Video-Assisted Thoracoscopic Thymectomy -Report of Two Cases- (흉강내시경을 이용한 흉선 절제술)

  • We report two cases of the thoracoscopic thymectomies for patients of myasthenia gratis with tracheostomy (Osserman's Group-ll-C-1). The Irst case was 47-year-old male wlth generalized myas- thenia gravis who was under the mechani al ventilator therapy with tracheostomy. The second case was 60-year-old male with deteriorating generalized myasthenia gratis after the mechanical ventilator therapy. We decided to resect the thymus by vldeo-assisted thoracoscopy to prevent the ouurrence of postoperative complications, especially mediastinitis because all two cases were under tracheostomy state. We could stop the mechanical ventilator therapy on the postoperati'fe 16th day and 3rd day respect- ively and they were recovered without mediastinitis. So we concluded that video-assisted thoracoscopic thymectomy is a good alternative surgical method for myasthenia gratis patients with tracheostomy.

Editor's Perceptions and Attitudes Toward Open Access Publishing (오픈액세스출판에 대한 학술지 편집인의 인식 연구)

  • This study surveyed editors of 309 gratis open access journals in Korea in order to examine the reason why they decided to open their journals to the public on the Web. Also this study examined their perceptions and attitudes toward open access publishing. The main reasons of open access publishing in Korea were expansion of free use, increasing citation, and international trends regarding open access in their scholarly fields. Also, 65% of all respondents were aware of the concepts of open access, and 75% supported open access. The reasons of supporting open access were sharing of research results with the public, increasing the availability of peer researchers, and enhancing journal reputations.

An Analysis of Income Models for Open Access in Korean Scholarly Journals (오픈액세스 수입원 분석을 통한 국내 학술지의 성향 연구)

  • The purpose of this study is to examine the tendency of open access income models in gratis open access journals and fee-based online journals in Korea. This study found that there was no clear difference between two groups and fee-based online journals had several of the same characteristics of open access journals. That is, the societies published fee-based journals have requested APC to authors and additional page charges for the articles with research funding. Also, in case of fee-based online journals, the journals received subsidies from external funding agencies were more than free OA journals. These findings show that fee-based online journals in Korea have a lot of capability to transition to open access journals.

A Proposal for the Acceptance and Processes of Open Access in Korean Societies (국내 학회의 오픈액세스 수용과 추진방식에 대한 제안)

  • The purpose of this study was to propose a guide for the acceptance and processes of open access in Korean Societies. Firstly, this study developed journal publishers' open access model focused on the relationship between types and strategies of open access. Secondly, this study investigated current status of journal publishers' open access strategies and the adoption degrees through SHERPA/RoMEO, DOAJ and PMC databases. Finally, on the basis of the result, this study proposed a guide to help Korean societies make journals more open or less open according to their situations as real. Korean societies can make informed decisions about how to derive open access.

Surgical Results for Myasthenia Gravis (중증근무력증의 수술적 치료 결과)

  • Myasthenia gravis is relatively rare disease which is related autoimmune response. There are various methods of management for myasthenia gravis, but nowaday radical thymectomy is the treatment of choice in the aspect of bringing out complete remission and clinical improvement. Sixteen patients of myasthenia gratis underwent radical thymectomy during last eight years, and its result was analysed. Complete rem ssion was achieved in five patients (31 %) and pharmacological or symptomatic improvement in seven patients (44%), thus giving a total remission in 12 patients (75%). Postoperative result was not correlated with age, sex, degree of preoperative symptom, surgical approach, pathologic diagnosis.

A Study on A Model Sample for Guidance System for Copyright of Domestic Journals and Open Access Policy (국내 학술지 저작권 및 오픈액세스 정책 안내시스템 모형 연구)

  • The study aimed to suggest a model sample for guidance system for copyright of domestic journals and open access policy. Toward this end, analyses were conducted to examine copyright of domestic journals and open access environment. As a result of analyses, it turned out that 33.8% of domestic journals had 'regulations on ownership of rights to papers published in journals,' and that the subject to ownership of rights to papers published in domestic journals accounted for 28.8%, which was the highest rate. Of domestic journals, 34% charged a subscription fee, and they were toll access journals, and 56% were free access journals. As for system examples of guide system for copyright of foreign journals and open access policy, analyses were conducted to examine and investigate SHERPA/RoMEO in the U.K. and SCPJ in Japan and generate considerations at a time of domestic application. What needs to be taken into account is that overseas examples are focused on collecting and introducing self-archiving policy by authors for academic journals, so there are limitations in offering information including open access publication policy for domestic journals. Based on the analytical result, the study designated the purpose, direction and four steps that need to be considered at a time of development of guide system models for copyright and open access policy for domestic journals before suggestion of the basic direction and operational methods by stage.

Clinical Analysis of Surgical Treatment and Risk Factors of Thymoma (흉선종의 수술적 치료 및 그 위험인자에 관한 임상적 고찰)

  • Though thymoma is considered benign In a histopathologic specimen, its unusual behavior makes it important for surgeons to manage this neoplasm as cancerous lesion. Hence we clinically analysed the surgical cases of thymoma in our hospital, And we suggest the risk factors for its prognosis From January 1987 to December 1994, we experienced 41 surgical cases of thymoma, excluding thymic carcinoma and cysts. There were 21 male and 20 female; age ranged from 16 to 64 years. Among them, myasthenia gratis was present in 22 patients(53.7%). Surgical treatment consisted of complete resection in 31 patients, partial resection In 7 patients, and biopsy only in 3 patients. According to Masaoka's classification, there were 27 patients in milage 1, 4 patients in stage II, and 10 patients In stage III. Histopathology was of epithelial type in 14 patients, Iymphocytic type in 11, and mixed type in 19. Eleven patients had adjuvant radiotherapy, chemotherapy, or b th and there was no surgical mortality. Postoperative follow-up ranged from 1 to 88 months (mean )6 months) and three patients died and 5 patients suffered recurrences during the follow-up period. Postoperative risk factors were advanced Masaoka stage, invasiveness, and surgical method.

Measurements on Legislation of User-Protection Act in the Era of ICT-Convergence (ICT융합에 따른 방송통신 이용자보호 법제의 합리적 개선방안)

  • This article aims at the legislation of User-Protection Act in area of ICT. In these days telecommunication and broadcasting are getting more and more convergent. The paradigm of ICT is turning over from the service provider to the end-user. User protection has been in each erea of ICT (C-P-N-D) individually regulated. In the area of telecommunication it is important to protect the interest of user, who stands in contract with the service provider. And in area of broadcasting it is important to protect the interest of viewers, who stands "gratis" with the broadcasters without any contracts. For the more efficient user-protection it is also necessary to make a dedicated organization under KCC(Korean Communications Committee). In this early year the government organization was divided into MSIP(Ministry of Science, ICT and future planing) and KCC. The user-protection act will be very important instrument of ICT regulation in the era of creative economy. It is necessary to establish a new frame act of user protection. It is also necessary to make start to establish a new system of user education in erea of ICT. It is strongly expected the new act will be a turning point of ICT development in Korea.

Angiographic Results of Radial Artery Grafts that are Used for Myocardial Revascularization (관상동맥 우회술 후 혈관조영술을 이용한 요골동맥의 개통률 분석)

  • Background: The radial artery is gaining widespread acceptance as complementary arterial conduits for surgical myocardial revascularization, but there have been limited reports about its angiographic patency compared with that of internal thoracic artery or saphenous vein. We tried to evaluate angiographic patency of radial artery graft and to compare that of radial artery and other gratis with retrospective manner. Material and Method: From January 2001 to Jure 2006, totally 132 patients (male 92, female 40) who underwent coronary artery bypass graft using radial artery were re-admitted to our hospital for follow up angiographic examination. Mean age was 58.2+8.87 and mean follow up duration was 32 month ($2{\sim}110$ month). Off pump and on pump bypass surgery were performed 74 and 58 patients respectively. Along with radial artery, left internal thoracic arteries were used in 57 cases, concomitant left internal thoracic artery and saphenous veins were used in 47 cases and bilateral internal thoracic arteries were used in 20 cases. Result: Totally 412 distal anastomosis were performed and 376 anastomosis remained patent (91.2%). Left internal thoracic artery showed the most excellent patency in all of the conduits (98.5%). Radial artery graft patency was 90.8% (169/186). There was no statistical difference of the patency by conduit between on-pump and off-pump group. But radial artery showed more higher patency rate (98/110, 89%) in the severe stenotic lesion that preoperatively revealed more than 90% stenosis than in the lesser severe (<90%) stenotic lesion (60/76, 78%)(p < 0.005). Radial artery conduit represented the worst result when it was grafted in the right coronary system. But when it was positioned in the left heart especially diagonal or obtuse marginal area, patency was comparable with left internal thoracic artery. Conclusion: Radial artery graft showed good midterm patency when it was used in the severe stenotic lesion more than 90% and left coronary system. But great notice should be taken when it is grafted in the right coronary system or less severe stenotic lesion.
