Balance Training Program for Community-Dwelling Elders with Risk of Falls: A Multi-center Randomized Controlled Trial

  • Objective: This study aimed to assess the effectiveness of a balance training program in improving balance and functional independence to reduce fall risks among community-dwelling elders. Design: A multi-center randomized controlled trial Methods: A total of 66participants were randomly assigned to a balance training group or a control group. The balance training program, conducted three times a week for 32 weeks, included warm-up exercises, main balance training exercises, and cooldown stretch exercises. Outcome measures included the Berg Balance Scale (BBS), Timed Up and Go Test (TUGT), and Modified Barthel Index (MBI). Results: The balance training group demonstrated significant improvements in all outcome measures, indicating enhanced balance, improved functional mobility, and increased independence in activities of daily living. In contrast, the control group showed only slight improvements in BBS, TUGT and MBI scores. Conclusions: These findings provide evidence supporting the effectiveness of balance training programs in reducing fall risk and promoting health and wellbeing among community-dwelling elders. Future research should aim to refine the design of these programs and assess the sustainability of the observed improvements.

  • Purpose : The purpose of present study was to determine effects of a visual feedback training on balance ability in poststroke hemiparetic subjects. Methods : Fourteen chronic stroke patients participated. Participants were randomly divided into either visual feedback training group(7 experimental group, 7 control group). All of participated were inpatients at local rehabilitation center and had been receiving a traditional rehabilitation program, five days a week. Exprimental group have additionally undergone for four weeks, three days a week, the visual feedback training but control group was not received any additional program except the traditional rehabilitation program. The Berg Balance Scale(BBS), the FICSIT-4, the MTD-balance system to measure a balance ability were carried out twice before and after training. Results : After participation in the program, subjects of visual feedback training demonstrated a significant improvement in the scores of the BBS, the FICSIT-4, the MTD-Balance system. The control group had no change on the any tests. After the training, the result to improve significantly in visual feedback training group compared to post-test of control group were the score of BBS and the FICSIT-4 and the MTD-Balance system. Conclusions : The present study suggests that the visual feedback training program may become a useful strategy for enhancing balance ability in the rehabilitation of stroke patiens.

전신진동이 결합된 흡기근 훈련이 뇌졸중 환자의 폐 기능에 미치는 즉각적인 효과 (The Immediate Effect of Inspiratory Muscle Training with Whole Body Vibration on Pulmonary Function of Stroke Patients)

  • PURPOSE: This study investigated the immediate effect of inspiratory muscle training with whole-body vibration on the pulmonary function of subacute stroke patients. METHODS: All participants (n=30) were allocated to the following groups: (1) the inspiratory muscle training group with whole-body vibration (n=10), wherein the patients received inspiratory muscle training with whole-body vibration comprising 3minutes of vibration per session and respiratory training of 30 times and 2 sessions for one day. (2) the inspiratory muscle training group with visual feedback (n=10), wherein the patients received inspiratory muscle training with visual feedback. (3) the inspiratory muscle training group (n=10), wherein the patients received inspiratory muscle training. RESULTS: After the experiment, the inspiratory muscle training group with whole-body vibration exhibited significantly higher forced vital capacity, forced expiratory volume at 1 second, peak inspiratory flow rate, maximal inspiratory pressure, and chest expansion (p<.05), compared to the other groups. Inspiratory muscle training group with whole-body vibration had significantly higher peak expiratory flow rate and maximal voluntary ventilation than the other groups (p<.05). CONCLUSION: These results show that pulmonary function, maximal inspiratory pressure, and chest expansion were significantly better in the inspiratory muscle training group with whole-body vibration than in the other groups. Thus, this treatment will help recovery of pulmonary function in stroke patients.

Group Locomotor Imagery Training-Combined Knowledge of Performance in Community-Dwelling Individuals With Chronic Stroke: A Pilot Study

  • We evaluated whether group locomotor imagery training-combined knowledge of performance (KP) lead to improvements in gait function in community dwelling individuals with chronic stroke. Ten adults who had suffered a hemiparetic stroke at least 6 months earlier participated in group locomotor imagery training-combined KP for 5 weeks, twice per week, with 2 h intensive training. Dynamic gait index scores increased significantly after the group locomotor imagery training-combined KP. However, times for the timed up-and-go test did not improve significantly after the training. Group locomotor imagery training-combined KP may be a useful option for the relearning of gait performance for community dwelling individuals with chronic hemiparetic stroke.

Effect of Weight Shift Training with Electrical Sensory Stimulation Feedback on Standing Balance in Stroke patients

  • PURPOSE: The purpose of this study was to investigate the effect of weight shift training with electrical sensory simulation feedback on quiet standing balance in hemiplegic stroke patients. METHODS: 30 stroke patients were equally allocated at random to an experimental group or a control group. Patients in both two groups underwent comprehensive rehabilitation physical therapy for 30 minutes per day for 5 days per week for 4 weeks. Members of the experimental group received additional weight shift training with electrical sensory simulation feedback was conducted for 15 minutes after 30 minute sessions, whereas members of the control group underwent additional leftward/rightward weight shift training by themselves after 30 minutes per day for four weeks. COP (center of pressure) path lengths, COP velocities, and foot forces were measured before and immediately after the 4-week training period in both groups and results were compared. RESULTS: COP path lengths significantly decreased by 3% after training in the experimental group and this was significantly greater than that observed in the control group (p<0.05). In both groups, foot forces of affected sides showed significant increases after intervention, whereas foot forces of unaffected sides showed significant decreases (p<0.05). No significant difference was observed between the two groups with respect to these changes. CONCLUSION: Weight shift training using electrical sensory simulation feedback has a positive effect on quiet standing balance in hemiplegic stroke.

휠체어 이동 및 조작 훈련을 통한 모의 대피훈련 향상 활동의 효과 (Effect of Simulation Evacuation Training Improvement Activity through Wheelchair Skill Training)

  • Purpose: The purpose of this study was to determine the effect of simulation evacuation training improvement activity through wheelchair skill training. Methods: The study included 40 patients with early stroke who were randomly allocated to a wheelchair skill training group (Experimental group, n=20) and a general exercise group (Control group, n=20). Both groups performed the exercise 3 times a week for 3 weeks. Outcomes were assessed using Wheelchair Skills Test Skills Performance (WSTSP), Wheelchair Skills Test Skills Safety (WSTSS) and Wheelchair Propulsion Velocity (WPV). Results: After 3 weeks of training, both groups showed significantly improved WSTSP, WSTSS and WPV (p<.01 in both groups). However, the WSTSP, WSTSS and WPV in the experimental group were very significantly better than in the control group (p<.01). Conclusion: These findings indicate that wheelchair skill training may be effective at improving wheelchair skill ability and wheelchair propulsion velocity in stroke patients who cannot walk independently. Therefore, short-term wheelchair skill training could be useful for patient safety in simulated evacuation situations.

The Effects of Coordinative Locomotor Training using Elastic Band on Dynamic Balance and Grip Strength in Elementary School Baseball Player

  • Objective: The purpose of this study was to investigate the effects of coordinative locomotor training(CLT) using elastic bands on dynamic balance and grip strength for Elementary school baseball players and to provide correct posture guidance and reference on the prevention and rehabilitation program of sports damage and injury in the future. Design: Two groups pre-post randomized controlled design. Methods: Forty-six subjects were randomly divided in two groups;1) CLT using Elastic Band group(Experimental group, n=23), 2) Routine baseball training group(Control group, n=23). The intervention was conducted total 16 times for sixty minutes a day, 2 times a week, for 8 weeks. Evaluations of dynamic balance ability and grip strength were performed with all subjects before the commencement of training and 8 weeks after training. Results: Compared to the control group after training, the dynamic balance ability and dominant handgrip strength of the experimental group were significantly more improved(p<0.05). Conclusions: We confirmed that the effects of CLT using elastic bands on dynamic balance ability and grip strength in Elementary school baseball player. This study should be used for improving the quality of the Elementary school baseball player's training and would be contributed prevention and rehabilitation program of sports damage and injury.

고등학교 투척선수들의 plyometric training과 weight training이 순발력, 민첩성 및 최대 근력에 미치는 영향 (The Differential Effects of Plyometric Training and Weight Training on Muscular Power, Agility and Maximal Muscular Strength of the Male and Female High-school Throwers)

  • 본 연구는 육상 투척선수들의 plometric training과 weight training 프로그램 훈련이 순발력 및 민첩성과 최대 근력의 운동능력 향상에 어떠한 영향을 가져오는지 검증하기 위하여 고등학교 재학 중인 투척선수 21명을 대상으로 plyometric training과 weight training을 10주간의 트레이닝을 실시하여 여자 집단(10명)과 남자 집단(11명)으로 분류하여 다음과 같은 결론을 얻었다. 순발력에 있어서 제자리멀리뛰기, 서전트 점프, 30 m 달리기는 트레이닝 전에 비해 두 집단 모두 향상도를 보였으며, 여자 집단이 더 높은 향상도를 나타냈다. 민첩성에 있어서 사이드스텝, 버피 스텝, 전신반응 테스트는 트레이닝 전에 비해 모두 향상도를 보였으며, 사이드스텝과 버피 스텝은 여자 집단이 더 높은 향상도를 나타냈으며, 전신반응 테스트는 남자 집단이 더 높은 향상도를 나타냈다. 하지 근력에 있어서 squat, leg press는 트레이닝 전에 비해 모두 향상도를 보였으며, 여자 집단이 더 높은 향상도를 나타냈다. 무릎 관절 등속성 근력에 있어서 왼쪽, 오른쪽 신근력과 굴근력은 트레이닝 전에 비해 모두 향상도를 보였으며, 왼쪽 신근력은 남자 집단이 더 높은 향상도를 나타냈으며, 왼쪽 굴근력, 오른쪽 신근력, 오른쪽 굴근력은 여자 집단이 더 높은 향상도를 나타냈다. 상지의 근력에 있어서 Bench press, 배근력은 트레이닝전에 비해 모두 향상도를 보였으며, 여자집단이 더 높은 향상도를 나타냈다.

Robotic-assisted gait training applied with guidance force for balance and gait performance in persons with subacute hemiparetic stroke

  • Objective: Robot assisted gait training is implemented as part of therapy for the recovery of gait patterns in recent clinical fields, and the scope of implications are continuously increasing. However clear therapy protocols of robot assisted gait training are insufficent. The purpose of this study was to investigate the effects of robot-assisted gait training applied with guidance force on balance and gait performance in persons with hemiparetic stroke. Design: Two group pre-test post-test design. Methods: Nineteen persons were diagnosed with hemiparesis following stroke participated in this study. The participants were randomly assigned to the unilateral guidance group or bilateral guidance group to conduct robot-assisted gait training. All participants underwent robot-assisted gait training for twelve sessions (30 min/d, 3 d/wk for 4 weeks). They were assessed with gait parameters (gait velocity, cadence, step length, stance phase, and swing phase) using Optogait. This study also measured the dynamic gait index (DGI), the Berg balance scale (BBS) score, and timed up and go (TUG). Results: After training, BBS scores were was significantly increased in the bilateral training group than in the unilateral guidance group (p<0.05). Spatiotemporal parameters were significantly changed in the bilateral training group (gait speed, swing phase ratio, and stance phase ratio) compared to the unilateral training group (p<0.05). Conclusions: The results of this study suggest that robot-assisted gait training show feasibility in facilitating improvements in balance and gait performance for subacute hemiparetic stroke patients.

운동심상이 만성 경수 손상 환자의 근활성도와 일상생활에 미치는 영향 (Effect of a Motor Imagery Program on Upper Extremity Strength and Activities of Daily Living of Chronic Cervical Spinal Cord Injury Patients)

  • Purpose: The purpose of this study is to determine the effect of motor imagery training on residual upper extremity strength and activities of daily living of chronic cervical spinal cord injury patients. Methods: Twelve ASIA A B patients, who had more than a 12-month duration of illness and C5 or 6 motor nerve injury level, were randomly divided into experimental group (n=6) and control group (n=6). Patients in the experimental group performed motor imagery training for five minutes prior to general muscle strengthening training, while those in the control group performed general muscle strengthening training only. The training was performed five times per week, 30 minutes per day, for a period of four weeks. General muscle strengthening training consisted of a progressive resistive exercise for residual upper extremity. Motor imagery training consisted of imagining this task performance. Before and after the training, EMG activity using BTS Pocket Electromyography and Spinal Cord Independent Measure III(SCIM III) were compared and analyzed. Results: The residual upper extremity muscle strengths showed improvement in both groups after training. Comparison of muscle strength improvement between the two groups showed a statistically significant improvement in the experimental group compared to the control group (p<0.05). SCIM III measurements showed significant improvement in the scores for Self-care and Transfer items in the experimental group. Conclusion: Motor imagery training was more effective than general muscle strengthening training in improving the residual upper extremity muscle strength and activities of daily living of patients with chronic cervical spinal cord injury.
