The 2019 novel coronavirus (2019-nCoV) outbreak in Wuhan, Hubei Province, China in 2019 led to large numbers of people being infected and developing atypical pneumonia (coronavirus disease 2019, COVID-19). Typical imaging manifestations of patients infected with 2019-nCoV has been reported, but we encountered an atypical radiological manifestation on baseline computed tomography (CT) images in three patients from Wuhan, China infected with the 2019-nCoV. Surprisingly, the only similar CT finding was a solitary sub-centimeter ground-glass nodule adjacent to bronchovascular bundles, which could be easily overlooked. In addition, the follow-up images in these patients showed how COVID-19 pneumonia evolved from these small nodules. The radiologic manifestation of the three cases will expand contemporary understanding of COVID-19.
Background: A recent increase in the incidental detection of ground glass nodules (GGNs) has created a need for improved diagnostic accuracy in screening for malignancies. However, surgical diagnosis remains challenging, especially via video-assisted thoracoscopic surgery (VATS). Herein, we present the efficacy of a novel electrical navigation system for perioperative percutaneous transthoracic nodule localization. Methods: Eighteen patients with GGNs who underwent electromagnetic navigated percutaneous transthoracic needle localization (ETTNL), followed by 1-stage diagnostic wedge resections via VATS between January and December 2020, were included in the analysis. Data on patient characteristics, nodules, procedures, and pathological diagnoses were collected and retrospectively reviewed. Results: Of the 18 nodules, 17 were successfully localized. Nine nodules were pure GGNs, and the remaining 9 were part-solid GGNs. The median nodule size was 9.0 mm (range, 4.0-20.0 mm); and the median depth from the visceral pleura was 5.2 mm (range, 0.0-14.4 mm). The median procedure time was 10 minutes (range, 7-20 minutes). The final pathologic results showed benign lesions in 3 cases and malignant lesions in 15 cases. Conclusion: Perioperative ETTNL appears to be an effective method for the localization of GGNs, providing guidance for a 1-stage VATS procedure.
Ground-glass opacity nodules(GGNs) in chest CT images are associated with lung cancer, and have a different malignant rate depending on existence of solid component in the nodules. In this paper, we propose a method to classify pure GGNs and part-solid GGNs using multiview images and texture analysis in pulmonary GGNs with solid components of 5mm or smaller. We extracted 1521 features from the GGNs segmented from the chest CT images and classified the GGNs using a SVM classification model with selected features that classify pure GGNs and part-solid GGNs through a feature selection method. Our method showed 85% accuracy using the SVM classifier with the top 10 features selected in the multiview images.
Thoracic surgeons need to be aware of several important points regarding intraoperative lymph node dissection during surgery for non-small cell lung cancer with ground-glass opacities. The first point relates to the need for lymph node dissection during sublobar resection. Since even patients undergoing sublobar resection may benefit from lymph node dissection, it should be selectively performed according to adequate indications, which require further study. Second, there seems to be no difference in postoperative morbidity between systematic sampling and systematic dissection, but the survival benefit from systematic dissection remains unclear. The results of randomized controlled trials on this topic are conflicting, and their evidence is jeopardized by a high risk of bias in terms of the study design. Therefore, further randomized controlled trials with a sound design should investigate this issue. Third, more favorable survival outcomes tend to be positively associated with the number of examined lymph nodes. Minimum requirements for the number of examined lymph nodes in non-small cell lung cancer should be defined in the future. Finally, lobe-specific lymph node dissection does not have a negative prognostic impact. It should not be routinely performed, but it can be recommended in selected patients with smaller, less invasive tumors. Results from an ongoing randomized controlled trial on this topic should be awaited.
As diagnoses of small ground glass nodule (GGN)-type lung adenocarcinoma are increasing due to the increasing frequency of computed tomography (CT) screening, surgical treatment for GGN-type lung adenocarcinoma has rapidly become more common. However, the appropriate surgical extent for these lesions remains unclear; therefore, several retrospective studies have been published and prospectively randomized controlled trials are being undertaken. This article takes a closer look at each clinical study. Convincing evidence must be published on 2 issues for sublobar resection to be accepted as a standard surgical option for GGN lung adenocarcinoma. In the absence of such evidence, it is better to perform lobar resection as long as the patient has sufficient lung function. The first issue is the definition of a sufficient resection margin, and the second is whether lymph node metastasis is conclusively ruled out before surgery. An additional issue is the need for an accurate calculation of the total size and solid size on CT. Given the results of clinical studies so far, wedge resection or segmentectomy shows a good prognosis for GGNs with a total size of 2 cm or less. Therefore, sublobar resection will play a key role even in patients who can tolerate lobectomy.
본 논문에서는 흉부 CT 영상에서 결절의 밝기값, 재질 및 형상 증강 영상 기반의 GGN-Net을 이용해 간유리음영 결절 자동 분류 방법을 제안한다. 첫째, 입력 영상에 결절 내부의 고형 성분의 유무 및 크기 정보가 포함될 수 있도록 밝기값, 재질 및 형상 증강 영상의 활용을 제안한다. 둘째, 다양한 입력 영상을 여러 개의 컨볼루션 모듈을 통해 획득한 특징맵을 내부 네트워크에서 통합하여 훈련하는 GGN-Net를 제안한다. 제안 방법의 분류정확성 평가를 위해 순수 간유리음영 결절 90개와 고형 성분의 크기가 5mm 미만인 혼합 간유리음영 결절 38개, 5mm 이상 고형 성분의 크기를 가지는 혼합 간유리음영 결절 23개의 데이터를 사용하였으며, 입력 영상이 간유리음영 결절 분류 결과에 미치는 영향을 비교하기 위해 다양한 입력 영상을 구성하여 결과를 비교하였다. 실험 결과, 밝기값, 재질 및 형상 정보가 함께 고려된 입력 영상을 사용한 제안 방법이 정확도가 82.75%로 가장 좋은 결과를 보였다.
Fine needle aspiration cytology(FNAC) is preferred because of simplicity, safety, and reliability in the evaluation of patients with thyroid nodule or hyperplasia. However, there are a few limitations such as false-negative or false-positive cases and non-diagnostic material. To evaluate the usefulness of FNAC in thyroid lesions, we reviewed 704 FNAC cases of thyroid nodules from 1988 to 1994 at Soonchunhyang University Hospital. The results are as follows. 1. Among 704 FNAC cases of thyroid gland, 571(81.1%) cases were benign, 12(1.7%) were suspicious, 71(10.1%) were malignancy, and 50(7.1%) were material insufficiency. The cytologic diagnoses of the benign lesions included 168 cases of follicular neoplasm, 139 cases of adenomatous goiter, 162 cases of follicular lesion such as follicular neoplasm or adenomatous goiter, 61 cases of Hashimoto's thyroiditis, 13 cases of subacute thyroiditis, and 28 cases of colloidal nodule or benign nodule. The malignant lesions included 68 cases of papillary carcinona, two medullary carcinomas and a case of metastatic colon cancer. 2. The average number of cytologic smear slides was $4.12{\pm}1.81$ in material insufficiency and $5.63{\pm}1.79$ in diagnostic cases. This difference was statistically significant(p<0.00001). 3. Histological assessment of 150 cases revealed 2 false negative and 1 false positive cases. The false negative cases were a case of marked sclerosis in papillary carcinoma and an occult case of papillary carcinoma. The false positive case resulted from pseudo-ground glass nuclei due to marked dry artifact. 4. Comparison between the FNAC and the histologic diagnosis revealed that FNAC had a sensitivity of 93.5%, a specificity of 99.2%, a false negative rate of 6.6%, a false positive rate of 0.8%, and an overall diagnostic accuracy of 98.0%. Therefore, FNAC of thyroid gland is a very reliable diagnostic method with excellent accuracy rate.
미국방사선의학회는 효과적인 국가 폐암 검진 시행을 위해 2019년도에 Lung CT Screening Reporting & Data System (이하 Lung-RADS) 1.0보다 폐암의 위양성을 줄이기 위해 개편된 Lung-RADS 1.1을 발표하였고, 2022년 12월에 새로운 Lung-RADS 1.1 개선안 Lung-RADSⓇ 2022를 발표하였다. Lung-RADSⓇ 2022은 Lung-RADS 1.0과 비교했을 때 결절의 크기는 소수점 첫째 자리까지 측정하고, 늑막근처 결절의 크기가 10 mm 미만인 경우까지 범주 2로 하며, 범주 2에서 간유리 결절의 크기 기준을 30 mm로 높이고, 범주 4B와 4X는 매우 의심으로 변경하며, 기도 결절의 위치와 비정형 폐 낭종의 형태와 벽 두께에 따라 범위를 나누었다. 이에 영상의학과 의사들의 개선된 Lung-RADSⓇ 2022에 대한 이해를 돕고자, 이 종설에서는 Lung-RADSⓇ 2022의 장점, 단점, 그리고 향후 개선점에 대해서 기술하고자 한다.
연구목적 : 기관지폐포암은 다양한 임상적, 방사선적인 양상으로 나타나며 대표적인 방사선적인 양상으로 또는 종괴, 국소적 경화, 미만성 결절 또는 경화 등으로 나타난다. 고립성 결절 또는 국소적 경화 병변으로 나타난 기관지폐포암은 다발성 병변과는 달리 예후가 좋으므로 조기에 진단하는 것이 매우 중요하다. 그러나 고립성 결절로 나타나는 기관지폐포암은 비특이적 임상증상과 다양한 방사선 양상으로 폐렴, 폐결핵, 양성병변으로 오인되어 치료의 시기를 놓치는 경우가 있다. 이에 저자들은 고립성 폐결절로 나타난 기관지폐포암의 임상적, 방사선적 특성을 알아보기 위하여 다음과 같은 연구를 시행하였다. 방 법 : 1995년 1월부터 1996 년 8월까지 서울삼성병원에서 병리학적으로 기관지폐포암이 진단되고 방사선 소견에서 고립성 폐결절로 나타난 환자를 대상으로 임상적, 방사선적인 특성을 조사였다. 결 과 : 환자는 총 11명으로 남자는 6명, 여자 5명이었고 환자들의 연령은 37세에서 69세로 중앙 연령값은 60세이었다. 대부분의 환자는 증상이 없이 단순흉부촬영에서 우연히 이상소견이 발견되어 진단되었다. 단순흉부촬영상 대부분 경계가 불명확한 결절 또는 음영증가의 소견을 보였고 단순흉부촬영후 추측진단 양성병변이나 결핵성 폐병변으로 오인되었던 예가 6예였고 5예에서만 악성 결절로 추정되었다. 전산화단층촬영상 경화, 간유리 모양, 기관지 공기 조영, open bronchus sign, internal bubble-like lucencies, spiculated margin 또는 pleural tag등의 소견이 대부분의 환자에서 관찰되었다. FDG-PET 검사를 시행한 8예중 3예에서 악성을 의심하는 소견이 보였고 5예에서 위음성의 소견을 보였다. 병리학적인 진단은 경기관지폐생검과 경피적 폐생검을, 통해 진단된 예가 각각 1예, 2예이었고 8예에서는 비디오흉강경을 이용한 폐생검을 통해 이루어졌다. 환자는 모두 폐엽절제술을 시행받았고 수술후 병기는 $T_1N_0M_0$가 8예, $T_2N_0M_0$가 3예로 근치적 절제술이 가능하였다. 결 론 : 고립성 결절 양상의 기관지폐포암은 증상 없이 우연히 발견되는 경우가 대부분이며 단순흉부촬영상 주로 경계가 불명확한 결절 또는 음영증가의 소견으로 나타나 양성병변으로 오인되는 경우가 많았다. 만약, 이런 병변이 추적검사에서 계속 남아 있거나 커지는 경우는 흉부전산화단층촬영을 시행하여 경화, 간유리 모양, 기관지 공기조영, internal bubble-like lucencies, spiculated margin 또는 pleural tag등 소견을 보인다면 개흉술 등을 포함한 적극적인 진단적 접근이 필요하다고 생각된다.
Background: A ground-glass nodule (GGN) represents early-stage lung adenocarcinoma. However, there is still no consensus for preoperative staging of GGNs. Therefore, we evaluated the need for the routine use of positron emission tomography/computed tomography (PET)/computed tomography (CT) scans and brain magnetic resonance imaging (MRI) during staging. Methods: A retrospective analysis was undertaken in 72 patients with 74 GGNs of less than 3 cm in diameter, which were confirmed via surgery as malignancy, at the Samsung Medical Center between May 2010 and December 2011. Results: The median age of the patients was 59 years. The median GGN diameter was 18 mm. Pure and part-solid GGNs were identified in 35 (47.3%) and 39 (52.7%) cases, respectively. No mediastinal or distant metastasis was observed in these patients. In preoperative staging, all of the 74 GGNs were categorized as stage IA via chest CT scans. Additional PET/CT scans and brain MRIs classified 71 GGNs as stage IA, one as stage IIIA, and two as stage IV. However, surgery and additional diagnostic work-ups for abnormal findings from PET/CT scans classified 70 GGNs as stage IA, three as stage IB, and one as stage IIA. The chest CT scans did not differ from the combined modality of PET/CT scans and brain MRIs for the determination of the overall stage (94.6% vs. 90.5%; kappa value, 0.712). Conclusion: PET/CT scans in combination with brain MRIs have no additional benefit for the staging of patients with GGN lung adenocarcinoma before surgery.
