위 절제술 환자의 진료계획표 개발 및 전자 의무 기록화 (Development of a Flexible Critical Pathway with Electronic Medical Record for Gastrectomy Patients in a University Hospital)

  • Objectives : This study was conducted to evaluate the effect of fixed critical pathway with emr (electronic medical record) on the length of hospital stay, the cost and quality of care provided to gastrectomy patients in a university hospital and to develop flexible critical pathway with emr which can be used excluded or drop-out patients. Methods : Thirty-eight patients with gastrectomy were included as case group and Thirty-four patients included as control group. The comparison between control and case with using fixed critical pathway were done. To develop and to evaluate usefulness of flexible critical pathway with flexible data base, simulation was done for flexible critical pathway with drop-out patients. Result : The major results of this study were as follows: There were no significant differences in patient clinical conditions and no sign of deterioration of quality from critical pathway. The length of hospital stay was 11 days in control group, 8 days in path group(P<0.01). The total costs during the hospital stay were reduced in path group. However the cost per day was significantly increased from reduction of hospital stay(554,352 won in control, 645,669 won in path group). One hundred percentage of drop out patients(60) in the simulation of flexible critical pathway was successful. Conclusion : Computerized critical pathway reduced the length of hospital stay, total hospital costs and resource utilization without harming quality of patient care. The flexible critical pathway program can be used as one of the powerful management tools for reducing the practice variations and increasing the efficiency of care process and decreasing the workload of doctors and nurses in Korean hospital settings.

  • In the event of a severe accident in Sodium Cooled Fast Reactors (SFR), the sodium combustion aerosols along with fission product aerosols would migrate to the environment through leak paths of the Reactor Containment Building (RCB) concrete wall under positive pressure. Understanding the characteristics of sodium aerosol transport through concrete leak paths is important as it governs the environmental source term. In this context, experiments are conducted to study the influence of various parameters like pressure, initial mass concentration, leak path diameter, humidity etc., on the transport and deposition of sodium aerosols in straight leak paths of concrete. The leak paths in concrete specimens are prepared by casting and the diameter of the leak path is measured using thermography technique. Aerosol transport experiments are conducted to measure the transported and plugged aerosol mass in the leak paths and corresponding plugging times. The values of differential pressure, aerosol concentration and relative humidity taken for the study are in the ranges 10-15 kPa, 0.65-3.04 g/m3 and 30-90% respectively. These observations are numerically simulated using 1-Dimensional transport equation. The simulated values are compared with the experimental results and reasonable agreement among them is observed. From the safety assessment view of reactor, the approach presented here is conservative as it is with straight leak paths.

Free-fatty-acid-regulating effects of fermented red ginseng are mediated by hormones and by the autonomic nervous system

  • Background: Understanding what causes changes in the flux of free fatty acids (FFA) is important to elucidate the etiology of metabolic syndrome. The first aim of this study was to test whether or not hormones and the autonomic nervous system influence blood FFA levels. A secondary aim was to test by means of a multiple group path analysis whether the consumption of fermented red ginseng (FRG; Panax ginseng) would influence those causal relationships. Methods: Ninety-three postmenopausal women (age 50e73 yr) were randomly divided into two groups. One group (44 women; age, $58.4{\pm}5.9yr$; body mass index, $3.6{\pm}2.5kg/m^2$) was supplied place capsules and the other group (49 women, age $58.4{\pm}5.5yr$; body mass index, $22.9{\pm}2.4kg/m^2$) was supplied FRG capsules. Both prior to and after the study (2 wk), blood samples were collected from the participants and several blood variables were measured and analyzed. Results: Squared multiple correlations of FFA were 0.699 in the placebo group and 0.707 in the FRG group. The unstandardized estimate of estradiol (E2) for FFA was 0.824 in both groups. Conclusion: The path coefficients of cortisol and the branchial pulse for FFA were significantly different between the FRG group and the placebo group.

IEEE802.11e를 기반으로 한 Wireless Mesh Networks에서 QoS 향상 방법 설계 (QoS Support to Design for IEEE802.11e based in Wireless Mesh Networks)

  • 무선 LAN(Local Area Network) 환경에서 QoS를 지원하기 위해 IEEE 802.11 Working Group은 IEEE 802.11e를 표준화하였다. IEEE 802.11e EDCA는 우선순위가 높은 트래픽의 채널 접근 기회를 확률적으로 높임으로써 QoS를 지원한다. 이는 우선순위가 높은 트래픽이 우선순위가 낮은 트래픽에 비해 채널 접근을 반드시 먼저 하는 것이 아니다. 따라서 모든 트래픽이 같은 채널, 같은 routing path를 가진다면, 우선순위가 낮은 트래픽은 우선순위가 높은 트래픽의 성능에 지속적인 영향을 미친다. 결과적으로 EDCA는 음성이나 비디오와 같은 멀티미디어 응용에 QoS를 보장하기에 무리가 따른다. 본 논문에서는 이러한 문제점을 해결하기 위해 Multi-Interface를 이용한 Multi-Path Routing Algorithm을 제안한다. 각 Interface 마다 IEEE 802.11e EDCA에서 사용하는 Access Category별로 각각의 Routing Path를 다르게 함으로써 높은 우선순위의 트래픽에 QoS를 보장한다.

On Finding a Convenient Path in the Hierarchical Road Network

  • In a hierarchical road network, all roads can be classified according to their attributes such as speed limit, number of lanes, etc. By splitting the whole road network into the subnetworks of the highlevel and low-level roads, we can reduce the size of the network to be calculated at once, and find a path in the way that drivers usually adopt when searching out a travel route. To exploit the hierarchical property of road networks, we define a convenient path and propose an algorithm for finding convenient paths. We introduce a parameter indicating the driver's tolerance to the difference between the length of a convenient path and that of a shortest convenient path. From this parameter, we can determine how far we have to search for the entering and exiting gateway. We also propose some techniques for reducing the number of pairs of entries and exits to be searched in a road network. A result of the computational experiment on a real road network is given to show the efficiency of the proposed algorithm.

Effect of Weight Shift Training with Electrical Sensory Stimulation Feedback on Standing Balance in Stroke patients

  • PURPOSE: The purpose of this study was to investigate the effect of weight shift training with electrical sensory simulation feedback on quiet standing balance in hemiplegic stroke patients. METHODS: 30 stroke patients were equally allocated at random to an experimental group or a control group. Patients in both two groups underwent comprehensive rehabilitation physical therapy for 30 minutes per day for 5 days per week for 4 weeks. Members of the experimental group received additional weight shift training with electrical sensory simulation feedback was conducted for 15 minutes after 30 minute sessions, whereas members of the control group underwent additional leftward/rightward weight shift training by themselves after 30 minutes per day for four weeks. COP (center of pressure) path lengths, COP velocities, and foot forces were measured before and immediately after the 4-week training period in both groups and results were compared. RESULTS: COP path lengths significantly decreased by 3% after training in the experimental group and this was significantly greater than that observed in the control group (p<0.05). In both groups, foot forces of affected sides showed significant increases after intervention, whereas foot forces of unaffected sides showed significant decreases (p<0.05). No significant difference was observed between the two groups with respect to these changes. CONCLUSION: Weight shift training using electrical sensory simulation feedback has a positive effect on quiet standing balance in hemiplegic stroke.

승마 운동이 여성 노인의 균형능력에 미치는 영향 (The Effect of Horse-Riding Exercise on the Balance Ability in the Frail Woman Elderly People)

  • Purpose : The purpose of this study was to investigate the effect of horse riding exercise for the prevention of falls in the frail woman elderly people. Method : 30 subjects in H-equestrian and N-equestrian were randomly divided two group, aero-step exercise group and horse-riding exercise group. Each group carried out 40 minutes exercise three times a week for 8 weeks. TUG were measured for dynamic balance ability, and sway path of COP movement during standing were measured for evaluation of static balance ability in balance performance monitor(BPM). Result : The results were as follows, the dynamic balance scales of TUG between aero-step exercise and horse-riding exercise groups in post-test, were significantly different in measures(p<.05). And there were significant in two group after exercise(p<.05). The static balance scales of sway path between aero-step exercise and horse-riding exercise groups in post-test, were significantly different in measures(p<.05). And there were significant in two group after exercise(p<.05). Conclusion : These finding revealed that horse-riding exercise was effective on dynamic and static balance abiility of elderly people so that these exercise can be new altematives for the prevention of falls in the frail woman elderly people.

고유수용성 신경근 촉진법의 하지패턴이 편마비환자의 균형능력에 미치는 영향 (Effect of Lower Extremity Patterns of Proprioceptive Neuromuscular Facilitation on Balance Ability in Patients with Hemiplegia)

  • Objective : The main purpose of this study was to investigate the influence on lower extremity pattern of PNF to balance ability of patients with chronic hemiplegia. Methods : The subjects were consisted of 52 patients with chronic hemiplegia. All subjects were randomly devide to two group which was ROM exercise group and PNF group. Each group had a treatment for 30 minutes per day and three times a week during 10 weeks period. BPM was used to measure sway area, path, velocity, and BBS FRT TUG to measure patient's balance ability, ABC to measure patient's self-confidence. Results : 1. Sway area was showed that PNF group had more significantly decreased than ROM exercise group(p<.05). 2. Sway path was showed that PNF group had more significantly decreased than ROM exercise group(p<.05). 3. Max velocity was showed that PNF group had more significantly decreased than ROM exercise group(p<.05). 4. BBS was showed that PNF group had more significantly increased than ROM exercise group (p<.05). 5. FRT was showed that PNF group had more significantly increased than ROM exercise group (p<.05). 6. TUG was showed that PNF group had more significantly decreased than ROM exercise group(p<.05). 7. ABC was showed that PNF group had more significantly increased than ROM exercise group (p<.05). Conclusion : it was showed that the lower extremity pattern of PNF would increase balance abilities of the chronic hemiplegia patients. So it would be possible to make good use of lower extremity pattern of PNF for improving balance abilities of chronic hemiplegia patients.

프로골프 경기기록을 활용한 다중집단분석 : 경로분석 적용 (Effectiveness of golf skills to average score using records of PGA, LPGA, KPGA, KLPGA : Multi-group path analysis)

  • 이 연구는 프로골프대회에서 기록되는 중요변인들 (드라이브거리, 페어웨이안착율, 그린적중율, 센드세이브율, 리커버리율, 평균퍼팅수)이 평균타수에 미치는 영향력에 대해 집단 간 차이를 분석하였다. 구체적으로 미국남자프로골프협회 (PGA), 미국여자프로골프협회 (LPGA), 한국남자프로골프투어 (KGT), 그리고 한국여자프로골프투어 (KLPGT)에서 측정, 공개하는 자료를 통해 PGA, LPGA, KPGA, KLPGA 집단 간에 중요변인들이 평균타수에 미치는 경로에 유의한 차이가 있는지를 검증하였다. 그 결과 PGA와 LPGA 모형 간에 드라이브거리와 그린적중율이 평균타수에 미치는 경로가 p <.05 수준에서 통계적으로 유의한 차이가 있는 것으로 나타났으며, KPGA와 KLPGA 모형 간에는 드라이브거리, 리커버리율, 그리고 평균퍼팅이 평균타수에 미치는 경로가 유의한 차이가 있는 것으로 나타났다. 또한 PGA와 KPGA 모형 간에 드라이브거리, 리커버리율, 그리고 평균퍼팅이 평균타수에 미치는 경로가p <.05 수준에서 통계적으로 유의한 차이가 있는 것으로 나타났으며, LPGA와 KLPGA 모형 간에는 모든 경로에서 p >.05 수준에서 통계적으로 유의한 차이가 없는 것으로 나타났다. 이 연구 결과는 남녀 국내 투어와 미국 투어의 골프 경기력의 구조 특성을 이해하는데 필요한 기초자료로 활용 할 수 있을 것이며 향후 투어 진출을 위한 유의한 정보가 될 수 있을 것이다.
