Pediatric lymphangioma can occur at any site. However the neck is the most common site. There are two treatment modalities (surgical excision and intralesional injection) for lymphangiomas. But, the treatment guide line for lymphangioma has not been established, yet. The aim of this study is to establish the treatment guide line based on our experience with lymphangiomas. Medical records of 82 cases of lymphangioma were reviewed retrospectively. On MRI (magnetic resonance image) findings, lymphangiomas were divided into 4 groups by the proportion of the cyst bigger than 2 cm in diameter of the tumor; group A-proportion of cyst occupies more than 75%, group B-proportion of the cyst 50~75%, group C-25~50%, and D in less than 25%. All patients were treated with OK-432 intralesional injection as the initial treatment. The effective response rates of OK-432 in group A & B were 88.2% and 88.8%, respectively. Group C response was 38.0% and D only 20.0%. Twenty-three patients received surgical excision. The result of surgical excision was generally satisfactory. Surgical site infection occurred in 1 case and postoperative bleeding in 1 case. Theses results indicate that intralesional injection of OK-432 could be the first line therapy in group A & B. In group C, OK-432 would be better as the first line therapy than surgery. For the group D, surgical excision should be the first line of treatment.
The latissimus dorsi flap is popular due to the versatile nature of its applications. When used as a pedicled flap it provides a robust solution when soft tissue coverage is required following breast, thoracic and head and neck surgery. Its utilization as a free flap is extensive due to the muscle's size, constant anatomy, large caliber of the pedicle and the fact it can be used for functional muscle transfers. In facial palsy it provides the surgeon with a long neurovascular pedicle that is invaluable in situations where commonly used facial vessels are not available, in congenital cases or where previous free functional muscle transfers have been attempted, or patients where a one-stage procedure is indicated and a long nerve is required to reach the contra-lateral side. Although some facial palsy surgeons use the trans-axillary approach, an operative guide of raising the flap by this method has not been provided. A clear guide of raising the flap with the patient in the supine position is described in detail and offers the benefits of reducing the risk of potential brachial plexus injury and allows two surgical teams to work synchronously to reduce operative time.
우리나라 치과의 CBCT 보급률은 세계적으로 높은 편으로, 최근 1만대 보급을 돌파하고 있는 것으로 파악된다. 또한 최근 각 치과 제조업체마다 DLP방식의 In-House방식의 3D 프린터 보급률 또한 가파르게 상승하고 있다. 이에 맞춰 최근 개원가에서 CBCT와 구강 스캔 정보를 이용한 컴퓨터 가이드 임플란트 수술의 활용도가 증가하고 있는 상황이다. 현재 컴퓨터 가이드를 이용한 임플란트 수술 방법에 대해 문헌상 고찰을 통해 리뷰해보고, 또한 정확성 및 신뢰성이 보장되는 가이드 수술을 위한 고려사항에 대해 정리해보고자 한다.

The fibula free flap has now become the most reliable and frequently used option for mandible reconstruction. Recently, three dimensional images and printing technologies are applied to mandibular reconstruction. We introduce our recent experience of mandibular reconstruction using three dimensionally planned fibula free flap in a patient with gunshot injury. The defect was virtually reconstructed with three-dimensional image. Because bone fragments are dislocated from original position, relocation was necessary. Fragments are virtually relocated to original position using mirror image of unaffected right side of the mandible. A medical rapid prototyping (MRP) model and cutting guide was made with 3D printer. Titanium reconstruction plate was adapted to the MRP model manually. 7 cm-sized fibula bone flap was designed on left lower leg. After dissection, proximal and distal margin of fibula flap was osteotomized by using three dimensional cutting guide. Segmentation was also done as planned. The fibula bone flap was attached to the inner side of the prebent reconstruction plate and fixed with screws. Postoperative evaluation was done by comparison between preoperative planning and surgical outcome. Although dislocated condyle is still not in ideal position, we can see that reconstruction was done as planned.
Objectives: This study, investigated the satisfaction of patients who received the procedure using an implant guide to identify factors affecting satisfaction in dental hospitals and clinics in Jeollanam-do. Methods: From December 2021 to February 2022, questionnaires randomly were collected from 128 patients aged 20 to 90 years visiting dental hospitals and clinics using surgical guides in some areas of Jeollanam-do. Results: There was a negative correlation between the overall satisfaction average and Visual Analog Scale (VAS) pain level (r=-0.414, p<0.001), and a negative correlation between VAS pain level and VAS satisfaction (r=-0.273, p<0.05), As subjective pain decreased, subjective satisfaction increased, and as subjective pain increased, the overall satisfaction average also decreased, showing an inverse correlation. Factors affecting satisfaction with the guide for implant surgery were whether you would like to recommend it to people around you (β=0.556), whether you would like to explain the guide (β=0.194), and whether you thought it was good to receive the guide (β=0.199). Conclusions: Therefore, in order to increase the satisfaction of the implant surgical guide procedure, it is considered important to perform a sufficient explanation of the implant surgical guide before the procedure.
Surgeon dentists usually rely on their experiential judgments from patients' oral plaster casts and medical images to determine the positional and directional information of implant fixtures and to perform drilling tasks during dental implant surgical operations. This approach, however, may cause some errors and deteriorate the quality of dental implants. Computer-aided methods have been introduced as supportive tools to alleviate the shortcomings of the conventional approach. In this paper, we present an approach of 3D dental implant simulation which can provide the realistic and immersive experience of dental implant information. The dental implant information is primarily composed of several kinds of 3D mesh models obtained as follows. Firstly, we construct 3D mesh models of jawbones, teeth and nerve curves from the patient's dental images using software $Mimics^{TM}$. Secondly, we construct 3D mesh models of gingival regions from the patient's oral impression using a reverse engineering technique. Thirdly, we select suitable types of implant fixtures from fixture database and determine the positions and directions of the fixtures by using the 3D mesh models and the dental images with software $Simplant^{TM}$. Fourthly, from the geometric and/or directional information of the jawbones, the gingival regions, the teeth and the fixtures, we construct the 3D models of surgical guide stents which are crucial to perform the drilling operations with ease and accuracy. In the application phase, the dental implant information is combined with the tangible interface device to accomplish 3D dental implant simulation. The user can see and touch the 3D models related with dental implant surgery. Furthermore, the user can experience drilling paths to make holes where fixtures are implanted. A preliminary user study shows that the presented approach can be used to provide dental students with good educational contents. With future work, we expect that it can be utilized for clinical studies of dental implant surgery.
증강현실 기술은 사용자의 시각에 가상의 영상 정보를 결합하여 다양한 영상 정보를 동시에 취득할 수 있게 한다. 이러한 증강현실 기술은 최근 의료융합을 통해 이미지 가이드 수술(Image-Guide Operation), 수술 교육 훈련, 영상 진단 분야에서 수술 및 진단 시 환자의 장기나 조직들을 구분하여 가시화하고 가장 효과적인 수술 방법을 제시 할 수 있다. 이에 본 논문을 통해 증강현실 기술의 의료 융합을 위한 각 요소 기술의 기술적 특징과 적용 방안 등을 고찰하고자 하였다. 의료융합을 위한 증강현실 기술에서는 효율적인 의료 영상의 구현을 위해 디스플레이, 마커인식 그리고 영상합성 인터페이스 기술의 유기적인 구동이 필수적이다. 이러한 증강현실 기술은 향후, 이미지 가이드 수술, 수술 교육 및 영상 진단 등의 분야에서 현재의 의료 기술을 획기적으로 증진시킬 수 있는 방안이 될 수 있으리라 여겨진다.
PURPOSE. The present experiment aimed to evaluate the placement accuracy of fully guided implant surgery using a mucosa-supported surgical guide when the protocol of osteotomy and installation was modified (MP) compared to when the protocol was sequentially and conventionally carried out (CP). MATERIALS AND METHODS. For 24 mandibular dentiform models, 12 dentists (6 experts and 6 beginners) performed fully guided implant placements two times at the right first and second molar sites using a mucosa-supported surgical guide, once by the CP (CP group) and at the other time by the MP (MP group). The presurgical and postsurgical stereolithographic images were superimposed, and the deviations between the virtually planned and actually placed implant positions and the procedure time were compared statistically (P < .05). RESULTS. The accuracies were similar in the CP and MP groups. In the CP group, the mean platform and apex deviations at the second molar site for the beginners were +0.75 mm and +1.14 mm, respectively, which were significantly larger than those for the experts (P < .05). In the MP group, only the mean vertical deviation at the second molar site for the beginners (+0.53 mm) was significantly larger than that for the experts (P < .05). The procedure time was significantly longer for the MP group (+94.0 sec) than for the CP group (P < .05). CONCLUSION. In fully guided implant surgery using a mucosa-supported guide, the MP may improve the placement accuracy when compared to the CP, especially at sites farther from the most-posterior natural tooth.
목적: 전방 십자 인대 재건술시 잔류조직 보존은 이식건 치유를 향상시키고 고유수용 감각 기능을 보존할 수 있어 저자들은 10시 혹은 2시 방향 경경골 대퇴 터널을 천공하고 잔류조직을 보존하는 술기를 소개하고자 한다. 수술 술기: 잔류조직은 가능한 많이 보존하고자 하였으며 그 중에서 경골 부착부위 전방부와 후방 십자 인대와 연결된 활액막 및 잔류조직은 반드시 보존하여 잔류조직의 장력을 어느정도 유지하고 흘러내림을 방지하여 과간 절흔의 충돌을 방지 하였다. 경골 drill guide의 각도를 40~45도로 하고 관절 내 guide tip을 고식적인 위치보다 1 mm씩 전방 및 내측으로 이동하였다. 경골에서 유도 강선의 시작점은 거위발 건의 근위부, 내측 측부 인대의 전방부로 하였다. 경골 터널은 먼저 이식건의 두께보다 1 mm 작은 확공기를 이용하여 만들되, 경골 관절면의 피질골에 닿으면 조심스럽게 진행하여 피질골을 뚫자마자 멈춤으로써 가능한 잔류조직에 손상이 가지 않도록 하였으며, 다시 이식물과 같은 크기의 확공기를 이용하여 경골 터널과 10시 혹은 2시 방향의 대퇴 터널을 완성하였다. 결론: 전방 십자 인대 재건술시 10시 혹은 2시 방향 경경골 대퇴 터널을 천공하고 잔류조직을 보존하는 술기를 소개하였다.
