머리-목 굽힘 기반 몸통 안정화 운동이 경직성 뇌성마비 아동의 대동작기능과 앉은 자세정렬에 미치는 효과 (The Effects of Cranio-Cervical Flexion based Trunk Stabilization Exercise on Gross Motor Function and Posture Alignment Change in Children with Spastic Cerebral Palsy)

  • Purpose: This study was to evaluate the effect of cranio-cervical flexion based trunk stabilization exercise on gross motor function and posture alignment change in children with spastic cerebral palsy. Design: Randomized Controlled Trial. Methods: Twenty-six children participated in this study. All subjects were randomly assigned to either the Cranio-Cervical Flexion Based Trunk Stabilization Exercise (CCFTS) group (n=13) or the Trunk Stabilization Exercise (TS) group (n=13). In both groups were trained general physical therapy for 10 minutes, in the CCFTS group was trained cranio-cervical flexion based trunk stabilization exercise for 20 minutes and in the TS group was trained trunk stabilization exercise for 20 minutes. The training was provided 2 times a week during 8 weeks. All subjects were measured with the Gross Motor Function Measure (GMFM) and Cranio-Vertebral Angle (CVA) before and after intervention. Results: The results showed that the CCFTS have increased significantly in GMFM (B, C, D and E-dimension) and CVA, and the TS group have increased significantly in GMFM (B, C D and E-dimension). In particular, the CCFTS group improved significantly than TS group in GMFM (B, C and D-dimension) and CVA. Therefore, the cranio-cervical flexion based trunk stabilization exercise improved gross motor function and posture alignment in children with spastic cerebral palsy. Conclusion: These results suggest that cranio-cervical flexion based trunk stabilization exercise is feasible and suitable for individuals with a spastic cerebral palsy and can be used in addition to conventional physical therapy.

The Effects of Sensory Integration Training on Motor, Adaptability and Language Development in 3-5 Year-old Children with Developmental Delay

  • The purpose of this study is to examine the effects of sensory integration training on children with developmental delays. To achieve this goal, an educational experiment is conducted in five main areas: gross motor ability, fine motor ability, adaptive ability, language and social ability in children with developmental delay. The study subjects were children with developmental delays aged 3-6 years diagnosed at Beijing Institute of Pediatrics and Beijing Medical University and received sensory integration intervention and homebased training at the Golden Rain Forest Beijing Tongzhou Center from 2018 to 2021. According to the purpose of the analysis, the data collected are subjected to descriptive statistics using SPSS 21.0 statistical program, Two-way MANOVA analysis, and data analysis method of multivariate analysis is used to process the collected data. In addition, a total of 39 subjects were selected, including 19 children who received sensory integration training and 20 children who only received family training. The results show that the sensory integration training group outperformed the home training group in all aspects and developmental quotient, but the home training group also showed higher levels of significance for improvements in gross motor, fine motor and developmental quotient.

대동작 기능 평가도구(GMFM)의 Rasch분석 (Application of Rasch Analysis to the Gross Motor Function Measure: A Preliminary Study)

  • The purpose of this study was to validate the Korean version of the Gross Motor Function Measure (GMFM) using Rasch analysis. The data was obtained from the assessments of 59 children with cerebral palsy in Korea and were applied to Rasch's rating scale model to estimate the difficulty and goodness-of-fit of each item. Rasch modeling helped us to identify 76 items from the original 88-item GMFM that form an unidimensional hierarchical scale to rearrange 76 items in order of difficulty. Reliability coefficients of the 88-item and 76-item GMFM were .99 and .99, respectively. In this preliminary report, the Korean version of GMFM seems to have significant validity and reliability. These results may be useful in assessment of gross motor functions in children with cerebral palsy.

  • Purpose : The purpose of this study was to examine the effects of lower trunk muscles strengthening in athetosis children with cerebral palsy. Methods : One children with cerebral palsy participated in the case study. The age was 6 year. GMFM (gross motor function measure) was used to measure the functional movement ability. Lower trunk strengthening exercise were performed 3 times a week for 12 months. The measurements were taken before and after the exercise program. Results : In this study, the lower trunk strengthening exercise program was effective for gross motor functions. The children with athetosis type showed improvement in the Walking, Running & Jumping in GMFM. Conclusion : Therefore, the lower trunk strengthening exercise program was effective for the gross motor in athetosis type of cerebral palsy.

  • Purpose : The purpose of this study was to examine the correlation between Pediatric Balance Scale(PBS) and Gross Motor Function Measurement(GMFM) with cerebral palsy(CP) children. Methods : 31 ambulatory children with CP were recruited for this study. PBS is a modified version of the Berg’s Balance Scale to access the children with motor impairments. To access the motor functions that directly influence to the functional independence of the ambulatory CP children, total scores of standing category and walking/running/jumping category of GMFM were selectively compared with PBS score by Spearman correlation coefficient analysis. Results : The results revealed high correlation between PBS and GMFM scores both in standing and walking/running/jumping categories(r=.9). Conclusions : Therefore, this study indicates that PBS can be applied not only as a tool to access balance, but also as a measurement to predict and access the level of standing and ambulatory related functions of children with CP.

선택적 운동 조절 척도와 기능적 수행도 평가 간의 상관: 경직형 뇌성마비 아동을 대상으로 (Correlation Between Selective Motor Control Test and Functional Performance Evaluation in Children With Spastic Cerebral Palsy)

  • 이 연구는 경직형 뇌성마비 아동의 운동 손상를 측정하는 도구로서 선택적 운동조절능력 척도에 대한 기초 자료를 제공하고 선택적 운동조절능력과 대동작 기능 분류 체계, 대동작 기능 평가 결과, 그리고 일상 생활동작 평가 결과 사이의 상관성을 알아보는데 그 목적이 있다. 이를 위해 68명의 경직형 뇌성마비 아동을 대상으로 선택적 운동조절능력, 대동작 기능 분류 체계 평가, 대동작 기능 평가, 그리고 일상생활동작 평가를 실시하고 상관을 알아보았다. 그 결과 선택적 운동 조절 척도 등급은 대동작 기능 분류 체계 평가(r = -.485)와 대동작 기능 평가(r = .482)와 유의한 상관을 보였다(p<.05). 하지만 일상생활동작과는 유의한 상관을 보이지 않았다(p>.05). 이는 이 척도가 경직형 뇌성마비 아동의 운동 손상 중 선택적 운동조절능력을 평가하는 평가도구로써 유용하게 사용될 수 있음을 시사한다.

Correlation Between the Activities of Daily Living Assessment and Gross Motor Function Measures in Children with Spastic Cerebral Palsy

  • Purpose: This study was to identify the whether there is any correlation between Gross Motor Function Measures (GMFM) and Functional independence Measurement for children (WeeFIM) in order to identify a relevance of daily life motor with gross motor function of children with cerebral palsy. Methods: Subjects were instructed to evaluated by an occupational therapist using WeeFIM (reliability 0.83-0.99) and by a physical therapist using GMFM (reliability 0.93). 10 male children and 13 female children among 23 children being diagnosed as cerebral palsy were targeted for this study. A correlation between GMFM and WeeFIM was analyzed with Kendall correlation. Results: Participants showed significant correlation between GMFM and WeeFIM in all categories, except in the GMFM's walking running jumping category, standing category of GMFM and self care, sphincter control, communication category of WeeFIM. Lying rolling and sitting and crawling kneeling area of GMFM showed a significant correlation with self care(r=0.35, 0.39, 0.4), sphincter control (r=0.45, 0.43, 0.61), mobility transfer (r=0.44, 0.36, 0.64), locomotion (r=0.41, 0.35, 0.74), communication (r=0.4, 0.44, 0.51), and social cognition (r=0.43, 0.51, 0.64) area of WeeFIM (p<0.05). Standing of GMFM and mobility transfer (r=0.74), locomotion (r=0.47) and social cognition (r=0.4) area of WeeFIM showed a significant correlation (p<0.05). Conclusion: These findings indicated significant correlation between activity of daily living (ADL) and gross motor function of children with cerebral palsy. However, because there are a few results that are not significant, both assessment tools should be used for evaluations so that treatment can be achieved with an accurate assessment and establish a therapeutic plan.

뇌성마비 아동의 대동작 기능에 대한 가정중심치료 효과 : 체계적 고찰 (Effectiveness of home-based therapy on gross motor function in children with cerebral palsy: A systematic review)

  • Background: Although children with cerebral palsy (CP) are able to walk independently, gait imbalance occurs due to abnormal muscle tone, musculoskeletal deformity, loss of balance, and selective motor control impairment. Gait restriction in the community and school is a major problem of rehabilitation in CP. Home-based therapy (HBT) provides a variety of interventions in which the therapist and the parent work together to resolve the activities and problems caused by the child's body structure. Therefore, we investigate the effectiveness of home-centered therapy on gross motor function in CP and try to present the possibility of clinical application. Design: A Systematic Review Methods: Research papers were published from Jan, 2012 to Jan, 2022 and were searched using Medline and PubMed. The search terms are 'family-centered' OR 'home-based' AND 'cerebral palsy'. A total of nine papers were analyzed in this study. The paper presented the quality level based on Physiotherapy Evidence Database (PEDro) scores to assess the quality of randomized clinical trials studies. Results: The results showed that HBT for strengthening exercise in lower extremity has a positive effect on the isokinetic torque and gross motor function. home-based treadmill therapy in CP is effective to perform at least 12 sessions of treadmill HBP in which the therapist determines the treadmill speed every week and the child's own gait pattern is modified. Conclusion: These results suggest that it will be important data for founding evidence on the effectiveness of home-centered therapy on gross motor function in children with cerebral palsy to advance clinical protocols.

보바스 접근방법이 정신지체 아동의 균형 및 운동능력에 미치는 영향: 단일사례연구 (The Effect of the Bobath Approach on Balance and Motor Ability in Mentally Retarded Child)

  • Background: The purpose of this study was to present a practical method of medical treatment to improve the balance and motor ability of the mentally retarded child with a single mentally retarded child-subject. Methods: The subject of the study was a 39-month-old mentally retarded female. This study included a 2-week basic period and a 13-week treatment period. The treatment method was based on the Bobath Approach. Gross motor function measurement (GMFM) was used to examine changes in motor ability, and the Pediatric Balance Scale (PBS) was used to measure changes in balance ability. The curative program was composed of normalization of muscle tone, strengthening of leg endurance and muscular strength, the improvement of trunk alignment, and the increase of balance. Visual rate of change was used to examine the results. Results: As a result of this study, balance ability increased on the Pediatric Balance Scale (PBS) by 24 points, and motor function increased in terms of Gross Motor Function Measurement (GMFM) by 6.9% (18 points). Standing increased by 41% (16 points), and walking, running, and jumping increased by 31.9% (23 points) compared to thebasic period. Therefore, the Bobath Approach appears to be an appropriate method to improve balance and motor ability in mentally retarded children. Conclusion: It is surmised that aggressive intervention by physical therapists and occupational therapists, and a follow-up study, are required for the growth of motor ability in mentally retarded children.

프로바디마사지가 뇌병변 장애우의 신체적 특성과 대동작 기능 및 관절 가동범위에 미치는 영향 : 사례연구 (Effects of the Probody Massage on the Physical Characteristics, Gross Motor Function and ROM in Youth with Cerebral Palsy : Case study)

  • Objective : The purpose of this study was to investigate the effects of the Probody Massage Program on the physical characteristics, gross motor function and ROM (Range Of Motion) of children with cerebral palsy. Method : The subjects of this study were two children who have been diagnosed with first grade cerebral palsy that utilized T development support center located in B Metropolitan city for 8 weeks, twice a week, to carry out the Probody Massage Program for 30 minutes. Physiological reactions (height, weight, BMI, blood pressure (an index of inflammation), pulse rate) and large operating functions (sitting, crawling and the joints' range of motion as an angle of the shoulders' upper limb articulation) were measured pretest, after 4 weeks, and after 8 weeks. Results : The Probody Massage Program showed positive changes in physical characteristics (blood pressure, sitting, and crawling), gross motor function (upper limb shoulder movement), joint range of motion, height, body weight, metabolic activation and blood circulation of children with cerebral palsy. Conclusion : We believe making a practical impact on the growth and development, functional recovery of daily life, and improvement of quality of life of children with cerebral palsy by utilizing Probody Massage Program improves blood pressure (an index of inflammation), pulse, sitting, crawling, and the joints' range of motion as an angle of the shoulder joints' upper limb movement of children with cerebral palsy.
