The Comparison of Plantar Foot Pressure Distribution in Adult and Elderly according Obstacle Heights (장애물 높이에 따른 성인과 노인의 족저압 분포 비교)

  • Purpose: The purpose of this study was to compare plantar foot pressure distribution in adults and elderly according to obstacle height. Methods: Nine healthy adults and nine older adults were recruited and the subjects provided written informed consent consent prior to participation. Both groups walked and crossed obstacles with heights of 0%, 10%, 20%, and 30% of their height. Foot pressure was measured by peak pressure using the Pedar System (Novel Gmbh, Germany) during obstacle walking with barefeet in shoes. Three trails were calculated on eight areas and then averaged for data analysis. Results: A significant difference in great toe, little toes, and lateral metatarsal area was observed between adults and elderly groups, but other areas did not show significant differences. Foot pressure was increased in groups according to obstacle height. Conclusion: These findings showed that change in foot pressure distribution is more lateral in elderly in order to maintain postural control during obstacle crossing.

  • Since the first report by Freeman of renal artery repair for renovascular hypertension in 1954, there has been a gradual improvement in the results of renovascular reconstruction. This is case report of renovascular hypertension due to diffuse primary arteritis, which was performed aorto-bllateral renal bypass graft with using of Y -woven dacron graft. This 33 y-o male has complained intermittent headache, facial edema & malignent hypertension symptoms for 1.5 years. He had the history of Rt. B-K amputation due to unhealed wound after trauma of the Rt. great toe about 7 years ago. The abdominal aortography revealed nearly not visualized the Rt. renal artery & severe narrowing of the Lt. renal arterly. During postop. course, Blood pressure was well controlled. At postop. 3rd week, systolic pressure was down to 130-140mmHg But, diastolic pressure was remained to 100-110mmHg. At postop. 30th day, exploration was done due to intestinal obstruction signs. But severe ischemic enteritis was occured due to fibrotic obstruction of the superior mesenteric artery. The next day, he was dead. in spite of Rt. common iliac artery-sup. mesenteric artery bypass graft.

  • Epidermal inclusion cyst is a common mass in life. It is covered with a stratified squamous epithelium, thus, there is a granular cell layer adjacent to the keratin-containing cyst lumen. It can be caused by mechanical force, trauma, or a spontaneous event. It can rupture spontaneously or be ruptured by external mechanical forces. Epidermal inclusion cysts that exhibit inflammation or recur should be removed by simple excision. In this case, the patient showed an epidermal inclusion cyst under an anterolateral thigh free flap, which can cause the palpable mass to go unnoticed. First we thought he had neuroma formation after a surgical procedure on his foot. However it was an epidermal inclusion cyst, which was diagnosed by a special pathologist. It is a curious and rare case.

  • Hallux valgus is a deformity characterized by lateral deviation of the great toe and medial deviation of the first metatarsal. When planning an operative treatment, it is important to realize that the deformity is tridimensional and diverse. Operative techniques include medial eminence resection, distal soft tissue procedure, first metatarsal osteotomy (distal, diaphyseal, proximal, or multiple), proximal phalanx osteotomy, arthrodesis (first metatarsophalangeal or metatarsocuneiform joint), and so on. Among these techniques, osteotomy is the main procedure for correcting the hallux valgus. The objective of this article is to describe the characteristics and recent advancements made for corrective osteotomies in the hallux valgus. The pathophysiology of the hallux valgus is also described.

  • Intradural extramedullary spinal metastasis from systemic tumor is extremely rare but epidural extramedullary cord tumor metastasis from the lung is relatively common. A 57 year-old male patient was admitted to department of internal medicine and neurosurgery in Chonbuk National University Hospital because of coughing, low back pain radiate to the right great toe, and numbness of the right calf area. Spinal MRI scan revealed round oval shaped mass lesion on just below the level of the conus medullaris. Authors present the clinical, histologic, radiologic features of spinal intradural metastatic tumor and operative total removal followed by chemothrapy with an extensive review of literatures.

  • In appilcation of wide latitude HR-C film to chest x-ray examination, former x-ray diagnosis area is larger and diagnostic information has great deal of promotion. HR-C film is compare to former x-ray film is larger latitude and density level is small, reading is very easily. Especially, high estimate that is in characteristic curve linearity of toe part is good, contrast of low density made good shape and not good describe to overlap is diagnostic information increase mediastinum portion etc.

  • A 55-year-old woman who was diagnosed as Diabetic foot was admitted to Dong-so Oriental-medical hospital on April 15th, 2002. Her great toe and secondary were to be tut off, and she was much depressed. We diagnosed her constitution as Taeumin(太陰人), and classified her symptom as Taeumin-Ban-Chang-Byung( 太陰人 ?瘡病 ) and prescribed Yuldahansotang(熱多寒少湯) for the purpose of chengganjoyoil(淸肝燥熱). During admission, the ulceration had nearly disappeared and so she could save her pre[ious toes. From this case, even though we could not say that Diabetic foot is equal to Ban-Chang-Byung but could suppose that there was some relationship between one and the other.

  • This study was intended to implement ankle joint dorsi flexion training against ankle muscule strength weakening that erodes stroke patients' gait performance to examine the effect of the training on stroke patients' plantar pressure and gait ability. In this study, 36 stroke patients diagnosed with stroke due to cerebral infarction or cerebral hemorrhage were divided to measure 10MWS which are stroke patients' gait variables maximum plantar pressure by area of the sole by collecting data using an F-scan system during gait. Given these results of the study, compared to other training groups, the ankle muscule strength reinforcing training group showed statistically significant increases of maximum plantar pressure in the great toe, the toe and the first metatasal areas too and thus it can be said that this training increases forward thrust during stroke patients' foot end taking off and positively affects stroke patients' ability to perform gait.

The Classification of Foot Types of Junior High School Boys for the Development of Shoes' Easy-Order Prototype (신발류 이지오더 Prototype 개발을 위한 청소년의 발의 형태 분류)

  • The health of feet is connected with individual's health and affects a man's activity. Shoes need to be designed to protect feet and to absorb the impact of land. In order to choose suitable shoes for feet, the foot size and shape must be considered, so it is essential to grasp the exact size and shape of the foot. This study aims to present fundamental data on shoes' easy order prototype development for choosing shoes of good wearing comfort, by classifying feet size and shape junior high school boys in the early adolescent period. The subject were 234 Korean junior high school boys age from 14 to 16 years old. The subjects were directly measured anthropometrically and indirectly analyzed photographically. 6 factors were extracted through factor analysis and those factors comprised 79.42% of total variance. The factors characterizing foot girth and width, foot length, foot height, foot shape around the fifth toes, angle of foot breadth and foot shape around the first toes. 3 clusters as their foot shape were categorized using 6 factor scores by cluster analysis. Type 1 was characterized by long large foot with deformed first toe. Type 2 had smaller in foot girth, width and length than other types and with deformed fifth toe. Type 3 had average size and high foot shape. Shoes prototype which is to be developed later on will be able to generate 2D flattening in the foot sole form. Therefore, it would be a great support in producing and choosing appropriate shoes if forms are classified by subdividing foot form classification and extract a factor which shows only the foot sole shape.
