Gait Analysis According to the changes of the carrying type and weight of bag (가방의 휴대 형태와 무게 변화에 따른 보행 분석)

  • The purpose of this study was to analyze the changes in gait according to four style of bag's carryied method and three different bag's weights. Twenty healthy adults participated in four conditions. The first condition, they wearing a bag on one side shoulder and walked. The second condition, they carried a bag sling across on shoulder and walked. The third condition, they carried a bag on a back using both shoulders and walked. The fourth condition, they hold a bag in their right hand and walked. During all four conditions participants wore a SmartStep insole in their right shoe and had a pressure control device strapped to their right ankle. Each participant walked 10 meters carrying a 2.5 kg, 5 kg and 7.5 kg bag under all four conditions. There were significantly differents in stance phase rate; swing phase rate and walking speed according to bag weight of 2.5 kg, 5 kg, 7.5 kg.

The Usability of Perforator-based Fasciocutaneous Flap for Trochanteric Pressure Sore (대전자부 압박궤양에서 천공지를 이용한 근막피부피판술의 유용성)

  • Purpose: Myocutaneous flap was widely used for trochanteric pressure sore but it had many drawbacks such as donor site morbidity, dog-ear deformity and functional muscle sacrifice. We have performed fasciocutaneous flap based on perforating vessels and succeeded in overcoming its drawbacks. Methods: We experienced 11 cases of perforator-based fasciocutaneous flap for the coverage of trochanteric pressure sore in 9 patients, 2 cases of which were bilateral. The ambulatory status of patient group is as follows: 6 of them used a wheelchair, 2 of them are free walking, 1 of them use a wheelchair or crutches. Flap was supplied by cutaneous perforating vessel of descending branch of the lateral circumflex femoral artery and the third perforating artery of the deep femoral artery. The size of wounds were from $4{\times}6.5cm$ to $10{\times}13cm$. Results: We did not find any flap loss or congestion except 2 partial wound dehiscences and 1 wound infection. Donor site morbidity was not found. We observed no recurrence of the pressure sore during the 2.5 year follow-up period. Conclusion: We considered that perforator-based fasciocutaneous flap could overcome the traditional drawbacks of the conventional myocutaneous flap and its modified flap for trochanteric pressure sore. And this flap has many advantages for covering trochanteric pressure sore without any donor site deformity and morbidity, which would greatly improve the aesthetic result.

The effects of sports on the General Features of low back pain (운동이 요통의 일반적 특성에 미치는 영향)

  • This study was performed to investigate the effects on the general features of low back pain. The study subjects were low back pain patients treated with physical therapy at general hospitals located in Gyeonggi-do from August 2, 2000 to September 10. Total number of distributed questionnaires were 300, and 281 questionnaires were collected and analysed finally. The results were follows: 1. 76.3$\%$ of male answers that participate in sports, 60.6$\%$ of female answers that participate in sports and there are significant differences of two group(p < 0.01). 2. In case of male, preferential sports were running, tennis, golf, the others, but in case of female, preferential sports were walking, ascent of a mountain, swimming(p < 0.01). 3. In case of male, the most frequency preferred to participate in sports by patients was 30minutes daily, but in case of female, 3 times a week(p < 0.05). 4. The onset time of low back pain was 28.3$\%$ in the morning, 23.7$\%$ in the afternoon, 23.2$\%$ in motion according. 5. The onset conditions of low back pain was 48.5% during exercise, 44.4$\%$ after exercise. 6. Regardless of gender, low back pain patients want to go to a hospital for treatment. Otherwise, male preferred conventional treatment than a hospital, but female preferred a hospital than conventional treatment(p < 0.05). 7. Regardless of gender, 97.5$\%$ of patients is answered that the most preferential treatment method was physical therapy, 8. 92.2$\%$ of patients is answered that physical therapy was effective.

Bee venom reduces burn-induced pain via the suppression of peripheral and central substance P expression in mice

  • Background: Scalding burn injuries can occur in everyday life but occur more frequently in young children. Therefore, it is important to develop more effective burn treatments. Objectives: This study examined the effects of bee venom (BV) stimulation on scalding burn injury-induced nociception in mice as a new treatment for burn pain. Methods: To develop a burn injury model, the right hind paw was immersed temporarily in hot water (65℃, 3 seconds). Immediately after the burn, BV (0.01, 0.02, or 0.1 mg/kg) was injected subcutaneously into the ipsilateral knee area once daily for 14 days. A von Frey test was performed to assess the nociceptive response, and the altered walking parameters were evaluated using an automated gait analysis system. In addition, the peripheral and central expression changes in substance P (Sub P) were measured in the dorsal root ganglion and spinal cord by immunofluorescence. Results: Repeated BV treatment at the 2 higher doses used in this study (0.02 and 0.1 mg/kg) alleviated the pain responses remarkably and recovered the gait performances to the level of acetaminophen (200 mg/kg, intraperitoneal, once daily), which used as the positive control group. Moreover, BV stimulation had an inhibitory effect on the increased expression of Sub P in the peripheral and central nervous systems by a burn injury. Conclusions: These results suggest that a peripheral BV treatment may have positive potency in treating burn-induced pain.

  • Objectives: This study aimed to identify the relationships of oral and systemic health-related characteristics with health-related quality of life (EQ-5D) in the elderly, and factors related to quality of life were compared according to age subgroups classified as either younger (young-old) or older (old-old) than 75 years of age. Methods: Data acquired by the Sixth Korean National Health and Nutrition Examination Survey(KNHANES) from 2013 to 2015 were used, and the research target was 3,124 people aged 65 years or older. A complex samples general linear model was used to identify health-related quality of life factors. Results: Education, economic activity, depression, stress, regular walking, self-rated oral health, pronunciation problems, and unmet dental care had significant effects on quality of life in both young-old and old-old participants. Marriage, income, number of systemic diseases, sleeping, and chewing inconveniences were significant factors for the young-old but not in the old-old participants. Instead, obesity and drinking were identified as significant factors in the old-old participants. Conclusions: We reaffirmed that factors affecting health-related quality of life in older adults differed by age group. We also confirmed the impact of oral health-related characteristics on this quality. Therefore, to improve quality of life for older adults, it would be efficient to divide groups by age and develop and implement programs that take relevant factors into consideration.

Nutrients Intake and Health Indices by Occupation in Middle-Aged Men - Using the Korean National Health and Nutrition Examination Survey (KNHANES) 2013-2016 - (중년남성의 직업에 따른 영양섭취실태와 건강지표의 비교 - 2013년~2016년 국민건강영양조사 자료 이용 -)

  • The objective of this study was to compare nutrients intake, health indices, and prevalence of chronic diseases by occupation in middle-aged men, using results from National Health and Nutrition Examination Survey. The subjects were divided into two groups by their occupation (Office worker (OW) and Agri-fishery worker (AFW)) and their percentages were 75.5% and 24.5% respectively. In health-related factors, drinking rate was higher in OW (p<0.001), while smoking rate was higher in AFW (p<0.001). Walking and strength exercise were both higher in OW group. In dietary behavior, the rate of skipping breakfast and eating out was higher in OW (p<0.001). Food supplement usage were consumed by OW more than AFW (p<0.001). Daily energy intake was higher in AFW (p<0.05). There were significant differences in nutrient density per 1,000 kcal of minerals and vitamins between the two groups (p<0.05~p<0.001), except calcium, potassium, vitamin A and thiamin. In body weight, BMI, waist circumference and blood pressures, OW was higher than AFW (p<0.05~p<0.001). Whereas, HDL and LDL-cholesterol were higher in AFW (p<0.001). The prevalence of diabetes was higher in AFW (p<0.001). The results of this study can be used as data to establish nutrition and health strategies for occupation in middle-aged men.

Fall prevention strategies in community-dwelling older adults aged 65 or over with type 2 diabetes mellitus: a systematic review and meta-analysis

  • Objective: Independent walking is the most essential prerequisite to maintain quality of life in older persons. The purpose of this review was to investigate the effect of fall prevention strategies on fall risk for type 2 diabetes mellitus (T2DM) within community-dwelling older adults aged 65 and over. Design: A systematic review and meta-analysis. Methods: PubMed and three other databases were searched up to October 31st, 2018 and randomized controlled trials (RCTs) evaluating fall prevention strategies for fall risk in persons who were 65 years of age or above with T2DM were included. The review extracted the following information from each study selected: first author's surname, published year, country, study population, type of intervention, intensity of intervention, comparison, measurement variables, additional therapy, summary of results, and mean and standard deviation from selected studies. Results: This review selected fourteen RCTs with 460 older adults with diabetes mellitus. Of the 14 studies, the types of intervention used to improve the risk of falls were strengthening (5), aerobic exercises (2), multimodal exercises (4), one virtual reality exercise (1), whole body vibration with balance exercise (1), and Tai Chi exercise (1). Seven RCTs were eligible for the meta-analysis. Therapeutic interventions were more effective than the control group for the Timed Up-and-Go test (-1.11; 95% CI, -1.82 to -0.41) and the 6-minute Walk Test (-1.89; 95% CI, -8.33 to 4.54). Conclusions: The results of the review suggest that interventions to prevent fall risk in older adults with T2DM should focus on strengthening, balance, aerobic, and multimodal exercises.

A Systematic Review of the Application Dual Task Assessment for Screening Mild Cognitive Impairment (경도 인지장애 선별에 적용된 이중과제 평가에 대한 체계적 고찰)

  • Purpose: This study aims to systematically review the dual-task evaluation applied to the screening of mild cognitive impairment. It also aims to present various evaluation items and results analysis methods for dual tasks applied to patients with mild cognitive impairment. Methods: We conducted a systematic search of published studies in PubMed databases and KISS from January 2000 to August 2020 using the main keywords such as "Dual task," "Mild Cognitive impairment," "Elderly," and "Screening." We selected a total of 10 studies for the analysis from 1314 searched articles. Results: We analyzed the qualitative level of 10 studies that were nonrandomized two-group studies with evidence level II (100.0%). These results suggest that the evidence level of the studies was high. We analyzed 10 studies and identified 12 motor tasks and 19 cognitive tasks. Walking was the most commonly used evaluation motor task and counting backward by ones and naming animals were the most commonly used evaluation cognitive tasks. Moreover, the velocity speed was the most used result analysis method. The results indicate that there were significant differences in dual-task performance between patients with normal and mild cognitive impairment. Conclusion: The results of this study can be used as a basis for the selection of dual-task evaluation items and methods of analyzing the results for screening mild cognitive impairment. Furthermore, they are expected to be used for research on the development of dual-task evaluation tools. It is necessary to compare and analyze the usage trends of dual-task evaluation by cultural differences in future studies.

Identification of Subgroups with Poor Glycemic Control among Patients with Type 2 Diabetes Mellitus: Based on the Korean National Health and Nutrition Examination Survey from KNHANES VII (2016 to 2018) (제 2형 성인 당뇨병 유병자의 혈당조절 취약군 예측: 제7기(2016-2018년도) 국민건강영양조사 자료 활용)

  • Purpose: This study was performed to assess the level of blood glucose and to identify poor glycemic control groups among patients with type 2 diabetes mellitus (DM). Methods: Data of 1,022 Korean type 2 DM patients aged 30-64 years were extracted from the Korea National Health and Nutrition Examination Survey VII. Complex samples analysis and a decision-tree analysis were performed using the SPSS WIN 26.0 program. Results: The mean level of hemoglobin A1c (HbA1c) was 7.22±0.25%, and 69.0% of the participants showed abnormal glycemic control (HbA1c≥6.5%). The characteristics of participants associated with poor glycemic control groups were presented with six different pathways by the decision-tree analysis. Poor glycemic control groups were classified according to the patients' characteristics such as period after DM diagnosis, awareness of DM, sleep duration, gender, alcohol drinking, occupation, income status, low density lipoprotein-cholesterol, abdominal obesity, and number of walking days per week. Period of DM diagnosis with a cut-off point of 6 years was the most significant predictor of the poor glycemic control group. Conclusion: The findings showed the predictable characteristics of the poor glycemic control groups, and they can be used to screen the poor glycemic control groups among adults with type 2 DM.

Combined Effects of Gastrocnemius Stretch and Tibialis Anterior Resistance Exercise in Subjects with Limited Ankle Dorsiflexion

  • Objective: Limited ankle dorsiflexion is related to ankle injuries. There are various exercises to increase the flexibility of the gastrocnemius for improving the passive range of motion in ankle dorsiflexion. However, to performances in daily activities and athletic sports and higher efficiency of walking and running, both ankle dorsiflexion passive and active range of motion are needed. To investigate the effects of combined gastrocnemius stretching and tibialis anterior resistance exercise on ankle kinematics (passive and active range of motion of ankle dorsiflexion) and tibialis anterior muscle activity in subjects with limited ankle dorsiflexion. Design: Cross-sectional single-group repeated measures design. Methods: Fourteen subjects with limited ankle dorsiflexion were recruited (in the right ankle in 7 and the left ankle in 7). All subjects performed gastrocnemius stretching alone and tibialis anterior resistance exercise after gastrocnemius stretching. The passive and active range of motion of ankle dorsiflexion were measured after interventions immediately. The tibialis anterior activity was measured during active range of motion of ankle dorsiflexion measurement. Results: There was no significant difference of ankle dorsiflexion passive range of motion between gastrocnemius stretching alone and the tibialis anterior resistance exercise after gastrocnemius stretching. The tibialis anterior resistance exercise after gastrocnemius stretching significantly increased active range of motion of ankle dorsiflexion compared to gastrocnemius stretching alone (p<0.05). The tibialis anterior resistance exercise after gastrocnemius stretching significantly increased tibialis anterior activity better than did gastrocnemius stretching alone. Conclusions: Thus, subjects with limited ankle dorsiflexion should be encouraged to perform tibialis anterior resistance exercises.
