The Effects of a Horseback Riding Simulation Exercise on the Spinal Alignment of Children with Cerebral Palsy

  • Purpose: The purpose of this study is to examine the effects of postural control training using a horseback riding simulation on the spinal alignment of children with cerebral palsy. Methods: This study was conducted with 30 children with cerebral palsy at levels I~IV in the Gross Motor Function Classification System (GMFCS), and they were randomly divided into a control group and a hippotherapy group. Both the control group and the experimental group received NDT for 30 minutes per session, four times per week for ten weeks, while the experimental group also received hippotherapy 15 minutes per session, four times per week for ten weeks, after the neurodevelopmental treatment (NDT). The horseback riding simulators (JOBA, EU7805, Panasonic) used in this study simulated actual horse movements. Trunk imbalance, pelvic torsion, and pelvic tilt were measured in each group before the exercise and five weeks and ten weeks after the beginning of the exercise using a spinal structure analysis system (ABW Mapper). Results: The Intra-group effects on trunk imbalance, pelvic torsion, and pelvic tilt according to the exercise periods after the hippotherapy were tested, and the results showed significant interaction effects between the groups and the periods (p<0.05). Conclusion: The horseback riding simulation exercise was shown to be effective for the spinal alignment of children with cerebral palsy. Therefore, additional studies should be conducted with more children with CP divided by type.

Accessory Respiratory Muscle Activation during Chest Expansion Exercise using Elastic Bands in Children with Cerebral Palsy

  • PURPOSE: The aim of this study was to evaluate activation of accessory respiratory muscles using electromyography during chest expansion upper extremity flexion, abduction, and external rotation exercises with an elastic band in children with cerebral palsy. METHODS: The subjects were 10 children with cerebral palsy. The inclusion criterion for participation was a Gross Motor Function Classification System level of I to III. The subjects were instructed to perform upper extremity flexion, abduction, and external rotation exercises with inspiration, and extension, adduction, and internal rotation exercises with expiration while seated on a chair without a backrest. PM (Pectoralis major), SCM (sternocleidomastoid), RA (rectus abdominis), and EO (external oblique) muscle activities were measured using electromyography. RESULTS: All tested muscles showed a statistically significant increase in activity after elastic band exercise. There were significant differences in PM, SCM, RA, and EO results after chest expansion exercise using elastic band. SCM showed the largest increase in activity after use of elastic bands, at $52.37{\pm}45.88$%, followed by the RA ($50.56{\pm}79.31$), EO ($35.42{\pm}35.45$), and PM ($31.72{\pm}25.64$). The increase in the SCM was greatest, followed by increases in the RA, EO, and PM CONCLUSION: These finding suggest that activity of accessory respiratory muscles increases with use of elastic bands during chest expansion exercise in cerebral palsy.

뇌성마비 환아 어머니의 사회적 지지와 부담감 (Burden and Social Support of Mothers with Cerebral Palsy Children)

  • Purpose: The purpose of this study was to identify the level of burden and social support of mothers with Cerebral Palsy (CP) children, and to determine whether providing social support is an effective strategy to relieve the burden of those mothers. Method: From October to November 2007, the data were collected from 63 mothers with CP children who were admitted and treated for CP at Y rehabilitation center in Seoul. The data were analyzed with t-test, ANOVA, Scheffe's test, Pearson's Correlation Coefficient, and Stepwise Multiple Regression using SPSS 12.0. Results: Burden of mothers was related to the Gross Motor Function Classification System (GMFCS) of the CP children. Social support was negatively correlated and GMFCS was positively correlated with burden of mothers. Social support and GMFCS was statistically significantly related with burden of mothers with CP children by multiple regression analysis. Conclusion: There was a significantly negative correlation between social support and burden. It demonstrated that social support, an important factor relating to burden imposed on mothers of CP children, can be considered as an intervening variable which can contribute to decrease of burden. In addition, applying developmental screening measures are needed to decrease disability severity.

Immediate Effect of Fabric Ankle Foot Orthosis on Balance in Children With Unilateral Cerebral Palsy

  • Balance control deficits have been indicated to be a primary problem among cerebral palsy (CP) patients. Fabric ankle foot orthosis (AFO) can allow more efficient balance control by facilitating proprioception. The purpose of this study was to investigate the immediate effect of fabric AFO on balance, compared to a barefoot condition in children with unilateral CP. Twelve children with unilateral CP participated in this study. Their balance ability was evaluated using pediatric balance scale and bubble test. Both pediatric balance scale and bubble test showed significant improvement with the use of the fabirc AFO (p<.05). The fabric AFO could improve functional balance ability, and promote better balance among children with unilateral CP. We demonstrated that fabric AFO contributed to improving balance among children with unilateral CP, classified as Gross Motor Function Classification System level I and II. Consequently, fabric AFO might be an assistive device leading to the improvement of balance instead of the typical AFOs.

  • Purpose: The purpose of this study is to examine the effects of using a horseback riding simulation on static balance in children with cerebral palsy. Methods: This study was conducted with 30 children with cerebral palsy at levels I~IV in the Gross Motor Function Classification System (GMFCS), who were randomly divided into a control group and a hippotherapy group. Results: The intergroup effects on static balance were tested, and the results showed no significant differences (p<0.05). Conclusion: The horseback riding simulation exercise was shown to be effective for the static balance of children with cerebral palsy. Therefore, additional studies should be conducted with more children with CP divided according to type.

Assessment Using the ICF-core set for Children and Youth with Cerebral Palsy: A Single Case Study

  • Purpose: This study evaluated the usefulness of International Classification of Functioning, disability and health core set for children and youths with cerebral palsy (ICF-core set for CP) by comparing the Gross Motor Function Measure-88 (GMFM-88), which is the most widely used outcome measure in children with cerebral palsy (ICF-core set for CP). Method: One subject (Female, 14) was evaluated by GMFM-88 and ICF-core set for CP. In addition, the concept of GMFM-88 was compared with the concept of the ICF-core set for CP in compliance with ICF linking rules. The numerical values both of the GMFM-88 categories and the ICF-core set for CP's items were compared. Results: The ICF b760 was linked to the GMFM-88 59~61. d415 was linked to 57~58 of GMFM-88. d450 was linked to GMFM-88's 65~72. d455 was linked to GMFM-88 at 77and 80~83. Conclusion: The association of the ICF-core set for CP and GMFM-88 was confirmed. In addition, the number of categories of ICF-core set for CP was smaller than the number of items of GMFM-88. In conclusion, the ICF-core set for CP is an evaluation tool that can identify the various features of children. In addition, GMFM-88 was linked to the ICF core set for CP according to the ICF linking rules.

아동을 대상으로 한 감각통합치료의 중재효과에 대한 체계적 고찰: 국내 연구를 중심으로 (A Systematic Review of Sensory Integration Intervention for Children in Korea)

  • 목적 : 본 연구에서는 최근 10년간 국내에서 발표된 아동 감각통합치료에 대한 중재효과를 체계적으로 분석하여 이해하기 쉽도록 제시하고자 한다. 연구방법 : 학술연구정보서비스(RISS)와 디비피아(DBpia)의 검색엔진을 사용하여 2011년에서 2020년 6월까지 출판된 국내 논문을 대상으로 하였다. 검색어는 "감각통합", "감각처리", "ASI(Ayres Sensory Integration)"이었다. 총 19개의 논문을 분석하였다. 연구결과는 PICO(Population, Interventions, Outcomes, Comparisons), 국제기능장애 건강분류(International Classification of Function, Disability and Health; ICF)방법과 수정한 The Evidence Alert Traffic Light Grading System을 사용하여 그림으로 제시하였다. 결과 : 아동 감각통합치료를 가장 많이 적용한 진단은 발달지연이었고, 중재법은 개별 감각통합치료가 가장 많았다. ICF의 신체구조 및 기능에 해당하는 중재는 91%에서 긍정적인 효과를 보였다, 세부 영역은 감각조절 및 감각처리, 소운동과 대운동, 신체도식, 신체-자기개념, 균형, 기본 움직임, 자세조절 및 손기능, 집중, 자존감이었다. 결론 : 아동 감각통합치료의 중재효과를 ICF의 분류와 그림으로 간단히 제시하여 치료사, 연구자, 아동의 가족들이 쉽게 이해하고 이용할 수 있는 근거를 마련하고자 하였다.

The effect of low-dose intravenous bisphosphonate treatment on osteoporosis in children with quadriplegic cerebral palsy

  • Purpose: Quadriplegic children with cerebral palsy are more susceptible to osteoporosis because of various risk factors that interfere with bone metabolism. Pamidronate is effective for pediatric osteoporosis, but there are no guidelines for optimal dosage or duration of treatment in quadriplegic children with osteoporosis. We aimed to evaluate the efficacy of low-dose pamidronate treatment in these patients. Methods: Ten quadriplegic patients on antiepileptic drugs (6 male, 4 female patients; mean age, $10.9{\pm}5.76years$), with osteoporosis and gross motor function classification system level V, were treated with pamidronate (0.5-1.0 mg/kg/day, 2 consecutive days) every 3-4 months in a single institution. The patients received oral supplements of calcium and vitamin D before and during treatment. The lumbar spine bone mineral density (BMD) z score and biochemical markers of bone metabolism were measured regularly during treatment. Results: The main underlying disorder was perinatal hypoxic brain damage (40%, 4 of 10). The mean cumulative dose of pamidronate was $4.49{\pm}2.22mg/kg/yr$, and the mean treatment period was $10.8{\pm}3.32months$. The BMD z score of the lumbar spine showed a significant increase from $-4.22{\pm}1.24$ before treatment to $-2.61{\pm}1.69$ during treatment (P=0.008). Alkaline phosphatase decreased during treatment (P=0.037). Significant adverse drug reactions and new fractures were not reported. Conclusion: Low-dose pamidronate treatment for quadriplegic children with cerebral palsy increased lumbar BMD and reduced the incidence of fracture.

뇌성마비 아동의 대동작운동기능 수준에 따른 흉곽발달 양상 (Development of the Chest Wall in Children with Cerebral Palsy according to GMFCS Levels)

  • Purpose: The purpose of this study was to provide quantitative data regarding development of the chest wall in children with cerebral palsy (CP) according to Gross Motor Function Classification System (GMFCS) levels and age using the radiological image diameter measurement method. Methods: Subjects included 112 children with CP and 110 healthy children, All of the children underwent simple chest x-ray. The diameters of the upper chest ($D_{apex}$) and lower chest ($D_{base}$) were measured on the anteroposterior (AP) view of a chest x-ray, and the $D_{apex}$ to $D_{base}$ ratio was calculated. Chest wall ratios were compared among children with CP at GMFCS levels I ~ III, GMFCS levels IV and V, and healthy children. Results: The results showed significant differences between the upper and lower chest wall diameters of children with CP at GMFCS levels IV and V, and healthy children (F=4.54, p=0.01; F=3.20, p=0.04). Results of comparison between the chest wall ratios of children with CP and healthy children, showed that the upper chest walls of healthy children were significantly larger in children younger than 48 months (p<0.05), and both the upper and lower chest walls of healthy children were significantly larger compared to children with CP in children older than 48 months (p<0.05). Conclusion: Radiographic measurement for examination of chest wall development is relatively simple, and the results yield quantitative data on development of the chest wall for children with CP. In addition, therapeutic interventions may be considered based on the results.

Research on the Quality of Life of the Parents by Functions, Activities, Participation, and Environmental Factors of Children with Cerebral Palsy Using ICF-CY Checklist

  • Purpose: This study examined whether the quality of life of parents of children with cerebral palsy is affected by the functions, activities, participation, and environmental factors using an ICF-CY check list. Methods: This study recruited in 26 parents of children with cerebral palsy. The functions, activities, participation, and environmental factors of the children were evaluated using ICF-CY checklist. To measure the quality of life of parents, this study used world health organization quality of life (WHOQOL-BREF), which was composed of a total of 26 questions. The GMFCS (gross motor function classification system) was used to assess the degree of disability in the children. Multiple regression analysis was performed to examine the effects of the ICF-CY checklist on the quality of life. Correlation analysis was performed to examine the correlation between GMFCS and WHOQOL. Results: The functions, activities, participation, and environmental factors were significantly different from WHOQOL-BREF. On the other hand, the contextual factor showed a significant difference in the neuromusculoskeletal and movement-related functions (b7), and service, systems, and policies (e5)(p<0.05). Conclusion: This study suggests that the functions and environmental factors affect the quality of life of parents of children with cerebral palsy. Therefore, these findings suggest that contextual factors, such as neuromusculoskeletal and movement-related functions (b7), and service, systems and policies (e5), which can be facilitators, should be considered for improving the quality of life of parents of children with cerebral palsy.
