study was to evaluate the effects of preoperative pain control education on the pain control barrier, postoperative pain and pain control satisfaction in gynecological patients. Method: The study was a quasi-experimental research design. There were 58 subjects who were admitted for gynecological surgery to D University Hospital in B city. Pain control education was provided individually to the experimental group one day before their operation day for 20 minutes with the 'Pain Control Guide Book' in the patient's admission room. The education book was made by researchers based on pain management references and patient interviews. For assessing the pain control barrier, a simplified version of Barriers Questionnaire was used, postoperative pain was assessed on a numeric scale(0-10) and satisfaction of pain control was assessed by one question. Results: The pain control barrier(F=15.828, p<.001) and the post pain score of the experimental group was lower than that of the control group. In addition, pain control satisfaction of the experimental group(t=3.612, p<.001) was higher than the control group's. Conclusion: With the above results, preoperative pain control education could be an effective nursing intervention for pain control of surgical patients.

부인암 전문간호사 제도 확립을 위한 기초조사 (A Fundamental Study for a System Establishment of Advanced Practice Nursing for Gynecological Cancer Patients)

  • Purpose: This study was conducted to provide fundamental information for a system establishment of advanced practice nursing for gynecological cancer patients (APN-GCP). Method: Data was collected by focus group and individual interviews and analyzed in the framework of the Grounded theory method mapped by Strauss and Corbin (1990). There were 13 subjects in this study (nurses, doctors, patient and her family). Result: We identified 87 concepts, 22 sub-categories, and 10 categories. Categories for role expectation were arrangement of diagnosis and treatment process, giving information of treatment course, support of treatment process, patients' right toward making a decision of treatment, counseling and teaching after discharge from hospital, medical insurance and financial problems, counseling about sexual problems and use of family and community resources. All subjects perceived the necessity of an APN-GCP. An APN-GCP requires over 2$\sim$7 years clinical experience and a master's degree. Services would be performed from initial registration to termination of treatment or death, and accomplished on an outpatient clinic basis. Conclusion: The nursing delivery system and curriculum should be developed for a women's health nurse practitioner including APN-GCP. As a further step, cost-effectiveness and projected estimation of manpower of APN-GCP should be studied in the future.

간호중재분류 (NIC)에 근거한 부인과 간호단위의 간호중재 분석 (Analysis of Nursing Interventions Performed by Gynecological Nursing Unit Nurses Using the Nursing Interventions Classification)

  • Purpose: The purpose of this study was to identify nursing intervention performed by nurses on gynecological nursing units. Methods: The instrument in this study is based on the fifth edition of Nursing Interventions Classification (NIC) (2008). Data was collected by Electronic Medical record from August, 2010 to October, 2010 at one hospital and analyzed by using frequencies in the Microsoft Excel 2010 program. Results: Of a total of 82 NIC, domains of the nursing interventions showed higher percentages for physiological: basic (36.3%) and physiological: complex (34.5%). The classes of nursing interventions showed higher percentage for health system medication (12.1%), perioperative care (10.0%), and drug management (8.6%). The most frequently used top interventions were Discharge Planning. The thirty least used interventions was environmental management. Top thirty most frequently used interventions belonged to the domain of physiological: basic (37.9%), physiological: complex (31.1%), and behavioral (5.4%). Conclusion: These findings will help in the establishment of a standardized language for gynecological nursing units and enhance the quality of nursing care.

부인암 환자의 삶의 질 예측요인 (Factors Influencing on Quality of Life in Gynecological Cancer Patients)

  • Purpose: The purpose of this study was to measure the quality of life (QOL) and to identify the factors influencing QOL in gynecological cancer patients. Methods: The subjects of this study were 242 people who were receiving medical therapy or follow-up after surgery from one general hospital in Daegu. Data were collected from August 1, 2010 to January 31, 2011. A questionnaire including questions on QOL, distress score, distress problem, depression, anxiety, insomnia, perceived health status and body image were completed by the subjects. Results: The mean score of QOL was $70.68{\pm}13.40$. Religion, job, presence of spouse, level of education, household income, financial compensation, disease stage and recurrence were the significant factors related to QOL. Distress score, distress problem, depression, anxiety, insomnia, perceived health status and body image were also significant factors influencing QOL. Sixty eight percent of the variance in subjective overall QOL can be explained by body image, distress problem, distress score, anxiety, level of education and perceived health status (Cum $R^2$=0.689, F=76.316, $p$ <.001). Body image was the most important factor related to QOL. Conclusion: An integrative care program which includes general, disease-related and psychosocial characteristics of patients is essential to improve QOL in gynecological cancer patients.

한방부인과 영역에 활용되는 수기요법의 국내외 연구 동향 (Research Trends Using Manual Therapy in Obstetrics and Gynecological Diseases)

  • Objectives: The purpose of this study is to analyze the research trends using manual therapy in obstetrics and gynecological diseases. Methods: We searched for studies that performed manual therapy on diseases related to obstetrics and gynecology. All relevant papers were selected and extracted to be analyzed. Results: 12 Randomized controlled trials, 8 cases, 3 pilot studies, 3 non-comparative studies, and 1 before-after comparative study were collected. The study subjects, results, techniques of manual therapy used were analyzed. There were studies related to pregnancy-related symptoms, postpartum pain, dysmenorrhea, chronic pelvic pain, abnormal uterine bleeding, sexual dysfunction, and childbirth. Conclusions: It has been found that various manual therapies can be used for obstetrics and gynecological diseases.
