8주간의 체중을 이용한 저항운동 시 전신진동 유·무에 따른 노인 여성하지의 발바닥쪽굽힘근의 생체역학적 특성 변화 (Changes in the Biomechanical Properties of Ankle Plantarflexors Following 8-week Resistance Training with or without Whole-Body Vibration in Older Women)

  • The aim of this study was to investigate the effect of resistance training with and without whole-body vibration(WBV) on the biomechanical properties of the plantarflexor in the elderly women (>60 yrs., n=35). Thirty-five volunteers were randomly assigned to a resistance training with WBV group (RVT, n=14), a resistance training without WBV (RT, n=11), and a non-training control group (CON, n=10). The RVT and the RT groups participated in the training sessions three times a week for 8 weeks, followed by a 4-week detraining period. The CON group was instructed to refrain from any type of resistance training. To assess strength and activation of the plantarflexor muscles, maximum isometric ankle plantarflexion torque and muscle activation of the triceps surae muscles were measured using dynamometry, twitch interpolation technique and electromyography at four different ankle joint angles. Also, the lower extremity function was assessed by vertical jumping. The measurements were performed prior to, 2 and 8 weeks after the training and after a 4-week detraining period. Following the 8-week training sessions, an increase in the isometric plantarflexion strength was found to be greater for the RVT compared with the RT group (p<.05). Muscle inhibition was significantly decreased after training than before training only for the RVT (p<.05). Following the detraining period, a decrease in isometric plantarflexors strength and a increases in muscle inhibition were significantly less in the RVT compared with the RT group. In conclusion, the exercise with WBV is a feasible training modality for the elderly and seems to have a boosting effect when used with conventional resistance training.

The Effects of Virtual Reality Based Treadmill Training on the Muscle Architecture of Gastrocnemius in Chronic Stroke Patients : Randomized Controlled Trial

  • Purpose: The purpose of this study was to investigate the effects of virtual reality based treadmill training on muscle architecture of gastrocnemius in chronic stroke patients. Methods: Thirty chronic stroke patients were randomly assigned to either the virtual reality based treadmill training (VRTT) group (n=15) or treadmill training (TT) group (n=15). Both groups participated in a standard rehabilitation program; in addition, the VRTT group participated in virtual reality based treadmill training for 30 minutes per day, three times per week, for 6 weeks, and TT group participated in treadmill walking training for 30 minutes per day, three times per week, for 6 weeks. Ultrasound image was used for measurement of pennation angle and muscle thickness of the medial gastrocnemius muscle at rest and during maximum voluntary contraction. Results: In the paretic side medial gastrocnemius muscle, greater improvement on the pennation angle and muscle thickness while resting and maximal voluntary contraction were observed in the VRTT group compared with the TT group. Conclusion: Findings of this study demonstrated that the virtual reality based treadmill training has an effect on muscle architecture of medial gastrocnemius in chronic stroke patients.

기능적 발목 불안정성에 대한 안정지지면과 불안정지지면에서의 균형훈련의 효과 비교 (Comparison of the Effects of Balance Training on the Stable and Unstable Supporting Surfaces for the Functional Ankle Instability)

  • Purpose: The purpose of this study was to compare the effects of the balance training on the stable and unstable supporting surfaces for the subjects with functional ankle instability. Methods: Twenty-nine subjects with functional ankle instability were randomly assigned to the stable group (n=14) and the unstable group (n=15). Balance training was conducted twenty minutes a day on the stable surface for the stable group and on the airostep for the unstable group three times a week for four weeks. Balance training program was consisted of ten steps by eye opened or closed and two or one leg standing. X-speed for transverse balance and Y-speed for longitudinal balance were measured with eye opened and closed in affected leg standing position. Results: There were significant improvements of balance ability in eye opened (p<.01) and eye closed standing (p<.01) in stable group. Unstable group also showed significant improvements of balance ability in eye opened (p<.01) and eye closed standing (p<.01). There were no statistical differences in the magnitude of improvement between the groups (p<.05). Conclusions: Based on such results, it can be said that balance training on the stable surface is effective as much as training on the unstable surface for the subjects with functional ankle instability.

호흡근 훈련이 뇌졸중환자의 폐기능과 수면의 질에 미치는 영향 (Impact of Respiratory Muscle Exercises on Pulmonary Function and Quality of Sleep among Stroke Patients)

  • PURPOSE: This study attempts to examine the impact of respiratory muscle exercises on the respiratory function and quality of sleep among stroke patients. METHODS: A total of 20 stroke patients were randomly divided into a breathing-exercise training group(n=10) and a breathing-device-training group(n=10). Changes in pulmonary function, as well as the quality, were measured before and after the intervention. The breathing exercise was performed three times a week for a total of eight weeks. Breathing-device exercises made use of a lung-capacity-strengthening device and were performed for three times a week for eight weeks. One Flow FVC was used as a measurement tool for the pulmonary function test. The Pittsburgh Sleep Quality Index(PSQI) and a sleep measurement tool were used for sleep evaluation. RESULTS: In a comparison of changes in pulmonary function before and after the training, the breathing-exercise group and the breathing-device-training group showed a significant difference. In changes of sleeping measure and PSQI, the breathing-exercise group and the breathing-device-training group also showed significant differences. CONCLUSION: Diverse index analyses confirmed that breathing exercises and exercises using a lung-capacity-strengthening device, were effective in improving respiratory function and quality of sleep among stroke patients experiencing respiratory function disorders and sleep disorders.

성인 편마비환자의 외적 되먹임 체중부하훈련이 낙상예방에 미치는 효과 (The Effect of External-feedback Weight-bearing Training Protecting a Fall in Hemiplegic Patients)

  • Purpose : This article was a quasi-experimental study that applies to external -feedback weight-bearing training among hemiplegic patients with a stoke. Methods : It examined whether position sense and balance can improve fall efficacy and prevent a fall. The subject of study was divided in two; Experimental group had 18 people and control group had 16 people. It took 8 weeks to finish training. Experimental group provides external-feedback weight-bearing training for ten minutes after neurological treatment for 20 minutes and control group provides nerves treatment for 30 minutes. To find the effect of a fall assessed position sense, balance and fall efficacy of the lower limbs before the fact, after 4 weeks and 8 weeks. Results : According to the conclusions, external-feedback weight-bearing training to prevent a fall was more effective intervention in experimental group than in control group. Conclusion : This study suggests that external-feedback weight-bearing training is a intervention to prevent the fall of hemiplegic patients with a stroke effectively.

회전보행 훈련이 초기 뇌졸중 환자의 균형 자신감, 낙상 효능에 미치는 영향: 무작위 대조 예비 연구 (Effect of Rotation Curved Walking Training on Balance Confidence and Falls Efficacy in Early Stroke Patients: A Randomized Controlled Pilot Study)

  • Purpose: This study aimed to determine the effect of curved walking training on balance confidence and fall efficacy in early stroke patients. Methods: The study included 16 early stroke patients who were randomly allocated to a curved walking training group (experimental group, N=8) and a straight walking training group (control group, N=8). Both groups performed the exercise 5 times a week for 3 weeks. Outcomes were assessed using the Activities-specific Balance Confidence (ABC) Scale, Fall Efficacy Scale (FES), Berg Balance Scale (BBS), and Timed Up and Go (TUG) test. Results: After 3 weeks of training, both groups showed significantly improved ABC, FES, BBS, and TUG (p<.05 in both groups). However, the ABC, FES, BBS, and TUG scores in the experimental group were significantly better than those in the control group (p<.05). Conclusion: These findings indicate that curved walking training may be effective at improving balance confidence and decreasing fall risk in early stroke patients. Therefore, curved walking training can be used as a recommended walking method in early stroke patients.

Effects of Repetitive Sit to Stand Training on the Knee Extensor Strength and Walking Ability in Subject with Total Knee Replacement Patients

  • Purpose: The purpose of this study was to verify the effectiveness of repetitive sit to stand training to improve knee extensor strength and walking ability of total knee replacement patients. Methods: In this study, 12 patients with total knee replacement patients were recruited from a rehabilitation hospital. They were divided into two groups: a repetitive sit to stand group (n=6) and a control group (n=6). They all received 30 minutes of continuous passive motion (CPM). After that, the repetitive sit to stand group performed repetitive sit to stand training, and the control group performed resistance exercise for 15 minutes five times a week for 2 weeks. After 2 weeks of training, knee extensor strength and spatiotemporal gait parameters were measured. Knee extensor strength was measured by Biodex system 3, walking ability was measured by Biodex gait trainer 2. Paired t test was performed to verify the difference between before and after intervention within the group, and analysis of covariance was used to verify the differences between the two groups. Results: After the training periods, the repetitive sit to stand group showed a significant improvement in knee extensor muscle strength, walking speed, step length of the operated side, and step length of the non-operated side (p<0.05). Conclusion: The results of this study showed that repetitive sit to stand training was more effective in improving knee extensor muscle strength and walking ability. Therefore, to strengthen knee extensor muscles and improve the walking ability of total knee replacement patients, it is necessary to consider repetitive sit to stand training.

가벼운 터치 자극을 이용한 균형훈련이 뇌졸중 환자의 자세동요와 보행속도에 미치는 영향 (The effect of balance training combined with light touch on postural sway and walking speed in patients with stroke)

  • Background: The purpose of the present study was to determine whether balance training combined with light touch improves the postural sway and gait speed in stroke patients. Design: Randomized Controlled Trial Study Methods: The current study included 40 stroke patients, who were randomly divided into two groups: the balance training with light touch (LT) group (n=20) and the balance training with heavy touch (HT) group (n=20). Both groups performed balance training on a stable and unstable ground. Additionally, the LT group provided light touch stimulation, and the HT group provided heavy touch stimulation. All the participants underwent 20 sessions of STS training (thirty minutes, five days per week for four weeks). In this study, postural sway was measured using a force platform, and walking speed was measured using a 10 meter walk test. Results: The improvements in postural sway and gait speed were observed to be significantly greater in the LT group, compared to the HT group (p<0.05). Conclusion: The results of the current study imply that balance training combined with light touch is a beneficial and effective therapeutic modality that can be employed to improve the postural sway and gait speed in stroke patients.

편마비 환자의 트레드밀과 체중지지의 트레드밀 훈련이 균형능력 및 보행능력에 미치는 영향 (The Effect of Treadmill and Body Weight Support Treadmill Training on Balance and Gait Ability in Hemiplegia Patients)

  • Purpose: The purpose of this study is to investigate and to verification of changes that the effect of treadmill and body weight support treadmill training on balance and gait ability for sub-acute stroke patients during 4 week. Method: 16 subjects who was diagnosed stroke were divided into 2 groups(8-treadmill training group, 8-body weight support treadmill group) by randomized control trial. Both training programs were consisted with 40 minuted, 5 times a week for 4 weeks and after training programs. Analysis: We analysed effects and changes on balance and gait ability. Analyses were performed using PASW ver. 18.0 and results were reported as mean ${\pm}$ standard deviation (S.D.). To investigate within group comparisons and to verification on effects of exercise, we did paired t test and repeated measured ANOVA test. Significance was set at p<.05. Result: Both training programs showed positive changes in Limit of Stability but significant results which is area of hemiside, area of intact side, area of posterior, total area were reported in body weight support treadmill training group. Changes of Berg Balance Scale was significantly increase and it had significant correlation between groups. Changes of 6 Minutes Walking Test was significant increase in both groups but there was no significant changes on Foot print and Romberg test. Conclusion: After considering all factors, both training programs showed effect on improvement of balance and gait ability in sub-acute stroke patients, but body weight support treadmill training group had better improvement in dynamic balance than treadmill training group. For the increase of balance and gait ability in sub-acute stroke patients, we need to continues study on difference of treadmill and body weight support treadmill and then we will give stroke patients a better satisfaction if we develop and provide a rehabilitation program for improvement of balance and gait ability.

무작위 속도 변화에 의한 트레드밀 보행훈련이 뇌졸중 환자의 폐기능에 미치는 영향 (Effects of Treadmill Walking Training with Randomized Walking Speed on Pulmonary Function in Persons with Chronic Stroke)

  • PURPOSE: The purpose of this study was to investigate which treadmill walking training mode is more effective strategy for improving pulmonary function of persons with chronic stroke. METHODS: Twenty-one chronic stroke patients were allocated and randomly to an experimental group (treadmill training with randomized speed control, n=11) and a control group (treadmill training without speed change, n=10). All participants received 30 minutes of neurodevelopmental treatment. In addition, the two groups performed treadmill training for 20 minutes each time with or without speed change. Speed change was applied 40%, 50%, 60%, 70% of Heart Rate Reserve. All the exercise programs lasted six weeks, with five times per week. Pulmonary function was assessed before and after exercise program by using forced vital capacity (FVC), forced expiratory volume in one second (FEV1), and maximal voluntary ventilation (MVV). RESULTS: In the both groups, FEV1 was significantly increased after training (p<.05). Compared to the control group, the experimental group showed 11.9% larger amount of change (p<.05). In the experimental group, FVC and MVV were significantly increased after training (9.9%, 7.6%, respectively) (p<.05). But in the control group, there was no significant difference in the FVC and MVV after training. CONCLUSION: Treadmill training with randomized speed control will be a better positive rehabilitation program than treadmill training without speed change to improve pulmonary function in persons with chronic stroke.
